Aim: Dentigerous cyst (DC) is a disembriogenetic lesion. The cyst wall encloses the crown of an i... more Aim: Dentigerous cyst (DC) is a disembriogenetic lesion. The cyst wall encloses the crown of an impacted tooth. Several therapeutic approaches have been mentioned in the literature for management of this lesion. Case Report This article describes the management of an adolescent with a mandibular DC surgically treated with extraction of the tooth, enucleation of the cyst and replantation of the permanent tooth involved. Final outcome shows complete healing of the bone socket with eruption of a vital tooth. No orthodontic traction was required. No recurrence was detected at the radiographic follow-up at 12 months, thus confirming the success of this therapeutic approach. After a 7-year follow-up period the tooth responded positively to the vitality test. Clinical implications: In selected cases surgical enucleation of the lesion without loss of involved tooth, might be considered as a viable treatment to obtain healing of the lesion, spontaneous eruption of the tooth and physiological restoration of bone.

Gli autori hanno realizzato uno studio unicentrico randomizzato con l’obiettivo di individuare la... more Gli autori hanno realizzato uno studio unicentrico randomizzato con l’obiettivo di individuare la gestione farmacologica ottimale per prevenire l’insorgenza dell’osteonecrosi da bifosfonati che precede, generalmente, un intervento chirurgico estrattivo. Sono stati arruolati 52 pazienti, suddivisi in modo random in due gruppi (gruppo 0: controllo; gruppo 1: studio). In ciascuno dei due gruppi sono state realizzate estrazioni chirurgiche. In nessun paziente oncologico in terapia con bifosfonati endovenosi è stata riscontrata l’insorgenza della complicanza odontoiatrica più temuta, ovvero l’osteonecrosi dei mascellari. Utilizzando il nostro protocollo siamo riusciti a ottenere lo stesso risultato in tutti i partecipanti, con il vantaggio che, nel gruppo studio, la terapia antibiotica sistemica è stata ridotta a 6 giorni

Many consolidated national and international research groups will be involved in the debate and discussion through special guest lecturers, academic dissertations, single clinical case presentations, posters, and degree thesis discussions. The SIPMO Congress took place from the 17th-19th of October 2019 in Bari (Italy), and the enclosed copy of Proceedings is a non-exhaustive collection of abstracts from the SIPMO 2019 contributions.

Wound closure after post‐traumatic injuries and/or localized at peculiar body sites (head‐and‐nec... more Wound closure after post‐traumatic injuries and/or localized at peculiar body sites (head‐and‐neck, oral cavity, legs) are particularly challenging and can often be delayed due to local and systemic factors. In case of deep wounds and/or hard‐to‐heal wounds, grafting of dermal acellular matrices (ADM) is often needed. Though a great variety of synthetic and semisynthetic dermal and skin equivalents are available, viable human dermis, is still considered the most physiological alternative to replace the loss of autologous dermis, by acting as a physiological scaffold that add structural support to soft tissues. To date, human ADMs (hADMs) have been employed in the reconstruction of skin defects affecting almost all body sites, ranging from visceral sites to the skin and subcutaneous tissues. This review aims to investigate the use of hADM at different body sites and their peculiar advantages. A literature search was using the search terms “acellular dermal matrices”, “dermal regeneration”, “advances wound healing”, “human acellular dermal matrices surgery”. A total of 50 out of 150 papers was included. Based on the current body if evidence, hADMs appear to bring several advantages, such as: protection of deep structures (eg, tendons, bones, cartilage and nerves); stimulation of a functional new dermis (rather than a scar); reduction of wound closure time; control of pain and exudate. Finally, hADMs may represent the best treatment option for hard‐to‐heal wound not only in terms of efficacy and patient satisfaction bout also in terms of sanitary costs, especially across Europe, where hADMs cannot be commercialized as medical devices.

Nel nostro studio clinico abbiamo trattato pazienti pedodontici non collaboranti alle cure odonto... more Nel nostro studio clinico abbiamo trattato pazienti pedodontici non collaboranti alle cure odontoiatriche attraverso un ausilio farmacologico che ci ha permesso di riuscire a trattare questa categoria di pazienti. 57 pazienti sono stati arruolati nello studio. Hanno ricevuto cure odontoiatriche come sigillature, otturazioni, terapie canalari ed estrazioni attraverso la somministrazione di bromazepam. I 57 pazienti si sono mostrati non collaboranti nella prima visita alle cure odontoiatriche, ma attraverso l’ausilio di una terapia farmacologica sono riusciti a ottenere cure odontoiatriche. Bromazepam quindi trova indicazione terapeutica in questa classe di pazienti

(1) Le cheiliti, oltre a costituire un problema estetico, possono anche influire negativamente sulle normali attività quotidiane come alimentarsi, parlare o sorridere. L'obiettivo dello studio è fornire un quadro delle diverse applicazioni terapeutiche antimicrobiche che i fitoterapici possono avere in campo odontoiatrico, nonché degli effetti collaterali che alcune terapie a base di erbe possono indurre a livello del cavo orale. Materiali e metodi. È stata condotta un'attenta revisione della letteratura scientifica internazionale a partire dal 1995 in PubMed, Embase e Scopus mediante l'utilizzo di appropriate parole chiave. Risultati e conclusioni. Gli studi esaminati hanno mostrato importanti proprietà medicinali delle piante per uso odontoiatrico. Ulteriori ricerche sono però necessarie per valutare meglio l'efficacia clinica dei fitoterapici (trial clinici, studi prospettici), la tossicità, gli effetti collaterali, le interazioni con altre categorie di farmaci, al fine di possedere basi scientifiche sempre più valide.

Oral Lichen Planus (OLP) is a chronic inflammatory condition implicating T cell-mediated cytotoxi... more Oral Lichen Planus (OLP) is a chronic inflammatory condition implicating T cell-mediated cytotoxicity, and involving oral mucosal surfaces. Several therapeutic regimens have been evaluated to treat OLP and pain related, but often without high level of evidence. Topical formulations are the favourite for the majority of cases; bioadhesive formulations have been considered very useful and practical for local drug delivery in oral mucosa, due to the increased residence time on the oral mucosa of the dosage forms and better therapeutic efficacy. In this narrative review, authors try to illustrate the current topical managements for OLP from the accessible literature on this topic. Steroids are very helpful in discomfort and making better quality of life: they are considered the first-line treatment even if they could cause secondary candidosis, and sometimes bad taste, nausea, dry mouth, sore throat or swollen mouth. Other substances or devices by topical administration are adopted especially when the first line approach is refractory. This is the case when retinol with its synthetic and natural analogues (retinoids), hyaluronic acid, or Aloe Vera are chosen. Recent topical applications for OLP therapy include phototherapy and low/high energy pulsing light; the treatment with extracorporeal photochemotherapy is also reasonable and promising. Finally, calcineurin inhibitors (i.e. cyclosporine, tacrolimus and pimecrolimus), antioxidant and biologics (i.e alefacept, efalizumab, basiliximab, TNF-inhibitors-infliximab, rituximab) may be alternative approaches when OLP does not respond to the standard protocols. In this scenario, there are several studies on molecules different from glucocorticosteroids, but not sufficient or statistically adequate to justify their evidence-based use in OLP; large randomized placebo controlled trials are required to evaluate the safety and effectiveness of these non conventional therapies. In conclusion, since OLP is a chronic disease and requires long-term management, the dental/medical practitioner, who treats OLP patients, needs to know the natural history of OLP, how to monitor, and how to treat, taking in account all of the available modalities conventional and not, with pros and cons.

Finally, calcineurin inhibitors (i.e. cyclosporine, tacrolimus and pimecrolimus), antioxidant and biologics (i.e alefacept, efalizumab, basiliximab, TNF-inhibitors-infliximab, rituximab) may be alternative approaches when OLP does not respond to the standard protocols. In this scenario, there are several studies on molecules different from glucocorticosteroids, but not sufficient or statistically adequate to justify their evidence-based use in OLP; large randomized placebo controlled trials are required to evaluate the safety and effectiveness of these non conventional therapies. In conclusion, since OLP is a chronic disease and requires long-term management, the dental/medical practitioner, who treats OLP patients, needs to know the natural history of OLP, how to monitor, and how to treat, taking in account all of the available modalities conventional and not, with pros and cons.

Wound closure after post‐traumatic injuries and/or localized at peculiar body sites (head‐and‐nec... more Wound closure after post‐traumatic injuries and/or localized at peculiar body sites (head‐and‐neck, oral cavity, legs) are particularly challenging and can often be delayed due to local and systemic factors. In case of deep wounds and/or hard‐to‐heal wounds, grafting of dermal acellular matrices (ADM) is often needed. Though a great variety of synthetic and semisynthetic dermal and skin equivalents are available, viable human dermis, is still considered the most physiological alternative to replace the loss of autologous dermis, by acting as a physiological scaffold that add structural support to soft tissues. To date, human ADMs (hADMs) have been employed in the reconstruction of skin defects affecting almost all body sites, ranging from visceral sites to the skin and subcutaneous tissues. This review aims to investigate the use of hADM at different body sites and their peculiar advantages. A literature search was using the search terms “acellular dermal matrices”, “dermal regeneration”, “advances wound healing”, “human acellular dermal matrices surgery”. A total of 50 out of 150 papers was included. Based on the current body if evidence, hADMs appear to bring several advantages, such as: protection of deep structures (eg, tendons, bones, cartilage and nerves); stimulation of a functional new dermis (rather than a scar); reduction of wound closure time; control of pain and exudate. Finally, hADMs may represent the best treatment option for hard‐to‐heal wound not only in terms of efficacy and patient satisfaction bout also in terms of sanitary costs, especially across Europe, where hADMs cannot be commercialized as medical devices.
