Objective: Evaluate the lipid profile, fasting blood glucose, Body Mass Index (BMI) and weight ga... more Objective: Evaluate the lipid profile, fasting blood glucose, Body Mass Index (BMI) and weight gain of pregnant women with obesity and the effect of a low dose (1,1 g/day) of omega-3 on these parameters. Methods: A randomized clinical trial was performed from May 1st, 2019 to October 31th, 2022 with pregnant women diagnosed with obesity by arrival BMI ≥ 30.0 kg/m2. The participants were randomized into two groups: omega-3 and control. All pregnant women received standard prenatal follow-up and the same diet and physical exercise instructions. Laboratory tests were performed at three different times: first moment with gestational age (GA 12-20 weeks), second (GA 24-28 weeks) and third (GA 32-34 weeks) measuring maternal serum levels of glucose, total cholesterol (TC), low density lipoprotein (LDL), high density lipoprotein (HDL) and triglycerides (TG) as well as weight gain and BMI were monitored. Durante a gestação, a obesidade materna está associada ao desenvolvimento do Diabetes Mellitus Ge... more Durante a gestação, a obesidade materna está associada ao desenvolvimento do Diabetes Mellitus Gestacional, doenças hipertensivas, intercorrências cardiovasculares, eventos tromboembólicos, cesarianas e complicações cirúrgicas durante o parto. Tendo em vista que a dieta materna, no período pré-natal, é de grande importância para o desenvolvimento fetal, pois determina o tipo de ácido graxo que se acumulará no tecido do feto, e que o transporte do ômega-3 se dá através da placenta, este trabalho tem como objetivo realizar uma revisão bibliográfica acerca dos efeitos do ômega-3 em gestantes obesas. Os resultados sobre os estudos analisados nos mostram que o ômega-3 é capaz de ajudar no controle do perfil lipídico materno e desenvolvimento fetal, conter reações inflamatórias, diminuir o risco de DMG e pré-eclâmpsia e possível redução da predisposição para doenças metabólicas na vida adulta. Metodos: trata-se de um ensaio clinico randomizado com mulheres gravidas obesas com indice de massa corporal superior ou igual a 30 kg/m2 divididos em dois grupos ­ um grupo de estudo, que utilizou metformina, e um grupo controle, que seguiu a rotina de pre-natal. As gestantes entraram no estudo com idade gestacional menor que 20 semanas e foram acompanhadas durante todo o periodo pre-natal. O diagnostico de pre-eclâmpsia foi feito de acordo com os criterios da Organizacao Mundial de Saude. Resultados: verificou-se a reducao de risco absoluto e o numero necessario para tratar em um intervalo de confianca de 95% para o resultado pre-eclâmpsia. Introduction: After treatment for Multiple Myeloma (MM), transformation into burkitt’s lymphoma (... more Introduction: After treatment for Multiple Myeloma (MM), transformation into burkitt’s lymphoma (LB) is uncommon, with rare cases reported. Objective: to report a rare case of LB secondary to MM, in a patient treated with the CyBorD protocol. Case report: We describe a case of a 37-year-old man, black, Senegalese and HIV positive, who in just over 2 months after the diagnosis of MM evolved to a medullary transformation that resulted in a B lymphoblastic lymphoma compatible with LB. Conclusion: This case report demonstrates the possibility of malignant transformation from MM to BL, thus encouraging future multicentric comparisons of similar cases in a broad effort to better define this pathological process.

RESUMO: Quais são os resultados preponderantes com a administração do cloridrato de metformina no... more RESUMO: Quais são os resultados preponderantes com a administração do cloridrato de metformina no índice de massa corporal elevado da gestante e no peso do recém-nascido? Foi feita uma revisão sistemática da literatura selecionada entre os anos de 2000 a 2017 sobre a obesidade e os efeitos da terapia na gestação nas bases MEDLINE, LILACS , Embase e Cochrane. Analisou-se artigos disponíveis em meio eletrônico, publicados em português, espanhol e inglês. Dentre os 2.127 artigos inicialmente escolhidos, somente 29 estudos estavam dentro dos critérios estabelecidos. Os desfechos avaliados foram resultados da terapia metformina versus placebo, terapia metformina versus insulina e o impacto de diferentes dosagens de metformina utilizadas nas pesquisas. A medicação apresentou resultados relevantes na redução do peso da mãe, mas não o do recém-nascido. Novos estudos clínicos multicêntricos são necessários, bem como uma estratégia terapêutica inovadora quanto às dosagens do cloridrato de metformina durante os três trimestres da gravidez.

Objective To evaluate the factors associated with the need for insulin as a complementary treatme... more Objective To evaluate the factors associated with the need for insulin as a complementary treatment to metformin in pregnant women with gestational diabetes mellitus (GDM). Methods A case-control study was performed from April 2011 to February 2016 with pregnant women with GDM who needed complementary treatments besides diet and physical exercise. Those treated with metformin were compared with those who, in addition to metformin, also needed the combination with insulin. Maternal characteristics and glycemic control were evaluated. Multinomial logistic regression models were developed to evaluate the influence of different therapies on neonatal outcomes. Results A total of 475 pregnant women who needed pharmacological therapy were evaluated. Randomized clinical trial including obese pregnant women with a body mass index (BMI) ≥ 30 kg/m, divided into two groups (control and metformin). Both groups received guidance regarding diet and physical exercise. The participants were assessed at two moments, the first at enrollment (gestational age ≤ 20) and the second at gestational weeks 24-28. The outcomes assessed were BMI and gestational diabetes mellitus (GDM) diagnosis. The data distribution was assessed with the Friedman test.
