Objective Vancomycin-resistant Enterococcus (VRE) is today one of the principal microorganisms implicated in nosocomial infections. Thus, a study was carried out with the objective of evaluating its epidemiology at a tertiary-level teaching hospital. Methods This was a three-year retrospective epidemiological study conducted from 2000 to 2002. Samples of VRE-positive clinical cultures at a 660-bed university hospital were analyzed. The incidence of VRE and the main anatomical sites and hospital units from which it was isolated were defined. Differences between the variables over the three years of the study were verified, and these were considered significant when p<0.05. Results There was a progressive increase in the vancomycin resistance in the clinical cultures that were positive for Enterococcus spp., over the three years of the study. The Brazilian cities, in their great majority, don't get to maintain a building cadastre with trustworthy data, mainly in areas of great urban expansion. This fact causes a deficient outturn and in consequence a smaller volume of investments by municipal government in benefit to the local population. Although in Brazil there are mechanisms that allow to the municipal districts to obtain the federal government's resources to implant cadastral updating politics, the results aren’t efficient. In many cases, with the obtained resources, practically carry out a new cadastral surveying of the municipal district, that generate high costs and it contributes by updating in short periods of time not be accomplished. Este trabalho foi feito a partir de um curso de extensão universitária cujo intuito foi introduzir os estudantes em um ambiente de geometria dinâmica e explorar potencialidades de sua utilização em aulas de matemática do Ensino Fundamental e Médio. Neste texto, serão destacados episódios ocorridos durante o curso, evidenciando determinadas tarefas investigativas no ambiente computacional que foram exploradas pelos participantes. Em particular, será destacado um episódio em que situações inesperadas colocaram os futuros professores no que a literatura denomina de zona de risco.

Sickle-cell disease is the most prevalent genetic disease in the Brazilian population. Lower limb... more Sickle-cell disease is the most prevalent genetic disease in the Brazilian population. Lower limb ulcers are the most frequent cutaneous complications, affecting 8% to 10% of the patients. These ulcers are usually deep and may take many years to heal. Evidence about the effectiveness of systemic or topical treatment of these wounds is limited, apart from stabilization of the anemia. A 28-year old woman with sickle-cell disease was admitted for treatment of three deep chronic lower leg ulcers. All wounds had tendon exposure and contained firmly adherent fibrin slough. Following surgical debridement and before grafting, the wounds were managed with three different dressings: a rayon and normal saline solution dressing, a calcium alginate dressing covered with gauze, and negative pressure therapy. Purpose: This study evaluated the inflammatory reaction caused by the implantation of iodoform an... more Purpose: This study evaluated the inflammatory reaction caused by the implantation of iodoform and calcium hydroxide in the back of rats. These drugs may be used as intracanal dressings to eliminate residual bacteria of the root canal system. Methods: Twenty albinic rats (Rattus norvegicus, var Wistar) were divided into four groups: control group 1 (CG1) had normal skin; control group 2 (CG2) had wounded tissue without drugs; in groups 3 and 4, iodoform (IG) and calcium hydroxide (CHG) were inserted into the wounds, respectively. After 3, 5 and 11 days, slices of the implanted areas were macroscopically and microscopically observed regarding to their qualitative and quantitative aspects. Results: In the macroscopical analysis, the CHG showed a large area of necrosis and swelling, which progressively decreased; in the IG the presence of iodoform surrounded by normal tissue was observed. The qualitative and quantitative histological analysis showed that IG promoted a shorter delay in the inflammatory response than the CHG. Conclusion: The inflammatory reaction for iodoform had a peak period five days after the drug insertion. By comparison, calcium hydroxide showed a very large area of necrosis that could only be partially eliminated after eleven days.

Vancomycin-resistant enterococci (VRE) are important pathogens causing nosocomial infections, and... more Vancomycin-resistant enterococci (VRE) are important pathogens causing nosocomial infections, and there is reason for concern about their resistance and great ability to spread in hospital environments, especially intensive-care units (ICU). To determine the prevalence of rectal colonization by VRE, and the risk factors associated with their presence, rectal surveillance swabs were taken from patients under treatment in two intensive-care units (one medical and another both medical and surgical) at São Paulo Hospital, over a two-year period. Thirty-three percent of the 147 patients evaluated had VRE. The only significant variable in the logistic regression was the length of stay in the ICU.

Purpose: The aim of this study was to verify the antimicrobial effect of calcium hydroxide and io... more Purpose: The aim of this study was to verify the antimicrobial effect of calcium hydroxide and iodoform on Enterococcus faecalis with different exposure times evaluating the bacterial morphologic alterations. Methods: The antibacterial action was investigated in culture broth after zero, seven, fourteen and twenty-one days. Five mL samples were analyzed morphologically on the seventh day by transmission electron microscopy. The data of the antibacterial test were analyzed by Fisher's exact test. Results: The results revealed that between the seventh and fourteenth day, there was a decrease in bacterial growth with both medicaments (P=0.098), where they were eliminated between the fourteenth and twenty-first day. Transmission electronic microscopy showed alterations in the morphologic structures. Conclusion: It concluded that both medicaments kill Enterococcus faecalis, with an exposure time of 7 to 14 days, where no cell viability is seen after this period due to irreversible alterations in bacterial cell morphology.

PURPOSE:To investigate the influence of 650 nm GaAlAs laser on the action of bone morphogenetic p... more PURPOSE:To investigate the influence of 650 nm GaAlAs laser on the action of bone morphogenetic protein (BMP) in bone defects produced in rat femurs. METHODS: The sample consisted of 12 male albino Wistar rats (Rattus norvegicus). The animals were randomly divided into four experimental groups. After undergoing anesthesia, the fur was removed from the lateral face of the right thigh and surgical dissection was performed to view the femur region. A bone defect was created using a spherical diamond-tipped drill bit. In groups 1 and 2, the defect was filled with a paste of Gen-Tech bone-inducing substance. The animals were treated with GaAlAs laser, at a predetermined dose of joules/cm² for 80 seconds, over an area of 1 cm². Groups 2 and 4 were used as controls. As porfirias são divididas em eritropoiéticas, hepáticas agudas e hepáticas crônicas. Os subtipos de maior relevância clínica são a porfiria cutânea tarda e a porfiria intermitente aguda. O diagnóstico das porfirias pode ser bastante difícil, dada a sobreposição de quadros clínicos e achados bioquímicos. A precisão do diagnóstico depende da dosagem de porfirinas urinárias e fecais, da análise da atividade enzimática de eritrócitos e, eventualmente, da pesquisa de mutações. O objetivo do presente artigo é realizar revisão literária das porfirias, com ênfase no diagnóstico e tratamento de seus diversos subtipos.

A insuficiência cardíaca é uma síndrome complexa definida pela falência do coração em proporciona... more A insuficiência cardíaca é uma síndrome complexa definida pela falência do coração em proporcionar suprimento adequado de sangue às necessidades metabólicas tissulares, ou fazê-lo por meio de elevadas pressões de enchimento. Geralmente, resulta de disfunção estrutural ou funcional do coração que compromete a sua capacidade de comportar o volume de enchimento diastólico de sangue ou de realizar sua ejeção. Caracteriza-se por disfunção ventricular e alterações da regulação neurohumoral, acompanhada de sintomas de cansaço aos esforços, retenção hídrica e redução da expectativa de vida. Em nosso meio, a sobrevida da insuficiência cardíaca com o tratamento clínico é estimada em 84%, 63% e 52%, em um, dois e cinco anos, respectivamente. Tem-se observado que a insuficiência cardíaca apresenta maior mortalidade do que muitas modalidades de câncer, como bexiga, mama e próstata,perdendo apenas ao câncer de pulmão. Assim, realizou-se estudo com o objetivo de avaliar sua epidemiologia em um hospital terciário de ensino. MÉTODOS: Trata-se de um estudo epidemiológico retrospectivo, realizado de 2000 a 2002, que analisou amostras de culturas clínicas positivas para enterococo resistente à vancomicina (VRE) em um hospital universitário com 660 leitos. Procurou-se definir sua incidência e os principais sítios e unidades de isolamento. Foi verificada a significância entre as variáveis nos três anos de estudo, sendo considerado como significante p<0,05. Vancomycin-resistant enterococci (VRE) are important pathogens causing nosocomial infections, and... more Vancomycin-resistant enterococci (VRE) are important pathogens causing nosocomial infections, and there is reason for concern about their resistance and great ability to spread in hospital environments, especially intensive-care units (ICU). To determine the prevalence of rectal colonization by VRE, and the risk factors associated with their presence, rectal surveillance swabs were taken from patients under treatment in two intensive-care units (one medical and another both medical and surgical) at São Paulo Hospital, over a two-year period. Thirty-three percent of the 147 patients evaluated had VRE. The only significant variable in the logistic regression was the length of stay in the ICU.

Purpose: The aim of this study was to verify the antimicrobial effect of calcium hydroxide and io... more Purpose: The aim of this study was to verify the antimicrobial effect of calcium hydroxide and iodoform on Enterococcus faecalis with different exposure times evaluating the bacterial morphologic alterations. Methods: The antibacterial action was investigated in culture broth after zero, seven, fourteen and twenty-one days. Five mL samples were analyzed morphologically on the seventh day by transmission electron microscopy. The data of the antibacterial test were analyzed by Fisher's exact test. Results: The results revealed that between the seventh and fourteenth day, there was a decrease in bacterial growth with both medicaments (P=0.098), where they were eliminated between the fourteenth and twenty-first day. Transmission electronic microscopy showed alterations in the morphologic structures. Conclusion: It concluded that both medicaments kill Enterococcus faecalis, with an exposure time of 7 to 14 days, where no cell viability is seen after this period due to irreversible alterations in bacterial cell morphology.

METHODS: The sample consisted of 12 male albino Wistar rats (Rattus norvegicus). The animals were randomly divided into four experimental groups. After undergoing anesthesia, the fur was removed from the lateral face of the right thigh and surgical dissection was performed to view the femur region. A bone defect was created using a spherical diamond-tipped drill bit. In groups 1 and 2, the defect was filled with a paste of Gen-Tech bone-inducing substance. The animals were treated with GaAlAs laser, at a predetermined dose of joules/cm² for 80 seconds, over an area of 1 cm². Groups 2 and 4 were used as controls. As porfirias são divididas em eritropoiéticas, hepáticas agudas e hepáticas crônicas. Os subtipos de maior relevância clínica são a porfiria cutânea tarda e a porfiria intermitente aguda. O diagnóstico das porfirias pode ser bastante difícil, dada a sobreposição de quadros clínicos e achados bioquímicos. A precisão do diagnóstico depende da dosagem de porfirinas urinárias e fecais, da análise da atividade enzimática de eritrócitos e, eventualmente, da pesquisa de mutações. O objetivo do presente artigo é realizar revisão literária das porfirias, com ênfase no diagnóstico e tratamento de seus diversos subtipos.

Geralmente, resulta de disfunção estrutural ou funcional do coração que compromete a sua capacidade de comportar o volume de enchimento diastólico de sangue ou de realizar sua ejeção. Caracteriza-se por disfunção ventricular e alterações da regulação neurohumoral, acompanhada de sintomas de cansaço aos esforços, retenção hídrica e redução da expectativa de vida. Em nosso meio, a sobrevida da insuficiência cardíaca com o tratamento clínico é estimada em 84%, 63% e 52%, em um, dois e cinco anos, respectivamente. Tem-se observado que a insuficiência cardíaca apresenta maior mortalidade do que muitas modalidades de câncer, como bexiga, mama e próstata,perdendo apenas ao câncer de pulmão. Assim, realizou-se estudo com o objetivo de avaliar sua epidemiologia em um hospital terciário de ensino. MÉTODOS: Trata-se de um estudo epidemiológico retrospectivo, realizado de 2000 a 2002, que analisou amostras de culturas clínicas positivas para enterococo resistente à vancomicina (VRE) em um hospital universitário com 660 leitos. Procurou-se definir sua incidência e os principais sítios e unidades de isolamento. Foi verificada a significância entre as variáveis nos três anos de estudo, sendo considerado como significante p<0,05. RESULTADOS: Houve aumento progressivo na resistência à vancomicina nas culturas clínicas positivas para Enterococcus spp. nos três anos de estudo.
