Purpose Patients undergoing major orthopedic surgeries, such as total hip replacement (THR), tota... more Purpose Patients undergoing major orthopedic surgeries, such as total hip replacement (THR), total knee replacement (TKR), and trauma surgery, are at an elevated risk of venous thromboembolism (VTE), causing significant morbidity and mortality. Previous studies have investigated aspirin as a thromboprophylactic agent for arthroplasty, besides trauma surgery. Therefore, we sought to analyze the efficacy of aspirin compared to that of other anticoagulants for VTE prophylaxis in patients undergoing major orthopedic surgeries. Methods This study was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The study protocol was registered with the PROSPERO register. Randomized controlled trials that investigated the use of aspirin for thromboprophylaxis in major orthopedic lower limb surgeries were included and analyzed. Microscopic decompression (MD) has been widely used as an alternative to open decompression. Lately, biportal endoscopic spinal surgery (BESS) - a new approach in minimal-invasive spinal surgery – has also been used with good results. Although both groups can achieve adequate lumbar decompression, there is still a lack of evidence regarding their comparison. We aim to compare the outcomes of both techniques in a one-year follow-up. Methods. This is a retrospective study in 100 consecutive patients with symptomatic lumbar spine compression due to herniated nucleus pulposus (HNP) and lumbar canal stenosis (LCS) that was treated by either BESS or MD. Clinical evaluations using Visual Analog Score (VAS), Oswestry Disability Index (ODI), and SF-36 questionnaire were obtained. In this way, the segments will be brought into alignment with the other vertebrae with the pedicle mats being tightened. The aim of this study is to acknowledge whether reduction surgery for degenerative lumbar spondylolisthesis (DLS) using long arm pedicle screws is able to correct the listhesis and spinopelvic parameters. Methods: We carried out a retrospective study of patients with degenerative lumbar spondylolisthesis who went through reduction surgery using long arm pedicle screws in our institutions from January 2019 to March 2022. Preoperative and postoperative radiological outcomes consisting of listhesis and spinopelvic parameters were assessed. Results: We found a statistical difference between the magnitude of listhesis immediately after surgery and preoperatively (p<0.001), with a successful correction of 85.85%. There was significant decrease in the value of pelvic tilt (p=0.044) and increase in the value of sacral slope (p=0.008) after surgery. Conclusion: Reduction surgery using long arm pedicle screws for DLS was able to reduce the listhesis effectively up to 85.5%, and also to restore the parts of spinopelvic parameters, the pelvic tilt and sacral slope, approaching normal values.

The study aimed to identify the practice of servant leadership of school principals in Kuwait fro... more The study aimed to identify the practice of servant leadership of school principals in Kuwait from their teachers' perspectives and its relation to their job satisfaction. The current study followed the descriptive approach. The tool of this study is the Organizational Leadership Assessment (OLA) which consisted of (41) items. The study sample included (794) teachers in public schools in Kuwait. The study results are as following: Teachers' perspectives toward servant leadership practiced is in moderate levels. Based on six categories of servant leadership, building community ranked first, followed by providing leadership, valuing others, developing people, displaying authenticity, and shares leadership. Teachers expressed a high level of job satisfaction. A statistically significant positive correlation was found between school principal servant leadership practices from teachers' perspectives and teachers' job satisfaction. No differences were found in teachers' perspectives toward school principals servant leadership due to their years of experiences.

The purpose of this study was to evaluate the feasibility of the virtual clinic for outpatient follow-up care after TKR surgery. Methods. A total of 546 TKR surgeries were performed from January 2017 to December 2019. 30 patients were not able to go the hospital for routine follow-up. The data collections were taken for age, gender, year of surgery, functional score (Oxford Knee Score (OKS)), and active range of motion (ROM). The virtual clinic was conducted with the physician assistant and the operating surgeon via WhatsApp video call (WhatsApp Inc., Mountain View, California, USA) or Zoom (Zoom Video Communications, Inc., San Jose, California, USA). Result. The average follow-up period was 39.1 months. The earliest follow-up was 20 months, while the longest follow-up was 97 months. The average OKS score was 45. An accurate and secured pedicle screw placement as part of the correction tools is essential to accommodate reduction while preventing screw-related complications. Correction is not always easy as the structural anatomy differs from the general population. We present a female 23 years old with chief complain of curved back since 10 years before admission. The patient had high stature and positive Marfan syndrome’s criteria. The patient also complained of shortness of breath. The curvature was progressive becoming more severe over time. In radiographs we found that the main thoracic curve Cobb’s angle of 920 and main lumbar curve Cobb’s angle 760. We performed scoliosis correction with pedicle screws and rod by posterior approach. Intra-operative blood loss was 1900 cc. Postoperatively, the patient had weakness at the lower limbs but resolved after several days and gained full motoric function after one week. Posterior-only approach with rod and screw corrective manipulation to add strength of contra bending manipulation has correction achievement similar to that obtained by conventional combined anterior release and posterior approach. It also avoids the complications related to the thoracic approach. We reported a case of 25-year-old male adult idiopathic scoliosis with double curve. It consists of main thoracic curve of 150 degrees and lumbar curve of 89 degrees. The clinical implication is multiplied: avoiding anesthesia complication in SCFE surgical treatment, assessing the need of prophylactic pinning at contralateral hip, and early detection of underlying disease. This case report will discuss the lessons learned from the management of endocrine-related SCFE with coexisting pituitary tumor, along with the long-term clinical outcome. Methods: A 17-year-old male was diagnosed with unstable SCFE and underwent in situ pinning of the femoral head using 3 cannulated screws. Results: Hormonal examination showed hypogonadism, hyperprolactinemia, and growth hormone deficiency. Brain scan displayed macroadenoma of pituitary gland.
