A laparoscopic fixation of the rectum was carried out in seven women with complete rectal prolaps... more A laparoscopic fixation of the rectum was carried out in seven women with complete rectal prolapse. For fixation a Marlex Mesh was used, which was stapled to the pre-sacral fascia in all patients. The mesh was sutured to the rectum in three patients and stapled in four. There were no complications and in a follow-up period of 1 to 14 months no recurrence of the prolapse occurred.

Vesalius through his life and his works as a surgeon. He was the father of the anatomical revolution against Galen but was also an eminent clinician and surgeon. He was immediately able to adapt his surgical practice whenever the promising methodology was identified (see Consilia). The author concludes with a critical analysis of the Chirurgia magna in septem libros digesta attributed to A. Vesalius.

Experiments were performed to examine the possible interaction between the cholinergic transmitte... more Experiments were performed to examine the possible interaction between the cholinergic transmitter and the reactivity of gastric arteries to adrenergic activation. Helical strips of dogs' gastric arteries were mounted for isometric tension recording. The strips contracted on exposure to norepinephrine; these contractions were inhibited by phenoxybenzamine. Electrical stimulation caused an increase in tension, which was abolished by tetrodotoxin and phenoxybenzamine, indicating that electrical stimulation causes contraction by liberation of norepinephrine. Acetylcholine did not alter basal tension; it caused relaxation during responses to electrical simulation, but not during those to norepinephrine. The relaxation caused by acetylcholine was abolished by atropine. This indicates that acetylcholine inhibits adrenergic neurotransmission in the dog's gastric artery. Already in ancient times did medical liability occupy mankind. Various civilizations did give the... more Already in ancient times did medical liability occupy mankind. Various civilizations did give their own interpretation on the subject and proposed solutions. Original writings are rare and articles concerning ancient medical liability equally are hard to find. The only relatively trustworthy sources are of legal nature and find their origin in Greek philosophy and Roman Law. At a later stage, Arabic philosophers gave a renewed view on the statements of these previous civilizations and added their own way of thinking. All these influences still reflect in our modern western way of medical acting. Some of these ancient customs concerning medical liability will be discussed in this article.

Three types of historical events are of interest, when dealing with the topic of abdominal compar... more Three types of historical events are of interest, when dealing with the topic of abdominal compartment syndrome. The first concerns the evolution in the understanding of the pathophysiologic features of a compartment syndrome in general. The symptoms were well described by Richard von Volkmann in the 19th century. The second consists of the experimental studies devoted to measurements of intra-abdominal pressure. Different devices and techniques were used, in particular using detection locations approachable via natural body orifices (bladder/uterus/rectum etc). A third point of interest has come forward after the empirical assessment, in particular by sir Heneage Ogilvie that open laparostomies were beneficial in the healing of great abdominal war wounds, or later, of congenital abdominal wall defects. Non-traumatic intramural hematoma of the duodenum is an unusual clinical entity. Indeed, in a maj... more Non-traumatic intramural hematoma of the duodenum is an unusual clinical entity. Indeed, in a majority of 70% of patients intramural hematoma of the duodenum is caused by a blunt, frequently minor abdominal trauma. The main etiology of non-traumatic intramural hematoma of the duodenum in the adult is overdose anticoagulant therapy. Rarer causes include pancreatic disease, blood dyscrasia or vascular collagen disease. In this presentation a case of pancreatitis-induced intramural duodenal hematoma is discussed and compared with corresponding data in the literature.

In an attempt to make the interventions as little invasive as possible, the ergonomics of the procedure, for the surgeon, are often not dealt with at all. In an attempt to improve the surgeon-patient interface during endoscopic procedures, a new concept is presented to allow the surgeon a much more natural visual interaction with the inside of the patient. It is made possible for the surgeon to look around into the cavity, with a stereo endoscope, mounted on a robot arm. This robot arm is directed by the motion of the head of the surgeon.

A series of 34 patients with pancreatic resections was evaluated with respect to the occurrence o... more A series of 34 patients with pancreatic resections was evaluated with respect to the occurrence of local and general complications. Two groups were compared, depending on whether or not treatment with somatostatin was instituted. Postoperative morbidity and mortality were less frequent when somatostatin was given (resp. 50.0 and 5.5%) than when it was omitted (resp. 68.7 and 31.2%). Less complications were observed after resections performed for chronic pancreatitis than for pancreatic cancer in the non somatostatin-treated group. It is concluded that somatostatin treatment may be beneficial in preventing complications after elective pancreatic surgery.

A series of 18 patients with unexpected well-differentiated cancers of the thyroid is presented. ... more A series of 18 patients with unexpected well-differentiated cancers of the thyroid is presented. This proved to be a high percentage of all patients operated for "benign" thyroid disease (6.5%), and a substantial percentage of all patients operated for thyroid cancer (44%). Preoperative investigations consisting mainly of thyroid function tests, ultrasound, scintigraphy, and fine needle aspiration biopsy, were not contributive for cancer. Intraoperative suspicion of malignancy induced extended surgery in one patient and frozen section biopsies in another 5 patients, with 3 positive answers resulting in immediate more radical operations. In 12 patients unsuspected cancer was diagnosed at paraffin section. This resulted in reoperation in 8 patients. Considering a number of arguments, we advocate a therapeutical guideline which makes a distinction between tumours which should be reoperated and tumours which are adequately treated by the initial resection.

Several complications, resulting from drainage, are discussed. These include severe tissue reacti... more Several complications, resulting from drainage, are discussed. These include severe tissue reactions, leakage from bowel anastomoses, obstruction and perforation of small or large bowel, herniation, leaving behind a foreign body, severe bleeding and the induction of infection. Several of these complications are illustrated with case histories. Moreover a review of the literature on the subject is given. In view of these complications, the author warns against too liberal or too long drainage procedures.

Conservative management of a traumatic biliopleural fistula. A female patient, 13 years of age, d... more Conservative management of a traumatic biliopleural fistula. A female patient, 13 years of age, developed a right-sided haemothorax after a stab wound in the right para-vertebral region. Pleural effusion reoccurred after thoracic drainage was stopped. Ten days after the injury, thoracic puncture enabled a diagnosis of biliopleural fistula to be made, confirmed by ERCP and CT-scan. The fistula closed after repeat thoracic drainage combined with prophylactic antibiotic therapy.
