Buerger's disease, also called thromboangiitis obliterans (TAO), is an inflammatory disease affec... more Buerger's disease, also called thromboangiitis obliterans (TAO), is an inflammatory disease affecting small and medium size blood vessels of the limbs. Several risk factors are strongly associated with this disease. In fact, heavy smokers represent 99% of patients diagnosed with Buerger's disease and smoking cessation can slow down the progression of the pathology. Ulcers and gangrene are very common complications and TAO may also cause a vasculitis as well as ischemic damage to the distal part of the limbs that can eventually lead to limb amputation. Here, we report the case of a 43 years old patient, a flight personnel working in a Tunisian airline company who consults for a lesion of the left toe that appeared 2 weeks ago. He is a chronic inveterate smoker with 36 pack-years. The diagnosis of TAO was made based on the clinical presentation and the results of the Doppler Ultrasound of the left toe. This disease has a major impact on the quality of life due to the risk of necrotic ulcers and amputation. Navigation is a profession that requires a high level of physical and mental fitness. According to the international and Tunisian standards, the applicant or the holder of medical certificate must not have a cardiovascular condition that may affect the safety of the exercise.

AIM To evaluate the quality of the IMC and their compliance with the legal provisions and to develop good practice recommendations for the drafting of the occupational accident certificate. METHOD An epidemiological, descriptive and retrospective study was conducted in the University Hospital Center Farhat Hached (Tunisia) based on the occupational accident certificates completed by physicians during a period of 12 months. RESULTS In our study, 241 certificates were collected. The number of the patient's ID card was mentioned in 12% of cases. The date of the accident was noted in almost all cases (99.6%). The aim of this study was to assess trends in the frequencies of sensitization to the different allergens in the European standard battery over a 26 year period from 1989 to 2015. Methods: It’s a descriptive study of the trend of sensitization to BSE allergens over a period of 26 years, at the dermato-allergology unit of occupational medicine department of Farhat Hached university hospital-Sousse (Tunisia), based on the results of patch tests of all patients who consulted the unit between 1989 and 2015. Objectives: Low back pain is a major occupational problem especially among nursing staff. The obj... more Objectives: Low back pain is a major occupational problem especially among nursing staff. The objectives of our study are to evaluate the prevalence of low back pain among nurses and to look for physical and psychosocial risk factors. Methods: It is a cross-sectional study based on a self-administered questionnaire destined for all nurses working in Farhat Hached Teaching hospital of Sousse (Tunisia). Results: Our study included 203 nurses with an average age of 39.8 years. The prevalence of low back pain over the last twelve months was 58.1%. The factors that are significantly associated to low back pain were: high BMI, number of pregnancies, arthritis, poor physical condition, daily frequency of inappropriate posture for the activity being performed, and the layout of materials in the workplace. Conclusion: Our study evidenced the high prevalence of LBP among nurses and allowed bringing to light the role of individual and ergonomic physical factors in the genesis of LBP. Such identification permits to undertake targeted preventive actions. The association between psychosocial factors and LBP was not emphasized.

Le syndrome d’apnees obstructives de sommeil (SAOS) est une situation clinique hautement prevalen... more Le syndrome d’apnees obstructives de sommeil (SAOS) est une situation clinique hautement prevalente, entrainant des repercussions sur le plan fonctionnel, psychologique, socioprofessionnel. La reconnaissance de la qualite de vie (QDV) comme objectif therapeutique est une notion assez recente. But Notre objectif est d’evaluer la QDV des patients suivis pour SAOS et determiner les facteurs predictifs d’une alteration de la QDV. Il s’agit d’une etude descriptive transversale sur des patients suivis au service de Pneumologie pour SAOS. L’evaluation de la qualite de vie s’est basee sur un questionnaire generique (le SF-36). Resultats Au total, 103 cas ont ete inclus. Ces patients sont caracterises par un âge moyen de 55 ± 11 ans avec une predominance feminine (58 %), un IMC a 39 ± 7,19 Kg/m 2 (dont 95 % obeses), 72 % ont des comorbidites, le score de somnolence d’Epworth moyen etait de 14,4 ± 3,9, 85 % des cas avaient une somnolence diurne excessive. L’IAH moyen etait de 54,7 ± 2,06/h. L’etude de la qualite de vie par le SF-36 nous a permis de constater que les dimensions de la qualite de vie les plus alterees etaient : les limitations dues a l’etat physique, les limitations liees a l’activite physique et la vitalite. Les facteurs independants influant sur les composantes physique et mentale du SF-36 reveles par les analyses par regression logistique etaient : le genre, l’âge, l’IMC et les troubles anxieux et depressifs. Des facteurs d’alteration de la qualite de vie ont ete identifies nous laissant envisager la possibilite d’une prise en charge precoce, adapte a la gravite potentielle du SAOS et la plus proche des preoccupations du patient.

Introduction: The association between occupational asthma (OA) and occupational allergic rhinitis... more Introduction: The association between occupational asthma (OA) and occupational allergic rhinitis (OAR) is often described in the literature supporting the concept of "United airways disease" based on a parallel significant reaction of the nose and lungs after challenges with occupational agents and involving several occupational, host-related and environmental factors and thus exacerbating the medical and socioeconomic impact of the OR. In Tunisia, only limited studies were carried out to explore association between OA and OR. Objectives: Assess the incidence and prevalence of OR among workers in the textile sector, to determine their socioprofessional characteristics, and to study the association between this pathology and OA. Material and methods: We carried out a descriptive epidemiological study about charts of occupational allergic rhinitis cases in the textile sector recognized by the competent commissions of the National Medical Care Fund (CNAM) in the region of the Tunisian center during the period from January 1 st , 2008 to December 31, 2012. Results: One hundred seventy-three cases of OAR declared in the central region of Tunisia were enrolled in the study during the study period representing a prevalence of 3.9% of all occupational diseases, recognized during this same period. Our study population was mainly women with a mean age of 40 ± 8.32 years. The mean delay of respiratory complaints onset was of 13.46 years. The average duration of OR evolution before it is reported to CNAM was of 5.13 years. Rhinitis was isolated in 34.10% of the cases. The association between OA and OR was present among 59.5% of the cases. Rhinitis symptoms preceded asthma symptoms in most of the cases (96.12%). According to the results of rhinomanometry, 40.5% of OR (i.e. n = 70) were severe. Most of reported OR cases were attributable to Vegetable textile dusts. After multiple binary logistic regression, the association was 2.29 times more frequent in the subjects aged more than 39 years (IC at 95% [1.08-4.86]) and 3.22 times more frequent in subjects poorly or not skilled (IC at 95% [1.38-7.5] (p = 0.05)). Conclusion: Our study is the first one to evaluate the association between rhinitis and asthma among workers in the textile sector in Tunisia. The standard profile of our cases reporting allergic occupational rhinitis is a young unskilled worker in the textile sector. There is a strong association between OAR and OA. A rigorous and adequate prevention is necessary and allows reducing the importance of these occupational pathologies and their serious consequences.

Conclusion: Our study is the first one to evaluate the association between rhinitis and asthma among workers in the textile sector in Tunisia. The standard profile of our cases reporting allergic occupational rhinitis is a young unskilled worker in the textile sector. There is a strong association between OAR and OA. A rigorous and adequate prevention is necessary and allows reducing the importance of these occupational pathologies and their serious consequences.
