In this paper, we propose a novel method for video anomaly detection motivated by an existing arc... more In this paper, we propose a novel method for video anomaly detection motivated by an existing architecture for sequence-to-sequence prediction and reconstruction using a spatio-temporal convolutional Long Short-Term Memory (convLSTM). As in previous work on anomaly detection, anomalies arise as spatially localised failures in reconstruction or prediction. In experiments with five benchmark datasets, we show that using prediction gives superior performance to using reconstruction. We also compare performance with different length input/output sequences. Overall, our results using prediction are comparable with the state of the art on the benchmark datasets.

G protein coupled receptor kinase type 4 (GRK4) phosphorylates the D 1 R and reduces its membrane expression, but little is known about regulation of GRK4. We hypothesized that GRK4 is targeted for elimination through ubiquitylation. Using D 1 R and GRK4γ stably transfected human embryonic kidney cells (HEK293), we demonstrated GRK4 and ubiquitin coimmunoprecipitation. Addition of clasto-lactacystin beta-lactone (CLBL, an inhibitor of the 26S proteasome) increased GRK4 expression levels (western blot) using two different GRK4 fusion protein constructs. Consequently, the vast diversity of Amoebozoa genomes remain unexplored. A draft genome of Cochliopodium minus, an amoeba characterized by extensive cellular and nuclear fusions, is presented. C. minus has been a subject of recent investigation for its unusual sexual behavior. Cochliopodium’s sexual activity occurs during vegetative stage making it an ideal model for studying sexual development, which is sorely lacking in the group. Here we generate a C. minus draft genome assembly. From this genome, we detect a substantial number of lateral gene transfer (LGT) instances from bacteria (15%), archaea (0.9%) and viruses (0.7%) the majority of which are detected in our transcriptome data. DESIGN A cross-sectional survey based on a two-stage cluster sampling design. SETTING Secondary high schools in Ho Chi Minh City. SUBJECTS There were 1504 adolescents involved in the study, of which 50% were girls and the mean age of participants was 13.1 years. RESULTS Overall, 4.9% of the students were overweight and 0.6% were obese, while 13.1% of the students were underweight. The prevalence of underweight was significantly higher in boys than girls (p=0.001) and overweight and obesity were also higher in boys although these differences were not statistically significant (p=0.074). However, an optimal surgical approach to kidney and pancreas transplantation can minimise this risk. We performed a systematic review and meta-analysis to examine factors contributing to incisional hernia formation in kidney and pancreas transplant recipients. Bias appraisal of studies was conducted via the Newcastle-Ottawa scale. We considered recipient factors, surgical methods, and complications of repair. The rate of incisional hernia formation in recipients of kidney and pancreas transplants was 4.4% (CI 95% 2.6-7.3, p < 0.001). Age above or below 50 years did not predict hernia formation ( (1) = 0.09, = 0.77). Body mass index (BMI) above 25 (10.8%, CI 95% 3.2-30.9, p < 0.001) increased the risk of an incisional hernia. Variants of the electrogenic sodium bicarbonate cotransporter NBCe2 gene, SLC4A5, are associated with increased blood pressure in several ethnic groups. SLC4A5 variants are also highly associated with salt sensitivity, independent of hypertension. In developing countries models to improve hand hygiene compliance to reduce healthcare acquired infections are required. The aim of this study was to determine hand hygiene compliance following an educational program in an obstetric and gynecological hospital in Vietnam. Health care workers from neonatal intensive care, delivery suite and a surgical ward from Hung Vuong Hospital, Ho Chi Minh City, Vietnam undertook a 4-h educational program targeting hand hygiene. Compliance was monitored monthly for six months following the intervention. Hand hygiene knowledge was assessed at baseline and after six months of the study. There were 7124 opportunities over 370 hand hygiene recording sessions with 1531 opportunities at baseline and 1620 at 6 months following the intervention. However, the most commonly used method of treatment of diastasis of the recti involves plication with non-absorbable sutures as part of an abdominoplasty procedure. This case report describes single-port laparoscopic repair of diastasis of recti and umbilical hernia with prosthetic mesh after plication with slowly absorbable sutures combined with abdominoplasty. Technique Description: Our patient is a 36-year-old woman with severe diastasis of the recti, umbilical hernia and an excessive amount of redundant skin after two previous pregnancies and caesarean sections. After raising the upper abdominal flap, a single-port was placed in the left upper quadrant and the ligamenturn teres was divided. The diastasis of the recti and umbilical hernia were plicated under direct vision with continuous and interrupted slowly absorbable sutures before an antiadhesive mesh was placed behind the repair with 6 cm overlap, transfixed in 4 quadrants and tacked in place with non-absorbable tacks in a double-crown technique. The left upper quadrant wound was closed with slowly absorbable sutures. The excess skin was removed and fibrin sealant was sprayed in the subcutaneous space to minimize the risk of serorna formation without using drains. Discussion: Combining single-port laparoscopic repair of diastasis of recti and umbilical hemia repair minimizes inadvertent suturing of abdominal contents during plication, the risks of port site hernias associated with conventional multipart repair and permanently reinforced the midline weakness while achieving "scarless" surgery.

Therefore, a growing number of patients are presenting with recurrent hernias after conventional anterior and laparoscopic repairs have failed. This study reports our experience with single-incision laparoscopic (SIL) intraperitoneal onlay mesh (IPOM) repair of these hernias. Patients referred with two or more recurrences of inguinal hernia underwent SIL-IPOM from November 1, 2009, to June 24, 2014. A 2.5-cm infraumbilical incision was made, and an SIL port was placed intraperitoneally. Background Multiple prospective studies have confirmed safety and efficacy of laparoscopic inguin... more Background Multiple prospective studies have confirmed safety and efficacy of laparoscopic inguinal herniorraphy with single-port compared to multiport surgery. This prospective randomized controlled trial aimed to assess safety, efficacy and potential benefits of singleport total extraperitoneal inguinal herniorraphy beyond the learning curve. Methods All referred patients with inguinal/femoral hernias were enrolled from December 2011 to February 2013. Exclusion criteria included workers compensation cases. Identical balloon dissector, lightweight mesh and non-absorbable tacks were used in all cases. For single-port cases Triport TM was used while structural balloon trocar/inflation bulb for multiport cases. Results were analyzed with IBM Ò SPSS Ò version 22 for Windows. Results Participation rate was 100 % with 157 inguinal/femoral hernias in 100 patients: 51 randomized to single-port and 49 to multiport group. There was no conversion to open surgery/need for additional ports. There were no statistical differences between single-port and multiport groups with respect to age, sex, body mass index, American Society of Anesthesiologists scores, preoperative pain, hernia defect sizes and length of hospital stay. Operation times were equivalent for single-port and multiport 60.0 vs 61.0 min, P = 0.23, respectively. Significantly, single-port patients ingested fewer pain killers: 6 tablets vs 14 Dextropropoxyphene tablets, P \ 0.001, experienced less pain (visual analog scores) on day 1 and 7 post-op op: 2.5 and 0, P \ 0.001 compared to 4.5 and 2.5, P \ 0.001, respectively, returned to work/normal physical activities 7 days quicker: 7.0 vs 14.0, P \ 0.001 and had higher cosmetic scar scores at 6-week follow-up: 24 vs 21, P \ 0.001, compared to multiport patients. There were no mortalities, morbidities or recurrences after followup of 6-21 months. Conclusions Compared to multiport, single-port laparoscopic total extraperitoneal inguinal herniorraphy, when performed by a high-volume and highly dedicated hernia surgeon, resulted in significantly reduced postoperative pain, analgesic requirements, quicker return to work/normal activities, improved cosmesis, and equivalent safety and efficacy.

This pilot study assessed the occurrence of adhesions to full-sized Polypropylene and Gore-tex Du-alMesh Plus meshes and the possibility for adhesion prevention using fibrin sealant. Methods: Two 10-cm to 15-cm pieces of mesh were placed and fixed laparoscopically in pigs (25kg to 55kg). Group I: 2 animals with Polypropylene mesh on one side and DualMesh on other side. Group II: 2 animals with DualMesh on each side with fibrin sealant applied to the periphery of mesh and staples to one side. Group III: 1 animal with 2 pieces of Polypropylene mesh with fibrin sealant applied to the entire mesh. All animals underwent laparoscopy 3 months later to assess the extent of adhesions, and full-thickness specimens were removed for histological evaluation. Results: More Polypropylene mesh was involved in adhesions than DualMesh. However, with the DualMesh involved in adhesions, more of the surface area was involved in forming adhesions than with Polypropylene mesh. None of the implanted DualMesh had visceral adhesions, while 2 out of 3 Polypropylene meshes had adhesions to both the liver and spleen but none to the bowel. Implanted Polypropylene mesh with fibrin sealant had no adhesions. DualMesh had shrunk more significantly than Polypropylene mesh. Histological evaluation showed absence of acute inflammatory response, significantly more chronic inflammatory response to DualMesh compared to Polypropylene and complete mesothelializa-tion with both meshes. There was extensive collagen deposition between Polypropylene mesh fibers, while fibrosis occurred on both sides of DualMesh with synovial metaplasia over its peritoneal surface akin to encapsulation. Conclusions: DualMesh caused fewer omental and visceral adhesions than Polypropylene mesh did. Fibrin sealant eliminated adhesions to DualMesh and prevented adhesions to Polypropylene mesh when applied over the entire surface. These results support our current use of DualMesh and fibrin sealant in LVHR.

Little is known regarding how the kidney shifts from a sodium and water reclaiming state (antinat... more Little is known regarding how the kidney shifts from a sodium and water reclaiming state (antinatriuresis) to a state where sodium and water are eliminated (natriuresis). In human renal proximal tubule cells, sodium reabsorption is decreased by the dopamine D 1 -like receptors (D 1 R/D 5 R) and the angiotensin type 2 receptor (AT 2 R), whereas the angiotensin type 1 receptor increases sodium reabsorption. Aberrant control of these opposing systems is thought to lead to sodium retention and, subsequently, hypertension. We show that D 1 R/D 5 R stimulation increased plasma membrane AT 2 R 4-fold via a D 1 R-mediated, cAMP-coupled, and protein phosphatase 2A–dependent specific signaling pathway. D 1 R/D 5 R stimulation also reduced the ability of angiotensin II to stimulate phospho-extracellular signal–regulated kinase, an effect that was partially reversed by an AT 2 R antagonist. DESIGN A cross-sectional survey based on a two-stage cluster sampling design. SETTING Secondary high schools in Ho Chi Minh City. SUBJECTS There were 1504 adolescents involved in the study, of which 50% were girls and the mean age of participants was 13.1 years. RESULTS Overall, 4.9% of the students were overweight and 0.6% were obese, while 13.1% of the students were underweight. The prevalence of underweight was significantly higher in boys than girls (p=0.001) and overweight and obesity were also higher in boys although these differences were not statistically significant (p=0.074). We performed a systematic review and meta-analysis to examine factors contributing to incisional hernia formation in kidney and pancreas transplant recipients. Bias appraisal of studies was conducted via the Newcastle-Ottawa scale. We considered recipient factors, surgical methods, and complications of repair. The rate of incisional hernia formation in recipients of kidney and pancreas transplants was 4.4% (CI 95% 2.6-7.3, p < 0.001). Age above or below 50 years did not predict hernia formation ( (1) = 0.09, = 0.77). Body mass index (BMI) above 25 (10.8%, CI 95% 3.2-30.9, p < 0.001) increased the risk of an incisional hernia. In developing countries models to improve hand hygiene compliance to reduce healthcare acquired infections are required. The aim of this study was to determine hand hygiene compliance following an educational program in an obstetric and gynecological hospital in Vietnam. Health care workers from neonatal intensive care, delivery suite and a surgical ward from Hung Vuong Hospital, Ho Chi Minh City, Vietnam undertook a 4-h educational program targeting hand hygiene. Compliance was monitored monthly for six months following the intervention. Hand hygiene knowledge was assessed at baseline and after six months of the study. There were 7124 opportunities over 370 hand hygiene recording sessions with 1531 opportunities at baseline and 1620 at 6 months following the intervention. However, the most commonly used method of treatment of diastasis of the recti involves plication with non-absorbable sutures as part of an abdominoplasty procedure. This case report describes single-port laparoscopic repair of diastasis of recti and umbilical hernia with prosthetic mesh after plication with slowly absorbable sutures combined with abdominoplasty. Technique Description: Our patient is a 36-year-old woman with severe diastasis of the recti, umbilical hernia and an excessive amount of redundant skin after two previous pregnancies and caesarean sections. After raising the upper abdominal flap, a single-port was placed in the left upper quadrant and the ligamenturn teres was divided. The diastasis of the recti and umbilical hernia were plicated under direct vision with continuous and interrupted slowly absorbable sutures before an antiadhesive mesh was placed behind the repair with 6 cm overlap, transfixed in 4 quadrants and tacked in place with non-absorbable tacks in a double-crown technique. The left upper quadrant wound was closed with slowly absorbable sutures. The excess skin was removed and fibrin sealant was sprayed in the subcutaneous space to minimize the risk of serorna formation without using drains. Discussion: Combining single-port laparoscopic repair of diastasis of recti and umbilical hemia repair minimizes inadvertent suturing of abdominal contents during plication, the risks of port site hernias associated with conventional multipart repair and permanently reinforced the midline weakness while achieving "scarless" surgery.

This prospective randomized controlled trial aimed to assess safety, efficacy and potential benefits of singleport total extraperitoneal inguinal herniorraphy beyond the learning curve. Methods All referred patients with inguinal/femoral hernias were enrolled from December 2011 to February 2013. Exclusion criteria included workers compensation cases. Identical balloon dissector, lightweight mesh and non-absorbable tacks were used in all cases. For single-port cases Triport TM was used while structural balloon trocar/inflation bulb for multiport cases. Results were analyzed with IBM Ò SPSS Ò version 22 for Windows. Results Participation rate was 100 % with 157 inguinal/femoral hernias in 100 patients: 51 randomized to single-port and 49 to multiport group. There was no conversion to open surgery/need for additional ports. There were no statistical differences between single-port and multiport groups with respect to age, sex, body mass index, American Society of Anesthesiologists scores, preoperative pain, hernia defect sizes and length of hospital stay. Operation times were equivalent for single-port and multiport 60.0 vs 61.0 min, P = 0.23, respectively. Significantly, single-port patients ingested fewer pain killers: 6 tablets vs 14 Dextropropoxyphene tablets, P \ 0.001, experienced less pain (visual analog scores) on day 1 and 7 post-op op: 2.5 and 0, P \ 0.001 compared to 4.5 and 2.5, P \ 0.001, respectively, returned to work/normal physical activities 7 days quicker: 7.0 vs 14.0, P \ 0.001 and had higher cosmetic scar scores at 6-week follow-up: 24 vs 21, P \ 0.001, compared to multiport patients. There were no mortalities, morbidities or recurrences after followup of 6-21 months. Conclusions Compared to multiport, single-port laparoscopic total extraperitoneal inguinal herniorraphy, when performed by a high-volume and highly dedicated hernia surgeon, resulted in significantly reduced postoperative pain, analgesic requirements, quicker return to work/normal activities, improved cosmesis, and equivalent safety and efficacy.

This pilot study assessed the occurrence of adhesions to full-sized Polypropylene and Gore-tex Du-alMesh Plus meshes and the possibility for adhesion prevention using fibrin sealant. Methods: Two 10-cm to 15-cm pieces of mesh were placed and fixed laparoscopically in pigs (25kg to 55kg). Group I: 2 animals with Polypropylene mesh on one side and DualMesh on other side. Group II: 2 animals with DualMesh on each side with fibrin sealant applied to the periphery of mesh and staples to one side. Group III: 1 animal with 2 pieces of Polypropylene mesh with fibrin sealant applied to the entire mesh. All animals underwent laparoscopy 3 months later to assess the extent of adhesions, and full-thickness specimens were removed for histological evaluation. Results: More Polypropylene mesh was involved in adhesions than DualMesh. However, with the DualMesh involved in adhesions, more of the surface area was involved in forming adhesions than with Polypropylene mesh. None of the implanted DualMesh had visceral adhesions, while 2 out of 3 Polypropylene meshes had adhesions to both the liver and spleen but none to the bowel. Implanted Polypropylene mesh with fibrin sealant had no adhesions. DualMesh had shrunk more significantly than Polypropylene mesh. Histological evaluation showed absence of acute inflammatory response, significantly more chronic inflammatory response to DualMesh compared to Polypropylene and complete mesothelializa-tion with both meshes. There was extensive collagen deposition between Polypropylene mesh fibers, while fibrosis occurred on both sides of DualMesh with synovial metaplasia over its peritoneal surface akin to encapsulation. Conclusions: DualMesh caused fewer omental and visceral adhesions than Polypropylene mesh did. Fibrin sealant eliminated adhesions to DualMesh and prevented adhesions to Polypropylene mesh when applied over the entire surface. These results support our current use of DualMesh and fibrin sealant in LVHR.

In human renal proximal tubule cells, sodium reabsorption is decreased by the dopamine D 1 -like receptors (D 1 R/D 5 R) and the angiotensin type 2 receptor (AT 2 R), whereas the angiotensin type 1 receptor increases sodium reabsorption. Aberrant control of these opposing systems is thought to lead to sodium retention and, subsequently, hypertension. We show that D 1 R/D 5 R stimulation increased plasma membrane AT 2 R 4-fold via a D 1 R-mediated, cAMP-coupled, and protein phosphatase 2A–dependent specific signaling pathway. D 1 R/D 5 R stimulation also reduced the ability of angiotensin II to stimulate phospho-extracellular signal–regulated kinase, an effect that was partially reversed by an AT 2 R antagonist.
