A comunicacao buco-sinusal (CBS) e uma condicao patologica caracterizada pelo acesso entre a cavi... more A comunicacao buco-sinusal (CBS) e uma condicao patologica caracterizada pelo acesso entre a cavidade oral e o seio maxilar devido ao defeito em tecido osseo e tecido mole, que normalmente garantem esta separacao anatomica. O tratamento deve considerar fatores como a localizacao, etiologia e extensao, alem disso, a CBS deve ser diagnosticada e tratada de forma imediata a fim de se obter melhor prognostico. O objetivo do presente trabalho e descrever a tecnica cirurgica para o fechamento de uma CBS de longa data utilizando a Bola de Bichat, associado a reconstrucao do assoalho do seio maxilar com tela de titânio e enxerto osseo particulado. O paciente apresentou CBS em lado direito de maxila decorrente de um procedimento de levantamento do assoalho do seio maxilar e simultânea instalacao de implantes. A inserção imediata de próteses parciais removíveis pode evitar a interrupção de um estilo de vida normal, além de proteger a área cirúrgica. Este artigo descreve um método prático de fornecer próteses parciais removíveis imediatas para pacientes que se submeterão a extrações dentárias.(AU)
This study compared the effectiveness of the three analgesics in the control of postoperative pai... more This study compared the effectiveness of the three analgesics in the control of postoperative pain in impacted third molar surgeries. Thirty patients were selected who needed removal of only two impacted third molars at the same side. Randomized, double-blind trials were applied to evaluate the efficacy of analgesics. Analgesics were divided in: A (tramadol/acetaminophen), B (sodic dipyrone) e C (acetaminophen) -1 tablet each 6 hours during 48 hours. The intensity of the pain was evaluated by the patients according with a visual analogical scale (score 0 – 4: no pain –insupportable). Statistical analysis at InStat software, the significance level of α=0.05. Kruskal-Wallis test showed that the three medicaments presented the same result during the first stage of measurements (P=0.135). Dental implant surgery is a common procedure in oral and maxillofacial surgery practices. Extensi... more Dental implant surgery is a common procedure in oral and maxillofacial surgery practices. Extensive training, skill, and experience allow this procedure to be performed with an atraumatic approach, but like any surgical technique, it is subject to accidents and complications. This is an unusual clinical case of an accidental displacement of an implant into the submandibular space that progressed to Ludwig’s angina, and it has not yet been described in the literature. This case report describes a clinical case of dental implant displaced into the submandibular space after healing cap removal. After seven days, it progressed to Ludwig’s angina. The removal was performed through extraoral access in the submandibular area by using hemostatic forceps and radioscopic technique. After implant removal, the clinical case showed a satisfactory repair emphasizing the importance of a meticulous clinical planning to achieve an appropriate treatment plan, which is essential for a favorable progno...

The objective of this study is to discuss and analyze whether extrusion of endodontic material co... more The objective of this study is to discuss and analyze whether extrusion of endodontic material constitutes avoidable intercurrence, discussing the clinical, ethical and legal implications. Patient LSR, 31 years old, female, attended a dental consultation complaining of pain in the second left maxillary premolar (tooth 25). Radiographically, a single root canal and thickening of the periodontal ligament associated with extravasation of 8 mm of gutta percha to the maxillary sinus were observed. The first endodontic session aimed to performing the desobturation, root canal preparation and intracanal medication placement. The root canal obturation was performed in the second session. Was carried out an apical surgery that removed 2 mm from the root apex and also performed the covered with a collagen membrane. A 22 months follow-up revealed a tooth function, absence of painful symptomatology or infection, and radiographically normal periodontal ligament and bone neoformation. The second single-root premolar is the type of premolar with less distance with the floor of the maxillary sinus. In this case, the extrusion of the obturator material occurred due to the superinstrumentation of the root canal associated with the proximity of the root with the membrane of the maxillary sinus. From the ethical and legal point of view, the patient has the right to be informed about any intercurrences that may arise during or after the treatment, and the informed consent form is essencial. This document will allow the patient to make a decision about performing an endodontic treatment in which the risk of an accident or complication is imminent or that treatment failure is already expected. It is important that professionals make appropriate diagnosis and treatment plan for each case, since this conduct may avoid clinical intercurrences. In addition, if the intercurrences occur, the patient should be advised of how to proceed.

OBJECTIVE: To evaluate epidemiological aspects of maxillofacial injuries in hospitalized patients... more OBJECTIVE: To evaluate epidemiological aspects of maxillofacial injuries in hospitalized patients.METHODS: The sample was composed of 405 patients treated at the Department of Oral and Maxillofacial Surgery of the Emergency Hospital of Aparecida de Goiânia, Brazil, between 2011 and 2013. The following informations were collected from the patients’ medical records: gender, age, cause of injury, seasonal distribution and type of injury. The statistical treatment analyzed data from frequency distribution and chi-squared test. The level of significance was set at 5% for all analyses.RESULTS: A higher occurrence of maxillofacial injuries was observed in males (72.59%) and with 21-30 years old (26.17%). The main etiologic factors involved were vehicle traffic accidents (30.62%), falls (22.72%) and violence (21.48%). The seasonal distribution showed that most cases occurred in autumn (38.02%), followed by summer (34.07%). The most common injuries were facial fractures (80%), with the nasal...

Keratocystic odontogenic tumors (KCOT) comprise a unique pathological entity characterized by agg... more Keratocystic odontogenic tumors (KCOT) comprise a unique pathological entity characterized by aggressive/destructive behavior and propensity to recurrence. This study describes the diagnostic and treatment features of a KCOT lesion. A 22-year old man was referred for surgical treatment of pericoronitis on tooth no. 37. Panoramic radiography revealed a unilocular, large radiolucent area extending from tooth no. 36 to the left mandibular ramus. Aspiration and incisional biopsy were performed, and the tissue sample was sent for microscopic evaluation. Microscopically, a cystic lesion was observed, lined by keratinized squamous epithelium and fi lled with keratin lamellae, confi rming the diagnosis of KCOT. Surgery was performed in an outpatient setting and involved osteotomy, detachment of the cystic lesion, and removal of teeth no. 36, 37, and 38. The patient was clinically and radiographically followed for 12 months, and no evidence of recurrence was observed. KCOTs should be considered in the differential diagnosis of lesions affecting the posterior region of the mandible. Accurate clinical, radiographic, and microscopic examinations are essential to establish the defi nitive diagnosis and choose the most effective therapy.

O presente trabalho descreve as particularidades de diagnostico e tratamento de um TOQ. Um paciente do sexo masculino, com 22 anos de idade, foi encaminhado para tratamento cirurgico de pericoronarite no dente 37. O exame radiografico panorâmico revelou uma area ampla, unilocular, estendendo-se do dente 36 ate o ramo esquerdo da mandibula. Puncao ossea aspirativa e biopsia incisional foram realizadas, e a amostra de tecido foi encaminhada para analise microscopica. Microscopicamente, observou-se lesao cistica, revestida por epitelio escamoso queratinizado e preenchida por lamelas de queratina, confirmando o diagnostico de TOQ. O procedimento cirurgico foi realizado em ambiente ambulatorial e envolveu osteotomia, descolamento da luz da lesao e exodontia dos dentes 36, 37 e 38. O paciente foi acompanhado clinica e radiograficamente por um periodo de 12 meses, e nao foi observada recorrencia da lesao. O TOQ deve ser considerado no diagnostico diferencial de alteracoes da regiao posterior da mandibula. Exames clinicos, radiograficos e microscopicos precisos sao essenciais no estabelecimento do diagnostico e na escolha da modalidade terapeutica mais eficaz.(AU)
Introduction: primary care must assume the centrality of care so that the health system meets and... more Introduction: primary care must assume the centrality of care so that the health system meets and resolves the demands and needs of the population. The quality and efficiency of this level of care must be constantly evaluated and monitored by managers. Instruments such as the Primary Care Assessment Tool (PCATool) have been used in Brazil for this purpose. It is a questionnaire that evaluates structural aspects of process and results in primary care. Objective: to evaluate primary health care from its attributes, from the perspective of professionals, checking factors of better health care between quality of services and professional qualification. Methodology: a cross-sectional study was conducted with 41 nurses and 31 physicians in the Southwest region of the state of Goiás, using interviews using the PCATool instrument. The data were analyzed in the Stata program, version 14.0. The Cronbach's alpha coefficient was used to analyze the internal consistency of the instrument's total score. Results: in the evaluation of the attributes of primary care, it was evidenced that the lower overall primary health care score was for the accessibility attribute (3.71). The bivariate analysis showed statistical difference between the variables profession and the essential, derived and general scores. Physicians of the Mais Médicos Program had higher mean scores (8.04 p = 0.001) when compared to other medical professionals and nurses. Professionals with post-graduation in any area, obtained a better score in the derived score (p = 0.021). Conclusion: a high general orientation score for primary care is observed by health professionals, with exception for the attribute accessibility. Medical professionals linked to the Mais Médicos Program obtained better average scores on all attributes. From the monitoring of the attributes of the primary attention it is possible to know the critical points that deserve intervention, as well as to evaluate if the strategic actions implemented have been effective in the scope of the primary attention.

Numerous "in vit... more Numerous "in vitro" investigations have been conducted to evaluate the role of screw size and pattern in determining optimal resistance to deformation, often these have been controversial. The aim of this study was to evaluate the effect of screw size and insertion technique on the stability of sagittal split osteotomies. This study used twenty polyurethane replicas of human hemimandibles with a prefabricated sagittal split ramus osteotomy (SSRO). The hemimandibles were stabilized with 1.5 mm and 2.0 mm titanium screws inserted in an inverted L configuration. All specimens were tested to determine the strength and stability of the fixation. In all cases there was failure of the synthetic bone before there was any evidence of screw failure. There were no significant differences in the load necessary to make the construct fail between the 1.5 or 2.0 mm screw sizes. There was no statistically significant difference between the strengths achieved with screws of 1.5 and 2.0 mm diameters for fixation of SSRO performed in synthetic mandibles. There was no fracture of the 1.5 mm or 2.0 mm diameter screws in any of the tests. 1.5 mm diameter screws in an inverted L pattern have as much stability and mechanical resistance as a 2.0 mm screw, may be safely used for this procedure.

O procedimento cirurgico foi realizado em ambiente ambulatorial e envolveu osteotomia, descolamento da luz da lesao e exodontia dos dentes 36, 37 e 38. O paciente foi acompanhado clinica e radiograficamente por um periodo de 12 meses, e nao foi observada recorrencia da lesao. O TOQ deve ser considerado no diagnostico diferencial de alteracoes da regiao posterior da mandibula. Exames clinicos, radiograficos e microscopicos precisos sao essenciais no estabelecimento do diagnostico e na escolha da modalidade terapeutica mais eficaz.(AU)
Introduction: primary care must assume the centrality of care so that the health system meets and... more Introduction: primary care must assume the centrality of care so that the health system meets and resolves the demands and needs of the population. The quality and efficiency of this level of care must be constantly evaluated and monitored by managers. Instruments such as the Primary Care Assessment Tool (PCATool) have been used in Brazil for this purpose. It is a questionnaire that evaluates structural aspects of process and results in primary care. Objective: to evaluate primary health care from its attributes, from the perspective of professionals, checking factors of better health care between quality of services and professional qualification. Methodology: a cross-sectional study was conducted with 41 nurses and 31 physicians in the Southwest region of the state of Goiás, using interviews using the PCATool instrument. The data were analyzed in the Stata program, version 14.0. The Cronbach's alpha coefficient was used to analyze the internal consistency of the instrument's total score. Results: in the evaluation of the attributes of primary care, it was evidenced that the lower overall primary health care score was for the accessibility attribute (3.71). The bivariate analysis showed statistical difference between the variables profession and the essential, derived and general scores. Physicians of the Mais Médicos Program had higher mean scores (8.04 p = 0.001) when compared to other medical professionals and nurses. Professionals with post-graduation in any area, obtained a better score in the derived score (p = 0.021). Conclusion: a high general orientation score for primary care is observed by health professionals, with exception for the attribute accessibility. Medical professionals linked to the Mais Médicos Program obtained better average scores on all attributes. From the monitoring of the attributes of the primary attention it is possible to know the critical points that deserve intervention, as well as to evaluate if the strategic actions implemented have been effective in the scope of the primary attention.

Numerous "in vit... more Numerous "in vitro" investigations have been conducted to evaluate the role of screw size and pattern in determining optimal resistance to deformation, often these have been controversial. The aim of this study was to evaluate the effect of screw size and insertion technique on the stability of sagittal split osteotomies. This study used twenty polyurethane replicas of human hemimandibles with a prefabricated sagittal split ramus osteotomy (SSRO). The hemimandibles were stabilized with 1.5 mm and 2.0 mm titanium screws inserted in an inverted L configuration. All specimens were tested to determine the strength and stability of the fixation. In all cases there was failure of the synthetic bone before there was any evidence of screw failure. There were no significant differences in the load necessary to make the construct fail between the 1.5 or 2.0 mm screw sizes. There was no statistically significant difference between the strengths achieved with screws of 1.5 and 2.0 mm diameters for fixation of SSRO performed in synthetic mandibles. There was no fracture of the 1.5 mm or 2.0 mm diameter screws in any of the tests. 1.5 mm diameter screws in an inverted L pattern have as much stability and mechanical resistance as a 2.0 mm screw, may be safely used for this procedure.
