Conclusions: The functional interplay between the insular region and the thalamus underpins salience attribution, which is a key cognitive asset for survival in the extrauterine environment. Accordingly functional synchronisation of these regions, as part of the Salience Network, has been previously reported already at birth. We report novel evidence showing how maternal mental states may impact on fetal functional neurodevelopment. We argue that maternal anxiety may influence functional connectivity patterns through the increase of fetal cerebral blood flow, stemming from reduced uteroplacental perfusion as a consequence of norepinephrine release.

Her previous baby had had a double outlet right ventricle. Voluson E8 ultrasound equipment with a volumetric RAB 4–8D MHz probe was used. The scan showed a normal four-chamber view of the heart with the pulmonary vein draining into the left atrium (Figure 1) and normal angiographic study of the left ventricle and ascending aorta (Figure 2). Additional structures also appeared normal. The newborn showed normal postnatal echocardiographic and cardiologic outcomes. HDlive differs from conventional rendering methods because it uses a fixed virtual light source that calculates the propagation of light through skin and tissue. Operators can freely select the light source at any angle relative to the ultrasound volume to enhance anatomical details.

Fetal cerebral midline structures from 300 consecutive normal low-risk pregnant women were studied prospectively by 2D and 3D ultrasound between 19-23 weeks of gestation. All the newborn infants underwent pediatric follow-up and were considered normal up to 2 years of life. In addition, five confirmed pathologic cases were evaluated and the abnormal features using this technique are described in this clinical series. Off-line volume data sets displaying the corpus callosum and the cerebellar vermis anatomy were accurately reconstructed in 98.5% and 96% of cases from sagittal and axial planes, respectively. For pathological cases, an agreement rate of 0.96 and 0.91 for midsagittal and axial planes, respectively, was observed. This study demonstrates the feasibility of including 3D ultrasound as an adjunct technique for the evaluation of cerebral midline structures in the second trimester fetus. Future prospective studies will be necessary to evaluate if the application of this novel 3D reconstructing technique as a step forward following 2D second trimester screening scan will improve the prenatal detection of cerebral midline anomalies in the low-risk pregnant population.

Research paper thumbnail of Early Prenatal Diagnosis of Orofacial Clefts: Evaluation of the Retronasal Triangle Using a New Three-Dimensional Reslicing Technique

Fetal Diagnosis and Therapy, 2013

The role of three-dimensional ultrasound in the prenatal diagnosis of orofacial clefts is increas... more The role of three-dimensional ultrasound in the prenatal diagnosis of orofacial clefts is increasing in recent time. We obtained three-dimensional data sets of the fetal face from 100 low-risk and 50 high-risk first-trimester fetuses to evaluate the offline reformatting accuracy of the retronasal triangle (RNT) using a novel reslicing technique. Adequate volume data sets for offline analysis were captured in 98% of cases. The RNT view was reformatted in 96% of cases by offline analysis. The secondary palate could be assessed in 93% by offline analysis and an abnormal RNT in the coronal plane was detected in 2 cases of lethal aneuploidy. The false-positive rate was 1.33% in the two studied groups. Abnormal RNT seems to be a valuable ultrasound marker for the early diagnosis of facial clefting. This novel reslicing technique has proven to be easy, fast and accurate, which suggest that this technology could be included in daily practice and integrated with other applications such as multiplanar mode and volume NT. Prospective studies are needed to confirm these promising results and to demonstrate if these diagnostic armamentarium tools will lead to a diagnostic enhancement of cleft lip and palate in early pregnancy.

Abnormal intracranial translucency (IT) (fourth ventricle) and a Blake&am... more Abnormal intracranial translucency (IT) (fourth ventricle) and a Blake's pouch cyst with normal brain stem cavity may be valuable first-trimester call signs of defects in the skull base. Here, we report a case of presumptive two-dimensional sonographic diagnosis of occipital cephalocele that was posed at the time of 11-13 weeks aneuploidy scan. The two-dimensional sonographic finding elicited a detailed fetal neuroscan that was performed using either multiplanar mode or a novel three-dimensional reslicing and lightening technique. The use of three-dimensional sonographic software and offline "navigation" within the volume of interest enabled operators to capture a diagnostic snapshot of the condition, enhancing quality imaging and early detection of the encephalic lesion.

In this series, there were no signs of polydramnious or fetal hydropsis. Associated malformations were found in 23% of the fetuses, being the cardiac malformation the more prevalent. Preterm delivery occurred in 23% of the pregnancies, as consequence of obstetrical complications as preeclampsia (7.6%), placenta previa (7.6%) and preterm rupture of membranes (7.6%). 15.4% of the fetuses had involution of the SRM during the pregnancy, one fetus had surgical treatment with a different diagnosis of the ultrasound and MRI. Of the 69.2% of the cases, the spontaneous resolution occurred in 30.7% during the first year of life and in 38.4% the masses presented without modifications or signs of regression since the neonatal period. Conclusions: Prenatal suprarenal masses are related with preterm labour and congenital heart defects. The spontaneous regression and the absence of modifications in the masses were a common long-term outcome of SRM. OP18. 02 Sonographic features of fetal hepatic hemangioma before and after birth
