Elective termination of pregnancy is prohibited in many countries, even after prenatal diagnosis ... more Elective termination of pregnancy is prohibited in many countries, even after prenatal diagnosis of oral clefts. Though some studies address registries on termination of pregnancy, many investigations include only livebirths. This may lead to underestimation of the overall occurrence of oral clefts, influencing their reported prevalence. This paper does not intend to discuss if termination of pregnancy because of the presence of an oral cleft is justifiable from ethical, moral, or religious standpoints. Rather, its main goal is to promote a reflection on how the prevalence of oral clefts has been addressed and to rethink the reported differences in prevalence. Authors publishing on the prevalence of oral clefts might indicate the regulations and practices on pregnancy termination in their countries, so that readers may have an idea of what is beyond those findings.

Este estudo investigou a prevalência de disgenesias dentárias, alterações de tecido mole e caract... more Este estudo investigou a prevalência de disgenesias dentárias, alterações de tecido mole e características cefalométricas em pacientes com síndrome velocardiofacial e síndrome G/BBB. A presença de disgenesias dentárias foi avaliada em pacientes acima de seis anos de idade; para a análise cefalométrica, foram avaliadas telerradiografias laterais obtidas antes de qualquer intervenção ortodôntica. A amostra de estudo para a síndrome velocardiofacial incluiu 26 pacientes para análise de disgenesias dentárias e alterações de tecido mole e 18 pacientes para análise cefalométrica. Para a síndrome G/BBB, foram analisados 21 pacientes quanto à presença de disgenesias dentárias e alterações de tecido mole, e 23 pacientes para a análise cefalométrica. Foram incluídos apenas pacientes de etnia branca. Pain is an unpleasant, sensitive and emotional experience associated with or described in terms o... more Pain is an unpleasant, sensitive and emotional experience associated with or described in terms of tissue lesion, and may be acute or chronic. It may also be classified as nociceptive, neuropathic or psychogenic. Nociceptive pain involves the transformation of environmental stimuli into action potentials carried to the central nervous system, where they are modulated and integrated up to final interpretation in the cerebral cortex. Neuropathic pain may arise as a consequence of the direct lesion of axons, or of an increase in the production of neurotrophic factors. Chronic pain is always associated with anxiety and some degree of depression. Drug therapy should be selected according to its efficacy; nonetheless, the professional should also consider the tolerability and adverse effects that may occur, for example, in elderly individuals. It is necessary to emphasize the safety-considering the possibility of drug interactions-and define the posology to promote better adherence. However, the treatment of neuropathic pain should not be limited to the use of analgesic drugs, which are just one among several options enabling patients to participate in bio-psycho-social rehabilitation programs.

To investigate the prevalence of the presence of a soft tissue bridge (Simonart's band) in patien... more To investigate the prevalence of the presence of a soft tissue bridge (Simonart's band) in patients with complete cleft lip and alveolus and complete cleft lip and palate. Design: Cross-sectional. Sample: We assessed 407 consecutive unoperated patients first attending the Hospital for Rehabilitation of Craniofacial Anomalies of University of São Paulo, in Bauru, Sã o Paulo, Brazil, in the year 2000. The patients were classified as presenting complete cleft of the primary palate or of the primary and secondary palate, unilateral or bilateral, as follows: unilateral cleft lip, bilateral cleft lip, unilateral cleft lip and palate, and bilateral cleft lip and palate. Method: Clinical examination. The band was considered as present whenever there was a soft tissue bridge between the separated alveolar ridges, regardless of volume and position. Results were analyzed by descriptive statistics and were expressed as percentages, according to the type of cleft. Conclusion: 31.2% of patients presented with Simonart's band. The band was observed more frequently in patients with unilateral clefts than in patients with bilateral clefts, and in patients with complete cleft lip and alveolus than in patients with complete cleft lip and palate.

Introdução: A simulação vem sendo implementada como uma ferramenta de ensino, inclusive na área o... more Introdução: A simulação vem sendo implementada como uma ferramenta de ensino, inclusive na área odontológica. As práticas para aquisição de habilidades ocorrem, normalmente, em laboratórios de ensino antes que os estudantes iniciem as práticas clínicas. A prática de laboratório estimula o raciocínio clínico e prático. Porém, a ausência da simulação de condições peculiares e a falta de realismo podem tornar a prática clínica mais difícil, como no atendimento de indivíduos com fissuras labiopalatinas. Objetivo: Assim, foi proposto a elaboração de um manequim simulador de um indivíduo com fissura labiopalatina completa unilateral, que possa contribuir para o ensino e aprendizagem da prática reabilitadora odontológica de forma mais realista. Relato de experiência: Desta forma, foi realizada a fissura de lábio, sobre uma estrutura de silicone já curada. Para a caracterização do manequim intrabucal foi utilizado como referência um manequim articulado da empresa Pronew desenvolvido para especialidade de Endodontia. Os dentes foram montados com o cuidado de manter as seguintes características oclusais: Classe III de Angle com mordida cruzada posterior bilateral. Na região da fissura, o incisivo central foi montado com giroversão e o incisivo lateral mesializado com severa inclinação para palatina, que este simulador permite a troca da caracterização facial e o manequim odontológico. Considerações finais: A criação dos manequins com caracterizações intra e extrabucais é um grande aliado para o treinamento profissional para tratamento e reabilitação de paciente com fissuras labiopalatinas.

This study investigated the prevalence of oral habits in children with clefts aged three to six y... more This study investigated the prevalence of oral habits in children with clefts aged three to six years, compared to a control group of children without clefts in the same age range, and compared the oral habits between children with clefts with and without palatal fistulae. The sample was composed of 110 children aged 3 to 6 years with complete unilateral cleft lip and palate and 110 children without alterations. The prevalence of oral habits and the correlation between habits and presence of fistulae (for children with clefts) were analyzed by questionnaires applied to the children caretakers. The cleft influenced the prevalence of oral habits, with lower prevalence of pacifier sucking for children with cleft lip and palate and higher prevalence for all other habits, with significant association (< 0.05). There was no significant association between oral habits and presence of fistulae (> 0.05). The lower prevalence of pacifier sucking and higher prevalence of other oral habits agreed with the postoperative counseling to remove the pacifier sucking habit when the child is submitted to palatoplasty, possibly representing a substitution of habits. There was no causal relationship between habits and presence of palatal fistulae.

The purpose of this paper was to report the case of a 6-year-old male patient who was clinically ... more The purpose of this paper was to report the case of a 6-year-old male patient who was clinically diagnosed as having Rapp-Hodgkin syndrome-a variant of ectodermal dysplasia characterized by dysplastic disorders of the skin, hair, and nails associated with cleft lip and/or palate. The patient exhibited only the primary maxillary and mandibular canines bilaterally. Restorative treatment was performed on all primary canines, followed by aquisition of maxillary and mandibular impressions for fabrication of acrylic removable partial dentures with circumferential clasps on the primary canines. The dentures allowed recovery of the vertical dimension and excellent esthetics. Hypodontia of several teeth associated with ectodermal dysplasia, as in this case involving Rapp-Hodgkin syndrome, causes several functional and esthetic alterations. These disorders should be diagnosed and treated as early as possible to restore the normal function, allow adequate mastication and speech, enhance esthetics, and, consequently, promote the patient's self-esteem and social integration.
