Pharmacologic treatment of acromegaly is currently based upon assay-error strategy, the first-gen... more Pharmacologic treatment of acromegaly is currently based upon assay-error strategy, the first-generation somatostatin receptor ligands (SRL) being the first-line treatment. However, about 50% of patients do not respond adequately to SRL. Our objective was to evaluate the potential usefulness of different molecular markers as predictors of response to SRL. We used somatotropinoma tissue obtained after surgery from a national cohort of 100 acromegalic patients. Seventy-one patients were treated with SRL during at least 6 months under maximal therapeutic doses according to IGF1 values. We analyzed the expression of SSTR2, SSTR5, AIP, CDH1 (E-cadherin), MKI67 (Ki-67), KLK10, DRD2, ARRB1, GHRL, In1-Ghrelin, PLAGL1 and PEBP1 (RKIP) by RT-qPCR and mutations in GNAS gene by Sanger sequencing. La calidad de vida y la morbimortalidad en la acromegalia están relacionadas con el control bioqu... more La calidad de vida y la morbimortalidad en la acromegalia están relacionadas con el control bioquímico (IGF-1 normal para edad y sexo)1. El tratamiento de primera línea incluye la cirugía transesfenoidal y/o análogos de somatostatina (ASS) de primera generación, que actúan principalmente a través de la isoforma 2 del receptor de somatostatina (RSS). Estudios epidemiológicos y amplios registros nacionales2--5 indican que esta estrategia no supera el 50% de remisiones. La segunda línea incluye pegvisomant en monoterapia o asociado a ASS, que mejora el porcentaje de remisiones, pero supone una mayor complejidad terapéutica. Los estudios pivotales de pasireótida6,7, un ASS con afinidad para varios RSS, sobre todo RSS5, evidencian superioridad frente a ASS de primera generación, lo cual nos permitiría controlar un mayor número de casos. En nuestra práctica clínica habitual, los pacientes con acromegalia mal controlados con ASS están en su gran mayoría en tratamiento con pegvisomant en monoterapia o combinado con ASS. Prácticamente no existen datos sobre la utilidad en la práctica clínica de pasireótida. Presentamos 4 pacientes acromegálicos con control deficiente tras cirugía y ASS y/o pegvisomant, en los que pasireótida ha permitido controlar las concentraciones de IGF-1 y el tamaño de los restos tumorales.

There is no consensus about the treatment and the follow-up of differentiated thyroid carcinoma (... more There is no consensus about the treatment and the follow-up of differentiated thyroid carcinoma (DTC), because in some patients, currently known prognostic factors cannot predict the course of the disease. The aim of this work has been to review the results of the treatment of our patients with DTC and to assess the prognostic factors. We include, retrospectively, 204 patients with DTC (54 males and 150 women) with a men (SD) age of 38 (23) years, attending to a terciary university hospital. The follow-up is 9.2 (6) years (range, 2-22 years). They all underwent near total thyroidectomy, ablative doses of radioactive iodine (131INa) and L-thyroxine to suppress thyroid stimulating hormone (TSH) levels. At least once a year, a clinical examination, a whole-body scan (WBS) with 131INa and serum thyroglobulin (Tg) measurements were performed in hypothyroid state. To evaluate our experience regarding the treatment of pituitary macroadenomas with cavernous sinu... more To evaluate our experience regarding the treatment of pituitary macroadenomas with cavernous sinus invasion in a series of 23 cases of transphenoidal resection. Twenty two patients, fifteen males and seven females, with ages ranging from 27 to 75 (mean of 48), were operated under protocol by a single surgeon between May of 2002 and December of 2004. Preoperatively all lesions were diagnosed by MRI and staged according to the Knosp classification. All tumors had extension to one or both cavernous sinuses. Four patients were considered to be grade 1, two grade 2, one grade 3 and sixteen grade 4. Twenty three operations were performed on twenty-two patients. Twenty cases were the standard transsphenoidal approach, and three were endoscopic. Postoperatively, the excision was classified as Complete or Total, Subtotal or Partial. The aim of this retrospective study is to compare recombinant human thyrotropin versus endogenous stimulation. Thirty-three patients with DTC with previous thyroidectomy and thyroid ablation were selected. All patients underwent whole-body radioiodine scanning and third day serum thyroglobulin (TG) measurement by two techniques, the first one after conventional thyroid hormone withdrawal (TSHe, TGe), and the second one after rhTSH stimulation (TSHr, TGr). Measurement of TG was performed on the third day due to the infrastructure. We only included patients with stable disease, without therapeutic interventions between two consecutive controls in an interval inferior to one year.
