The aims of this study were (1) to study the immunohistochemical expression of the bcl-2 oncoprotein in colorectal tumors, (2) to correlate it with that of p53 protein overexpression, and (3) to compare it with histopathologic prognostic factors, such as TNM classification and grade. Prospective study of expression of bcl-2 and p53 oncogenes in colorectal tumors. We examined 6 colorectal hyperplastic polyps, 33 adenomas, and 61 carcinomas. Regional academic medical center. The aim of this study was to correlate current morphologic data relating to the lumen of the audi... more The aim of this study was to correlate current morphologic data relating to the lumen of the auditory tube. Four methods were used: dissection under the operating microscope; microendoscopy of the tubal lumen; optical and electron microscope histology; and MR or CT imaging. The auditory tube consists of two unequal cones, a small posterior third, fixed and osseous (protympanum), and a mobile fibrocartilaginous anterior two-thirds, both joined by the tubal isthmus, a short constriction which is pseudosphincteric at endoscopy. The tensor veli palatini muscle (TVPM) and the levator veli palatini muscle (LVPM) are the chief muscles that vary the tubal lumen of the fibrocartilaginous portion, which is collapsed at rest. CT and especially MR imaging allows their observation in static conditions. Serial histologic sections reveal the continuity between the TVPM and the tensor tympani muscle. The main cartilage framing the lumen varies in shape according to the level surveyed. The tubal mucosa is lined with an epithelium combining ciliated and mucus cells, involved in mucociliary drainage and gas exchanges in the auditory tube. These morphologic elements represent a basis for study of tubal physiology and for planning treatment in dysfunctions of the auditory tube.

ABSTRACT IntroductionLa tumeur triton maligne (TTM) est un sous-type de tumeur maligne des gaines... more ABSTRACT IntroductionLa tumeur triton maligne (TTM) est un sous-type de tumeur maligne des gaines nerveuses périphériques qui présente une composante rhabdomyosarcomateuse. C’est une tumeur maligne rare, qui fut décrite pour la première fois en 1932, et dont le pronostic reste sombre. Les localisations ethmoïdo-nasales sont exceptionnelles avec seulement 10 cas rapportés dans la littérature.ObservationNous rapportons le cas d’une patiente âgée de 80 ans qui présentait une obstruction de la fosse nasale droite évoluant depuis plusieurs années sans anosmie. La consultation a été motivée par des épistaxis récidivantes. La tumeur, au scanner, occupait l’ensemble de l’ethmoïde droit, du sinus maxillaire droit et de la fosse nasale homolatérale. L’exérèse a été effectuée par voie médio-faciale. Le diagnostic histologique a été schwannosarcome de bas grade avec un contingent rhabdo-myosarcomateux discrètement étendu, c’est-à-dire une TTM. Il n’y a pas eu de traitement complémentaire. Huit mois plus tard, la tumeur a récidivé avec une atteinte cérébrale. Malgré une reprise chirurgicale en double équipe avec les neurochirurgiens et une radiothérapie la tumeur a de nouveau récidivé au niveau ethmoïdo-nasale. Après 5 ans d’évolution, la patiente est toujours en poursuite évolutive locale sans métastase.DiscussionLa TTM des cavités naso-sinusiennes est une maladie rare mais qui doit faire partie des diagnostics différentiels des tumeurs malignes des cavités naso-sinusiennes. La plupart des cas, en dehors des localisations ethmoïdo-nasales, sont associés à une neurofibromatose de von Recklinhausen et surviennent chez le sujet jeune (35 ans en moyenne). Mais il existe des cas sporadiques notamment chez les femmes plus âgées ou après radiothérapie. Le pronostic est un peu moins sombre pour les localisations ethmoïdo-nasales.IntroductionThe combination of a malignant peripheral nerve sheath tumor with rhabdomyosarcomatous elements has been termed Malignant Triton Tumor (MTT). This tumor was first reported in 1932 and the prognosis was poor. Involvement of the sinonasal tract is rare with only 10 case reports.CaseWe report the case of a 80-year-old woman who presented an obstruction of the right nasal fossa which developed over several years without anosmia. She consulted because of recurrent epistaxis. The CT-scan revealed a tumor occupying the right ethmoid the right maxillary sinus and the homolateral nasal fossa. After five years, the patient is alive with local progression without metastasis.DiscussionMTT of the sinonasal tract is a rare disease that clinicians should be aware of and that must be included in the differential diagnosis of malignant lesions involving the sinonasal tract. Most of the reported cases involving localizations other than the ethmoidonasal cavities, are associated with von Recklinhausen neurofibromatosis and occurr in young subjects (mean age 35 years). There have however been sporadic cases, mainly in older women or after radiotherapy. The prognosis is somewhat better for ethmoidonasal localizations.

Ces métastases sont un défi pour le pathologiste et posent un problème de prise en charge multidisciplinaire. Notre travail consistait à mettre au point une trousse d'auto-apprentissage sur les métastases thoraciques dont le primitif était indéterminé. L'objectif principal est de fournir un outil éducatif pratique pour les étudiants en médecine, les omnipraticiens, les pathologistes, les oncologues, les radiothérapeutes, les chirurgiens thoraciques, les radiologues et les médecins nucléaires. Il propose une démarche claire et rationnelle afin d'améliorer la pertinence de la prise en charge. Ils pourront grâce à la méthode « E-Learning » compléter leur formation à leur rythme, selon leurs besoins et améliorer ainsi leurs connaissances. Le kit contient les modules théoriques suivants : généralités sur les métastases thoraciques d'études primaires, cliniques, biologiques et radiologiques inconnues, un chapitre détaillé sur les différents types histopathologiques riches en algorithmes décisionnels, des tableaux, des cas cliniques illustratifs et une évaluation à la fin. Tous illustrés par une iconographie riche de 186 images avec 181 collectées dans les départements de pathologie anatomique, radiologie, oncologie et médecine nucléaire à l'hôpital universitaire Mohammed VI. OBJECTIVES Supracentimetric cervical lymph node metastasis is classically a poor prognostic facto... more OBJECTIVES Supracentimetric cervical lymph node metastasis is classically a poor prognostic factor for locoregional recurrence and survival in head and neck cancer. Causality, however, is more controversial for infracentimetric cervical lymph node metastases. The objective of this study was to evaluate the incidence and prognostic value of infracentimetric lymph node metastasis. MATERIALS AND METHODS Two hundred and forty-three neck dissections from 150 head and neck cancer patients were analyzed. A single pathologist exhaustively inventoried the number and size of all adenopathies in the surgical specimen. RESULTS Cervical lymph node metastases were infracentimetric in 38% of cases, with 72% extracapsular spread (versus 91% for supracentimetric adenopathies; P<0.01). Infracentimetric metastases were more often associated with other cervical lymph node metastases (mean 5.3 versus 3.9; P=0.14). Fifty three percent of specimens showed only supracentimetric metastases (versus 13% infracentimetric metastases; P<0.01). Disease-specific and failure-free survival were lower in case of infracentimetric metastasis, associated with supracentimetric metastasis or not, than in case of macrometastasis only. CONCLUSION Infracentimetric cervical lymph node metastasis is a factor of poor prognosis, and may represent a different, more aggressive lymphatic process. We suggest complete neck dissection by the surgeon and meticulous analysis by the pathologist, the results of which guide complementary therapy. Close surveillance of recurrence is also recommended.

To analyze histopathologic invasion of the anterior laryngeal commissure on surgical specimens fr... more To analyze histopathologic invasion of the anterior laryngeal commissure on surgical specimens from patients operated on for stage-2 squamous-cell carcinoma managed by supracricoid partial laryngectomy (SCL). Twenty-five patients with previously untreated stage-2 squamous-cell carcinoma were selected. Preoperative endoscopy confirmed anterior commissure involvement; CT found no cartilage lysis. SCL was performed in all cases: 15 anterior frontal SCLs with epiglottoplasty, 8 with cricohyoidepiglottopexy, and 2 with cricohyoidopexy. Histopathology analyzed resection margins (< 1mm, 1-5mm, > 5mm), cartilage extension and vascular embolism. Mean time to observation was 18 months (range, 12-36 months). Resection margins were < 1mm in 7 cases (28%), 1-5mm in 9 and > 5mm in 9 patients. Vascular emboli were found in 15 patients (60%). Observation.-Nous rapportons une observation d'un tel anévrisme sur la coronaire droite, survenu chez un homme de 56 ans et compliqué d'une thrombose à l'origine d'un infarctus étendu, révélé par des signes d'insuffisance cardiaque congestive, dont le diagnostic formel d'anévrisme athéromateux a été réalisé à l'examen histologique après exérèse chirurgicale. Discussion.-Ces anévrismes peuvent être associés à de nombreuses maladies inflammatoires ou dysplasiques, mais l'athérosclérose reste l'étiologie la plus fréquente. Les différentes techniques d'imagerie complètent utilement le bilan préopératoire de ces formes pseudotumorales, dont le diagnostic n'est souvent possible que par l'examen anatomopathologique.

Surgery is commonly indicated after failure of medical treatment. The aim of this study was to analyze the indications and complications of surgery as well as endocrine results. Patients operated on for hyperthyroidism between 2004 and 2012 were included in a retrospective study. Total thyroidectomy was performed for Graves' disease, toxic multinodular goiter and amiodarone-associated thyrotoxicosis; patients with toxic nodule underwent hemithyroidectomy. Pathologic analysis assessed surgical specimens; postoperative complications and resolution of hyperthyroidism were noted. Two hundred patients from 15 to 83 years old were included. Histologically, these lesions are composed of mature adipocytes and spindle cells associated with collagen bundles. Spindle cell lipomas are benign tumors that can be difficult to distinguish from malignant tumors such as spindle cell liposarcomas, myxoid liposarcomas or well-differentiated liposarcomas. We report herein the description of two new cases. The first case was a deeply situated and infiltrating tumor located in the retromastoidian area. The karyotype showed the presence of two chromosomal abnormalities, a partial deletion of the long arm of chromosome 13, del(13)(q12) and a balanced reciprocal translocation t(2;6)(p16~21;p21). The second case was a subcutaneous tumor of posterior cervical localization. The karyotype showed a 13q deletion associated with a complex rearrangement of chromosomes 5, 6 and 10. Méthodes et résultats.-À partir de six observations provenant de notre expérience, nous illustrons son intérêt d'un point de vue clinique, diagnostique et didactique, ainsi que ses apports dans le domaine de la santé publique. Conclusions.-Nous tentons de comprendre les facteurs limitant aujourd'hui sa réalisation. Nous soulignons aussi l'importance d'une politique claire des pouvoirs publics et des instances sanitaires. Cette condition est indispensable pour améliorer les pratiques actuelles et alléger les protocoles administratifs afin de conserver la « vérification anatomique » dans notre arsenal diagnostique et pédagogique.

Causality, however, is more controversial for infracentimetric cervical lymph node metastases. The objective of this study was to evaluate the incidence and prognostic value of infracentimetric lymph node metastasis. MATERIALS AND METHODS Two hundred and forty-three neck dissections from 150 head and neck cancer patients were analyzed. A single pathologist exhaustively inventoried the number and size of all adenopathies in the surgical specimen. RESULTS Cervical lymph node metastases were infracentimetric in 38% of cases, with 72% extracapsular spread (versus 91% for supracentimetric adenopathies; P<0.01). Infracentimetric metastases were more often associated with other cervical lymph node metastases (mean 5.3 versus 3.9; P=0.14). Fifty three percent of specimens showed only supracentimetric metastases (versus 13% infracentimetric metastases; P<0.01). Disease-specific and failure-free survival were lower in case of infracentimetric metastasis, associated with supracentimetric metastasis or not, than in case of macrometastasis only. CONCLUSION Infracentimetric cervical lymph node metastasis is a factor of poor prognosis, and may represent a different, more aggressive lymphatic process. We suggest complete neck dissection by the surgeon and meticulous analysis by the pathologist, the results of which guide complementary therapy. Close surveillance of recurrence is also recommended.

Twenty-five patients with previously untreated stage-2 squamous-cell carcinoma were selected. Preoperative endoscopy confirmed anterior commissure involvement; CT found no cartilage lysis. SCL was performed in all cases: 15 anterior frontal SCLs with epiglottoplasty, 8 with cricohyoidepiglottopexy, and 2 with cricohyoidopexy. Histopathology analyzed resection margins (< 1mm, 1-5mm, > 5mm), cartilage extension and vascular embolism. Mean time to observation was 18 months (range, 12-36 months). Resection margins were < 1mm in 7 cases (28%), 1-5mm in 9 and > 5mm in 9 patients. Vascular emboli were found in 15 patients (60%). Observation.-Nous rapportons une observation d'un tel anévrisme sur la coronaire droite, survenu chez un homme de 56 ans et compliqué d'une thrombose à l'origine d'un infarctus étendu, révélé par des signes d'insuffisance cardiaque congestive, dont le diagnostic formel d'anévrisme athéromateux a été réalisé à l'examen histologique après exérèse chirurgicale. Discussion.-Ces anévrismes peuvent être associés à de nombreuses maladies inflammatoires ou dysplasiques, mais l'athérosclérose reste l'étiologie la plus fréquente. Les différentes techniques d'imagerie complètent utilement le bilan préopératoire de ces formes pseudotumorales, dont le diagnostic n'est souvent possible que par l'examen anatomopathologique.

Surgery is commonly indicated after failure of medical treatment. The aim of this study was to analyze the indications and complications of surgery as well as endocrine results. Patients operated on for hyperthyroidism between 2004 and 2012 were included in a retrospective study. Total thyroidectomy was performed for Graves' disease, toxic multinodular goiter and amiodarone-associated thyrotoxicosis; patients with toxic nodule underwent hemithyroidectomy. Pathologic analysis assessed surgical specimens; postoperative complications and resolution of hyperthyroidism were noted. Two hundred patients from 15 to 83 years old were included. Histologically, these lesions are composed of mature adipocytes and spindle cells associated with collagen bundles. Spindle cell lipomas are benign tumors that can be difficult to distinguish from malignant tumors such as spindle cell liposarcomas, myxoid liposarcomas or well-differentiated liposarcomas. We report herein the description of two new cases. The first case was a deeply situated and infiltrating tumor located in the retromastoidian area. The karyotype showed the presence of two chromosomal abnormalities, a partial deletion of the long arm of chromosome 13, del(13)(q12) and a balanced reciprocal translocation t(2;6)(p16~21;p21). The second case was a subcutaneous tumor of posterior cervical localization. The karyotype showed a 13q deletion associated with a complex rearrangement of chromosomes 5, 6 and 10. Méthodes et résultats.-À partir de six observations provenant de notre expérience, nous illustrons son intérêt d'un point de vue clinique, diagnostique et didactique, ainsi que ses apports dans le domaine de la santé publique. Conclusions.-Nous tentons de comprendre les facteurs limitant aujourd'hui sa réalisation. Nous soulignons aussi l'importance d'une politique claire des pouvoirs publics et des instances sanitaires. Cette condition est indispensable pour améliorer les pratiques actuelles et alléger les protocoles administratifs afin de conserver la « vérification anatomique » dans notre arsenal diagnostique et pédagogique.
