Background We evaluated taste functions of patients with perennial allergic rhinitis (AR) before ... more Background We evaluated taste functions of patients with perennial allergic rhinitis (AR) before and after allergen-specific immunotherapy (AIT). Methods The study was designed as a prospective clinical study in our tertiary care hospital. Patients (n = 21) who were diagnosed with perennial AR on the basis of physical examination, skin prick test of at least 3* for HDM allergen and treated with AIT were enrolled in this study. A control group (n = 21) was selected from patients who were given intranasal steroids (INS) for perennial AR. Both groups had self-reported hyposmia and subjective loss of the sense of taste before treatment. Taste strips (Burghart, Wedel, Germany) were used for the taste identification scores before and after 6 months treatment. Results A total of 42 subjects were included, with a mean age of 24.1 ± 7.9 years (range 15-43 years). Overall, the AIT group showed more of an improvement of taste function, observed in the total average test scores, compared to the INS group (p < 0.05), but no change was detected between the groups before treatment. No difference was found for the bitter taste scores between the study groups (p = 0.053). Conclusion Subcutaneous allergen immunotherapy resulted in more of an improvement in taste function than intranasal steroids. Further studies are needed.

Management of recurrent epistaxis in patients on anticoagulant therapy is a challenging problem. ... more Management of recurrent epistaxis in patients on anticoagulant therapy is a challenging problem. In this article, we report our experience concerning the use of bivalve septal teflon splint (BSTS) for the treatment of recurrent mild epistaxis in a group of patients who underwent anticoagulant therapy after cardiac valve surgery. The study included 18 patients whose epistaxes recurred despite standard methods such as local pressure, vasoconstrictors, sedation, packing or cauterization. BSTS was sutured on the both sides of the nasal septum and held in place during one month. Epistaxis was controlled in all of the patients. We believe that the use of BSTS is an effective, easily applied, non-traumatic, well-tolerated additional method for the treatment of recurrent mild epistaxis in patients undergoing anticoagulant therapy, when other conventional treatments fail. Further controlled studies with larger groups are warranted in order to further evaluate this method.

We report a rare case of isolated extramedullary plasmacytoma (EMP) of the right true vocal fold ... more We report a rare case of isolated extramedullary plasmacytoma (EMP) of the right true vocal fold in a 38-yearold male with a one-year history of hoarseness. Immunohistochemical staining of plasma cells in the tumour, showed over 90 per cent of them to be positive for kappa light chains. After two attempts at local surgical excision and recurrence within 10 months, the tumour was irradiated. Only seven reported cases of isolated EMP of the true vocal fold are reported in the literature. The therapeutic options are discussed.

Isolated sphenoid sinusitis is a rare disorder. There are some difficulties in its diagnosis and ... more Isolated sphenoid sinusitis is a rare disorder. There are some difficulties in its diagnosis and the first presentation of this disorder might be with complications. These complications are essentially due to the anatomical location of the sinus and its proximity to the intra-cranial and orbital contents, to which infection may easily spread. In this paper, we report a case of isolated sphenoid sinusitis in a 12-year-old girl who was presented with unilateral VIth nerve palsy. She was initially treated with parenteral antibiotherapy, the abducens nerve palsy recovered but as sphenoiditis persisted she underwent an endoscopic sphenoidotomy. We discuss the clinical features, the diagnostic tools, and the treatment options for this entity.

We present a case with a tonsillar metastasis from unusually localized primary signet ring-cell carcinoma of the cecum.

Silver-Russell sendromu (SRS), büyüme ve gelişme geriliği, beşinci parmakta klinodaktili ve üçgen... more Silver-Russell sendromu (SRS), büyüme ve gelişme geriliği, beşinci parmakta klinodaktili ve üçgen yüz ile karakterize nadir bir sendromdur. Silver-Russell sendromu tanısıyla takip edilen 3.5 yaşındaki erkek hasta kliniğimize horlama, burun tıkanıklığı ve uyku apnesi yakınmaları ile başvurdu. Fizik muayene ve diğer incelemelerde SRS semptomlarının yanı sıra adenotonsiller hipertrofi ve sağ efüzyonlu otitis media tespit edildi. Sağ ventilasyon tüpü takıldı ve adenotonsillektomi (AT) uygulandı. Ameliyat sırasında ağız açacağı yerleştirilirken hastada bradikardi gelişti. Muhtemelen vagal iritasyon sonucu ortaya çıkan bradikardi, dil bir tarafa ekarte edilip ağız açacağı yerleştirilince düzeldi. Hastanın takibinde büyüme hızında artış görüldü. Silver-Russell sendromlu hastalarda tıkayıcı uyku apne sendromunun erken tanı ve tedavisi önem taşımaktadır. Gelişme geriliği ve mikrognatisi olan SRS'li hastalarda AT yapılırken, hastanın ağız boşluğunun da hacmen küçük olduğu göz önünde bulundurulmalı, cerrah ağız açacağını yerleştirirken dikkatli olmalı ve anesteziyoloji uzmanları olası bradikardi gibi komplikasyonlara karşı hazırlıklı olmalıdır. Objective: Tumour markers play an important role in the diagnosis of cancer and the early detecti... more Objective: Tumour markers play an important role in the diagnosis of cancer and the early detection of recurrences during follow-up. This study aimed to assess the clinical value of measuring urinary N-acetyl-beta-d-glucosaminidase (U-NAG) levels in patients with laryngeal squamous cell carcinoma. Design: Prospective, controlled study. Setting: Tertiary university hospital. Patients and methods: Our study included 21 patients with primary laryngeal squamous cell carcinoma and 17 cases with recurrent laryngeal squamous cell carcinoma. U-NAG levels of patients with tumours at different stages were compared with a control group of 19 healthy individuals with no known cancer. Main outcome measures: Patients' age, TNM stage, blood urea, glucose, alanine aminotransferase (ALT), and U-NAG. Results: A statistically significant difference was found between U-NAG levels of patients with a primary tumour (preoperative period) and the control group (one-way analysis of variance, p = .00; Dunnett's t-test, p = .00). In the postoperative period, the observed trend was that of a significant decrease among primary cases (paired t-test, p = .00). U-NAG levels of patients with a recurrent tumour and patients with a primary tumour (preoperative period) were not significantly different (Tukey honest significant difference test, p = .841). There was no statistically significant difference for blood urea (t = -1.95, p = .064), glucose (t = -1.84, p = .074), or ALT (t = -1.79, p = .080). No significant relationship was found between the TNM stage and preoperative U-NAG levels (p > .05). Conclusions: These results suggest that U-NAG might be used in the diagnosis of laryngeal carcinoma and the early detection of recurrences during follow-up. Further investigations are warranted to clarify the prognostic significance of U-NAG levels.

We present an unusual type of first branchial cleft anomaly involving the external auditory canal, the middle ear and the nasopharynx through the eustachian tube.

An important hydrotomu should be done in the retroauricular sulcus to obtain a plane surface that goes from the posterior face of the pinna to the skin over the mastoid surface. This plane surface is the donor site and allows the harvesting with the electric dermatome. The characteristics of this graft are: The graft have the same color of the face, the same operative field, yielding an unremarkable scar in the donor site. The thickness is limited to 3 tenths of mm, and his size to 6 x 4 cm approximately.

The effectiveness of odiosoft-rhino (OR) in the evaluation of nasal obstruction was investigated in a blind comparison at a referral center, institutional practice. Patients: Fortyeight patients with perennial allergic rhinitis and 52 healthy subjects were studied. Nasal endoscopic examination, acoustic rhinometry (AR), and OR were performed and symptom scores compared. Main Outcome Measure: Assessment of nasal blockage with OR method was the main outcome measure. Results: Using the OR technique, significant differences were observed between the patient and control groups. OR data correlated with symptom scores and endoscopic examination. However, this was not observed with AR. A 15.5-dB cutoff point for the left side and 14.5 dB cutoff point for the right side at the 2,000 to 4,000 Hz frequency interval resulted in 93.8% sensitivity and 92.3% specificity and 72.9% sensitivity and 80.8% specificity, respectively. A 8.5-dB cutoff point for the left and right sides at the 4,000 to 6,000 Hz frequency interval resulted in 87.5% sensitivity and 80.8% specificity and 70.8% sensitivity and 78.8% specificity, respectively. Conclusion: OR is a simple, noninvasive test for assessing nasal obstruction. The OR technique can detect nasal obstruction with high sensitivity and specificity, and these findings correlate with symptoms and physical examination.

The effectiveness of odiosoft-rhino (OR) in the evaluation of nasal obstruction was investigated in a blind comparison at a referral center, institutional practice. Patients: Fortyeight patients with perennial allergic rhinitis and 52 healthy subjects were studied. Nasal endoscopic examination, acoustic rhinometry (AR), and OR were performed and symptom scores compared. Main Outcome Measure: Assessment of nasal blockage with OR method was the main outcome measure. Results: Using the OR technique, significant differences were observed between the patient and control groups. OR data correlated with symptom scores and endoscopic examination. However, this was not observed with AR. A 15.5-dB cutoff point for the left side and 14.5 dB cutoff point for the right side at the 2,000 to 4,000 Hz frequency interval resulted in 93.8% sensitivity and 92.3% specificity and 72.9% sensitivity and 80.8% specificity, respectively. A 8.5-dB cutoff point for the left and right sides at the 4,000 to 6,000 Hz frequency interval resulted in 87.5% sensitivity and 80.8% specificity and 70.8% sensitivity and 78.8% specificity, respectively. Conclusion: OR is a simple, noninvasive test for assessing nasal obstruction. The OR technique can detect nasal obstruction with high sensitivity and specificity, and these findings correlate with symptoms and physical examination.
