It was also discovered that the article was not submitted by Sher Wali, but by Rahil Barkat while using the account of Sher Wali. Mr. Barkat was involved in data theft and misuse in two recently published Cureus articles, which have since been retracted. As the origin of this article's data and verified IRB approval cannot be confirmed, we have made the decision to retract this article. Cureus has confirmed that the co-authors were asked by Mr. Barkat to proofread the article and provide payment in exchange for authorship. Antiphospholipid syndrome (APS) is a rare autoimmune disorder characterized by thromboembolic eve... more Antiphospholipid syndrome (APS) is a rare autoimmune disorder characterized by thromboembolic events, fetal loss during pregnancy, and evidence of antiphospholipid (aPL) antibodies such as beta-2-glycoprotein I (B2-GPI) and anticardiolipin (aCL). The diagnosis and treatment of this condition in the pediatric population have limited literature evidence due to the rarity of the condition in this age group. Guidelines have been adopted from the adult counterpart of the affected population, thereby giving rise to diagnostic and therapeutic challenges. In this report, we describe a rare case of a 15-year-old male who presented with lepromatous leprosy and developed deep vein thrombosis in his right leg. The laboratory evidence of positive aPL antibodies guided our diagnosis of APS and treatment with oral anticoagulants. This report highlights the importance of screening and timely diagnosis of APS in the pediatric population presenting with venous thrombosis in the backdrop of infection.

Sensor nodes residing near a sink receives more traffic than other nodes and depletes their batte... more Sensor nodes residing near a sink receives more traffic than other nodes and depletes their battery quickly which leads to an energy hole problem. The objective is to collect data from all over the sensor field, aggregate and forward the same to a mobile sink to extend the network lifetime. A sensor network is logically divided into subgrids and each is identified uniquely and the sink is aware of the number of sensors under each subgrid. During data collection, a mobile sink move towards a potential subgrid so that the energy of all the sensor nodes is equally utilised. It is implemented through reconstructing the network and new routing paths are generated. The sink moves to a new subgrid where sensor nodes have adequate residual energy and starts collecting data, which prolongs the network lifetime. Parameters like average lifetime of nodes, packet delivery ratio and throughput of the network are considered for performance analysis. Residual energy obtained from the simulation shows that the lifetime of the network is improved.

A 46-year-old gentleman presented to the emergency department with a 3-day history of severe abdo... more A 46-year-old gentleman presented to the emergency department with a 3-day history of severe abdominal pain, progressive abdominal distention, vomiting, and fever. He was a chronic smoker, alcoholic for 20 years, and nonsteroidal anti-inflammatory drug (NSAID) abuser for the past 2 years. On examination, he was tachypneic and had tachycardia with mild hypotension. Abdominal examination showed a tense abdomen with guarding and rigidity. After adequate resuscitation, abdominal erect x-ray identified pneumoperitoneum and abdominal supine x-ray identified air (pneumo) tracking along the Arantius ligament (ligamentum venosum) (Fig. 1). Subsequently, contrast-enhanced computed tomography (CECT) abdomen confirmed these findings (Fig. 2). Exploratory laparotomy identified a 1 1 cm perforation in prepyloric segment which was repaired by a Graham’s patch. The patient recovered and was discharged on post-operative day 9 and is asymptomatic at follow-up. Peritonitis due to perforation of the gastrointestinal tract is a common surgical emergency all over the world that occurs in about 2%–14% of patients with active ulcer disease. Peptic perforation is most commonly present in the first part of duodenum (35%– 65%), with 25%–45% located in the pylorus and 5%–25% in the stomach. The factors that precipitate in occurrence of peptic perforation are Helicobacter pylori infection and chronic use of NSAIDs as in that of our patient. Other factors influencing perforation generally occurs as a result of stress, excessive smoking, and consumption of alcohol or coffee. Perforation peritonitis is diagnosed clinically with abdominal signs such as guarding and rigidity along with radiographic signs. The radiographic sign of intra-abdominal free air is commonly associated with a perforated viscus, demonstrated as free gas under the diaphragm in 69%–90% of patients in erect abdominal x-ray. Features of pneumoperitoneum on a supine abdominal x-ray are well documented in major radiological textbooks. Menuck and Siemers reported many rare abdominal radiographic signs such as falciform ligament sign (1/59) and air outlining the ligamentum teres. However, we report a rare abdominal radiographic sign of air tracking along the ligamentum venosum or the Arantius ligament, hence named it as Arantius sign (Fig. 1). Arantius ligament is a fibrous remnant of fetal ductus venosus, located within the fossa between the left lateral lobe and caudate lobe of liver, traveling from the left portal vein to the inferior vena cava. These findings are confirmed through CECT abdomen (Fig. 2). To the best of our knowledge, this is the first reported case of an Arantius sign. This rare abdominal radiographic sign is to be well appreciated in the most common emergency surgical condition, as these signs can be easily missed by the inexperienced. We believe that the knowledge of this rare radiographic sign empowers surgeons ultimately for earlier diagnosis and operative management if required. Abstract Dysembryoplastic neuroepithelial tumors (DNET) are Grade 1 tumors and second most common... more Abstract Dysembryoplastic neuroepithelial tumors (DNET) are Grade 1 tumors and second most common cause of drug resistant tumor associated epilepsy in young population. The clinical and radiological characteristics are not specific and therefore leads to diagnostic dilemma. Microhemorrhages are present in almost 3% of these patients, however clinically significant bleed in the lesion is almost unheard of. Early surgery remains the mainstay of treatment. Patients with good seizure control may however not be motivated to surgery and may opt for conservative line of approach. This can be catastrophic as happened in this index case. This patient was on medical line of management over 10 years when he presented with tumor bleed. This case is reinforcing the fact that early surgery aiming towards the gross total resection gives a favorable result in almost 90% of the cases.

Therefore, in today’s time it is very important for every individual to check the air quality of the surroundings. It leads to the need of an air quality monitoring system. This work proposes a portable Smart phone accessible Air quality monitoring and notification system, which notifies the user when the CO2 level and Total Volatile Organic Compound (TVOC) exceeds the maximum safety level. The concentration of carbon dioxide CO2 in the surrounding spans from 300-500 parts per million(ppm), whereas the global average holds 400 ppm of CO2. The quality of the air concerning carbon dioxide and TVOC value can be examined and notified using a small portable IoT sensor kit when the quality of air crosses the maximum safety levels.

This study therefore defined typical isoniazid exposure parameters after two months of treatment among fully-adherent patients in both hair and plasma (plasma area under the concentration-time curve, AUC, estimated using pharmacokinetic data collected 0, 2, 4, and 6 hours after drug administration). We found that INH levels in hair among highly-adherent individuals did not correlate well with plasma AUC or trough concentrations, suggesting that each measure may provide incremental and complementary information regarding drug exposure in the context of TB treatment.

The objective of this study was to albumin, pre-albumin and CRP levels of the patients who underwent surgery for gastric cancer were prospectively recorded and retrospectively evaluated.Methods: Study was done for period of 1 year in patients underwent surgery for gastric cancer. Details noted from records are as age, gender, duration of hospital stay, TNM stage, and preoperative albumin, pre-albumin and CRP levels.Results: A total number of 44 patients, 32 (72.7%) men and 12 (27.7%) women, were included in the study. Introduction: Metabolic Syndrome (MetS) is strongly associated with obstructive sleep apnea (OSA)... more Introduction: Metabolic Syndrome (MetS) is strongly associated with obstructive sleep apnea (OSA), a combination often termed syndrome Z. There is wide individual variation in the duration of the apneas and hypopnoeas. Some have few but prolonged apneas, and others frequent but brief apneas. Prolonged apneas are associated with more severe hypoxia. Desaturation index (DSI) may correlate better with some of the complications and comorbidities of OSA than the Apnea Hypopnea Index(AHI), as the intermittent hypoxias causing oxidative stress may underlie the pathogenic mechanisms in OSA. Objective: To compare the relationship of DSI and AHI to MetS. Materials and methods: Polysomnography recordings in a level I sleep laboratory of 104 proven OSA patients were studied. AHI, DSI and desaturations were analyzed and compared for their association with MetS. Conclusion: Our study shows DSI and degree of desaturation are better predictors of MetS, and reducing nocturnal hypoxia is important in managing OSA.

A fifteen years old boy presented to the emergency department with blunt trauma abdomen following... more A fifteen years old boy presented to the emergency department with blunt trauma abdomen following cricket ball injury. At the time of presentation, patient was hemodynamically stable with diffuse abdominal tenderness and right hypochondrial guarding. On exploratory laparotomy, he was found to have isolated duodenal laceration and underwent pancreas preserving total duodenectomy with ampullojejunostomy. In the postoperative period, patient developed fever with respiratory tract infection and eventually succumbed to multiorgan dysfunction. To the best of our knowledge, this is the first case report describing isolated duodenal injury resulting from blunt abdominal trauma due to cricket ball. OHRQoL includes different domains like esthetics which affects smiling, laughing and talking which in turn affects our social well being. It also includes the functional aspect of teeth and oral cavity like efficacy of mastication and other aspects like absence of symptoms like pain and hypersensitivity [1]. Smile is one of the most effective measures by which people convey their emotions and from a social point of view a beautiful smile is an added asset to a beautiful face. Children, with or without smiling, communicate whether they are happy or sad, confident or uncertain [2]. Children's smiling patterns are influenced by oral health related quality of life in two ways, the psychological aspect which includes the child's self perception of their own smile and the social aspect which includes the degree to which they have an open and relaxed smile. To obtain information on child's oral health related quality of life, it is necessary to develop questionnaires, which take into account a number of domains i.e. oral symptoms, functional limitations, emotional well being, and family well being/parental distress [3,4].

Intestinal loop stoma is a common surgical procedure performed for various benign and malignant a... more Intestinal loop stoma is a common surgical procedure performed for various benign and malignant abdominal problems, but it rarely undergoes spontaneous closure, without surgical intervention. Two male patients presented to our emergency surgical department with acute abdominal pain. One of them was diagnosed as having rectosigmoid perforation and underwent diversion sigmoid loop colostomy after primary closure of the perforation. The other was a known case of carcinoma of the rectum who had already undergone low anterior resection with covering loop ileostomy; the patient underwent second loop ileostomy, this time for complicated intestinal obstruction. To our surprise, both the loop colostomy and ileostomy closed spontaneously at 8 weeks and 6 weeks, respectively, without any consequences. Spontaneous stoma closure is a rare and interesting event. The exact etiology for spontaneous closure remains unknown, but it may be hypothesized to result from slow retraction of the stoma, added to the concept of a tendency towards spontaneous closure of enterocutaneous fistula.

This study was undertaken to examine the association between plasma nevirapine (NVP) and efavirenz (EFV) concentrations and virological outcomes in patients infected with HIV-1 and TB. This was a nested study undertaken in a clinical trial of patients with HIV-1 and TB, randomized to two different once-daily antiretroviral treatment (ART) regimens along with anti-TB treatment (ATT). Trough concentrations of plasma NVP and EFV were estimated at months 1 (during ATT and ART) and 6 months (ART only) by HPLC. The objective of this study was to albumin, pre-albumin and CRP levels of the patients who underwent surgery for gastric cancer were prospectively recorded and retrospectively evaluated.Methods: Study was done for period of 1 year in patients underwent surgery for gastric cancer. Details noted from records are as age, gender, duration of hospital stay, TNM stage, and preoperative albumin, pre-albumin and CRP levels.Results: A total number of 44 patients, 32 (72.7%) men and 12 (27.7%) women, were included in the study. There is wide individual variation in the duration of the apneas and hypopnoeas. Some have few but prolonged apneas, and others frequent but brief apneas. Prolonged apneas are associated with more severe hypoxia. Desaturation index (DSI) may correlate better with some of the complications and comorbidities of OSA than the Apnea Hypopnea Index(AHI), as the intermittent hypoxias causing oxidative stress may underlie the pathogenic mechanisms in OSA. Objective: To compare the relationship of DSI and AHI to MetS. Materials and methods: Polysomnography recordings in a level I sleep laboratory of 104 proven OSA patients were studied. AHI, DSI and desaturations were analyzed and compared for their association with MetS. Conclusion: Our study shows DSI and degree of desaturation are better predictors of MetS, and reducing nocturnal hypoxia is important in managing OSA.

At the time of presentation, patient was hemodynamically stable with diffuse abdominal tenderness and right hypochondrial guarding. On exploratory laparotomy, he was found to have isolated duodenal laceration and underwent pancreas preserving total duodenectomy with ampullojejunostomy. In the postoperative period, patient developed fever with respiratory tract infection and eventually succumbed to multiorgan dysfunction. To the best of our knowledge, this is the first case report describing isolated duodenal injury resulting from blunt abdominal trauma due to cricket ball. OHRQoL includes different domains like esthetics which affects smiling, laughing and talking which in turn affects our social well being. It also includes the functional aspect of teeth and oral cavity like efficacy of mastication and other aspects like absence of symptoms like pain and hypersensitivity [1]. Smile is one of the most effective measures by which people convey their emotions and from a social point of view a beautiful smile is an added asset to a beautiful face. Children, with or without smiling, communicate whether they are happy or sad, confident or uncertain [2]. Children's smiling patterns are influenced by oral health related quality of life in two ways, the psychological aspect which includes the child's self perception of their own smile and the social aspect which includes the degree to which they have an open and relaxed smile. To obtain information on child's oral health related quality of life, it is necessary to develop questionnaires, which take into account a number of domains i.e. oral symptoms, functional limitations, emotional well being, and family well being/parental distress [3,4].

Intestinal loop stoma is a common surgical procedure performed for various benign and malignant a... more Intestinal loop stoma is a common surgical procedure performed for various benign and malignant abdominal problems, but it rarely undergoes spontaneous closure, without surgical intervention. Two male patients presented to our emergency surgical department with acute abdominal pain. One of them was diagnosed as having rectosigmoid perforation and underwent diversion sigmoid loop colostomy after primary closure of the perforation. The other was a known case of carcinoma of the rectum who had already undergone low anterior resection with covering loop ileostomy; the patient underwent second loop ileostomy, this time for complicated intestinal obstruction. To our surprise, both the loop colostomy and ileostomy closed spontaneously at 8 weeks and 6 weeks, respectively, without any consequences. Spontaneous stoma closure is a rare and interesting event. The exact etiology for spontaneous closure remains unknown, but it may be hypothesized to result from slow retraction of the stoma, added to the concept of a tendency towards spontaneous closure of enterocutaneous fistula.

This study was undertaken to examine the association between plasma nevirapine (NVP) and efavirenz (EFV) concentrations and virological outcomes in patients infected with HIV-1 and TB. This was a nested study undertaken in a clinical trial of patients with HIV-1 and TB, randomized to two different once-daily antiretroviral treatment (ART) regimens along with anti-TB treatment (ATT). Trough concentrations of plasma NVP and EFV were estimated at months 1 (during ATT and ART) and 6 months (ART only) by HPLC.
