The present study, involving investigations of recent foraminifera from the sediments of the inne... more The present study, involving investigations of recent foraminifera from the sediments of the inner shelf off Nagapattinam on the southeast coast of India, was undertaken to document the composition, distribution, and ecology of the foraminifera and their relationship to the climatic conditions of the study area. Thirty sediment samples were collected from depths ranging from 3.4 to 30.8 m during the fieldwork conducted in July 2001 and various ecological parameters were determined. Sediment samples from the sediment-water interface yielded 151 benthic foraminifera species and varieties belonging to 82 genera, 42 families, 21 superfamilies, and 5 suborders, which have been described and illustrated in this study. The spatial and temporal variations of total (“live” + dead) foraminifera populations were analysed. The study shows significant total populations in the collected samples. Enhanced recovery after surgery (ERAS ®) is a paradigm shift based on evidence showing reduced re... more Enhanced recovery after surgery (ERAS ®) is a paradigm shift based on evidence showing reduced regeneration times and complications after the operation. In a range of surgical disciplines, ERAS' functions have been studied. In the field of gastric surgery, the ERAS protocol for gastrectomies was developed in 2014, with a subsequent meta-analysis demonstrating its efficacy. The use of ERAS in an emergency situation is feasible and effective; however, some improvements to the protocol may be needed. As a result, further research is necessary. There is also insufficient data to say whether ERAS improves patients' long-term results. As most of the centers have been using the ERAS protocol for less than 5 years, more data are required.

Abstract Polymer matrix composites with different fibre glass reinforcements like wool, woven rin... more Abstract Polymer matrix composites with different fibre glass reinforcements like wool, woven ring and mesh has been prepared through hand layup method for the experimental investigation on compression strength and weight evaluation. EPOXY/PVC blend has been used as matrix material with different weight percentages of PVC such as 5%, 15% and 25%. The prepared specimen has been subjected to heat treatment in a oven for different timing such as 15, 30 and 45 min. The present study considers the percentage of PVC, fibre glass type and heating time as input factors to observe their effect over weight and compression strength of the specimen through Taguchi’s L27 Orthogonal array. The statistical analysis of experimental results through signal to noise ratio method and ANOVA reveals that the type of fibre used for making the specimen has more influence over both the output parameters. The heat treatment provided to the specimen and the percentage of PVC has no significant effect and the residual error in analysis is around 10%.The investigation carried out reveals that 5% PVC composition with woven rings reinforcement material has shown the maximum compression strength and highest specimen weight and the changes in matrix material combinations with different PVC compositions, fiber glass with increased or decreased thickness may be considered as the future directions of the current study.

Diabetic Retinopathy is a common complication of diabetes and the primary cause for visual impair... more Diabetic Retinopathy is a common complication of diabetes and the primary cause for visual impairment and blindness in adults that is caused by changes in the blood vessels of the retina. Regular screening is essential in order to detect the early stages of diabetic retinopathy for timely treatment to prevent further damage of vision. An important aspect of DR is the micro-vascular changes that cause detectable changes in the appearance of retina blood vessels. Prolongation of Non proliferative Diabetic Retinopathy may result in permanent blindness. A group of eye conditions that affect people with diabetes which causes progressive damage to the tiny blood vessels in the retina. The placid and brutal juncture are identified using Pixel Count Method. This Method scrutinizes the retina to know the number of red pixels in placid and brutal juncture of non proliferative Diabetic Retinopathy. The Microaneurysms appear as small blood clots on the surface of the retina layers. The Hemorrhages appear as large round red dots. The placid stage is indicated by the presence of at least 1 Microaneurysms on the surface of the retina layer and the brutal stage is indicated by the presence of Hemorrhages on the surface of the retina layer. The level of Microaneurysms and Hemorrhages are identified through the count of red pixels. The number of red pixels in brutal juncture is higher than placid juncture. This method pigeonholes the placid and brutal juncture of non proliferative diabetic retinopathy through the count of red pixels.

Aim: To analyse the post operative outcome in patients with CSOM-Atticoantral type and compare th... more Aim: To analyse the post operative outcome in patients with CSOM-Atticoantral type and compare the outcomes in terms of disease clearance and improvement in hearing. Material and Methods: 50 Patients who were diagnosed as Chronic Suppurative otitis Media of Attico-antral type, detailed history & clinical examination and investigations were performed. Per-operative findings (status of ossicles, middle ear, antrum, mastoid), Post operative follow up (1, 3 and 6 months)-disease clearance and hearing status was done. Results: Out of total 50 patients, Canal Wall Down Procedures (Modified radical mastoidectomy with Tympanoplasty) had a good success rate in disease clearance when compared with Canal Wall Up Procedures (Cortical Mastoidectomy/ Atticotomy With Tympanoplasty). Irrespective of canal wall up (or) canal wall down procedures, Type II Tympanoplasty show better results of hearing improvement when compared to other surgical procedures. Conclusion: In our study, postoperative surgical outcome were statistically analyzed using Chi square test, in which Canal wall down procedures showed good result in terms of disease clearance and Type II tympanoplasty showed good result in terms of hearing improvement.

Duodenal injury with leakage of bowel content along with bacterial contamination in retroperitoneum makes repair difficult. Mortality is usually due to the associated organ injuries .Successful surgical repair of a duodenal rupture was first reported in 1896, but it was not until 1905 that the first detailed case report of a survival appeared. Since this time, the number of patients with duodenal injuries increased markedly largely because of increased numbers of automobile accidents and the increased violence on our streets. Thus, physicians treating civilian trauma are being confronted with increasing numbers of duodenal injuries. Being best known for its neurological consequence, hypoglycemia is an important and not uncommon ... more Being best known for its neurological consequence, hypoglycemia is an important and not uncommon side effect with insulin or oral anti-diabetic therapy in patients with diabetes mellitus. However, hypoglycemia-induced myocardial infarction should be also taken into clinical consideration despite being rarely reported, especially non-ST segment-elevation myocardial infarction. Herein, we report a 68-year-old lady who underwent an episode of hypoglycemia manifested as reversible conscious disturbance and acute non-ST segment-elevation myocardial infarction, which was documented by typical ECG changes and dynamic cardiac-specific enzyme elevation, during long-term insulin therapy. The ST segment depression completely resolved as soon as the hypoglycemia was corrected. Subsequent coronary angiography showed no significant coronary stenosis responsible for the patient's ECG changes, suggestive of the strong relationship between hypoglycemia and myocardial infarction. Telemedicine is increasingly used across the developing world to expand access to healthcare, to ... more Telemedicine is increasingly used across the developing world to expand access to healthcare, to improve outcomes and to reduce costs. However, the impact of telemedicine in these settings – particularly on existing physical healthcare delivery channels – has not been thoroughly examined. We use data from one of the largest tele-ophthalmology implementations in the world to examine this issue. Using a quasi-experimental difference-in-differences approach on a dataset of more than 4.8 million visits over almost a decade (Jan 2006–May 2015), we find that telemedicine centers generate a 31% increase in the overall network visit rate, of which 62% comprise new patients, suggesting a substantial increase in access. We also find a 5.1% reduction in hospital visit rates suggesting that some patients substitute hospital visits with telemedicine center visits. We find substantial heterogeneity in the impact of treatment depending on the clinical complexity of patient needs. The rate of eye glasses prescriptions to correct for simple refractive errors increases by 18.5% but the rate of cataract surgery to replace the natural lens in a patient's eye with an artificial lens remains unchanged. The increase in access and treatment rates does not significantly impact the direct costs incurred by patients but reduces their indirect costs (measured as the average distance travelled to care) by 30% (12km). Finally, we find significant spatial heterogeneity in these effects depending on the relative distance of a patient's location from the telemedicine center as well as from the hospital. These results have important implications for the design of telemedicine networks and the portfolio of healthcare services provided through them.

Muen is representative of the class of modern separation kernels that leverage hardware virtualization support, and are generative in nature in that they generate a specialized kernel for each system configuration. These features pose substantial challenges to existing verification techniques. We propose a verification framework called conditional parametric refinement which allows us to formally reason about generative systems. We use this framework to carry out a conditional refinement-based proof of correctness of the Muen kernel generator. Our analysis of several system configurations shows that our technique is effective in producing mechanized proofs of correctness, and also in identifying issues that may compromise the separation property.

The variations in anatomy due to extensive fibrous tissue growth, soft tissue contractures around the fracture site and bony alterations like smoothening and sclerosis of the fracture ends must each be addressed as a whole if good outcome is to be expected. Here we present a series of 11 patients who had bimalleolar fracture of the ankle following which they had native splinting. These patients presented to us with established non-union. There were 7 males and 4 females in the study. The average age was 44.63 years. Ten out of the 11 patients went on to union (90.1%) following internal fixation with or without immobilization in a plaster cast at an average of 13.8 weeks (range 12-17 weeks). They are mostly due to high violence trauma in the younger age group. They almost always require open reduction and internal fixation. Due to the increase in the emergence of native bone setters, these fractures are increasingly been managed by these spurious bone setters using native splints. As a result, non-union rate is high among such patients. These patients definitely need open reduction with internal fixation +/- bone grafting. The choice of implants used can be either a dynamic condylar screw plate (DCS) orproximal femoral nail (PFN). Here we have used a surgical grade 316 L stainless steel proximal femoral anatomical locked compression plate (PF-LCP). We analyzed 13 patients with established non unions of subtrochanteric fractures treated in our centre by the use of the PF-LCP. There were 10 males and 3 females. The average age was 48.23 years. Telemedicine is increasingly used across the developing world to expand access to healthcare, to ... more Telemedicine is increasingly used across the developing world to expand access to healthcare, to improve outcomes and to reduce costs. However, the impact of telemedicine in these settings – particularly on existing physical healthcare delivery channels – has not been thoroughly examined. We use data from one of the largest tele-ophthalmology implementations in the world to examine this issue. Using a quasi-experimental difference-in-differences approach on a dataset of more than 4.8 million visits over almost a decade (Jan 2006–May 2015), we find that telemedicine centers generate a 31% increase in the overall network visit rate, of which 62% comprise new patients, suggesting a substantial increase in access. We also find a 5.1% reduction in hospital visit rates suggesting that some patients substitute hospital visits with telemedicine center visits. We find substantial heterogeneity in the impact of treatment depending on the clinical complexity of patient needs. The rate of eye glasses prescriptions to correct for simple refractive errors increases by 18.5% but the rate of cataract surgery to replace the natural lens in a patient's eye with an artificial lens remains unchanged. The increase in access and treatment rates does not significantly impact the direct costs incurred by patients but reduces their indirect costs (measured as the average distance travelled to care) by 30% (12km). Finally, we find significant spatial heterogeneity in these effects depending on the relative distance of a patient's location from the telemedicine center as well as from the hospital. These results have important implications for the design of telemedicine networks and the portfolio of healthcare services provided through them.

Muen is representative of the class of modern separation kernels that leverage hardware virtualization support, and are generative in nature in that they generate a specialized kernel for each system configuration. These features pose substantial challenges to existing verification techniques. We propose a verification framework called conditional parametric refinement which allows us to formally reason about generative systems. We use this framework to carry out a conditional refinement-based proof of correctness of the Muen kernel generator. Our analysis of several system configurations shows that our technique is effective in producing mechanized proofs of correctness, and also in identifying issues that may compromise the separation property.

The variations in anatomy due to extensive fibrous tissue growth, soft tissue contractures around the fracture site and bony alterations like smoothening and sclerosis of the fracture ends must each be addressed as a whole if good outcome is to be expected. Here we present a series of 11 patients who had bimalleolar fracture of the ankle following which they had native splinting. These patients presented to us with established non-union. There were 7 males and 4 females in the study. The average age was 44.63 years. Ten out of the 11 patients went on to union (90.1%) following internal fixation with or without immobilization in a plaster cast at an average of 13.8 weeks (range 12-17 weeks). They are mostly due to high violence trauma in the younger age group. They almost always require open reduction and internal fixation. Due to the increase in the emergence of native bone setters, these fractures are increasingly been managed by these spurious bone setters using native splints. As a result, non-union rate is high among such patients. These patients definitely need open reduction with internal fixation +/- bone grafting. The choice of implants used can be either a dynamic condylar screw plate (DCS) orproximal femoral nail (PFN). Here we have used a surgical grade 316 L stainless steel proximal femoral anatomical locked compression plate (PF-LCP). We analyzed 13 patients with established non unions of subtrochanteric fractures treated in our centre by the use of the PF-LCP. There were 10 males and 3 females. The average age was 48.23 years.
