Postanesthesia nurses are expected to critique nursing research before applying findings to clinical practice. As consumers of research, postanesthesia nurses must be able to employ critical evaluation skills to judge the merit and relevance of research to their clinical practice. This article presents criteria for critiquing clinical nursing research reports to determine their relevance to practice.

therapies and usage of suboptimal doses were abstracted from notes in the medical chart and from ... more therapies and usage of suboptimal doses were abstracted from notes in the medical chart and from telephone review of previous medication trials with the patient. Results: For ACE-i/ARB and beta blockers, rates of prescription in eligible patients were 81% and 94% respectively. However, 71% of patients on an ACE-i/ARB and 81% of patients on a beta blocker were prescribed lower than target doses of these medications. By contrast, utilization of other categories of GDMT was lower in eligible patients, with 59% prescribed MRAs and 16% prescribed an ARNi. As summarized in the Figure, in most cases, the reasons for non-prescription or under-dosing of GDMT by treating clinicians were not apparent on detailed review of the EHR and discussion with the patient. Conclusion: Despite widespread utilization of ACEi/ ARB and beta blockers in HFrEF, optimization of pharmacologic therapy according to guidelines is inconsistent in clinical practice. The reasons for nonprescription and under-dosing of evidence-based therapies are frequently not apparent from detailed chart review, and only rarely related to documented medication intolerance or contraindications. These data suggest a substantial opportunity for quality improvement.

Family caregivers are susceptible to negative outcomes as a result of their caregiving role. A stress process model was utilized to identify characteristics of stroke caregivers who are at risk for poor physical and mental health-related quality of life (QOL). Research Method-Individuals who experienced an incident stroke event within the previous year were identified from a larger epidemiologic study of stroke incidence. These stroke survivors were enrolled in the Caring for Adults Recovering from the Effects of Stroke (CARES) study along with their primary family caregivers (N=146 dyads). Caregivers completed a baseline telephone interview that assessed physical and mental health-related QOL, problems their family members were experiencing, appraisals of those problems, and caregiver resources. Results-Objective stressors, appraisals, and caregiver resources were related to caregiver physical and mental health-related QOL, p's <.05. Objective stressors were found to have a stronger association with caregiver mental health than physical health. Hierarchical regression models showed the relative importance of each category of predictors. In the final models, older age and receiving more support were associated with worse physical health-related QOL while African American race and fewer stroke survivor problems were associated with better mental health. Implications-The correlates of health-related QOL identified in this national sample of caregivers can help identify stroke caregivers who are at-risk for poor adjustment to the caregiving role and aid in identifying areas that can potentially be intervened upon for these caregivers.

The purpose of this study was to identify high priority problems experienced by informal caregive... more The purpose of this study was to identify high priority problems experienced by informal caregivers when providing care for individuals with heart failure in the home. This secondary analysis was part of a cross-sectional, descriptive study using online self-report instruments (N = 530), including one researcher-developed item identifying top priority problems for heart failure caregivers. Content and quantitative data analyses were conducted. Performing multifaceted activities and roles that evolve around daily heart failure demands (n = 463) and maintaining caregiver physical, emotional, social, and financial well-being (n = 138) were the two most common themes experienced by caregivers of individuals with heart failure. Each of these two problems had several dimensions. Another theme was providing unending care (n = 40), with two dimensions.

Therefore, determining factors that promote effective self-management behaviors may be significant for improving the well-being of patients with type 2 diabetes mellitus. This study examined relationships among self-efficacy, social support, social problem solving, and diabetes self-management behaviors. Further, this study evaluated whether social support and social problem solving were mediators of the relationship between self-efficacy and diabetes self-management behaviors in those living with type 2 diabetes mellitus. Using a cross-sectional, descriptive correlational design, data from a convenience sample of 152 rural people living with type 2 diabetes mellitus were examined. Findings indicated that self-efficacy was a strong predictor of diabetes self-management. The effect of social support on diabetes self-management differed among men and women in the sample. Social support and social problem solving were significantly associated with diabetes self-management in men. Neither social support nor social problem solving were mediators of the relationship between self-efficacy and diabetes

Objective-To examine the effectiveness of an individualized problem-solving intervention provided... more Objective-To examine the effectiveness of an individualized problem-solving intervention provided to family caregivers of women living with severe disabilities. Design-Family caregivers were randomly assigned to an education-only control group or a problem-solving training (PST) intervention group. Participants received monthly contacts for 1 year. Participants-Family caregivers (64 women, 17 men) and their care recipients (81 women with various disabilities) consented to participate. Main outcome measures-Caregivers completed the Social Problem-Solving Inventory-Revised, the Center for Epidemiological Studies Depression scale, the Satisfaction with Life scale, and a measure of health complaints at baseline and in three additional assessments throughout the year. Results-Multilevel modeling was used to conduct intent-to-treat analyses of change trajectories for each outcome variable. Caregivers who received PST reported a significant linear decrease in depression over time; no effects were observed for caregiver health or life satisfaction. Caregivers who received PST also displayed an increase in constructive problem-solving styles over the year. Conclusions-PST may benefit community-residing family caregivers of women with disabilities, and it may be effectively provided in home-based sessions that include face-to-face visits and telephone sessions.
