The Housing Program Measure (HPM) was designed to document critical elements of a range of housin... more The Housing Program Measure (HPM) was designed to document critical elements of a range of housing program types and associated services. Qualitative methods, including literature review and open-ended interviews, were used to determine pertinent HPM domains and to develop the pool of items. The measure was pre-tested, and reliability and validity analyses were applied to revise and strengthen the measure. The resulting measure furthers homelessness research by providing a tool that can be used to define housing and housing services interventions across diverse projects and disciplines, to facilitate program management by matching housing resources to the needs of homeless individuals, and to support model development by measuring progress to goals.

Background: The use of alcohol by persons with schizophrenia is common and has been associated wi... more Background: The use of alcohol by persons with schizophrenia is common and has been associated with increased severity of psychiatric symptoms, multiple psychosocial problems, abuse of other drugs, and poor treatment outcomes. Most of the previous research in this area has been with urban patients. Method: The authors examined the correlates and outcomes of alcohol use in a rural sample of 75 DSM-III-R outpatients with schizophrenia. Based on multiple measures, 25% (N = 19) of 75 rural patients with schizophrenia were diagnosed with current co-occurring alcohol use disorders. Clinicians' ratings and self-reported symptoms were used to examine correlates of alcohol use, and the study group was followed prospectively for 1 year to identify all episodes of rehospitalization, incarceration, or literal homelessness. Results: Alcohol use disorder was statistically significantly associated with unstable housing, conceptual disorganization, denial of mental illness, and rehospitalization during 1-year follow-up. Several trends suggested that alcohol use was also related to positive symptoms of psychosis. Conclusion: Among rural patients with schizophrenia, alcohol use appears to play a significant role in destabilizing psychosocial adjustment. These results replicate similar findings in urban settings.

The cases represent a 2-year referral period which began July 1, 1980, when DSM-Ill criteria were instituted. Delirium and dementia are most commonly diagnosed and features of these, particularly in the geriatric population, me described. Delirium was more frequent in patients with multiple medical problems, was alz indicator of poor prognosis having the highest mortality rate, and was usually undiagnosed by the referring physician.

As the peer specialist workforce continues to expand, it is critical to better understand peer pr... more As the peer specialist workforce continues to expand, it is critical to better understand peer providers' working conditions and workplace experiences. The current study utilized a targeted non-probability sample of 801 peer specialists to explore whether key organizational climate and support variables would yield distinct multivariate groups, and to investigate the correlates of these groups. Analyses yielded a seven-group solution, with peer run organizations comprising a substantially greater proportion of the groups with high organizational climate and support scores. In direct comparisons, peer-run programs outranked all other groups in the areas of perceived organizational climate, supports for career development and perceived service quality.

Assertive community treatment (ACT) is an evidence-based practice that consists of a multidiscipl... more Assertive community treatment (ACT) is an evidence-based practice that consists of a multidisciplinary team of professionals who provide intensive and comprehensive services to people with serious mental disorders living in the community. ACT has been shown to be effective in reducing hospital days and increasing housing stability for service recipients. However, more than half of the people in these programs typically have a co-occurring substance use disorder, and evidence for the model's effectiveness in treating dual disorders is less consistent. One reason cited for this shortcoming is the apparent failure to provide care consistent with the principles and practices of integrated dual disorders treatment, itself an evidence-based practice with demonstrated effectiveness. This is a problem of treatment fidelity, one that is addressed in a new ACT fidelity measure, the Tool for Measurement of Assertive Community Treatment (TMACT), which assesses not only the structural features of ACT but also the quality of clinical processes and services. With the TMACT, evaluators assess particular aspects of staff roles and team functioning as well as integration of critical elements of other evidence-based services, including integrated dual disorders treatment and recovery-oriented, person-centered practices. The measure is described, with particular detail provided for items that assess integrated dual disorders treatment, and a case example is presented to illustrate how the TMACT is used to guide consultation for ensuring effective integrated dual disorders treatment implementation within ACT.

Continuous treatment teams serving persons with co-occurring severe mental disorders and substanc... more Continuous treatment teams serving persons with co-occurring severe mental disorders and substance abuse disorders at seven sites in New Hampshire were evaluated to determine their fidelity to a model based on the Program for Assertive Community Treatment. Continuous treatment teams and standard case management programs at the seven sites were evaluated on 13 criteria for fidelity to the continuous treatment team model over a 27-month period. Data sources included clinicians' activity logs, agencies' management information systems, interviews, observation of staff activity and practices, and clinical records and other documents. The continuous treatment teams scored significantly higher than the case management programs on ten of the 13 criteria. The teams were more effective than the case management programs in implementing substance abuse treatment. Evaluation of the programs' fidelity to the model criteria allowed differentiation of successfully implemented continuous treatment teams from standard case management and from an unsuccessfully implemented team. The results confirm the need for careful measurement of model implementation and for investigation of organizational issues such as administrative support and clarity of program mission.

The number of empowerment-oriented consumeroperated service programs (COSPs) in mental health has... more The number of empowerment-oriented consumeroperated service programs (COSPs) in mental health has increased dramatically over the past decade; however, little empirical evidence exists about the effects of such programs on their intended outcomes. This study examined the effects of COSPs on various aspects of empowerment within the context of a multisite, federally funded, randomized clinical trial of COSPs. Results suggest that the individuals who received the consumeroperated services perceived higher levels of personal empowerment than those in the control intervention; overall, effect sizes were very modest when all sites were examined together in intent-to-treat analyses. However, we noted variations in outcomes by intensity of COSP use and also by study site, which suggest that specific programs had significant effects, while others did not. The implications of these results for the mental health field and for service providers and policy makers are discussed. Clinical Trial Registration: The International Committee of Medical Journal Editors (ICMJE) has proposed a policy for the registration of clinical trials stating that any trial enrolling participants after July 1, 2005, should be registered. For trials that began enrollment before this date, the ICMJE member journals required registration by September 13, 2005. This study was funded from 1998 until 2004 and thus was concluded before the development of the ICMJE policy on registration. Therefore, this study was not registered as a clinical trial.
