Previous and current studies have shown satisfaction with telemedicine, especially from a parental perspective. The largest barrier for implementation is technology. Some studies have found telemedicine equal to in-person visits. Only few studies have measured clinical outcomes, and the perspective of the child and adolescents as well as healthcare personal is rarely examined. There is a need of developing guidelines and education in paediatric telemedicine, as argued in this review.

Even so, little is known about the current treatment practice. We aimed to explore the current organisation of clinical care for children and adolescents with functional somatic symptoms in a hospital setting. Methods: A de novo questionnaire with 60 items about practice patterns for children and adolescents with functional somatic symptoms was developed, tested and distributed to all Danish paediatric departments. Results: All 19 paediatric departments in Denmark participated. Two departments had no available treatment programme, whereas the remaining 17 departments offered a treatment programme. Overall, substantial variation was reported in the number of patients in treatment, the composition of multidisciplinary teams and in the tasks and training of these teams. A future increase in paediatric patients with functional somatic symptoms was expected by 95% of the departments. Still, 59% reported a reduction in resources allocated to this group over the past five years. Two thirds of the departments expressed a need for clinical guidelines. Conclusions: Substantial variation was observed in the healthcare services offered to paediatric patients with functional somatic symptoms referred to the Danish hospital setting. Clinicians called for guidelines, which, in the light of the decreasing resources allocated and the increasing patient numbers, may help ensure a systematic approach and better quality of care for this patient group. FUNDING none. TRIAL REGISTRATOIN not relevant.

We investigated predictors and diabetes outcomes in a pediatric population with and without psychiatric comorbidities. RESEARCH DESIGN AND METHODS Data from the Danish Registry of Childhood and Adolescent Diabetes (DanDiabKids) and National Patient Register were collected (1996–2015) for this population-based study. We used Kaplan-Meier plots to investigate whether age at type 1 diabetes onset and average glycated hemoglobin (HbA1c) levels during the first 2 years after onset of type 1 diabetes (excluding HbA1c at debut) were associated with the risk of being diagnosed with a psychiatric disorder. Mixed-effects linear and logistic regression models were used to analyze HbA1c, BMI, severe hypoglycemia (SH), or ketoacidosis as outcomes, with psychiatric comorbidities as explanatory factor. RESULTS Among 4,725 children and adolescents with type 1 diabetes identified in both registers, 1,035 were ...

This study described, implemented and evaluated a transition programme for adolescents.MethodsWe conducted a single centre, nonrandomised and noncontrolled prospective programme at the cystic fibrosis centre at Copenhagen University Hospital Rigshospitalet from 2010 to 2011, assessing patients aged 12–18 at baseline and after 12 months. Changes implemented included staff training on communication, a more youth‐friendly feel to the outpatient clinic, the introduction of youth consultations partly alone with the adolescent, and a parents’ evening focusing on cystic fibrosis in adolescence. Lung function and body mass index (BMI) were measured monthly and adolescents were assessed for their readiness for transition and quality of life at baseline and 12 months.ResultsWe found that 40 (98%) of the eligible patients participated and youth consultations were successfully implemented with no ...

Research paper thumbnail of Increasing risk of psychiatric morbidity after childhood onset type 1 diabetes: a population-based cohort study

In a matched, population-based cohort study based on Danish national registers, we identified children and adolescents who had been diagnosed as an in- or outpatient with type 1 diabetes before the age of 18, and afterwards diagnosed with a psychiatric disorder (n = 5084). Control individuals were matched according to sex and date of birth (n = 35,588). The Cox proportional hazards model was used to assess associations between type 1 diabetes and the incidence of psychiatric disorders as well as the effects of age at onset and duration of type 1 diabetes on the risk of subsequently developing psychiatric morbidities. An increased risk of being diagnosed with mood disorders and anxiety, dissociative, eating, stress-related and somatoform disorders was observed in both sexes in the years following type 1 diabetes onset, with the highest risk observed five years or more after on...

Figure 1. Newborn with marked swelling and discoloration around the right hip and thigh. ### Prenatal and Birth Histories ### Presentation Infant was discharged home and did well during the first week; she was well-appearing and feeding by demand every 3 to 4 hours. On day 8 a health visitor found a significant weight loss of 650 g. The infant was admitted to a local neonatal ward. She was moderately dehydrated with a sodium level of 160 mEq/L (160 mmol/L). There were no other significant clinical or laboratory findings, and the dehydration was thought to be caused by difficulties with breastfeeding. Oral rehydration was performed via nasogastric tube, and the infant was subsequently successfully transitioned to breastfeeding and discharged on day 15. On day 20 the parents noticed swelling and bluish discoloration of the infant’s right thigh, but she remained otherwise well. Four days later she held her leg in a fixed flexed position and she was crying during diaper changes. They consulted their general practitioner, who referred the infant to a neonatal intensive care unit. ### Vital Signs

In Denmark, less than 40% of pregnant women meet physical activity (PA) recommendations. The FitMum study aims to explore strategies to increase PA during pregnancy among women with low PA and assess the health effects of PA. This paper presents the FitMum protocol, which evaluates the effects of structured supervised exercise training or motivational counselling supported by health technology during pregnancy on PA level and health of mother and offspring. Methods and analysisA single-site three-arm randomised controlled trial that aims to recruit 220 healthy, pregnant women with gestational age (GA) no later than week 15 and whose PA level does not exceed one hour/week. Research paper thumbnail of ‘I Think It Will Be Like This Forever’: How Family Narratives Affect Participation in a Childhood Weight Management Intervention

The Social Sciences, 2021

Involving the family also entails involving the individual family members’ experiences with, attitudes towards, and understandings of obesity. This study explores how families with life-long experiences of overweight manage and experience a family-based childhood weight management intervention in Northern Zealand in Denmark. The analysis is focused on family narratives and their temporal character. The families’ narratives about overweight and past weight management interventions are crucial to how they understand and manage the present intervention. Additionally, the families expect the focus on weight management to continue to be a constant part of their everyday life. The paper concludes that the understanding of weight management in interventions should take its point of departure in the life-world, which the individual family creates...
