Study Design: The study used a descriptive cross-sectional study design. Place and Duration of Study: Uyo capital city territory, Akwa Ibom State, Nigeria from August 1 st to 30 th , 2019. Methodology: We recruited 42 butchers (all males; mean age 25+/-6 years) from an abattoir, 2 slaughter slabs and meat processing sites on 2 major streets in Uyo capital city territory. Data collection was done using interviewer administered questionnaires and interview guide. Data was analyzed with Stata statistical software version 10.0.

Background: The breast is a common site for pathologies which predominantly involves the female b... more Background: The breast is a common site for pathologies which predominantly involves the female breast. While benign diseases are more common, malignancies are of utmost concern for the patient, clinician, and the pathologist. Aim: The aim of this study is to audit the clinical and pathological features of patients with breast lumps in a tertiary center in Nigeria. Materials and Methods: This is a retrospective observational study of histologically diagnosed breast lumps over a period of 8 years. Results: Lumps were common in the 20-39 years age group (>50%), with a mean age of 34.95 years. Females accounted for 759 (98.1%) of cases. In 467 (60.4%) cases, the lumps measured >5 cm and only 175 (22.6%) cases presented to the surgeon within 12 weeks of noticing a breast lump. Ninety-one (11.8%) cases also presented with axillary lymph nodes, breast pain 79 (10.2%), peau d' orange 56 (7.2%), and ulceration 47 (6.1%). Benign breast disease (BBD) occurred in 401 (51.8%) cases and malignant breast diseases (MBD) occurred in 344 (44.3%) of cases. Conclusion: BBD and MBD are common among the young in Uyo. Lumps are the primary complaint and late presentation is the norm.

These hernias are rare in our practice, and a definitive preoperative diagnosis is often not made as the symptoms are not specific. Early assessment and prompt and adequate resuscitation and surgery obviate the risk of strangulation and intestinal resection. This report highlights a rare cause of intestinal obstruction in a young male who presents with all the classical features of obstruction: colicky abdominal pain, distension, vomiting, and inability to pass stool or flatus. The diagnosis of paraduodenal hernia was made intraoperatively. We do not routinely request for barium examination or abdominal computed tomography scan in acute abdominal pain. These modalities can suggest a preoperative diagnosis.

Impalement presents a combination of injuries severe enough to cause the homeostatic mechanisms t... more Impalement presents a combination of injuries severe enough to cause the homeostatic mechanisms to malfunction or fail. It challenges the abilities of the managing team as well as the available medical resources. Its management is even more demanding in environments where health-care personnel, facilities for emergency care, and communications infrastructure are absent or poorly funded and equipped. The primary objective of managing impalement injuries is to prevent further injuries and preserve or restore homeostasis. Leaving the impaling object in situ until in the operating room is a time proven approach as it allows for managing life-threatening hemorrhage in the operating room. Adequate volume replacement, good visualization, debridement of injuries, and anti-infective measures are complementary. We present a case of type I abdominal impalement in a young man who sustained minimal visceral or neurovascular injuries and underwent exploratory laparotomy for his injuries; he developed a traumatic incisional hernia. He defaulted on the scheduled hernia repair.

The use of prosthetic biomaterials for reconstructing and reinforcing the posterior wall of the i... more The use of prosthetic biomaterials for reconstructing and reinforcing the posterior wall of the inguinal canal reduces the incidence of hernia recurrence. Cost, availability of mesh, and perhaps reluctance to adopt a new technique are factors which prevent widespread practice of hernioplasty in low-resource settings. Use of resterilized mesh significantly reduces the cost of hernioplasty and is safe. Sheets of 30 cm × 30 cm polypropylene mesh were cut into 16 cm × 8 cm to produce mesh strips which were repackaged into SELFSEAL (Medical Action Industries Inc., USA) sterilizing pouches measuring 90 mm × 230 mm and autoclaved. At repair, the strips are shaped to fit the anatomy of the posterior wall of the inguinal canal, a slit created at one end and applied in Lichtenstein repair of inguinal hernias. A rare complication of ilioinguinal nerve block is transient femoral nerve palsy (TFNP). The aim of this study is to examine the incidence of TFNP among adults undergoing ambulatory inguinal hernia repair under ilioinguinal nerve block. Patients 18 years and older in the American Society of Anesthetists classes I and II who underwent ambulatory inguinal hernia repair over a 3-year period under ilioinguinal nerve block only were assessed for evidence of TFNP. It is rare in our practice, and its diagnosis may be delayed if the index of suspicion is low among emergency room physicians. Imaging studies can easily confirm the diagnosis of diaphragmatic rupture. Herniation of organs and viscera through the diaphragmatic defect occurs commonly, and same may obstruct or strangulate and worsen the outcome. We present two adults with complaints of severe acute abdominal pain and a history of high-speed vehicular trauma whose definitive diagnoses were delayed. The presence of air-fluid levels in the left hemithorax on a chest radiogram and a history of blunt abdominal injury made us arrive at the diagnosis of intestinal obstruction secondary to traumatic diaphragmatic hernia.

The patient developed an ulceration of the anterior abdominal wall necessitating prolonged hospitalization. She was delivered by emergency lower segment cesarean section at 35 weeks gestational age because of premature labor. The patient unfortunately died from primary postpartum hemorrhage.

Background: Cancer patients in Nigeria are known to present late and have poor awareness of their... more Background: Cancer patients in Nigeria are known to present late and have poor awareness of their disease. Among the factors which limit early presentation are aversion for orthodox medicine due to perceived complexity of health institutions, and belief in superiority of traditional medicine. Patient navigation has been shown to improve patient treatment adherence. There is limited evidence on the impact of PN in Nigeria, and other resource-limited settings. Aim: Study sought to introduce a hospital-based patient navigation program (PNP) and evaluate its impact. Methods: PNP was initiated at the University of Uyo Teaching Hospital in June 2016 with focus on breast cancer patients. A young, male public health officer, who spoke the local language was recruited and trained to guide cancer breast patients through the hospital. Training focused on hospital services, sensitive communications, patient management process, confidentiality and resource identification. The service was announced to the hospital community, and his contact information was advertised. Patients and hospital community could contact the navigator via phone calls and text messaging. Impact data collection included services provided, patient characteristics and feedback on services. Data analysis included descriptive statistics and thematic analysis. Results: In the first year of the PNP (June 2016-17), 202 breast cancer cases were seen, and 137 connected with the navigator; service utilization rate was 67.8% (137/202). More patients in the PNP received diagnosis within 90 days of their first visit, 58.4% (80/137) presented with stage 4 disease, and 20 had mastectomy. Most patients had low income and low education. Two patients died after completing three courses of chemotherapy. Finance, transportation and poor social support were the most common barriers to access. Health care providers positively reviewed the PNP, often saying that “it made their work” and “added value to hospital care and cancer management”. Physicians who did not speak the local language often used the navigator as a translator. Most patients echoed “God bless you” in prayers offered to appreciate the navigator. They agreed that the PNP reduced some barriers to hospital service utilization, although 32 patients (23%) expressed frustration over unmet expectations financial support from the navigator. Physicians who manage other cancers have started referring patients to the PNP since its initiation. Conclusion: This evaluation showed that PNP program was well used and led to improvements in breast cancer patient management. It proves that patient navigation is feasible in a resource-limited setting. Funding to sustain and expand the program is a limitation. More patient satisfaction and outcome studies are needed to evaluate PNP impact.

The patient developed an ulceration of the anterior abdominal wall necessitating prolonged hospitalization. She was delivered by emergency lower segment cesarean section at 35 weeks gestational age because of premature labor. The patient unfortunately died from primary postpartum hemorrhage.

Among the factors which limit early presentation are aversion for orthodox medicine due to perceived complexity of health institutions, and belief in superiority of traditional medicine. Patient navigation has been shown to improve patient treatment adherence. There is limited evidence on the impact of PN in Nigeria, and other resource-limited settings. Aim: Study sought to introduce a hospital-based patient navigation program (PNP) and evaluate its impact. Methods: PNP was initiated at the University of Uyo Teaching Hospital in June 2016 with focus on breast cancer patients. A young, male public health officer, who spoke the local language was recruited and trained to guide cancer breast patients through the hospital. Training focused on hospital services, sensitive communications, patient management process, confidentiality and resource identification. The service was announced to the hospital community, and his contact information was advertised. Patients and hospital community could contact the navigator via phone calls and text messaging. Impact data collection included services provided, patient characteristics and feedback on services. Data analysis included descriptive statistics and thematic analysis. Results: In the first year of the PNP (June 2016-17), 202 breast cancer cases were seen, and 137 connected with the navigator; service utilization rate was 67.8% (137/202). More patients in the PNP received diagnosis within 90 days of their first visit, 58.4% (80/137) presented with stage 4 disease, and 20 had mastectomy. Most patients had low income and low education. Two patients died after completing three courses of chemotherapy. Finance, transportation and poor social support were the most common barriers to access. Health care providers positively reviewed the PNP, often saying that “it made their work” and “added value to hospital care and cancer management”. Physicians who did not speak the local language often used the navigator as a translator. Most patients echoed “God bless you” in prayers offered to appreciate the navigator. They agreed that the PNP reduced some barriers to hospital service utilization, although 32 patients (23%) expressed frustration over unmet expectations financial support from the navigator. Physicians who manage other cancers have started referring patients to the PNP since its initiation. Conclusion: This evaluation showed that PNP program was well used and led to improvements in breast cancer patient management. It proves that patient navigation is feasible in a resource-limited setting. Funding to sustain and expand the program is a limitation. More patient satisfaction and outcome studies are needed to evaluate PNP impact.
