He developed marked cortical thickening of many bones, which raised the suspicion of a battered child syndrome. Unusual progression of bone thickening and hitherto undescribed excessive bone remodeling led to the diagnosis of Menkes'kinky hair disease, a disorder of the connective tissue caused by a decreased copper bioavailability, to which disease the infant finally succumbed.

The neutroeyte mobilization test was performed in 10 children to investigate the bone marrow func... more The neutroeyte mobilization test was performed in 10 children to investigate the bone marrow function during viral hepatitis. In the initial stage pathologic test results were obtained in 6 out of 10 patients. As the hepatitis improved the test responses normalized, or initially adequate responses increased. These findings are discussed as a manifestation of reversible bone marrow suppression during viral hepatitis.

To assess the potential benefits of a reassortant tetravalent rotavirus vaccine, we investigated ... more To assess the potential benefits of a reassortant tetravalent rotavirus vaccine, we investigated stool specimens from children in three different groups by reverse transcription-PCR (RT-PCR) for rotavirus G and P types: (i) children not hospitalized with community-acquired rotavirus-acute gastroenteritis (RV-AGE), (ii) children hospitalized for RV-AGE, and (iii) children with nosocomially acquired RV-AGE. From a total of 553 samples investigated, 335 were positive by enzyme-linked immunosorbent assay, of which 294 (88%) were positive by RT-PCR. Among the RT-PCR-positive samples, the predominant types were G1P[8] (84%), followed by G4P[8] (9%) and G3P[8] (2%). No differences between the three groups were observed, suggesting that community vaccination will diminish the most cost-relevant cases of hospitalizations and nosocomial infections.

At the age of three months an infant rapidly developed signs of cardiac failure as well as in- an... more At the age of three months an infant rapidly developed signs of cardiac failure as well as in- and exspiratory stridor, caused by an intrathoracic tumor. Thoracotomy and biopsy revealed an intrapericardial tumor, histologically myxosarcoma. In spite of chemotherapy and radiation the infant died at the age of seven months due to multiple intracerebral metastases now histologically rhabdomyosarcoma. This is one of the rare cases of primarily malignant intrapericardial tumors in infancy, and also shows the possible pleomorphism of childhood rhabdomyosarcoma. We know only one further case of pericardial rhabdomyosarcoma where similar histologic changes have been observed.

The patient suffered from recurrent stridor since the eighth month of life; during the seventh ho... more The patient suffered from recurrent stridor since the eighth month of life; during the seventh hospitalization at the age of 2 1/2 years laryngeal papillomas were found. Surgical removal of the papillomas was attempted several times. Despite this bronchopulmonary papillomas were seen on chest X-ray films at the age of seven years. After intravenous bleomycin therapy (7 mg/sqm twice weekly for nine weeks) the infiltrates disappeared and the patient has remained free of symptoms til now (18 months later).

The formation of an Austrian study group for the treatment of leukemia in children with internati... more The formation of an Austrian study group for the treatment of leukemia in children with international cooperation is reported. An essential aim is the centralisation of treatment at the 3 Austrian University clinics, the St. Anna children's hospital in Vienna and other children's hospitals particularly interested in the treatment of leukemia. Secondly the group undertakes randomised therapeutic studies, at present on two new variants of the Pinkel treatment scheme. Within the first 9 months 33 cases were included in this study in cooperation with the Zagreb University children's clinic (Doz. TIEFENBACH).

Within the last 20 years 56 children with neuroblastoma were seen at the Paediatric Departments o... more Within the last 20 years 56 children with neuroblastoma were seen at the Paediatric Departments of the University Hospitals of Graz, Innsbruck and Vienna. Case histories, clinical details and diagnostic procedures are discussed. The poor prognosis (only 18 out of the 56 children i.e. 32% are still alive) is due to the high incidence of metastasis (55% of the patients were admitted in stage IV of the disease). Diagnosis within the first year of life, mediastinal localization and histological differentialtion to ganglioneuroblastoma are good prognostic features, whereas therapeutic measures are of less importance. Radical surgery still provides the patient with the best chance of survival, but is rarely feasible (complete primary removal was possible in only 7 out of the 56 patients; in futher 2 patients the tumour proved to be resectable at a 2nd look operation). Two boys, aged 5 1/4 and 10 1/4, respectively, with acute immature-cell leukosis, died of massive... more Two boys, aged 5 1/4 and 10 1/4, respectively, with acute immature-cell leukosis, died of massive cerebral haemorrhage. Their eyes showed extensive leukaemic involvement of the retina with development of numerous miliary leukaemic nodules; the vitreous of case 2 was also involved. The lesions were considered to have developed because of high circulating white cell counts, a tendency of blast cells to produce nodules, an oxygen poor avascular matrix in which to grow, and a physical environment which permitted growth without haemorrhage or distortion due to tissue pressures etc. Using histological methods, an attempt is made to trace the pattern of formation of the nodule, from an initial growing focus through "colonial" form resembling a bacterial colony to lysis. Necrotic blast cells are apparently a metabolic source for viable tumour cells. Their localization in the eye makes access by cytotoxic drugs difficult; thus they should be considered as blast cell pools.

The causal relationship of immunosuppressive side effects of the treatment and this cytomegalovirus infection is discussed.

Cow's milk allergy is the most common type of food allergy in infants. Most infants develop s... more Cow's milk allergy is the most common type of food allergy in infants. Most infants develop symptoms one week after initiating the feeding of cow's milk based formulas though sensitisation in utero and via mother's milk are also possible. We report on three newborns who received whey baths after birth and developed allergic skin reactions. Cow's milk allergy was diagnosed. In our opinion the whey baths could be responsible for the sensitisation via skin and the allergic skin reactions. The risks and benefits of whey baths for newborns and infants should therefore be carefully considered.

In late 1990 a screening program for the early detection of neuroblastoma in infants was introduc... more In late 1990 a screening program for the early detection of neuroblastoma in infants was introduced in Austria. The program is performed on a voluntary basis in collaboration with general pediatricians and practitioners. Filter strips for urine collection are distributed to parents of infants aged seven to nine months on the occasion of a routine check up. The samples are sent to the laboratory by parents and analysed for vanillylmandelic acid (VMA) and homovanillic acid (HVA). Between January 1991 and December 1995 125,201 infants were screened. The compliance rate was 26.8% for Austria, but great differences were seen for different regions (65% in Carinthia, 10% in Vorarlberg). 30 children were admitted to hospital for investigation of repeatedly elevated urine catecholamines. A neuroblastoma was identified in 16 cases. During 14 years the department of otolaryngology at Leoben performed 7743 tonsillectomies and/or ... more During 14 years the department of otolaryngology at Leoben performed 7743 tonsillectomies and/or adenoidectomies in children in cooperation with the department of pediatrics. 97 patients were treated for postoperative bleeding = 1.25%; one child died as a consequence of severe bleeding. The analysis of age, timing and severity of the bleeding includes 15 additional children whose surgery had been performed elsewhere. Only 7 of the 112 bleeding episodes occurred during the first 24 hours after surgery. Most cases (18) occurred on the fifth postoperative day. The latest episode occurred on the 20th day after tonsillectomy. Because of severe blood loss, 18 patients received a blood transfusion. Despite pre- and postoperative coagulation tests no patient suffering from a coagulation disorder was identified. Neither the coagulation screening nor a 48 to 72 hour postoperative in hospital observation could prevent the risk of postoperative bleeding.
