We worked with the account "@distritovisual" where graphic pieces of bachelor’s students of Graphic Design in a private university in Guayaquil city where exhibited. A mixed approach with an exploratory-descriptive-causal scope was applied, an interview with an expert on the subject and the application of a rubric for the review and quantification of the comments of those who visited the profile, later these were analyzed by students for the respective conclusion in the decision making on the pedagogical relevance of instagram as a tool of collaborative learning that involves didactic interaction empowering education through feedback.

RESUMEN La granulomatosis con poliangitis (granulomatosis de Wegener), es una vasculitis necrotiz... more RESUMEN La granulomatosis con poliangitis (granulomatosis de Wegener), es una vasculitis necrotizante sistémica. Es una patología rara, sin causa definida; afectando vasos sanguíneos de mediano y pequeño calibre, pulmones, tracto respiratorio, riñones, entre otros órganos. Presenta una incidencia anual de 2.4 a 11.3 casos por millón de habitantes aproximadamente, afectando tanto a hombres y mujeres por igual. Las manifestaciones clínicas son variadas, llegando a presentar signos y síntomas inespecíficos por lo que llega a ser difícil su diagnóstico. Se reporta el caso de un paciente varón de 53 años de edad, sin comorbilidades, con un cuadro clínico inicial de fiebre y dolor lumbar, cuyo diagnóstico inicial fue de una pielonefritis aguda. Sin embargo, finalizado el tratamiento el paciente seguía presentando fiebre de forma intermitente, motivo por el cual es hospitalizado, donde se le realizó distintos exámenes auxiliares entre los cuales se encuentran una biopsia de piel y c-ANCA, los cuales fueron clave para el diagnóstico definitivo, tratamiento adecuado y buena evolución del paciente. Palabras clave: Granulomatosis con poliangitis, Anticuerpo Anticitoplasma de Neutrófilo, vasculitis (DeCS) SUMMARY Granulomatosis with Polyangiitis (Wegener's Granulomatosis) is a systemic necrotizing vasculitis. It is a rare condition affecting small and medium sized vessels, the lungs, respiratory tract, kidneys and other organs. The annual incidence is approximately 2.4-11.3 cases/1,000,000 with the disease affecting men and women equally. Clinical presentation is variable, usually with unspecific signs and symptoms that make difficult diagnosis. Here we report a clinical case regarding a 53 years old male, without comorbidities, who presented with an initial clinical picture of fever and lumbar pain and whose initial diagnosis was acute pyelonephritis. However, on completion of treatment the patient continued presenting intermittent fever for which he was hospitalized. During hospitalization distinct auxiliary examinations were performed including a skin biopsy and c-ANCA which were key to determining the definitive diagnosis, providing adequate treatment and therefore favourable development of the patient's clinical condition.

The objective of the work is to demonstrate, through a bibliographic review, that there are intraindividual, personal, environmental, socioeconomic and motivational factors that relate to drug use in adolescents. The review also aims to demonstrate that cannabis use in adolescence will be a precursor to schizophrenia disease, finding differentiation between people with schizophrenia with a history of cannabis use and people with this disorder but no history of consumption.

Negative symptoms are core features of schizophrenia-spectrum disorders that are frequently obser... more Negative symptoms are core features of schizophrenia-spectrum disorders that are frequently observed across all phases of illness. By their nature, COVID-19 social isolation, physical distancing, and health precautions induce behavioural aspects of negative symptoms. However, it is unclear whether these prevention measures also lead to increases in experiential negative symptoms, whether such effects are equivalent across individual negative symptom domains, and if exacerbations occur equivalently across phases of illness. The current study compared negative symptom severity scores obtained during the pandemic to pre-pandemic assessments in two samples: (1) outpatients with chronic schizophrenia (SZ: n = 32) and matched healthy controls (CN: n = 31) and (2) individuals at clinical high risk for psychosis (CHR: n = 25) and matched CN (n = 30). Pre-pandemic ratings of negative symptoms were clinically elevated in SZ and CHR groups, which did not differ from each other in severity. In SZ, ratings obtained during the pandemic were significantly higher than pre-pandemic ratings for all 5 domains (alogia, blunted affect, anhedonia, avolition, and asociality) and item-level analyses indicated that exacerbations occurred on both experiential and behavioral symptoms of anhedonia, avolition, and asociality. In contrast, CHR only exhibited increases in anhedonia and avolition items during the pandemic compared to pre-ratings. Findings suggest that negative symptoms should be a critical treatment target during and after the pandemic in the schizophrenia spectrum given that they are worsening and critically related to risk for conversion, functional outcome, and recovery.

Psychotic disorders are characterized by a generalized neurocognitive deficit (i.e., performance ... more Psychotic disorders are characterized by a generalized neurocognitive deficit (i.e., performance 1.5 SD below controls across neuropsychological domains with no specific profile of differential deficits). A motivational account of the generalized neurocognitive deficit has been proposed, which attributes poor neuropsychological testing performance to low effort. However, findings are inconsistent regarding effort test failure rate in individuals with psychotic disorders across studies (0-72%), and moderators are unclear, making it difficult to know whether the motivational explanation is viable. To address these issues, a meta-analysis was performed on data from 2,205 individuals with psychotic disorders across 19 studies with 24 independent effects. Effort failure rate was examined along with moderators of effort test type, forensic status, IQ, positive symptoms, negative symptoms, diagnosis, age, gender, education, and antipsychotic use. The pooled weighted effort test failure rate was 18% across studies and there was a moderate pooled association between effort failure rate and global neurocognitive performance (r = .57). IQ and education significantly moderated failure rate. Collectively, these findings suggest that a nontrivial proportion of individuals with a psychotic disorder fail effort testing, and failure rate is associated with global neuropsychological impairment. However, given that effort tests are not immune to the effects of IQ in psychotic disorders, these results cannot attest to the viability of the motivational account of the generalized neurocognitive deficit. Furthermore, the significant moderating effect of IQ and education on effort test performance suggests that effort tests have questionable validity in this population and should be interpreted with caution.

However, the 2-dimensional model has yet to be evaluated using more complex mathematical techniques capable of testing structure. In the current study, network analysis was applied to evaluate the latent structure of negative symptoms using a community-detection algorithm. Two studies were conducted that included outpatients with schizophrenia (SZ; Study 1: n = 201; Study 2: n = 912) who were rated on the Brief Negative Symptom Scale (BNSS). In both studies, network analysis indicated that the 13 BNSS items divided into 6 negative symptom domains consisting of anhedonia, avolition, asociality, blunted affect, alogia, and lack of normal distress. Unfortunately, current treatments result in only modest improvement in social cognition. Oxytocin, a neuropeptide with pro-social effects, has significant benefits for social cognition in the general population. However, studies examining the efficacy of oxytocin in schizophrenia have yielded inconsistent results. One reason for inconsistency may be that oxytocin has typically not been combined with psychosocial interventions. It may be necessary for individuals with schizophrenia to receive concurrent psychosocial treatment while taking oxytocin to have the context needed to make gains in social cognitive skills. However, it is unclear whether this is also true in individuals meeting criteria for "prodromal" psychosis, who are considered to be at symptomatic high risk for developing the disorder. In this study, we examined neurophysiological and self-reported response to emotional stimuli in UHR ( = 23) and healthy control (CN: = 30) participants who passively viewed pleasant, unpleasant, and neutral images for 500 ms while the electroencephalogram was recorded and then provided self-reports of valence and arousal to the stimuli. The Late Positive Potential (LPP) event related potential (ERP) component was used as a neurophysiological marker of emotional reactivity. Social functioning scale confirmed with these findings: a significant difference in total score was found between groups, with Group 1 scoring similarly to control subjects, higher than the other 3 (F(3, 560) = 3.882, P = .009). Various cortical regions involved in motor and executive functions, including middle temporal and superior frontal areas, showed drastic differences in volume and thickness across groups (all Ps < .001). Conclusion: Four clusters within probands showing non-unified structural differences, increments in cognitive deficits, and low reactivity to auditory inputs in only one group suggested a high degree of heterogeneity among psychotic populations that does not necessarily conform to traditional diagnoses. Investigation to psychosis phenotypes at network and circuit level will help further discern disease subtypes and refine diagnoses and prescription guidelines.

Este artículo explora las relaciones entre pintura y poesía a partir del análisis de una obra del... more Este artículo explora las relaciones entre pintura y poesía a partir del análisis de una obra del artista peruano Fernando de Szyszlo titulada Piedra de sol (1974), la cual toma el título del poema homónimo de Octavio Paz, publicado por primera ocasión en 1957. El trabajo ofrece al lector un análisis riguroso de la dimensión plástica de la obra de Szyszlo, pues en esa superficie textual-delimitada se pone en cuestión la asociación inherente con el poema de Paz. Con ayuda de una “hipótesis- visual” nutrida de diversas observaciones de la pintura Piedra de sol, se intenta demostrar una compleja tarea de traducción a partir de la cual la mirada del pintor accede a la tradición a través de la modernidad, o lo que Paz denominó “tradición moderna”. Con ello, la pintura logra distanciarse del poema homónimo y crear así sus propias figuras. The annual incidence is approximately 2.4-11.3 cases/1,000,000 with the disease affecting men and women equally. Clinical presentation is variable, usually with unspecific signs and symptoms that make difficult diagnosis. Here we report a clinical case regarding a 53 years old male, without comorbidities, who presented with an initial clinical picture of fever and lumbar pain and whose initial diagnosis was acute pyelonephritis. However, on completion of treatment the patient continued presenting intermittent fever for which he was hospitalized. During hospitalization distinct auxiliary examinations were performed including a skin biopsy and c-ANCA which were key to determining the definitive diagnosis, providing adequate treatment and therefore favourable development of the patient's clinical condition.

The review also aims to demonstrate that cannabis use in adolescence will be a precursor to schizophrenia disease, finding differentiation between people with schizophrenia with a history of cannabis use and people with this disorder but no history of consumption.

Negative symptoms are core features of schizophrenia-spectrum disorders that are frequently obser... more Negative symptoms are core features of schizophrenia-spectrum disorders that are frequently observed across all phases of illness. By their nature, COVID-19 social isolation, physical distancing, and health precautions induce behavioural aspects of negative symptoms. However, it is unclear whether these prevention measures also lead to increases in experiential negative symptoms, whether such effects are equivalent across individual negative symptom domains, and if exacerbations occur equivalently across phases of illness. The current study compared negative symptom severity scores obtained during the pandemic to pre-pandemic assessments in two samples: (1) outpatients with chronic schizophrenia (SZ: n = 32) and matched healthy controls (CN: n = 31) and (2) individuals at clinical high risk for psychosis (CHR: n = 25) and matched CN (n = 30). Pre-pandemic ratings of negative symptoms were clinically elevated in SZ and CHR groups, which did not differ from each other in severity. In SZ, ratings obtained during the pandemic were significantly higher than pre-pandemic ratings for all 5 domains (alogia, blunted affect, anhedonia, avolition, and asociality) and item-level analyses indicated that exacerbations occurred on both experiential and behavioral symptoms of anhedonia, avolition, and asociality. In contrast, CHR only exhibited increases in anhedonia and avolition items during the pandemic compared to pre-ratings. Findings suggest that negative symptoms should be a critical treatment target during and after the pandemic in the schizophrenia spectrum given that they are worsening and critically related to risk for conversion, functional outcome, and recovery.

Psychotic disorders are characterized by a generalized neurocognitive deficit (i.e., performance ... more Psychotic disorders are characterized by a generalized neurocognitive deficit (i.e., performance 1.5 SD below controls across neuropsychological domains with no specific profile of differential deficits). A motivational account of the generalized neurocognitive deficit has been proposed, which attributes poor neuropsychological testing performance to low effort. However, findings are inconsistent regarding effort test failure rate in individuals with psychotic disorders across studies (0-72%), and moderators are unclear, making it difficult to know whether the motivational explanation is viable. To address these issues, a meta-analysis was performed on data from 2,205 individuals with psychotic disorders across 19 studies with 24 independent effects. Effort failure rate was examined along with moderators of effort test type, forensic status, IQ, positive symptoms, negative symptoms, diagnosis, age, gender, education, and antipsychotic use. The pooled weighted effort test failure rate was 18% across studies and there was a moderate pooled association between effort failure rate and global neurocognitive performance (r = .57). IQ and education significantly moderated failure rate. Collectively, these findings suggest that a nontrivial proportion of individuals with a psychotic disorder fail effort testing, and failure rate is associated with global neuropsychological impairment. However, given that effort tests are not immune to the effects of IQ in psychotic disorders, these results cannot attest to the viability of the motivational account of the generalized neurocognitive deficit. Furthermore, the significant moderating effect of IQ and education on effort test performance suggests that effort tests have questionable validity in this population and should be interpreted with caution.

Various cortical regions involved in motor and executive functions, including middle temporal and superior frontal areas, showed drastic differences in volume and thickness across groups (all Ps < .001). Conclusion: Four clusters within probands showing non-unified structural differences, increments in cognitive deficits, and low reactivity to auditory inputs in only one group suggested a high degree of heterogeneity among psychotic populations that does not necessarily conform to traditional diagnoses. Investigation to psychosis phenotypes at network and circuit level will help further discern disease subtypes and refine diagnoses and prescription guidelines.

Este artículo explora las relaciones entre pintura y poesía a partir del análisis de una obra del... more Este artículo explora las relaciones entre pintura y poesía a partir del análisis de una obra del artista peruano Fernando de Szyszlo titulada Piedra de sol (1974), la cual toma el título del poema homónimo de Octavio Paz, publicado por primera ocasión en 1957. El trabajo ofrece al lector un análisis riguroso de la dimensión plástica de la obra de Szyszlo, pues en esa superficie textual-delimitada se pone en cuestión la asociación inherente con el poema de Paz. Con ayuda de una “hipótesis- visual” nutrida de diversas observaciones de la pintura Piedra de sol, se intenta demostrar una compleja tarea de traducción a partir de la cual la mirada del pintor accede a la tradición a través de la modernidad, o lo que Paz denominó “tradición moderna”. Con ello, la pintura logra distanciarse del poema homónimo y crear así sus propias figuras.
