However, extended rest periods may prolong recovery and contribute to persistent symptoms. Emerging evidence suggests early active rehabilitation that increases heart rate without exacerbating symptoms may improve mTBI patient recovery. This review aims to: (1) appraise evidence on active rehabilitation intervention for mTBI recovery within one-month of injury (i.e., exercise type, duration, intensity, etc.); and (2) recommend evidence-based rehabilitation protocols. Method: Pubmed, CINAHL, PsychARTICLES, SportDISCUS, and AMED databases were searched using key terms “mild Traumatic Brain Injury”, “Rehabilitation”, “Acute”, and their synonyms. Evidence was appraised using Cochrane RoB-2 and ROBINS-I. Results: 434 citations were initially identified with seven papers systematically reviewed. Within the rev...

Over a seven day period that included four training days, two rest days and a match day, energy intake (self-reported weighed food diary and 24-h recall) and expenditure (tri-axial accelerometry) were recorded in 10 male players from a professional English Premier League club. The mean macronutrient composition of the dietary intake was 318 ˘ 24 g¨day´1 (5.6 ˘ 0.4 g¨kg´1 BM) carbohydrate, 86 ˘ 10 g¨day´1 (1.5 ˘ 0.2 g¨kg´1 BM) protein and 70 ˘ 7 g¨day´1 (1.2 ˘ 0.1 g¨kg´1 BM) fats, representing 55% ˘ 3%, 16% ˘ 1%, and 29% ˘ 2 % of mean daily energy intake respectively. A mean daily energy deficit of ´1302 ˘ 1662 kJ (p = 0.035) was observed between energy intake (9395 ˘ 1344 kJ) and energy expenditure (10679 ˘ 1026 kJ). Match days (´2278 ˘ 2307 kJ, p = 0.012) and heavy training days (´2114 ˘ 2257 kJ, p = 0.016) elicited the greatest defi...

Ho... more Exercise Referral Schemes (ERS) are a prevalent method of increasing physical activity levels. However, they suffer from participant dropout and research predicting dropout or barriers to adherence are limited. This study aimed to focus upon the effect of referral characteristics on dropout, dropout predictors and whether self-reported barriers to exercise predict dropout. ERS data from 2009 to 2014 were retrieved for analysis. Chi-squared and t-tests were used to investigate differences between referral characteristics, and logistic regression used to investigate dropout predictors. Of 6894 participants, 37.8% (n = 2608) dropped out within 6 weeks and 50.03% (n = 3449) by the final 12th week. More males adhered (P < 0.001) with dropouts being significantly younger (P < 0.001). Dropout predictors were smoking (OR = 1.58, 95% CI: 1.29-1.93) or being a Tier 3 referral (OR = 1.47, 95% CI: 1.25-1.73). Increasing age (OR = 0.98, 95% CI: 0.98-0.99), drinking alcohol (OR = 0.82, 95% ...

Some will be upgrading their supplementary prescribing qualifications, while others will be training for the first time in prescribing. This article discusses the role of the extended scope physiotherapist as an independent prescriber in a specialist musculoskeletal setting. The case study outlines the assessment and treatment of a patient with lower back pain and neuropathic leg pain, and details the prescribing issues involved in the overall management.

The majority of athletes with exertional dyspnoea will be initially assessed and managed in primary care. This report provides a practical and pragmatic approach to the assessment and management of a young athlete presenting with suspected EIB in this setting.
