We report a multicentric prospective study which aimed to evaluate Doppler-assisted ligation of t... more We report a multicentric prospective study which aimed to evaluate Doppler-assisted ligation of the terminal haemorrhoidal arteries (THD) for II and III degree haemorrhoids. A total of 112 patients from five colorectal units, including 81 men, mean age 48 +/- 13 years, with II degree (39) and III degree (73) haemorrhoids were treated by Doppler-guided transanal de-arterialization and anopexy using a new device (THD). The mean operative time was 33.9 +/- 8.8 minutes, and the mean number of ligatures applied was 7.2 +/- 1.5. Postoperatively, 72% of patients did not need analgesics and the other 28% used nonsteroidal antiinflammatory drugs 1-3 times/day for less than 2 days. All the patients were operated as a day case. Early postoperative complications included haemorrhoidal thrombosis (2 patients), bleeding (1) treated by haemostatic suture, dysuria (6) and acute urinary retention (1). After a mean follow-up of 15.6 +/- 6.5 months (range 6-32), 2/105 (20.9%) patients complained of minor bleeding, while mild pain was still present in 4/51 patients (7.8%). There were no statistically significant differences in the sample population regarding the gender or stage of the disease. Tenesmus was cured in 15/17 patients, dyschaezia in 20/22 patients and mucous soiling in 10/10 patients. No new cases of altered defaecation or faecal incontinence were recorded. Overall, 85.7% of patients were cured and 7.1% improved. Residual haemorrhoids were treated by elastic band ligation in nine (8%) patients and by surgical excision in further five patients (4.5%). Doppler-assisted ligation of the terminal branches of the haemorrhoidal arteries for II and III degree haemorrhoids is highly effective and painless. Complications are few and the technique can be performed as a day case.

The authors investigated the effect of two extrahepatic cholestasis models (one by bile duct ligation and the other by choledocho-jugular fistula) on the hepatic clearance of horseradish peroxidase in male Sprague-Dawley rats divided into four groups. 2. In groups A (n = 5 rats) and B (n = 5), bile duct ligation was performed, while a choledocho-jugular fistula was created in groups C (n = 5) and D (n = 7). A 10 mg intravenous bolus of horseradish peroxidase was injected after 24 h (groups A and C), 48 h (groups B and D) or 1 h (Group E; five sham-operated rats). Serum and bile samples were then serially collected for 2 h. 3. In all groups, serum horseradish peroxidase levels increased soon after injection and then rapidly decreased, the curves being similar. Biliary excretion increased for 30 min and then slowly decreased. The highest horseradish peroxidase biliary concentrations and outputs were found in Group B followed by Group A; both groups had significantly higher levels than Group E. No difference was found between horseradish peroxidase biliary excretion of groups C and D and that of sham-operated rats. 4. When each group was considered separately, sampling times correlated with the corresponding ratios of bile/plasma HRP. Significant differences were found between the relative slopes of groups A, B and E, but not between those of groups C, D and E. 5. In conclusion, bile duct obstruction greatly affects the plasma-bile transfer of fluid phase markers, such as horseradish peroxidase, while single retention, caused by choledocho-jugular fistula, has no influence. The increased biliary hyperpressure related to the duration of cholestasis may account for the degree of horseradish peroxidase transfer which, in turn, probably depends on an enhanced paracellular passage.

As described in a recent study carried out in Connecticut [1], the incidence rate is 1.9 in males and 2.3 in females and is lower in white males (0.41/100000) and higher in black females (0.74/100000). The increased incidence rate has also been confirmed by a study on three cancer registers in Great Britain where it is emphasized that situations at risks are unmarried status, homosexual behavior, immunodeficiency syndromes and immunosuppression [2].

Methods We conducted a nationwide survey among members of the Italian Society of Colo-Rectal Surgery (SICCR) on the use of radiation therapy for rectal cancer in the year 2005. Demographic, clinical and pathologic data were retrospectively collected with an online database. Italy was geographically divided into 3 regions: north, center and south which included the islands. Hospitals performing 30 or more surgeries per year were considered high volume. Factors related to radiotherapy delivery were identified with multivariate analysis. Results Of 108 centers, 44 (41%) responded to the audit. We collected data on 682 rectal cancer patients

I present the Colo-Shower | (Sapi-Med, Alessandria, Italy), a modified, disposable proctoscope for intraoperative rectal irrigation that, by collecting liquid flowing out of the anus during rectal washout, minimizes operating theater pollution. The Colo-Shower | which has now been used on 100 patients who underwent ileorectal or colorectal anastomosis, is effective and easy to use.

The possible beneficial effect of reduced glutathione infusion on acute reflux pancreatitis was i... more The possible beneficial effect of reduced glutathione infusion on acute reflux pancreatitis was investigated in five groups of rats (8 each). In groups A, A’ and B, B’ acute pancreatitis was induced by means of a closed duodenal loop; group C was sham operated. In groups B and B’ reduced glutathione was infused at a flow rate of 0.2 ml/h (1,600 mg/kg/24 h) starting 2 h before the induction of pancreatitis and continuing for the subsequent 12 h. Groups A and A’ received equal volumes of saline solution. A, B and C rats were sacrificed 24 h after the induction of pancreatitis: serum amylase, AST, ALP and lipid peroxides together with pancreatic and liver histology were determined. A’ and B’ rats were observed for up to 15 days to evaluate survival. Serum amylase and liver function tests were higher in saline treated than in sham operated rats; while glutathione treated rats had lower values than saline treated, the differences were not significant. Lipid peroxides were not found to be significantly different among the three groups. Both groups with acute pancreatitis had pancreatic histological scores higher than sham-operated rats, with no significant difference between saline and glutathione-treated rat. Histological liver inflammation was a little more pronounced in acute pancreatitis rats, especially when they received only saline. It may be concluded that reduced glutathione infusion does not exert a substantial beneficial effect in the rat with acute reflux pancreatitis, probably due to the minor role played by oxygen-derived free radicals in the pathogenesis of this experimental model. Reduced glutathione has a protective effect on the liver; it is probably through this action that reduced glutathione seems to have some positive, although not substantial, effects in acute reflux pancreatitis induced by closed duodenal loop in the rat.Copyright © 1993 S. However, studies on this topic are underpowered and retrospective. This randomized trial evaluated whether the incidence of anastomotic leakage was reduced after colonic J pouch reconstruction compared with straight colorectal anastomosis following anterior resection for rectal cancer. Methods This multicentre RCT included patients with rectal carcinoma who underwent low anterior resection followed by colorectal anastomosis. Patients were assigned randomly to receive a colonic J pouch or straight colorectal anastomosis. The main outcome measure was the occurrence of major anastomotic leakage. The incidence of global (major plus minor) anastomotic leakage and general complications were secondary outcomes. Risk factors for anastomotic leakage were identified by regression analysis. The isolation of Fusarium fujikuroi from rice plants with elongated stems typical of bakanae dise... more The isolation of Fusarium fujikuroi from rice plants with elongated stems typical of bakanae disease in the Lao PDR is reported for the first time. The identification was based on both molecular and morphological markers. Koch's postulates were fulfilled. Bakanae disease has long been recognized in the Lao PDR based on symptomatology, but the etiology of the disease has not been clearly defined. This study was part of a continuing program to assist with the development of checklists of pathogens and plant diseases for Lao PDR for biosecurity purposes and integrated disease management.
