During the period of last fifteen years, at the Institute of Nephrology and Immunology in Sarajevo renal amyloidosis was diagnosed with 15 patients. The disease occurred more often with men than with women. Only during 1988, renal amyloidosis was revealed and followed up with five patients. The most common clinical manifestations of renal amyloidosis are nephrotic syndrome and chronic renal failure, with respective laboratory findings. Using immunofluorescent analysis of the kidney biopsy material, we discovered deposits of immunoglobulins of different intensity and deposits of lambda and kappa light chains of immunoglobulins. The intensity of lambda light chains is greater than that of kappa chains. The analysis of light microscopy showed nodular mesangial deposits and deposits along GBM without proliferation. The diagnosis of amyloidosis was confirmed by staining of amyloid. Application of therapy for amyloidosis was without any effect. Although renal amyloidosis is a rare disease, we want to point out disease as being an etiologic factor in nephrotic syndrome.

The cyclosporine levels are determined by using fluorescence polarization immune-assay and monoclonal whole blood test. We analyzed nine patients with transplanted kidneys who were on specific immunosuppressive cyclosporine therapy. The cyclosporine determination was performed in these patients three times in the span of 14 to 29 days. The cyclosporine levels in seven patients were in reference interval between 100 and 300 ng/ml of whole blood. In the first determination the values in two patients were out of reference interval, one value was lower, and the other one was higher then reference interval. In second determination after dose correction in patients who had lower cyclosporin level the new level was normal and the other patient cyclosporine value was lower then in the first determination, but still a little higher then reference values.

Our study is the prospective study for assessment of T lymphocyte immunological changes in patients with burns, with different degrees of %TBSA and depth of burns. Research objectives: Objectives of this study were to assess %CD3+Ly, %CD4+Ly, %CD8+Ly, %CD3+ HLA-DR+Ly, %CD4+Ly /CD8+Ly), of burned body with different %TBSA degrees, different depth burns and to establish predictive value of immune suppression these parameters. Patients and methods: According to %TBSA, patients were classified into three groups: mild burns with TBSA% < 15% (30 patients), group of medium burns with %TBSA from 15%-25% (30 patients) and group with %TBSA > 25% to 40% (30 patients). According to the depth of burns, patients were classified into two groups, partial-thickness burns, (39 patients), and full-thickness burns (51 patients). We followed laboratory parameters: % CD3+L...

Material and method: Total of 776 blood samples from inflammatory arthritis patients were obtained. Statistical analysis for positive and negative results was calculated and test values were compared. Results: Anti-CCP test was found positive (>25U/ml) in 32,8% of blood samples. The all positive test results were in rheumatoid arthritis patients with 95% specificity. Negative test results was found in 67,2% of blood samples that were drawn from all IgM RF negative individuals, reactive arthritis and osteoarthritis patients as well as in some end-stage rheumatoid arthritis disease. Mean value of positive results was very high: 599,62 U/ml. Conclusion: The anti-CCP test is highly specific test in Rheumatoid arthritis. The positive test in early undifferentiated inflammatory polyarthritis provides new laboratory diagnostic inflammatory marker and helps practitioners to confirm diagnosis of early rheumatoid arthritis. Microplate reader and Genf4 Software using ELISA method is essential automotive tool for extensive on-board data obtained analysis.

The value is measured by the total revenue. Organization is profitable if it the value it creates exceeds the costs involved in the creation of its products/services. Innovations that we use today, in the near future will be replaced by the completely new ones. Particular number of individual psychological factors has been studied in connection with resistance to the organizational innovation. For health care organizations in F.B&H, the obstacles in introduction of innovation, are very often political party disagreements, especially when the suggestions for innovations are coming from those who do not hold the political power. Health care is very complex and open macro-system with special importance for society. Health care system is composed of large number of its subsystems (micro-systems). Value and quality of any socio-system is being estimated by the degree of functioning of its components and system as the whole. Unfortunately, today in F.B&H entity, besides few honorable examples, we have a problem with a low and disharmonized complementary health care level, with lack of health care staff and system fragmentation. The ultilization of the modern innovation models, in which all must be included, from changing the patients' and health care workers' mindset, legislation, insurance funds and managers of health care institutions, can be the beginning point in improving the current situation.
