Background: Sexual assault against women is common all over the world. However, reliable data on ... more Background: Sexual assault against women is common all over the world. However, reliable data on the subject in developing countries including Nigeria is not available. Objective: To review the patterns of sexual violence against women treated at the hospital over a 7-year period. Materials and Methods: Review of hospital records of victims of sexual assault who presented at the hospital from 1 January 2003 to 31 December 2009. Data obtained were analyzed using descriptive statistics and Chi squire test. Results: Sexual assault cases constituted 2.1% of female consultation outside pregnancy during the period under study while proportion of cases increased over the years under review. Mean age of the victims was 15.8 (SD 8.1) years ranging from 5 to 48 years. Most (73.7%) were less than 18 years while 93.2% were single (never married). About 81% of the victims less than 18 years were sexually abused in the day time. Majority (79.6%) knew their assailant. About 40% of the victims presented within 24 h of sexual abuse but none had postexposure prophylaxis. Conclusion: Sexual assault among women is an important health problem in this environment. There is need for hospital based management protocol.

An incidence of 1 in 6,250 deliveries has been quoted in Ibadan, Nigeria (Aimakhu 1975). Attending possible complications include abortion, premature labour, intrauterine growth restriction, acute abdomen from uterine torsion and or strangulation, rupture of lower uterine segment, difficult labour and multiple abdominal skin ulcerations among others. Elective abdominal delivery in view of above complications is usually advised (Malhotra et al. 2003). Elective herniorrhaphy is also said to be most auspicious. We present a case of gravid uterus in an incisional hernia that presented at 20-week gestational age with ulcerative wounds of the skin covering the herniated uterus. The patient did not consent to elective delivery and presented in labour with a footling breech. She had an emergency lower caesarean section with concurrent herniorrhaphy with successful outcomes.

A comparative study was conducted to demonstrate the difference, if any, in effectiveness of trea... more A comparative study was conducted to demonstrate the difference, if any, in effectiveness of treatment of iron deficiency anaemia in pregnancy with either iron dextran or ferrous sulphate. Sixty pregnant women with iron deficiency anaemia were assigned randomly to either group and treated for 6 weeks. The age and parity distributions with mean packed cell volumes (PCVs) and gestational age at onset of treatment in the two groups were comparable. Comparing the mean PCVs at week 2, week 4 and week 6 of treatment the iron dextran group recorded higher and statistically significant mean PCVs (P<0.001). Thirty-six per cent of patients in the iron dextran group compared to 3.3% in the oral iron group (P=0.004) had their anaemia corrected by the sixth week. No significant side effects accompanied the use of intramuscular iron dextran. It was concluded that iron dextran corrects iron deficiency anaemia faster than ferrous sulphate. Parenteral iron should be considered in pregnant woman with moderate and asymptomatic severe anaemia between gestation ages of 28 weeks and 34 weeks; this may reduce the frequency of blood transfusion both in the antenatal and postnatal periods in these patients.

Four different cases to illustrate medical indications for use of vaginal pessary in the manageme... more Four different cases to illustrate medical indications for use of vaginal pessary in the management of Uterovaginal(U-V) prolapse are cited. Ring-shaped vulcanized rubber that serve different functions as vehicle parts were used in these case series due to paucity of modern vaginal pessary in Nigeria market. The first three cases benefited satisfactorily from the use of pessary. The fourth patient had deficient perineum and thus could not benefit from the modified ring pessary.

The patient had repeat embryo transfer eleven months afterwards that culminated in the delivery of living twins with a fetal papyraceous.

Background: Female age is a known independent index in determining outcome of fertility treatment... more Background: Female age is a known independent index in determining outcome of fertility treatment with ovulatory and fertility potentials declining rapidly after age 35 however it an assumed fact that men are capable of spermatogenic activity beyond age 60 with world oldest father procreating at age 97. (Myers, 1972) Objectives: Determine if age affected semen concentration and motility of male partners undergoing invitro fertilization. Determine if age of male partners had influence on percentage of oocytes that are fertilized following invitro fertilization cycles. Method: A descriptive retrospective review of data of IVF cycles performed between July 2015 and June 2017. Inclusion criteria was that age of oocytes producer must be less than 36, post sperm wash count of at least 10million per milliliters and 15% active motile sperm. Exclusion criteria were female age of oocyte producer more than 35, incomplete relevant data. Results: One hundred and twenty cycles met inclusion criteria. Mean husband age was 45.70 (±7.07). Mean percentage oocyte fertilized among the group was 51.95% (± 29.85%). Mean percentage of oocytes fertilized among husband equal to or less than 50 was 54.21% (± 28.87%) while mean percentage of oocytes fertilized among husband age 51 and above was 45.20% (± 32.20), the observed difference being statistically significant (p= 0.0000; X 2 =90.133). Conclusion: There is a significant drop in ability of men above 50years to fertilize oocytes compared to men below 51years of age during invitro fertilization cycles.

Since endoscopic and arthroscopic knots must be delivered over a distance to a tissue with minimal access maintaining tension is more important than the knot confi guration chosen. 5,6 The aim of this article review is to determine which hand tied knot confi guration and possibly, suture size, and suture type that would be safe in laparoscopic surgery. Materials and methods: A literature review was performed using PubMed, Springerlink, Highwire press and search engines like Google and Yahoo. The following search terms were used: extracorporeal knot, arthroscopic knots, Roeder's knot, Meltzer's knot, Mishra's knot, Duncan knot, Nicky's knot, SMC knot, Weston knot and Tennessee extracorporeal knot. A total of 48,100 citations were found. Selected papers were screened for further references. Publications that featured illustrations of sliding knots with statistical methods of analysis were selected. More than 20 different sliding knots were used for this review. Result: Eighty-one articles were reviewed. Most studies have evaluated knot security only and few studies have evaluated simultaneous both loop and knot security and also only a few compared knot and loop securities to the type of suture materials and their sizes. The addition of three RHAPs improves knot security of all sliding knots tested and improves the loop security of most of the sliding knots tested. Conclusion: The safety of extracorporeal knot depends on knot confi guration, especially when further RHAPs are included.
