The most common cause in adults is primary hyperparathyroidism, fo llo wed by renal tubular acidosis, medullary sponge kidney, and other causes of hypercalcemia (hypercalciuria). A 27 years o ld lady who presented with left lo in pain wh ich was later found to be bilateral medullary calcinosis. Patient experienced symptoms of colicky pain, polyuria, polydypsia, hyperthyroidism such as palpitations, tremor, sweating, loss of weight and heat intolerance. Physical examinations revealed fine tremors of hands, bilateral eyes lid lag and diffuse enlargement of the thyroid without retrosternal extension. Retro-aortic left renal vein [RLRV] is a congenital anomaly characterised by a left renal vein pa... more Retro-aortic left renal vein [RLRV] is a congenital anomaly characterised by a left renal vein passing posterior to abdominal aorta. It has been suggested that RLRV may cause varicocele and haematuria. The urological symptoms are probably due to the increased pressure in the left renal vein secondary to posterior nutcracker phenomenon. The diagnosis of RLRV is usually radiological. We report a case of a 53-year old man who presented with left scrotal swelling which was then confirmed to be varicocele by Doppler ultrasound. Two months later, the patient had microscopic haematuria and computed tomography [CT] abdomen and pelvis scan was done. CT showed presence of RLRV. Patients with Benign Prostatic Hyperplasia (BPH) commonly presents with lower urinary tract sympt... more Patients with Benign Prostatic Hyperplasia (BPH) commonly presents with lower urinary tract symptoms (LUTS), which can be of obstructive symptoms such as hesitancy, incomplete voiding, post void dribbling or of irritative symptoms such as urgency, frequency and nocturia. Various recent studies indicate that nocturia is a very important and bothersome lower urinary tract symptom especially among patients with Benign Prostatic Hyperplasia (BPH). The aims of the study was to determine the frequency of common urinary symptoms among patients with BPH in HUSM as well as to evaluate the extent of bothersomeness of each and every symptom to these patients. This study too was aimed at evaluating the success of TURP in resolving pre operative LUTS. This was a questionnaire-based survey using a validated ICSBPH model whereby patients with BPH were selected and quizzed personally by an investigator. A subset of patients who had undergone TURP were further questioned regarding their satisfaction with the procedure. When nocturia is defined as waking up at night once or more to pass urine, the prevalence of nocturia was about 90%, but only 1 in 6 patients considered this is a very serious symptom. Even if the definition was changed to waking up twice or more, the prevalence is still quite high at over 80%. Urgency were noted in half of the patients, but only a quarter of them consider it a serious problem. 1 in 5 patients experienced significant leak and almost all consider it serious. About one third of the studied population had to be catheterized due to urinary obstruction and interestingly only about half of them considered it as a very serious event. Overall, a great majority of these patients considered suffering from these urinary symptoms for the rest of their life as very devastating. This study conclude that although the prevalence of nocturia was high among BPH patients, but it was not considered serious by majority of them. In fact, LUTS in our BPH patients did not differ much from BPH patients elsewhere. On the whole, TURP resolved most of the LUTS effectively.

Mitochondrial DNA (mtDNA) polymorphisms were analyzed by polymerase chain reaction amplification ... more Mitochondrial DNA (mtDNA) polymorphisms were analyzed by polymerase chain reaction amplification and haplogroup-specific restriction screening in populations from Corsica and Sardinia. These included 56 individuals from the area of Corte, central Corsica (France), 51 individuals from Gallura, northern Sardinia (Italy), and 45 individuals from Barbagia, central Sardinia. The screening revealed that about 95% of mtDNAs could be grouped in 8 of the 9 European haplogroups, including H-K, T-V, and X. Our results confirmed that these haplogroups encompass virtually all the mitochondrial lineages present in Europe and can be detected in both northern and southern European populations. We also discovered 2 restriction sites (-73 Alw441 and +75 SphI) that allow the detection of informative nucleotide changes in the second hypervariable segment of the control region, which help to detect the haplogroup identity of mtDNAs without requiring further DNA sequencing. Objective: Systemic sclerosis (SSc) is a connective tissue disease that is characterized by vascu... more Objective: Systemic sclerosis (SSc) is a connective tissue disease that is characterized by vascular dysfunction and increased fibrosis. Cardiac manifestations are common in SSc, with a prevalence of 15-35%. We describe a patient with diffuse SSc and electrocardiographic findings of complete atrioventricular (AV) block associated with right bundle branch block (RBBB). Methods: A 68-year-old woman admitted to rheumatology clinic of our hospital due to generalized weakness, presyncopal episodes, and exertional dyspnea NYHA class IV. Past medical history revealed presence of diffuse SSc which was diagnosed 2 years ago when she presented Raynaud's phenomenon, arthralgias, sclerodactyly, and malaise; hypertension for 4 years. Her medication was including prednisone 5mg/d, cyclophosfomide 2×50mg/d, ramipril, aspirin, spironolactone and furosemide. Until the appearance of her recent symptoms, the patient was in dyspnea on exertion NYHA class II, whereas in previous ECG, RBBB was shown. On admission, the physical examination revealed jugular vein distension and peripheral edema. The liver was palpable 3 cm below the right costal margin. Her heart rate ranged from 34 to 45 beats/minute, and the blood pressure was 100/55mmHg. On auscultation, there was II/VI systolic murmur at the lower left parasternal border. Laboratory tests were normal besides a normochromic, normocytic anemia with 35%hematocrit and 48mm/h erythrocyte sedimentation rate. Antinuclear antibodies were present in a titer 1:1280, anti Scl-70 antibodies were also positive. Pulmonary function tests also disclosed extensive pulmonary fibrosis. Her ECG revealed complete AV block associated with RBBB. Transthoracic echocardiogram showed a LV ejection fraction of 65%, LVEDD of 5.3cm; 2°mitral regurgitation and 2°tricuspid regurgitation; mild RV dilatation. Multidetector computed tomography angiography showed normal coronary arteries. Due to her clinical presentation, permanent pacemaker implantation was planned. Results: On 2nd day of admission, dual chamber pacemaker was performed with no complication. Treatment was initiated with diuretics, ACE inhibitors, nifedipine and supplemental oxygen on which the patient improved both clinically and hemodynamically. The remaining hospital course was uneventful. At 6th month control, she was in dyspnea on exertion NYHA class I-II. Conclusions: Systemic sclerosis is mainly affecting pericardium, myocardium, and conduction system. SSc is connected with several atrial or ventricular arrhythmias and also conduction system abnormalities. Specifically, the conduction system lesions include first-degree heart block, RBBB, LBBB, and high-grade heart block. In our case, the complete AV block was associated with RBBB. The most probable explanation of the AV and intraventricular conduction abnormalities might be connected with the progressive fibrotic replacement of the myocardium.
