Zastarzałe zerwanie więzadła rzepki jest bardzo rzadkim schorzeniem, które jest technicznie trudn... more Zastarzałe zerwanie więzadła rzepki jest bardzo rzadkim schorzeniem, które jest technicznie trudne do leczenia. Zabieg operacyjny jest niezbędny do odtworzenia mechanizmu wyprostnego stawu kolanowego. Opisano wiele różnych technik rekonstrukcji więzadła rzepki. Prezentujemy przypadek 48-letniego mężczyzny, u którego doszło do zerwania więzadła rzepki osiem miesięcy przed operacją. Do rekonstrukcji więzadła rzepki wykorzystano przeszczep ze ścięgna mięśnia półścięgnistego i zmodyfikowaną technikę Ecker'a. Uzyskano pełny wyprost stawu kolanowego w połączeniu z zadowoleniem pacjenta. Jednakże wymagał on przedłużonej intensywnej rehabilitacji.

Currently, medicine uses typical industrial structure techniques, including reverse engineering, ... more Currently, medicine uses typical industrial structure techniques, including reverse engineering, data processing, 3D-CAD modeling, 3D printing, and coordinate measurement techniques. Taking this into account, one can notice the applications of procedures used in the aviation or automotive industries based on the structure of Industry 4.0 in the planning of operations and the production of medical models with high geometric accuracy. The procedure presented in the publication shortens the processing time of tomographic data and increases the reconstruction accuracy within the hip and knee joints. The procedure allows for the partial removal of metallic artifacts from the diagnostic image. Additionally, numerical models of anatomical structures, implants, and bone cement were developed in more detail by averaging the values of local segmentation thresholds. Background: The Knee Outcome Survey Activities of Daily Living Scale (KOS-ADLS) is a self-reporte... more Background: The Knee Outcome Survey Activities of Daily Living Scale (KOS-ADLS) is a self-reported questionnaire used for the evaluation of the overall health of patients with knee dysfunctions. The purpose of this study was to perform a cross-cultural adaptation of the Polish version of KOS-ADLS and to evaluate its psychometric properties in patients at the end-stage of knee osteoarthritis who were qualified for a total knee replacement (TKR). Material/Methods: Seventy patients consecutively qualified for surgical TKR procedure participated in this study. To adapt the KOS-ADLS, the following scales and reference tests were used: Knee injury and Osteoarthritis Outcome Score (KOOS), Visual Analogue Scale (VAS), Time Up and Go test (TUG), and Five Times Sit to Stand test (5×STS). The studies were conducted 3 times: 2 weeks before surgery (first study), 6 to 13 days later (retest), and 6 months after surgery. Results: The Polish version of KOS-ADLS showed excellent reliability (ICC=0.89 SEM=2.68, MDC=7.43) and high responsiveness (ES=4.76, SRM=3.18). The internal consistency was poor in the first assessment (Cronbach's al-pha=0.68), but acceptable in the post-surgery evaluation (Cronbach's alpha=0.86). There were fair and moderate correlations found between KOS-ADLS and VAS scales in the first examination, TUG, and 5×STS. Stronger correlations were observed between the results obtained in KOS-ADLS and KOOS. Conclusions: The Polish version of the KOS-ADLS demonstrated good reliability, validity, and responsiveness for use in patients who have undergone TKR surgery.

The aim of the study was to evaluate the impact of implant component alignment on objective and s... more The aim of the study was to evaluate the impact of implant component alignment on objective and subjective outcomes after total knee arthroplasty (TKA). The rotation of the femoral component and its influence on the final results were also examined. After exclusion, the study examined 102 patients (mean age, 66.28 years; range, 51–79 years) who had undergone unilateral TKA. All of the operative procedures were performed by one surgeon with one type of implant. One year after the operation, improvements in Knee Society's Knee Scoring System, functional score, Western Ontario and McMaster Universities Osteoarthritis Index, and Visual Analog Scale were observed; however, none showed a significant correlation with any of the parameters analyzed by X-ray or computed tomography (CT) (α, β, γ, δ angles and posterior condylar angle [PCA]). Endoprosthetics is currently most common method of the treatment of advanced osteoarthritis, including knee joint osteoarthritis. Its main goal is to restore patients functional independence in activities of daily living through reducing pain and disability. Objective. The aim of this study was functional assessment of the patients after total knee replacement (TKR) considering subjects age and gender, BMI and mean duration of the disease. Material and methods. 70 patients (59 females and 11 males), qualified for total knee replacement surgery due to the primary knee osteoarthritis (KO), participated in this study. Endoprotezoplastyka jest obecnie najbardziej rozpowszechnioną metodą leczenia zaawansowanych zmian zwyrodnieniowych stawów, w tym stawu kolanowego. Chronic patellar tendon rupture is a rare disabling injury that is technically difficult to repai... more Chronic patellar tendon rupture is a rare disabling injury that is technically difficult to repair. Many different surgical methods have been reported for the reconstruction of chronic patellar tendon ruptures. We are reporting the use of contralateral bone-tendon-bone (BTB) autograft for chronic patellar tendon rupture reconstruction followed by double-wire loop reinforcement and without postoperative immobilization. One year after the operation, our patient had full knee extension and up to 130 degrees of flexion. He had good quadriceps strength, and isokinetic muscle testing showed no deficit comparing to his right leg. Patient returned to playing basketball in his spare time, without having any limitation.
