Uno degli Autori (G.A.) fu consulente come epidemiologo occupazionale per l’Ufficio Speciale di S... more Uno degli Autori (G.A.) fu consulente come epidemiologo occupazionale per l’Ufficio Speciale di Seveso della regione Lombardia dal 1979 al 1983. Uno dei primi compiti fu quello di disegnare, realizzare e valutare uno studio sulla salute dei lavoratori addetti alla bonifica nella zona A fortemente contaminata da 2,3,7,8-TCDD. Il razionale dello studio era legato al fatto che le routinarie attivita di sorveglianza sanitaria nei confronti dei lavoratori potenzialmente esposti a TCDD non erano sufficienti a tutelare la salute degli addetti, dato che all’epoca non era possibile effettuare un adeguato monitoraggio biologico attraverso misure ematiche di TCDD. All’epoca l’unica possibilita di effettuare misure biologiche di TCDD consisteva nel sottoporre i soggetti ad una biopsia adiposa con almeno 5 grammi di tessuto adiposo, cosa evidentemente eticamente inaccettabile. Inorganic arsenic and its methylated metabolities were measured in 108 spot urine samples obtaine... more Inorganic arsenic and its methylated metabolities were measured in 108 spot urine samples obtained from the medical surveillance programme of workers exposed to inorganic Arsenic in July 2006. 15% of the samples showed levels higher than limit value of 35 microg/L (mean value 23,9 microg/L). After the improvement of the working conditions, in August-October 2006, we collected a urinary sample from each of the 108 workers enrolled. A questionnaire was also administrated, in order to investigate the influence of occupational and non occupational factors on the urinary arsenic excretion. The median value of urinary arsenic was 15,12 microg/L; among the 108 samples, 5% showed levels higher than limit value. A significant difference was observed in relation with sea-food consumption and aging stratification. In Italy there are eight tumor Population-Based Registries (PBRs) that publish incidence data, an... more In Italy there are eight tumor Population-Based Registries (PBRs) that publish incidence data, and only one of them (Ragusa) provides data for Southern Italy. Usually, PBRs are based on data collection from Pathologists and medical records. Our integrated system differentiates from traditional PBRs because the information comes from the General Practitioners (GPs) and is completed with the diagnosis provided by the Pathologists (Ps). During two years we have registered 1,057 new cancers on a middle period population of 212,644. GPs and Ps signed 395 and 879 incident cases, respectively. GPs alone provided 16.8%, Ps alone 62.6%, and either source 20.6% of total cases. After excluding non melanotic skin cancers and bladder carcinoma, the GPs-Ps integrated system counted 828 new cases in two years. These incidence data are the first in our region (Puglia). The 178 cases signed by GPs alone should have been lost if the informations of our PBR had been based only on local Ps' records...

Polycyclic aromatic hydrocarbons (PAHs) exposure in the coke industry poses a risk for workers... more Polycyclic aromatic hydrocarbons (PAHs) exposure in the coke industry poses a risk for workers' health as well as for subjects living in the plant vicinity. To assess PAHs exposure in coke-oven workers (CW) at the Taranto plant, Apulia, and in subjects from the general population living near (NC) and far away (FC) from the plant. Exposure was assessed by personal air sampling and urinary 1-hydroxypyrene (1-OHP) measured in 100 CW 18 NC and 15 FC. Median airborne benzo[a]pyrene (BaP) levels were 152, 1.5, and 3.6 ng/m3 in CW NC, and FC, respectively. In CW, median 1-OHP increased from 1.45 to 1.96 microg/g creatinine (crt) during the work shift (p > 0.05); in NC and FC, 1-OHP levels were 0.56 and 0.53 microg/g crt. No significant differences between NC and FC for both air and urinary indices were found. BaP exposure in CW exceeded the recently proposed German acceptable (70 ng/m3) and tolerable (700 ng/m3) risk-based limit values in 82 and 11% of subjects, respectively. In NC ...

This study aims at evaluating the occupational exposure to polycyclic aromatic hydrocarbons (PAH)... more This study aims at evaluating the occupational exposure to polycyclic aromatic hydrocarbons (PAH) through biological monitoring and at investigating the influence of selected genetic polymorphisms on the levels of internal dose biomarker. A cross-sectional study has been conducted on the basis of biological measurements in order to evaluate the levels of PAH exposure. PAH exposure assessment has been carried out in a coke-oven of one of the largest European steel plants (ILVA, Taranto), in the context of the program of medical surveillance carried in collaboration with the ILVA Occupational Health Service. 355 coke-oven workers exposed to PAH were enrolled from the 3 batteries of the coke oven plant. Maintenance workers and industrial cleaners were included. What is the prognostic significant of the histomorphology in the small cell carcinomas of the lun... more What is the prognostic significant of the histomorphology in the small cell carcinomas of the lung? After the WHO classification of the lung cancer (1981), several studies criticized the subdivision of the small cell carcinoma in three sub-types (oat-cell, intermediate cell and combined types). The role of histology in the prognostic predition has been devaluated. In order to verify the prognostic value of the morphology of the small cell types of lung cancer, we performed a multivariate analysis in 62 patients. The survival rate was analytically compared with the following parameters: nuclear maximum diameter, nuclear form, nuclear chromatism, chromatine distribution, presence of nucleolus, evidence of cytoplasm. The results showed that none of these parameters are able to express a prognostic value. According to the recent studies, we think that the small cell carcinoma of the lung is a neoplasia with a multiform histologic pattern. Differences observed in clinical management are ...

Eleven male workers in a battery storage plant with lead and erythrocyte protoporphyrin blood act... more Eleven male workers in a battery storage plant with lead and erythrocyte protoporphyrin blood actual levels greater than 50 and 100 micrograms %, respectively, and 18 male controls without lead exposure were tested by a clinical pendular eye tracking test (PETT). Each worker underwent a series of lead absorption measurements including blood lead, urinary lead, erythrocyte protoporphyrin, delta-aminolevulinic acid dehydratase activity, and urinary delta-aminolevulinic acid. The SPEMs were evaluated by an eye tracking technique. The subjects followed a horizontally moving target which, in the form of a luminous spot on a dark background, was projected onto a screen placed 1 m from the subject. The maximum predicted eye movement velocity during tracking was about 30 degrees/s. Skin electrodes were applied on the outer canthi of both eyes and SPEM were plotted on a polygraph, recording both the actual eye movements and the corresponding first derivative. Our findings suggest that lead workers display a disorder of motor coordination of SPEMs system, and the PETT is useful, when associated with biochemical data, to evaluate the degree of subclinical damage of nervous system during lead poisoning.

Long lived lymphocytes tend to have higher 32P postlabelling measured levels of adducts than shor... more Long lived lymphocytes tend to have higher 32P postlabelling measured levels of adducts than short lived granulocytes in environmental and life style associated i.e. smoking exposures. With the aim of investigating this issue for occupational exposure to PAH and contributing to further validation of some technical aspects of the 32P postlabelling assay, two Italian laboratories analysed PAH-DNA adducts from lymphocytes and total white blood cells WBC. Seventy seven blood samples from coke oven workers employed at a steel plant located in Taranto, Southern Italy, and 14 samples from control subjects were collected. At the University of Padua, DNA was purified from peripheral blood lymphocytes PBL. Two years later, at the University of Bari, white blood cells WBC were isolated from replicate blood samples stored at- 80 C and DNA purified by the same method. In both cases, the nuclease P1 modified postlabelling assay was used to determine aromatic DNA adduct levels. The mean adduct levels were 5.13 3.37 Padua and 2.48 1.27 Bari per 108 nucleotides. Both laboratories observed large inter individual variations of adduct levels ranging from 0.09 to 18.93 per 108 nucleotides. Both the correlation and the agreement of the two sets of data were assessed. Slight correlation r = 0.39; p 0.01 and a poor level of agreement were found, the intra class correlation coefficient being equal to 0.05. Better correlation coefficient r = 0.54, p 0.01 and intra class correlation coefficient r = 0.50 were observed comparing only the adduct levels determined on the diagonal zone DRZ. Our findings seem to confirm the same divergence reported in the literature on DNA adduct levels between lymphocytes and granulocytes.
