Un dosage de tryptase s6rique post-mortem est r6alis6 dans 10 cas dont la mort est due h une caus... more Un dosage de tryptase s6rique post-mortem est r6alis6 dans 10 cas dont la mort est due h une cause cardiaque (6), li6e une ndoplasie (2), un choc septique (1) et une mort subite du nourrisson (1). Le sang est prdlev6 par ponction cardiaque (7), carotidienne (5) et veineuse p6riph6rique (1). Le dosage est effectu6 par test radio-immunologique en phase solide Pharmacia. Sur 13 dosages 3 sont supfrieurs ~ 4 ng/ml (N < 2 ng/ml). Les auteurs attirent l'attention sur la possibilit6 d'616vation de tryptase quand le sang est prdlev6 longtemps apr6s la mort, ou par ponction intracardiaque, ou apr6s choc 61ectrique ou massage cardiaque. La sp6cificit6 de ce dosage pour le diagnostic post-mortem de choc anaphylacto~de est discut6e.

Despite the occurrence of a severe allergic reaction including an anaphylactic shock, a drug may ... more Despite the occurrence of a severe allergic reaction including an anaphylactic shock, a drug may remain essential and impossible to replace. This may be the case of insulin in a diabetic patient. We describe the case of an anaphylactic shock to human insulin in whom a desensitization protocol was successfully achieved. A 50-year-old type 2 diabetic man presented one year after initiation of the insulin therapy an anaphylactic shock following the subcutaneous administration of a human insulin containing protamine (Insulatard®). A desensitization protocol to human insulin was performed and allowed to use two human insulin analogues containing no protamine (asparte and glargine), with a two-year event-free follow-up. Positive skin tests with insulin and protamine, and the presence of insulin specific IgE were evidenced of an IgE-mediated mechanism. The prevalence of food allergies increases, relating to diet modifications. The consumption of ne... more The prevalence of food allergies increases, relating to diet modifications. The consumption of new foods--exotic foods or foods originally used for animal feed, new proteins, neo allergens due to the use of new technologies and soon, Genetically Modified Foods--are in the spotlight. It is essential to develop a system of food allergy vigilance encompassing the full range of foods being consumed. Understanding this imperative leads logically to the suggestion of developing an allergy vigilance network taking advantage of the ongoing experience of allergists "on the ground". The French Allergy Vigilance Network is subscribed to by 302 allergologists (267 of whom are French). The aims of the Network are to record cases of severe anaphylaxis, to establish an epidemiological data bank from prospective multicenter studies, and to monitor the allergic risk from novel foods. Several food proteins are used as food additives or may be contained in foods as contaminants. Th... more Several food proteins are used as food additives or may be contained in foods as contaminants. These masked allergens are at high risk of inducing severe clinical reactions: acute asthma, laryngeal angioedema, anaphylactic shock or even death. The present regulation requires labelling only if the level of food ingredients is superior to 25% of the final product. Several immunologic methods can detect low levels of proteins, equal or inferior to 2%. However a level of 1% or less may be noxious, suggesting a need for a specific line of food products which could be guaranteed free of ... and which might be recommended to patients with food allergy.

Forty-nine sera from patients with food allergies confirmed by challenge and of 14 with suspected... more Forty-nine sera from patients with food allergies confirmed by challenge and of 14 with suspected allergy were assayed with the DPC AlaSTAT liquid phase EIA and the Phadebas RAST CAP radioimmunoassay (RIA). Twenty-two foods were tested (four animals, 18 vegetables). Controls were performed on nonatopic subjects (N = 8). Specificity was 100%. Concordance RAST CAP/AlaSTAT was 79.6%; prick tests/AlaSTAT: 76%; prick tests/RAST CAP: 73.9%. AlaSTAT detected specific IgE even in sera with low total IgE (< 60 KU/L), while RAST CAP was inconclusive. Dilution studies and reproducibility were excellent. AlaSTAT EIA compares favorably with the Phadebas RAST CAP RIA for measurement of food-specific IgE antibodies.

Les deux autres femmes normalisalent aussi leurs anomalies biologiques h 1' arrrt de la CO, mais elles continualent ~ prSsenter des crises d'OAN (~t une frSquence moindre cependant). Ces OAN r@ondaient favorablement 5 la prise d'acide tranexamique au long cours. La susceptibilitS de ClInh ~ la coupure chez ces femmes fait poser la question d'une anomalie stmcturale. I1 pourrait avoir des interactions anormales entre le ClInh et ses cibles de la phase-contact de la coagulation : des tests fonctionnels de ClInh vis-~t-vis de ces protrines sont en cours d'Svaluation.

Histamine in food may be responsible for some cases of food intolerance. We previously demonstrat... more Histamine in food may be responsible for some cases of food intolerance. We previously demonstrated disturbances in the metabolism of ingested histamine in patients with chronic urticaria (CU) and proposed that this could be related to increased intestinal permeability to histamine. The present study was undertaken to look for ultrastructural changes in the intestinal tract that might explain this abnormality. We examined duodenal biopsies from seven patients with CU before and after intraduodenal administration of histamine (120 mg). Five subjects had clinical symptoms (diarrhea, urticaria, headache, accelerated heart rate. and drop in blood pressure) within 1 h of duodenal histamine challenge (DHC). Ultrastructural changes. including edema of the interstitial tissue, enlargement of the basal intercellular spaces, slight congestion of the endothelial cells, and pericapillary edema, were observed in six subjects 45 min after DHC. In all the biopsies, the epithelium was normal, and the tight junctions were not modified by DHC. This morphologic study demonstrates that histamine can induce edema in the basal intercellular spaces of the duodenal mucosa and in the submucosa without evident change in the integrity of intercellular junctions. The most plausible route for histamine to have taken would appear to be an intracellular one.

Exercise-induced anaphylaxis (EIA) is defined as the onset of allergic symptoms during, or immedi... more Exercise-induced anaphylaxis (EIA) is defined as the onset of allergic symptoms during, or immediately after, exercise, the clinical signs being various degrees of urticaria, angioedema, respiratory and gastrointestinal signs and even anaphylactic shock. Food-dependent exercise-induced anaphylaxis (FDEIA) introduces food in the syndrome and is revealed by a chronological sequence in which food intake, followed by exercise, induces symptoms after a varying period. When the food intake and the exercise are independent of each other, there are no symptoms. FDEIA is not very frequent. Identifying the culprit food allergen depends on the patient's eating habits. Crustaceans and wheat flour are the two commonest but others foods can be implicated. The patho-physiology of FDEIA has not been clearly established but it appears to result from degranulation of mast cells.
