Background: Brain tumors in general are rare tumors and their epidemiologic features in the Middl... more Background: Brain tumors in general are rare tumors and their epidemiologic features in the Middle East including the state of Qatar is rarely tackled. The objective of this study is to delineate the epidemiologic characteristic of brain tumors in the gulf state of Qatar. Methods: We run a retrospective analysis of brain tumors diagnosed histologically in the main Hospital in Qatar between January 1 st , 2006-December 31 st 2015. We studied the following indices: Histologic type, gender, age, and location. We obtained the demographic data from Qatar Planning and Statistic Authority (www.psa.gov.qa). Results: A total of 383 cases of primary brain tumors were identified and analyzed during the 10 years period studied. The incidence rate of primary brain tumors is 2.2/100,000 population. Normalized Male/Female ratio is 0.93:1. The commonest histological subtypes were Meningioma (21.9%) and Glioblastoma (18.3 %) followed by Astrocytoma (G2-G3) (18%) and Oligodendroglioma (15.1%). Conclusion: Brain tumors incidence in the state of Qatar is low (2.2/100,000) when compared to developed countries which may reflects the young age population.

A 66 year old Indian gentleman presented with a 3 days history of headache and gradual progressiv... more A 66 year old Indian gentleman presented with a 3 days history of headache and gradual progressive loss of vision in his eft eye, ophthalmological assessment showed no light perception in his left eye with papilledema and afferent papillary defect. Computed tomography (CT) and Magnetic Resonance Imaging (MRI) were done and showed an expanding lesion in the left anterior clinoid process encroaching upon the left orbital apex and optic nerve with features suggestive of a mucocele. Patient was started on dexamethasone, and urgent craniotomy was undertaken, where marsupialization and resection of left anterior clinoid mucocele was done, and histopathologic examination of the operative specimen was consistent with a mucocele. Post-operatively, patient was kept on dexamethasone for few days, with uneventful outcome, and his follow up at 6 months showed complete recovery of his vision from no light perception to 6/12 in the affected eye.

A 63-year old Yemeni male, known case of endstage renal disease (ESRD), hypertension and diabetes... more A 63-year old Yemeni male, known case of endstage renal disease (ESRD), hypertension and diabetes mellitus, underwent craniotomy and endoscopic removal of an intraventricular mass. Peri-operatively he was started on cefazolin, a normal diet and regular hemodialysis without heparin. The international normalising ratio (INR) increased rapidly, reaching 4.2 on day five but responded equally rapidly to parenteral Vitamin K and withdrawal of cefazolin. Cefazolin is an antibiotic commonly used in clinical practice because of its long half-life, narrow spectrum and its ability for dose adjustment in renal failure patients. We review the literature, discuss the risk factors of its use and suggest possible mechanisms causing the prolonged coagulation.

The purpose of this retrospective multicenter, pooled-data analysis was to determine the factors ... more The purpose of this retrospective multicenter, pooled-data analysis was to determine the factors associated with in-hospital mortality in decompressive hemicraniectomy (DHC) for malignant middle cerebral artery (MMCA) stroke. The authors reviewed pooled DHC database from 3 countries for patients with MMCA with hospital mortality in spite of DHC to identify factors that predicted in-hospital mortality after DHC. The identified factors were applied to the group of patients who were selected for DHC but either refused surgery and died or stabilized and did not undergo DHC. There were 137 patients who underwent DHC. Multiple logistic regression analysis showed middle cerebral artery (MCA) with additional infarcts (odds ratio [OR], 7.9: 95% confidence interval [CI], 2.4-26; P = .001), preoperative midline shift of septum pellucidum of 1 cm or more (OR, 3.83: 95% CI, 1.13-12.96; P = .031), and patients who remained unconscious on day 7 postoperatively (8.82: 95% CI; OR, 1.08-71.9; P = .04...

Spinal teratomas are more common in children as compared to adults and are usually associated with spinal dysraphism. Dermal Sinus is a closed type of Spinal dysraphsim, lined with epithelium and are mostly associated with Dermoid, Epidermoid and Posterior arch defect [1]. We present a case of an infant with recurrent meningitis. His examination was remarkable for a lumbar dimple. Imaging revealed a dermal sinus track continuous with a dumbbell appearance of extradural and intradural nodules, while the histopathology confirmed mature cystic Teratoma. In literature, only few cases been reported, most of them being adenocarcinoma from breast, lung and gastric cancer [1-3]. We present an uncommon case of Dural based mammary carcinoma with lymphatic involvement presenting as incidental subacute subdural hematoma in asymptomatic patient who was diagnosed on her work up for breast carcinoma. We would like to mention the importance of MRI and Dural biopsy in such cases as the treatment and management of these cases would change based on the result of Dural biopsy.

Symptomatic hydrocephalus due to vertebrobasilar dolichoectasia is a rare occurrence. We report a... more Symptomatic hydrocephalus due to vertebrobasilar dolichoectasia is a rare occurrence. We report a patient who presented with acute confusion and vomiting. Neuroimaging revealed elongated and tortuous basilar artery indenting and elevating the floor of third ventricle causing obstructive hydrocephalus. Initially, the patient was treated with external ventricular drain and then with ventriculo-peritoneal shunt. We suggest prompt surgical intervention upon diagnosis as a first choice of treatment in order to avoid further complications.

They are classified as a variant of PA. They have only been recently recognized, and they behave more aggressively than PAs. Most cases occur in the hypothalamic-chiasmatic area, although they may be located in any part of the neuraxis. Posterior fossa PMAs are very rare. The authors report a case of a 2-year-old girl with cerebellar PMA.

Fifty percent of traumatic brain injury (TBI) patients will have emotional disorders and post-traumatic agitations. Haloperidol is a neuroleptic antipsychotic medication commonly used in the traumatic brain injury patients due to its advantage of no effect on respiration and conscious level. But it is one of the common medications causing NMS. A 19-year-old male driver involved in the road traffic accident had an acute subdural hematoma, which was immediately evacuated. Postoperatively, he was awake. He was weaned from ventilator and extubated. He received 20 mg of intravenous haloperidol in divided doses with in 24 hours to control his agitation. Cystic mature teratomas presenting in the spine are rare, comprising less than 0.1% of all spinal... more Cystic mature teratomas presenting in the spine are rare, comprising less than 0.1% of all spinal tumors. They arise from all three germ cell layers and usually accompany other congenital abnormalities. Spinal teratomas are more common in children as compared to adults and are usually associated with spinal dysraphism. Dermal Sinus is a closed type of Spinal dysraphsim, lined with epithelium and are mostly associated with Dermoid, Epidermoid and Posterior arch defect [1]. We present a case of an infant with recurrent meningitis. His examination was remarkable for a lumbar dimple. Imaging revealed a dermal sinus track continuous with a dumbbell appearance of extradural and intradural nodules, while the histopathology confirmed mature cystic Teratoma.

We report a patient who presented with acute confusion and vomiting. Neuroimaging revealed elongated and tortuous basilar artery indenting and elevating the floor of third ventricle causing obstructive hydrocephalus. Initially, the patient was treated with external ventricular drain and then with ventriculo-peritoneal shunt. We suggest prompt surgical intervention upon diagnosis as a first choice of treatment in order to avoid further complications.

They are classified as a variant of PA. They have only been recently recognized, and they behave more aggressively than PAs. Most cases occur in the hypothalamic-chiasmatic area, although they may be located in any part of the neuraxis. Posterior fossa PMAs are very rare. The authors report a case of a 2-year-old girl with cerebellar PMA.

Although there has been a recent increase in the number of reported cases of vertebral artery (VA) dissection, the clinical variety of presentation and the early warning symptoms have not been well-described before. Our objectives in this study include: (1) To determine the early symptoms and warning signs which may help the clinician in the early identification and treatment of patients with VA dissection. (2) To explore the variety of clinical presentation of VA dissection and its relation to prognosis. Retrospective analysis of hospital records in a tertiary academic centre for the period 1989-1999. Twenty-six patients were identified (13 men and 13 women). The mean age was 48. Possible precipitating factors were identified in 14 patients (53%). Fifty percent of traumatic brain injury (TBI) patients will have emotional disorders and post-traumatic agitations. Haloperidol is a neuroleptic antipsychotic medication commonly used in the traumatic brain injury patients due to its advantage of no effect on respiration and conscious level. But it is one of the common medications causing NMS. A 19-year-old male driver involved in the road traffic accident had an acute subdural hematoma, which was immediately evacuated. Postoperatively, he was awake. He was weaned from ventilator and extubated. He received 20 mg of intravenous haloperidol in divided doses with in 24 hours to control his agitation. Next day, he became drowsy, spastic, febrile, and tachycardic with labile blood pressure.
