For chronic illness, the patients requested regular hospital visits and required long term use of steroids or immunosuppressive agents. Besides agents needing therapy, the patients also face the probability of recurrent disease, multiple complications, hospitalization, side effect of treatment and fall into chronic renal disease. The impact could be physical or psychological (emotional, social, school performance). Thus, in this article, we provide the quality of life (QoL) evaluation among children with NS in Prof Dr. I.G.N.G

Introduction: Chemotherapy is an important curative therapy modality in ALL patients. 8-Hydroxy-2... more Introduction: Chemotherapy is an important curative therapy modality in ALL patients. 8-Hydroxy-2'-Deoxyguanosine (8-OHdG) has been studied as a biomarker of oxidative stress as a trigger for DNA damage, which is statistically significantly increased in ALL patients after chemotherapy compared to healthy children. This study aims to prove that 8-OHdG levels in children with ALL post-chemotherapy induction phases were higher than in the post-chemotherapy maintenance phase. Methods: This study used a cross-sectional design. The subjects were children aged 1-18 years, with ALL who received treatment at Prof Dr. I.G.N.G Ngoerah Hospital from September 2019-January 2021. The Mann-Whitney test assessed the 8-OHdG levels in children with ALL post-chemotherapy induction and post-chemotherapy maintenance phase. Logistic regression multivariate analysis was used to evaluate the factors that influence 8-OHdG levels in pediatric patients with ALL. The value of p<0,05 determined the level of significance. Results: A total of 27 subjects who underwent the post-chemotherapy induction phase and 19 subjects who underwent the post-chemotherapy maintenance phase were analyzed in this study. In the post-chemotherapy induction group, the median 8-OHdG level was 0.163 (0.104-1.864), and the median 8-OHdG level in the post-chemotherapy maintenance phase group was 0.158 (0.094-0.512). There was no significant difference in 8-OHdG levels between the two groups. Logistic regression multivariate analysis found that the post-chemotherapy induction phase was a prognostic factor at risk of increasing 8-OHdG levels (RO 16.55; 95% CI 1.118-245.277). Conclusion: This study concluded that 8-OHdG levels were higher in children post-chemotherapy induction phase than in children post-chemotherapy maintenance phase.

Si... more Urinary tract infection (UTI) is the most common serious bacterial infection in young infants. Signs and symptoms are often nonspecific. To describe clinical, demographic and laboratory features of UTI in infants ≤ 3 months old. Cross-sectional study of infants ≤ 3 months old with UTI diagnosed in a pediatric emergency department, for the period 2010-2012. UTI was defined as ≥ 50,000 colony-forming units per milliliter of a single uropathogen isolated from bladder catheterization. Paired urinalysis and urine culture from group culture-positive and group culture-negative were used to determine the sensitivity and specificity of pyuria and nitrite tests in detecting UTI. Of 519 urine cultures collected, UTI was diagnosed in 65 cases (prevalence: 12.5%); with male predominance (77%). The most common etiologies were Escherichia coli (56.9%), Klebsiella pneumoniae (18.5%) and Enterococcus faecalis (7.7%). Henoch Schonlein Purpura (HSP) is a systemic disease with frequent renal involvement, its etiology is still unknown but several infections have been described as trigger includingGAS infection. A 4 year 10 month old Balinese boy presented with full blown acute nephritic syndrome, an elevation in serum creatinine and four fold increase of anti streptolysin-O, also low serum levels of complement C3 with normal C4 confirmed the diagnosis of APSGN. During hospitalization he developed palpable purpura, gastrointestinal symptoms as well as leucytoclastic vasculitis in skin biopsy conclude HSP diagnosis.He was treated with anti-hypertensions and metylprednisolone intravenous. The prognosis of the patient was excellent, he showed normal physical examination with normal complete blood count and urinalysis after 3 months follow up. G... more Background: Total prevalence of multiple congenital anomalies (MCA) was 15.8 per 10,000 births. Genitourinary anomalies were associated with MCA cases about 43.8% of the cases. The horseshoe kidney is one example of genitourinary anomalies related to the high incidence of MCA and up to one third also had hydronephrosis thought to be secondary to ureteric atresia. Case: A 10-month-old boy presented with a 5-month history of abdominal distention. On physical examination, there was a large non-tender cystic mass in the right upper abdomen extending down to the right lower abdomen. The other congenital anomalies on this patient were hydrocephalus, hypospadias, and bilateral undescended testis. Clinical symptom of UTI varies depending on age, location and intensity of inflammatory reaction in the urinary tract infection. Long-term effects may hold on renal function if not treated wisely. This study aims to determine the clinical profile and bacterial pattern of UTI among infants and children at Sanglah General Hospital Bali-Indonesia. Methods: A cross-sectional study was conducted at Department of Child Health in Sanglah General Hospital Bali-Indonesia from January 2010 to June 2012. Results: A total of 47 cases of UTI were enrolled in this study, (45%) of the subjects were boys. Median age was 1 year-old (IQR 1-4). Latar belakang.Acute kidney injury(AKI) merupakan merupakan suatu keadaan yang dapat meningkatkan... more Latar belakang.Acute kidney injury(AKI) merupakan merupakan suatu keadaan yang dapat meningkatkan angka morbiditas dan mortalitas pada anak yang dirawat di rumah sakit terutama dengan perawatan intensif.Tujuan.Mengetahui kejadian AKI pada anak yang mendapat perawatan di Unit Perawatan Intensif Anak.Metode.Penelitian deskriptif retrospektif dengan mengambil data dari rekam medis dan register perawatan ruang intensif selama tahun 2010Hasil.Dari 411 pasien yang dirawat di Unit Perawatan Intensif Anak, 149 (36,25%) dilakukan pemeriksaan blood urea nitrogen(BUN) dan serum kreatinin (SC). Terdapat 25 kasus (6,1%) merupakan kasus AKI. Sebagian besar subyek berusia di bawah 1 tahun (10/25) dan berjenis kelamin laki-laki 15/25. Berdasarkan kriteria pRIFLE, 3 termasuk dalam kriteria risk (R),12 injury (I),dan 10 dimasukkan failure (F).8 orang meninggal.Kesimpulan.Acute kidney injuryterbanyak dijumpai pada laki-laki dengan kasus terbanyak termasuk kriteria injury(I).

Elevated blood pressure in children is a worldwide problem, constitute as major, often overlooked... more Elevated blood pressure in children is a worldwide problem, constitute as major, often overlooked, public health problem in many countries. Normal blood pressure value in children varies based on age, sex and body height which available in standardized table. Measurement of blood pressure is crucial and needed to be done at least once a year in every child with risk factors. Risk factors for elevated blood pressure includes family history of hypertension, salt intake, physical activity, stress level and obesity. Therefore we aimed to determine risk factors for elevated blood pressure among children in Tulikup village Bali. This observational analytic study with cross-sectional design, was conducted in children age 1-18 years old whom visit child health community service program in Tulikup. We collected data from 81 children, age range from 1 to 13 years old, with median age was 4 years old. Data was collected by using questionnaire and physical examination. This study was dominated by female and elevated blood pressure (11.1%). On the other hand, nutritional status were both equal between wellnourished status and mild nutrition (44.4%). Multivariate analysis showed two factors associated with elevated blood pressure such as family history of hypertension with p=0.045 (OR 8.89; 95% CI 1.05-75.29) and excessive salt intake with p=0.046 (OR 5.24; 95% CI 1.02-26.77). Therefore risk factors such as family history of hypertension and excessive salt intake, significantly cause elevated blood pressure in children.

Abstract Background Hyperbilirubinemia is one of the most common problems in newborns. Severe hyperbilirubinemia, known as kernicterus, can suppress O2 consumption and oxidative phosphorylation, as well as damage brain cells, resulting in neuronal dysfunction and encephalopathy. Phototherapy is a common therapy for neonatal hyperbilirubinemia, but may rarely lead to the adverse effect of hypocalcemia. Objective To investigate serum calcium levels in full term neonates with hyperbilirubinemia, before and after phototherapy. It is the most common complication of late-onset neonatal sepsis. An understanding of the risk factors for bacterial meningitis in late-onset neonatal sepsis is required to provide comprehensive management. AIM: To identify the risk factors of bacterial meningitis in late-onset neonatal sepsis. METHODS: This is an analytical study with a case–control design, conducted in May 2019-February 2021, involving neonates aged 4–28 days who suffered from late-onset neonatal sepsis in Level II and III Neonatal Care Unit, Sanglah General Hospital Denpasar.
