Si certains spectateurs voient en l’evenement sportif un moyen de s’evader, d’eprouver des experi... more Si certains spectateurs voient en l’evenement sportif un moyen de s’evader, d’eprouver des experiences emotionnelles, d’autres y trouvent un moyen d’affirmation identitaire. L’objectif de ce travail est de voir d’une part, en quoi le besoin d'identite reste une motivation a la frequentation des evenements sportifs au Senegal et, d’autre part, en quoi cette expression identitaire pourrait etre un atout marketing pour les organisateurs. Obtenus par le biais d’une approche ethno-marketing, les resultats montrent qu’au Senegal, l’evenement sportif tend a se presenter comme un espace de visibilite privilegie de differentes communautes, en permettant a celles-ci de se faire reconnaitre et d’affirmer leur identite. Cette logique d’affirmation identitaire renforce la qualite du spectacle sportif, augmente la frequentation des stades et pousse les supporters a consommer les produits derives qui symbolisent leurs sportifs ou equipes favorites. A pseudocyst of the pancreas (PCP) is a rare affliction in Africa despite widespread malnutrition... more A pseudocyst of the pancreas (PCP) is a rare affliction in Africa despite widespread malnutrition and abdominal disorders. The authors cover 7 PCP observations on 3 men and 2 women between 18 and 32 years, and 2 boys of 5 and 6 years, in the clinical context of abdominal pain (7/7), an abdominal tumour (7/7) and as a sequence to abdominal contusion (4/7). In 5 cases, echotomography led the etiological investigation of the liquid mass to seek a pancreatic origin. Surgical treatment consisted of 2 termino-lateral cystojejunostomies on a shaped flexure++ and 3 latero-lateral cystojejunostomies (of which one had to be drained externally two times); and a corporeo-caudal pancreatectomy. Supervision of between 1 1/2 and 5 years (1 1/2 years: 3 patients; 2 1/2 years: 2 patients; 5 years: 1 patient; 1 patient lost track of) revealed only one recurrence after internal latero-lateral drainage, and that without clinical repercussions. The authors use this short experiment to study the clinical and paraclinical symptomatology, the etiology and the treatment of PCPs. They stress the role of medical pictures in diagnosis, and of interventional radiology in treatment, but above all the primacy of surgery to treat pseudocysts of the pancreas in Africa.

T... more The authors report on a retrospective study 20 cases of intussusception in senegalese children. The diagnosis is essentially clinical. In difficult cases plane abdominal X-rays, barium meal and echography are indispensable exams. In our context the diagnosis is most of the time late and it is due to the unknown nature of the affection and early consultation to the traditional practitionners. The lateness of the diagnosis leads to the lateness of the management. This explains the importance of intestinal necrosis noticed in our serie and the high mortality rate which is 10%. The authors suggest recommendations to ameliorate the prognosis of this ailment which is good elsewhere.

Appendiceal pathology&amp... more Appendiceal pathology's management has benefited in recent years from the advent of laparoscopic surgery. This study is to make a preliminary assessment of laparoscopic management of acute and complicated appendicitis in children after a few months of practice at the University Hospital Albert Royer, Dakar. This is a retrospective study of 22 cases of patients, all operated on by the same surgeon. The parameters studied were age, sex, clinical data and laboratory features, radiological data, and results of surgical treatment. The mean age of patients was 9.5 years with a male predominance. The series includes 14 cases of acute appendicitis and 8 complicated cases. Appendectomy anterograde is practiced in 81% of cases. Appendectomy was associated with peritoneal wash in 17 patients including 9 cases of acute appendicitis. Drainage of Douglas pouch is performed in 2 patients with complicated appendicitis; the average production was 300 cc of turbid liquids and any complications were not founded. An abscess of Douglas pouch is noted in 2 patients with complicated appendicitis undrained. These Douglas abscesses were treated medically. No conversion of laparotomy was performed in the series. After an average of 8 months no other problems were noted.

Complete traumatic amputation of the tongue is very exceptional, the intra-oral position generall... more Complete traumatic amputation of the tongue is very exceptional, the intra-oral position generally protecting the tongue. Most reported cases of trauma have involved partial section with satisfactory results after simple suture. Complete amputation of a portion of the tongue occurred in a 12-year-old boy during a fight. The fragment of the tongue was brought to the emergency unit. It was placed in heparinated saline solution and replanted successfully three hours after the trauma. Plastic surgery of the oral cavity was also performed to repair laceration injuries. The postoperative period was uneventful and the tongue regained vitality and normal motion. There was just a slight retraction. Replantation of the tongue after complete section may not be successful but should be attempted when possible.

This study was a retrospective evaluation of the quality of malaria case management at health facilities in four districts conveniently sampled to represent both urban and rural settings in different epidemiological zones and health facility coverage. The review period was from January to December 2008. The sample included twelve lower level health facilities from four districts. The Pearson Chi-square test was used to identify characteristics which affected the quality of case management. Out of 4 891 suspected malaria cases recorded at the 12 health facilities, more than 80% of the patients had a temperature taken to establish their fever status. About 67% (CI 95 66.1-68.7) were tested for parasitemia by either rapid diagnostic test or microscopy, whereas the remaining 22.5% (CI 95 21.3.1-23.7) were not subjected to any malaria test. Hepatitis B virus is the primary cause, and Hepatitis B and C together are responsible for 60% of cirrhosis and 80% of hepatocellular carcinomas. This study measured the prevalence of HBsAg among Senegalese military to develop an appropriate strategy to prevent cirrhosis and hepatocellular carcinoma. We conducted a descriptive cross-sectional study among Senegalese military aged 25 to 60 years. A sample of 1224 participants was selected following a two-level-stratification. The mark of surface HBs antigen using chemiluminescence concerned 1195 participants. The presence of HBsAg was analyzed according to age, marital status, alcohol consumption and glomerular filtration rate. Epi-info6fr and R software were used, respectively, for data capture and analyses. A Chi-square test was performed to compare proportions considering a significance level of 5% and a confidence interval of 95%. The average age was 39.8 ± 9.2 years. Participants in the age groups of 25-34 years, 45-60 years and 35-44 years were respectively 30.7%, 34.4% and 34.9% of the sample. Married persons represented 82.6% of participants and 17.08% were single. Most participants were educated (99%), and 56% had reached at least secondary school level. Alcohol consumption was at 11.5%. The HBsAg prevalence rate was 10.8% [9.1% to 12.7%] with a significant difference between age groups (P < 0.001), which ranged from 5.6% for 45-60 years, 9.62% for 25-34 years to 16.9% for 35-44 years. Marital status and alcohol consumption did not affect the carriage of HBsAg. HBsAg prevalence was more common among participants who had a glomerular filtration rate greater than 90 ml/min. Transaminases rate exceeded the normal threshold for 43 participants (3.6%); the increase was 6.6% [2.7% to 11.8%] for HBsAg carriers and 3.2% [1.2% to 6.7%] for alcohol users. The high prevalence of HBsAg in the military requires the implementation of an effective prevention and care program to reduce the risk of cirrhosis and hepatocellular carcinoma and contribute to reducing the burden of communicable diseases, such as hepatitis and HIV/AIDS, and non-communicable diseases.

Cannabis (Cannabis sativa) is the most frequently used drug worldwide. It has multiple dangers, r... more Cannabis (Cannabis sativa) is the most frequently used drug worldwide. It has multiple dangers, related to its power to involve abuse and dependency phenomena and to their social implications. Our study, which was carried out on a representative sample of 446 students living on the Dakar campus, aimed at measuring the prevalence of Cannabis use and at describing the main factors associated with it. We found a prevalence rate of 19.7%. Cannabis use starts relatively early, around 16-17 years. Young users are generally initiated by a close friend or relative and are motivated by curiosity for their first experience. The fact of having a grant or not and the field of study have no influence on the use of cannabis. On the other hand, religion might play a determining role, Christians seeming to be more affected than Moslems (p = 0.026). A similar prevalence among students has been noted in Kenya, but the rate is definitely lower than those found in developed countries. The identification of cannabis use predictors would make it possible to consider interesting prevention perspectives based on targeted education and on a more adequate legislation. .

Patients et méthodes: Il s'agit d'une étude rétrospective allant de juin 1994 à Juin 2004 portant sur les cas des hernies inguinales admis dans le seul service de Chirurgie Pédiatrique du Sénégal. Les dossiers de 135 enfants présentant une hernie inguinale étranglée ont été revus. L'âge, le sexe, la prévalence de l'étranglement, le délai de consultation, le mode de révélation de l'étranglement, les pathologies associées, la voie d'abord chirurgicale, le viscère hernié, les gestes pratiqués ainsi que les suites opératoires ont été analysés.

Le diagnostic était suspecté par la persistance d'une fistule cutanée productive en regard de la clavicule gauche, l'existence d'un fragment osseux extériorisé et confirmé par la radiographie standard de l'épaule. La bactériologie était négative et la vitesse de sédimentation accélérée. Le traitement était chirurgical et consistait en une séquestrectomie de la totalité de la clavicule laissant en place l'involucrum. L'évolution était favorable en trois semaines avec disparition du syndrome inflammatoire et reconstitution de la clavicule.

Cannabis (Cannabis sativa) is the most frequently used drug worldwide. It has multiple dangers, r... more Cannabis (Cannabis sativa) is the most frequently used drug worldwide. It has multiple dangers, related to its power to involve abuse and dependency phenomena and to their social implications. Our study, which was carried out on a representative sample of 446 students living on the Dakar campus, aimed at measuring the prevalence of Cannabis use and at describing the main factors associated with it. We found a prevalence rate of 19.7%. Cannabis use starts relatively early, around 16-17 years. Young users are generally initiated by a close friend or relative and are motivated by curiosity for their first experience. The fact of having a grant or not and the field of study have no influence on the use of cannabis. On the other hand, religion might play a determining role, Christians seeming to be more affected than Moslems (p = 0.026). A similar prevalence among students has been noted in Kenya, but the rate is definitely lower than those found in developed countries. The identification of cannabis use predictors would make it possible to consider interesting prevention perspectives based on targeted education and on a more adequate legislation. .

Patients et méthodes: Il s'agit d'une étude rétrospective allant de juin 1994 à Juin 2004 portant sur les cas des hernies inguinales admis dans le seul service de Chirurgie Pédiatrique du Sénégal. Les dossiers de 135 enfants présentant une hernie inguinale étranglée ont été revus. L'âge, le sexe, la prévalence de l'étranglement, le délai de consultation, le mode de révélation de l'étranglement, les pathologies associées, la voie d'abord chirurgicale, le viscère hernié, les gestes pratiqués ainsi que les suites opératoires ont été analysés.

Nous rapportons un cas d'ostéomyélite chronique de la clavicule chez un garçon de cinq ans, dû à ... more Nous rapportons un cas d'ostéomyélite chronique de la clavicule chez un garçon de cinq ans, dû à un traumatisme par chute de cheval. Le diagnostic était suspecté par la persistance d'une fistule cutanée productive en regard de la clavicule gauche, l'existence d'un fragment osseux extériorisé et confirmé par la radiographie standard de l'épaule. La bactériologie était négative et la vitesse de sédimentation accélérée. Le traitement était chirurgical et consistait en une séquestrectomie de la totalité de la clavicule laissant en place l'involucrum. L'évolution était favorable en trois semaines avec disparition du syndrome inflammatoire et reconstitution de la clavicule.
