Thus, maintaining ER homeostasis is essential for cellular functions. Several pathophysiological conditions and pharmacological agents are known to disrupt ER homeostasis, thereby, causing ER stress. The cells react to ER stress by initiating an adaptive signaling process called the unfolded protein response (UPR). However, the ER initiates death signaling pathways when ER stress persists. ER stress is linked to several diseases, such as cancer, obesity, and diabetes. Thus, its regulation can provide possible therapeutic targets for these. Current evidence suggests that chronic hyperglycemia and hyperlipidemia linked to type II diabetes disrupt ER homeostasis, thereby, resulting in irreversible UPR activation and cell death. Sleep is an integral part of human physiology; therefore, disorders of sleep may result in signif... more Sleep is an integral part of human physiology; therefore, disorders of sleep may result in significant derangement in human functionality. Sleep medicine has received little attention in Nigeria. Against this backdrop, this survey seeks to evaluate the quality of sleep among doctors. This was a cross-sectional descriptive study involving 59 doctors working with Federal Medical Centre Birnin Kebbi, Kebbi State, Nigeria; it was done between August 2017 and December 2017. Purposive sampling method was adopted. The Pittsburgh Sleep Quality Index (PSQI) and the Epworth Sleepiness Scale (ESS) were adopted. The questionnaire was self-administered. There were 34 (57.6%) males and 25 (42.4%) females. Their age ranged from 20 to 66 years, with a mean of 34.39 ± 8.00. The Epworth score ranged from 8 to 29, with a mean of 16.1 ± 4.4, while the PSQI score ranged from 5 to 19, with a mean of 9.5 ± 2.7. The objective of this study is to determine the prevalence of elevated mPAP in children with CKD compared with matched controls and to ascertain the relationship between elevated mPAP with right ventricular dysfunction and history of hemodialysis. A cross-sectional comparative study of mPAP and tricuspid annular plane systolic excursion of 21 children with CKD and age- and sex-matched controls asymptomatic for cardiac disease was conducted. Median mPAP was 27.69 (18.3-36.1) mmHg in CKD patients compared with 14.55 (13.5-17.1) mmHg in controls ( = 0.002). Elevated mPAP was present in 42.9% of CKD group and 0% in controls ( < 0.001). There may be other cardiac defects associated with it which are mostly right-sided cardiac defects such as atrial septal defect, Ebstein anomaly, and pulmonary stenosis. However, left-sided heart anomaly such as aortic regurgitation has been associated with it, but its association with transposition of the great vessels has not been documented before now. Therefore, the case of a 3-month-old girl with cor-triatriatum dexter with transposition of the great vessels, and atrial septal defect is reported.

Background: The blood group refers to the blood grouping system and their specificity which is ge... more Background: The blood group refers to the blood grouping system and their specificity which is genetically determined; however, the "blood phenotype" describes the reactivity of blood to specific testing antiserum. Although racial and regional variations have been established in the prevalence of the blood groups, most of these studies were done among blood donors, but the blood transfusion recipients who actually need these transfusions have been poorly studied. This study seeks to determine the common blood groups encountered among blood transfusion recipients in Federal Medical Centre (FMC), Birnin Kudu. Materials and Methods: This study was a retrospective analysis of the blood grouping pattern among blood transfusion recipients in FMC, Birnin Kudu over a 2-year period from January 2011 to December 2013. Blood grouping is determined commonly using venous blood through tile agglutination method. Standard commercially produced anti-A, anti-B monoclonal grouping reagents and anti-D sera are used for the tests and performed at room temperature. Evidence of agglutination is substantiated microscopically after waiting for 2 min. Results: There were 4129 blood requests during the study period; however, 28 of the entries were excluded due to grossly incomplete data. Among the 4101 entries analyzed, there were 1206 (29.4%) males and 2895 (70.6%) females; male/female ratio of 1:2.4. Blood group O (44.1%) was the most common blood group whereas blood group AB (5.9%) was the least; however, majority (96.2%) of the recipients were rhesus positive. Conclusion: Group O and rhesus positive antigens remain the most common blood group distribution among blood transfusion recipients.

It... more The term pyrexia of unknown origin (PUO) was first described by Petersdorf and Beeson in 1961. It is characterized by a temperature of more than 38.3°C on several occasions lasting for more than 3 weeks or for more than 1 week of inpatient investigation. The etiology of PUO is extensive but could be broadly classified into infectious and noninfectious causes. Infection accounts for 34% of cases; the case of a 6-month-old boy who presented with fever of 2 months duration is reported. He had series of investigations without the etiology identified. Furthermore, he was placed on empiric antibiotics on admission but the fever persisted; however, he was discharged after 3 weeks; and while on follow-up visit, the fever resolved. This report highlights how bizarre PUO may present.

These patients are at risk of cardiac tamponade. Apart from pericardiocentesis in the management of these patients, definitive drug treatment may pose a formidable challenge in a setting of coinfection as in patients with tuberculosis who are predisposed to secondary bacterial infections. This was the case of our patient. We here highlight the challenges faced with etiologic diagnosis in resource-limited settings.

A 4-year-old boy was referred to our pediatric emergency unit with the complaint of unintentional... more A 4-year-old boy was referred to our pediatric emergency unit with the complaint of unintentionally ingesting 6 tablets of (200 mg tablets) tramadol an hour before presentation. He was given lemon drink at home before presenting to the emergency unit. He vomited several times and convulsed twice on the way to the referring hospital and lapsed into unconsciousness. On examination, he was unconscious with Glasgow coma scale score of 3/15, and pupils were 1 mm in size bilaterally, respiratory rate of 19/min with oxygen saturation of 85% (in room air); the pulse rate was 110 b/ min, blood pressure-90/60 mmHg. He was hypoglycemic with random blood sugar (RBS) of 2.0 millimoles/liter. Electrocardiogram showed normal sinus rhythm, and other laboratory investigations, which included full blood count, electrolytes, urea and creatinine and liver function tests were normal. Hypoglycemia and hypoxia were corrected with dextrose infusion (RBS-5.5 mm/L) and intranasal oxygen (SPO 2 96%) respectively. However, approximately 20% of hospital admissions and 10% of hospital deaths in Nigeria have been attributed to malaria. This secondary data analysis was conducted to examine the pattern severe malaria in Plateau State in the face of declining global malaria infection. Severe malaria-specific Integrated Disease Surveillance and Response (IDRS) records of Plateau State of Nigeria over a five-year period were reviewed. A total of 38,467 cases of severe malaria were reported between January 2013 and December 2017. The highest number of cases 14098 (36.65%) was reported in 2016 and the least number of cases 950 (2.47%) were reported in 2014. A total of 362 severe malaria deaths was reported within the same period with a case fatality rate (CFR) of 0.94%. The highest CFR 1.43% was recorded in 2015 while the least CFR 0.00% was recorded in 2014. The 0-28 days age group had the highest CFR (3.13%). The comparative monthly trend of severe malaria cases did not follow any consistent pattern during the 5 years under review. However, as of 2017, the trends of total cases per year and CFRs were on the decline. In conclusion, the overall number of cases and deaths of severe malaria is declining in Plateau State but the CFR among neonates remains high. Therefore, prevention and control efforts should be intensified in Plateau State, in order to achieve malaria elimination in the State.

Prevalence of thrombocytopenia in congenital cyanotic heart disease is not known, and decreased platelet production due to right to left shunting of megakaryocytes away from the lung has also been implicated as a possible cause. [4,5] Furthermore, they are prone to infective endocarditis and coupled with right to left shunting of infected thrombi which may embolize to body organs including the brain resulting in cerebral abscess and mycotic aneurysm. His hematocrit still remained high despite the B-T shunt because the B-T shunt was now inadequate for his age. Availability of definitive surgery in our setting will certainly improve his quality of life and survival.

Although racial and regional variations have been established in the prevalence of the blood groups, most of these studies were done among blood donors, but the blood transfusion recipients who actually need these transfusions have been poorly studied. This study seeks to determine the common blood groups encountered among blood transfusion recipients in Federal Medical Centre (FMC), Birnin Kudu. Materials and Methods: This study was a retrospective analysis of the blood grouping pattern among blood transfusion recipients in FMC, Birnin Kudu over a 2-year period from January 2011 to December 2013. Blood grouping is determined commonly using venous blood through tile agglutination method. Standard commercially produced anti-A, anti-B monoclonal grouping reagents and anti-D sera are used for the tests and performed at room temperature. Evidence of agglutination is substantiated microscopically after waiting for 2 min. Results: There were 4129 blood requests during the study period; however, 28 of the entries were excluded due to grossly incomplete data. Among the 4101 entries analyzed, there were 1206 (29.4%) males and 2895 (70.6%) females; male/female ratio of 1:2.4. Blood group O (44.1%) was the most common blood group whereas blood group AB (5.9%) was the least; however, majority (96.2%) of the recipients were rhesus positive. Conclusion: Group O and rhesus positive antigens remain the most common blood group distribution among blood transfusion recipients.

It... more The term pyrexia of unknown origin (PUO) was first described by Petersdorf and Beeson in 1961. It is characterized by a temperature of more than 38.3°C on several occasions lasting for more than 3 weeks or for more than 1 week of inpatient investigation. The etiology of PUO is extensive but could be broadly classified into infectious and noninfectious causes. Infection accounts for 34% of cases; the case of a 6-month-old boy who presented with fever of 2 months duration is reported. He had series of investigations without the etiology identified. Furthermore, he was placed on empiric antibiotics on admission but the fever persisted; however, he was discharged after 3 weeks; and while on follow-up visit, the fever resolved. This report highlights how bizarre PUO may present.

He was given lemon drink at home before presenting to the emergency unit. He vomited several times and convulsed twice on the way to the referring hospital and lapsed into unconsciousness. On examination, he was unconscious with Glasgow coma scale score of 3/15, and pupils were 1 mm in size bilaterally, respiratory rate of 19/min with oxygen saturation of 85% (in room air); the pulse rate was 110 b/ min, blood pressure-90/60 mmHg. He was hypoglycemic with random blood sugar (RBS) of 2.0 millimoles/liter. Electrocardiogram showed normal sinus rhythm, and other laboratory investigations, which included full blood count, electrolytes, urea and creatinine and liver function tests were normal. Hypoglycemia and hypoxia were corrected with dextrose infusion (RBS-5.5 mm/L) and intranasal oxygen (SPO 2 96%) respectively. However, approximately 20% of hospital admissions and 10% of hospital deaths in Nigeria have been attributed to malaria. This secondary data analysis was conducted to examine the pattern severe malaria in Plateau State in the face of declining global malaria infection. Severe malaria-specific Integrated Disease Surveillance and Response (IDRS) records of Plateau State of Nigeria over a five-year period were reviewed. A total of 38,467 cases of severe malaria were reported between January 2013 and December 2017. The highest number of cases 14098 (36.65%) was reported in 2016 and the least number of cases 950 (2.47%) were reported in 2014. A total of 362 severe malaria deaths was reported within the same period with a case fatality rate (CFR) of 0.94%. The highest CFR 1.43% was recorded in 2015 while the least CFR 0.00% was recorded in 2014. The 0-28 days age group had the highest CFR (3.13%). The comparative monthly trend of severe malaria cases did not follow any consistent pattern during the 5 years under review. However, as of 2017, the trends of total cases per year and CFRs were on the decline. In conclusion, the overall number of cases and deaths of severe malaria is declining in Plateau State but the CFR among neonates remains high. Therefore, prevention and control efforts should be intensified in Plateau State, in order to achieve malaria elimination in the State.

Prevalence of thrombocytopenia in congenital cyanotic heart disease is not known, and decreased platelet production due to right to left shunting of megakaryocytes away from the lung has also been implicated as a possible cause. [4,5] Furthermore, they are prone to infective endocarditis and coupled with right to left shunting of infected thrombi which may embolize to body organs including the brain resulting in cerebral abscess and mycotic aneurysm. His hematocrit still remained high despite the B-T shunt because the B-T shunt was now inadequate for his age. Availability of definitive surgery in our setting will certainly improve his quality of life and survival.
