Purpose To investigate the experiences of participants of a self-management program for employees... more Purpose To investigate the experiences of participants of a self-management program for employees with complaints of the arm, neck or shoulder (CANS). The program consisted of six group sessions combined with an eHealth module. Methods Semi-structured interviews with the first 31 consecutive participants of the intervention group participating in a randomized controlled trial. Participants were interviewed after their last group session. Semi-structured interviews were guided by an interview guide and audio-recorded. Data were analyzed using thematic analysis and the emerging themes were discussed. All participants in the intervention group were asked about their experiences with a questionnaire at three (n = 58) and 12-months (n = 53) follow-up. Results Most participants appreciated the diversity of the program and benefited from the interaction with their peers. Literature searches were performed in Pubmed, CINAHL, EMBASE, and PsycInfo using Medical Subject Heading terms and text words relating to musculoskeletal conditions, interventions to promote work-focused care and work-related outcomes. Articles involving any interventions with elements of work-focused care delivered by healthcare providers to manage musculoskeletal conditions were reviewed for suitability and inclusion. A total of 22 articles (18 intervention trials) were identified. Physiotherapists (PTs), occupational therapists (OTs) and exercise therapists (ETs) are often involved in the early management of MSDs. There is a need for additional insights into therapists’ experiences, barriers and needs to work-focused care. Moreover, there is no evidence on how OTs and ETs address work participation. Therefore, the aim of this qualitative study was 1) to investigate how generalist PTs, OTs and ETs provide work-focused healthcare and 2) to obtain insight into their perceived barriers and needs that affect their ability to address occupational factors. SYNOPSIS Vascular serious adverse events (SAEs) can occur after manipulating, mobilizing, prescri... more SYNOPSIS Vascular serious adverse events (SAEs) can occur after manipulating, mobilizing, prescribing exercise for and examining the cervical spine. Patients presenting with neck pain and headache who develop a vascular SAE during or after treatment may have vascular flow limitations that were unrecognized and aggravated by treatment. Patients with neck pain and headache-the first non-ischemic symptoms of arterial dissection-frequently access a physical therapist as a first point provider who may or may not have specialist training in orthopedic manual physical therapy (OMPT). All physical therapists irrespective of their training, who are helping patients manage neck pain, headache and/or facial symptoms, must feel confident to identify potential vascular flow limitations of the neck prior to providing treatment. Background and objective: Serious pathologies of the neck can potentially result in cranial nerve... more Background and objective: Serious pathologies of the neck can potentially result in cranial nerve palsy. Knowledge about cranial nerve examination (CNE) seems sparse, and its use is still unknown. We aim to investigate the knowledge, skills, and utilization of CNE of Italian physiotherapists. Materials and Methods: An online cross-sectional survey. Results: 396 completed the survey, reaching the required sample size. Although Italian physiotherapists consider CNE relevant (mean ± SD = 7.6/10 ± 2.0), over half of all responders (n = 229 (57.8%)) were not trained in the fundamentals and around a third did not use it in their daily practice (n = 138 (34.8%)). Additionally, participants were unconfident and insecure in conducting (n = 152 (38.4%) and n = 147 (37.1%)), interpreting (n = 140 (35.4%) and n = 164 (41.4%)), and managing the CNE (n = 141 (35.6%) and n = 154 (38.9%)). Musculoskeletal disorders (MSDs) are characterized by pain and reduced physical functioning that ... more Musculoskeletal disorders (MSDs) are characterized by pain and reduced physical functioning that often results in decreased quality of life. Work-related musculoskeletal disorders (WRMDs) are health problems in which working conditions (ie, physically demanding or repetitive activities or a poor work environment) significantly contribute to the onset or progression of the disorder, but are not necessarily the sole cause. Even if work-related environmental factors do not directly cause MSDs, the symptoms can be work relevant (ie, the work can influence the symptoms, or the symptoms can affect the ability to work). WRMDs are burdensome to patients, employers and society as they are associated with high rates of disability, absenteeism, presenteeism, loss of productivity, longer recovery timeframes and workers' compensation costs. In this Point of View, we argue that physical therapists treating patients with MSDs should adopt a work-focused approach. There is ample evidence showing that being employed is associated with better health.1 Physical therapists can improve the health-related quality of life for patients with MSDs through adopting a work-focused approach and promoting participation in work.
