Personality disorders - description, definition and diagnosis Personality disorders are common, a... more Personality disorders - description, definition and diagnosis Personality disorders are common, associated with a poor quality of life, functional limitations, worse health, and often remain undiagnosed. The aetiology is complex, with the influence of both genetic and environmental factors. A person centred approach based on empathic understanding is important for the diagnostic and therapeutic alliance. In case of conceivable comorbidity, the relative influence of all possible comorbid conditions shall be evaluated to avoid an excessive number of comorbid diagnoses. A detailed and carefully taken history is crucial for a valid diagnosis and for assessment of severity.

Patients on long-term sick-leave (&am... more Patients on long-term sick-leave (> 90 days) from white collar work, with a diagnosis of work-related depression, dysthymia, or maladaptive stress reaction were randomized either to cognitive group therapy (CGT), focused psychodynamic group therapy (FGT), or to a comparison group. All patients were interviewed and responded to self-report questionnaires before the start of treatment and at 6 and 12 months. At the 12-month follow-up, 70% of the patients met the criteria for reliable change of the target symptom (depression), and the sample as a whole improved significantly on all measures used. However, there were no differences in outcome between the three treatment groups.

Empowerment ar ett nyckelord, och grundtanken ar att patienten betraktas som en kompetent partner i det gemensamma arbetet for halsa och livskvalitet. Fortroende, kunskap, kreativitet och flexibilitet ar grundlaggande for en organisation som stoder detta. DSM-III in practice: Views and use among Swedish psychiatrists. More than 40% of Swedish psychiatrists have received formal training in DSM-III diagnosing since 1986. A two-page questionnaire was distributed to all those who participated in two-day training programs arranged by the Swedish Psychiatric Association. The respondent rate was 70%, and 79% expressed a clearly positive attitude to DSM-III in general. Age or experience in psychiatry did not influence pre- or post-training use or views towards DSM-III. Females had more previous experience with DSM-III, and psychodynamically oriented psychiatrists were more in favor, in particular of the multiaxial aspect of the system. There was, however a general reluctance to use axis IV and V. After training 31% mostly and 64% sometimes used DSM-III in their daily clinical practice. The results are further compared with previous international studies on attitudes towards DSM-III.
