The lesions were classified as neuropathic, ischemic, or neuroischemic. Medication received by patients with diabetes control measured by blood glucose and glycosylated hemoglobin (HbA1c) was recorded. All daily data on ambient temperature and atmospheric pressure were obtained in the Autonomous City of Buenos Aires from the "Jorge Newbery Metropolitan Airport Meteorological Station" database from January 2014 to 2017. Introduction: Tibial stress fractures are a very common condition in military recruits. Our objec... more Introduction: Tibial stress fractures are a very common condition in military recruits. Our objective is to demonstrate that stress fractures are associated with other diseases. Materials and Methods: 42 stress fractures in 34 patients were retrospectively evaluated. Every patient had recently joined the Argentine Army and consulted for painful symptoms in the tibia. A clinical and scintigraphic diagnosis of stress fracture was made. Patient data, associated diseases, and risk factors were documented. Inclusion criteria: recent incorporation, same training, age between 16 and 23 years. Trauma, simulators, tumoral pathology, and cases with negative scintigraphy were excluded. Results: We studied 42 stress fractures in 34 patients, 14 were men and 20 were women. The average age was 20 years. There were no significant differences in the number of injuries regarding the affected limb. 64.7% had associated diseases, 73% in women and 27 % in men. Among the women with stress fractures, 80% had associated diseases, compared to 43% for men. Different diseases were found with lower limb varus and valgus imbalances. Varus was the most associated with fractures. Conclusions: A high rate of associated diseases was found in patients with tibial stress fractures with a predominance of lower limb varus imbalances. Associated diseases were more likely to be found in women with stress fractures than in men.

Our objectives were to evaluate the mortality rate in the first month, and 2 and 5 years after amputation for diabetic foot and to compare the mortality rate in patients with below-theknee (BKA) and above-the-knee amputations (AKA). Materials and Methods: Sixty patients with 64 major amputations, operated on between 2010 and 2018, were retrospectively evaluated. The following information was collected: personal data, laterality, University of Texas classification score, biochemical analysis results, Doppler studies, comorbidities, survival, and cause of death. The overall mortality rate and the mortality rate by type of amputation in the first month and after 2 and 5 years were calculated. results: 58 amputations were performed in men and 6 in women (37 right, 27 left), 39 were AKA and 25 were BKA. The average age was 68 years. There were 28 deaths: 15 patients with AKA and 13 with BKA. Mortality rates were: 13.33% in the first month, 33.3% after 2 years, and 46.42% after 5 years (patients with AKA, 26.78%; patients with BKA, 19.64%). The differences in the creatinine levels of the patients who died were statistically significant. The main causes of death were cardiovascular complications. conclusions: Mortality rates were significantly higher in patients with above-the-knee amputation. The sepsis-related mortality rate in the first month was high. Patients with kidney disease had a higher mortality rate.

Los objetivos de este estudio fueron evaluar la tasa de mortalidad al mes, y a los 2 y 5 años de una amputación por pie diabético y comparar la tasa de mortalidad en pacientes con amputación infrarrotuliana o supracondílea. Materiales y Métodos: Se evaluó retrospectivamente a 60 pacientes con 64 amputaciones mayores, operados entre 2010 y 2018. Se reunió la siguiente información: datos personales, lateralidad, puntaje de la clasificación de la Universidad de Texas, análisis bioquímicos, estudio Doppler, comorbilidades, supervivencia y causa del óbito. Se calculó la tasa de mortalidad general y por tipo de amputación al mes, a los 2 y 5 años. Resultados: Se realizaron 58 amputaciones en hombres y 6, en mujeres (37 derechas, 27 izquierdas), 39 fueron supracondíleas, y 25 infrarrotulianas. La edad promedio era de 68 años. El objetivo de este estudio fue analizar el empleo de la telemedicina como herramienta en pacientes con pie diabético, por parte de especialistas en pie y tobillo de la Argentina. Como objetivos secundarios se planteó valorar la satisfacción percibida por los profesionales con el uso de esta herramienta, evaluar qué estrategias se usaron para realizar la consulta médica remota y determinar la permanencia al retornar a la actividad asistencial habitual. Materiales y Métodos: Se enviaron tres encuestas a los socios de la Sociedad Argentina de Medicina y Cirugía de la Pierna y Pie, entre mayo 2020 y abril de 2022. El objetivo de este artículo es describir portales modificados para el diagnóstico y la reparación endoscópica de las lesiones del fascículo superomedial del ligamento calcaneonavicular y evaluar la seguridad de los portales utilizados. Materiales y Métodos: Se llevó a cabo un estudio cadavérico con seis preparados reproduciendo una lesión del fascículo superomedial del ligamento calcaneonavicular con una punta de corte de radiofrecuencia y la posterior reparación endoscópica. BACKGROUND "Morton's Neuroma" is a frequent cause of chronic forefoot pain. It can ... more BACKGROUND "Morton's Neuroma" is a frequent cause of chronic forefoot pain. It can affect general population, including athletes, and can lead to progressive foot pain and discomfort for daily life activities and sports. HYPOTHESIS/PURPOSE Our objective is to evaluate the long-term results in a series of 85 feet, operatively treated with minimally invasive interdigital approach for neurectomy. STUDY DESIGN Case series. METHOD 83 patients (85 feet) were treated between January 2003 and December 2019. The AOFAS score and VAS scale were used to evaluate the patients pre and postoperatively. Results were analyzed using the JASP software. RESULTS Mean age was 50.58 years (range 23-77). Eleven (11) were men, and seventy-four (74) women, with two bilateral cases. Mean follow up was 49 months. Our series was evaluated with AOFAS and VAS scores, both showing statistically significant improvement posterior to the operative procedure. CONCLUSIONS A series of 85 feet operatively treated for Morton's neuroma with minimally invasive interdigital approach for neurectomy is presented. Long term results were similar to other published series with different approaches, with the advantage of minimal incision, minimum soft tissue dissection and no need to release intermetatarsal ligament, immediate weightbearing and quick return to daily activities and sports. It is concluded that minimally invasive neurectomy with interdigital approach is a safe procedure for Morton's neuroma treatment with similar results that other operative procedures. LEVEL EVIDENCE Level IV.

Patients with acquired flatfoot deformity due to isolated injury of the spring ligament, with hea... more Patients with acquired flatfoot deformity due to isolated injury of the spring ligament, with healthy posterior tibialis tendon, are rarely identified. Between December 2004 and September 2011 (6 years and 9 months), we treated 10 patients with acquired flatfoot deformity due to spring ligament injury without tibialis posterior tendon tear. One patient (10%) was lost to follow-up. The mean age of the patients was 44.33 § 12.91 years; 4 (44.4%) were female, and 5 (55.56%) were male. Clinical presentation included mild to moderate hindfoot valgus and pain extending from the inferior part of the medial malleolus to the navicular, inferior to tibialis posterior. Forefoot abduction was not always present. Weightbearing radiographs and magnetic resonance imaging (MRI) scans were obtained in all cases. Six patients (66.66%) of the patients had a history of minor trauma. Spring ligament repair was performed in all cases, and 4 patients (44.44%) underwent adjunct procedures. After surgery, a cast was applied, and weightbearing was avoided for 6 weeks. The mean preoperative American Orthopaedic Foot and Ankle Society (AOFAS) score was 39.66 § 18.55, and this improved to 84.88 § 12.41 after surgery (p = .023). No statistically significant differences were found between males and females or between isolated repairs and adjunct procedures. The mean duration of follow-up was 45.33 § 37.11 months (range 15 to 120), and no complications were identified. Isolated injuries of the spring ligament with normal posterior tibialis tendon are rarely described and may be more prevalent than generally appreciated.

The symmetrical peripheral gangrene syndrome is characterized by ischemic distal lesions in two o... more The symmetrical peripheral gangrene syndrome is characterized by ischemic distal lesions in two or more extremities, without the obstruction of large vessels. The ischemia affecting extremities takes place when the arterial blood flow is insufficient to satisfy the metabolic demands of muscles in rest. We present a 22-year-old patient treated with norepinephrine after a septic shock suspicion who presented distal ischemia of fingers, toes, and scalp. There is no consensus about the initial treatment. Improved data gathering is necessary for a better understanding of the mechanisms involved, which could improve treatment and prevent amputation as the final therapeutic option.

Background: Traumatic dislocation of the posterior tibial tendon (PTT) is a highly unusual pathol... more Background: Traumatic dislocation of the posterior tibial tendon (PTT) is a highly unusual pathology. Diagnosis is based on suspicion and clinical examination. Case description: A 35-year-old male patient suffered a car accident and presented swelling, pain, and instability in the medial side of the right ankle and ecchymosis, on the affected side, associated with deltoid ligament injury. The mechanisms of the injury have not been completely defined but, in most cases, it has been associated with forced dorsiflexion and inversion of the ankle. He was surgically treated 2 weeks after the procedure. Conclusion: Surgical treatment is mandatory. The goal is to relocate the tendon in its normal anatomical position and recreate a competent flexor retinaculum. Clinical significance: We present an unusual case report with good clinical results after surgical treatment and a surgical tip not described before.

La tromboprofilaxis para las cirugías de tobillo y pie no está estandarizada, porque la incidencia es baja, según la bibliografía. El objetivo de este estudio es aportar un algoritmo de prevención para la trombosis venosa profunda y una revisión de la bibliografía.Materiales y Métodos: Se llevó a cabo una revisión de las historias clínicas de los pacientes operados entre 2011 y 2017. La muestra estaba integrada por 696 hombres (28,98%) y 1706 mujeres (71,02%), con un promedio de edad de 51 años (rango 20-82). Se realizaron 2402 procedimientos, 575 (23,94%) fueron por patologías traumáticas y 1827 (76,06%), por patologías ortopédicas.Resultados: Nueve pacientes (0,37%) presentaron trombosis venosa profunda y solo dos, tromboembolismo pulmonar (0,08%). Se realizaron 2402 procedimientos, 575 (23,94%) fueron por patologías traumáticas y 1827 (76,06%), por patologías ortopédicas.Resultados: Nueve pacientes (0,37%) presentaron trombosis venosa profunda y solo dos, tromboembolismo pulmonar (0,08%).
