Radioiodinated meta-iodobenzylguanidine (MIBG), an analogue of norepinephrine, has been used in m... more Radioiodinated meta-iodobenzylguanidine (MIBG), an analogue of norepinephrine, has been used in management of neuroendocrine tumors. Recent studies reveal that distribution of radioiodinated MIBG in animals depends on the specific activity of this radiopharmaceutical. In order to clarify the effect of specific activity on organ uptake of radioiodinated MIBG. the kinetics of no-carrier-added (n.c.a.) [I-123]MIBG (greater than or equal to 7.4 TBq/mu mol) were compared with those of commercial (com.) [I-123]MIBG (similar to 74 MBq/mu mol) in 3 healthy volunteers by serial imaging and blood sampling. The organ uptake of radioiodinated MIBG did not remarkably differ between the two specific activities. Due to rapid degradation a more pronounced accumulation of radioactivity was present in plasma alter n.c.a. than after com. [I-123]MIBG resulting in a higher background and thyroid activity. Compared with version 1 facultative options for RIT can be chosen in special cases: ablative RIT for papillary microcarcinoma </=1 cm, ablative RIT for mixed forms of anaplastic and differentiated thyroid cancer, and RIT in patients with a measurable or increasing thyroglobulin concentration but without detectable metastases by imaging. Therefore the data of 596 patients with papillary or follicular thyroid cancer are analysed retrospectively, which have been treated between 1980 and 1995 at the Clinic and Policlinic for Nuclear Medicine of the University of Würzburg. The influence of lymph node metastases on prognosis with respect to survival is analysed with the univariate Kaplan-Meier-method and with the multivariate discriminant analysis. In addition, the influence of the prognostic factor "lymph node involvement" on distant metastases is analysed by a stratified comparison and an univariate test. In papillary thyroid cancer, the 15 year-survival-rate for stage pN1 is significantly lower (p < 0.001) with 88.7% as compared to stage pN0 (99.4%). Thyroid cancer is a rare malignancy with wide interethnic and geographic variations. In Germany t... more Thyroid cancer is a rare malignancy with wide interethnic and geographic variations. In Germany thyroid carcinoma is the 13th most frequent malignancy (2.7 new cases yearly per 100,000 inhabitants). The overall temporal incidence is increasing slightly in recent years. The most common types of cancer are papillary (60-80%) and follicular cancers (10-20%). The relevant prognostic indicators are tumor stage and distant metastases. The mean survival rates in papillary thyroid cancer usually exceed 90%, whereas in follicular thyroid cancer they amount to approximately 80%. The standard treatment procedure in differentiated papillary and follicular thyroid cancer consists of total thyroidectomy followed by adjuvant ablative therapy with radioiodine. Only in papillary thyroid cancer stage pT1N0M0 lobectomy alone is considered to be appropriate. Both, dobutamine (D) as well as enoximone (E) mediate their inotropic action via an increase in intracellular c-AMP concentration based on a different action. In 10 patients with documented myocardial infarction either D (5 to 40 micrograms/kg/min, increments of 5 micrograms/kg/min every 3 min) or E (1 to 9 micrograms/kg/min, increments of 1 microgram/kg/min every 2 min) was administered intravenously on two consecutive days. Heart rate (HR), systolic and diastolic blood pressure (BP), as well as a wall motion score in 16 segment (WMS) and ejection fraction (EF) with 2D-echocardiography were determined at rest and during each increment. For this study, 24 patients with medullary thyroid cancer (MTC) and 10 with carcinoid-/GEP-tumour... more For this study, 24 patients with medullary thyroid cancer (MTC) and 10 with carcinoid-/GEP-tumours underwent scintigraphy with 123I-Tyr3-octreotide or 111In-DTPA-D-Phe1-octreotide (Octreoscan) or 99mTc-V-DMSA. Calcitonin and CEA were elevated in MTC patients, the other had tumour lesions on CT. Octreoscan-scintigraphy was positive in 68% of all suspicious cases. On the other hand, 123I-Tyr3-octreotide showed only rarely positive results. 99mTc-V-DMSA-scans in MTC patients were positive in 23%. Liver metastases could be seen only with Octreoscan in the non-MTC-group. These results showed better sensitivity of 111In-labelled octreotide.

First used by Fellinger et al. in 1952, the technique has now been applied for more than 50 y for treatment of resistant synovitis of individual joints after failure of long-term systemic pharmacotherapy and intraarticular steroid injections. RSV relieves pain and inflammation from rheumatoid arthritis (RA), for which it initially was used, and is accepted as an alternative to surgical synovectomy in cases of RA or other inflammatory arthropathies such as osteoarthritis and hemophiliac arthropathy. A good understanding of the clinical pathophysiology of the disease processes is mandatory, and close interdisciplinary collaboration with other clinicians who diagnose and treat patients is strongly suggested. The effect of specific activity of meta[I-123]iodobenzylguanidine ([I-123]MIBG) on neuroblastoma ... more The effect of specific activity of meta[I-123]iodobenzylguanidine ([I-123]MIBG) on neuroblastoma uptake was studied in a nude mouse model (NMRI nu/nu) xenografted subcutaneously with SK-N-SH cells. Groups of eight animals received [I-123]MIBG intravenously with a specific activity of greater than or equal to 260 GBq/mu mol (no-carrier-added), 3.7 GBq/mu mol, 37 MBq/mu mol, and 0.37 MBq/mu mol, respectively. All animals in the group injected with 0.37 MBq/mu mol died immediately after the injection. Al 4 and 24 h, there was no significant effect of specific activity on tumor uptake of [I-123]MIBG in the different groups. The uptake of non-tumor tissue was in general lower with 37 MBq/mu mol compared to higher specific activities. The differences in blood, heart, liver, spleen and lungs were statistically significant at 24 h, whereas at 4 h significant differences were only present in the heart, liver and lungs. The results suggest that for the treatment of children with neuroblastoma a lower specific activity of radioiodinated MIBG may minimize the radiation exposure to non-tumor tissue but not to the tumor. Higher mass of MIBG &amp;amp;gt;0.5 mu mol/g, however, is considered as lethal dose in our nude mice model and corresponding doses may cause toxic side effects in human.

Acute bone crisis and global indolent bone marrow displacement could both be assessed by bone marrow scintigraphy, whereas MRI could better clarify the corti-comedullary alteration after bone infarction. Thus, MRI and bone marrow scintigraphy could be used as complementary imaging methods in the management of patients with Gaucher disease.

ABSTRACT CONTROL ID: 1933354 TITLE: PEM tests show highest accuracy detecting breast cancer. BODY... more ABSTRACT CONTROL ID: 1933354 TITLE: PEM tests show highest accuracy detecting breast cancer. BODY: Objectives : The diagnostic performance of the first European PEM is evaluated in 79 females with 124 suspicious breast lesions. Methods : PEM tests of the breast were performed in 79 females with 124 suspicious breast lesions or known breast cancer (BC) 90 min after i.v. application of 3.5 MBq/kg F-18-FDG per kg body weight. The maximum PEM uptake value (PUV) was derived from a ROI around the target lesion and was correlated with a corresponding non-target ROI in the contra lateral healthy breast to determine the target/non-target ratio (PUV-ratio). Data were analyzed by 2 experienced independent readers and compared to histopathology in all cases. The between group analyses for all malignant, benign and corresponding non-target lesions were calculated by paired Student t-Test. The mean target/non-target ratio in patients with BC compared to healthy patients was calculated by independent Student t-Test. Significance level was considered at p value &lt;0.05. Receiver operating characteristic (ROC) analyses were employed to determine associations with PUV and PUV-ratio. Results : A total of 22 out of 124(17.7%) lesions indicates BC. Mean PUV was estimated to be 4.3±2.7 in BC and 1.2±0.4 for the contra-lateral healthy breast (p&lt;0.001). The mean PUV-ratio in patients with BC of 3.7±1.5 was significantly higher as compared to benign lesions 1.2±0.3 (p&lt;0.001). The area under the ROC curve was 0.997(0.000-1.000) for PUV and 0.986(0.965-1.000) for PUV-ratio. PEM was true-positive in 21 out of 22(1 false positive in case of papilloma), resulting in sensitivity of 95%, specificity of 99%, positive predictive value of 95%, and a negative predictive value of 99%. Conclusions : The different European approach in PEM using personalized FDG-dosage enables the comparability of FDG-metabolism in patients, resulting in highest accuracy of PEM tests also. We are reporting a case of pediatric thyroid cancer with diffuse skeletal metastasis associated with low serum Tg level bringing a point of caution in evaluation of patients with low Tg.

First used by Fellinger et al. in 1952, the technique has now been applied for more than 50 y for treatment of resistant synovitis of individual joints after failure of long-term systemic pharmacotherapy and intraarticular steroid injections. RSV relieves pain and inflammation from rheumatoid arthritis (RA), for which it initially was used, and is accepted as an alternative to surgical synovectomy in cases of RA or other inflammatory arthropathies such as osteoarthritis and hemophiliac arthropathy. A good understanding of the clinical pathophysiology of the disease processes is mandatory, and close interdisciplinary collaboration with other clinicians who diagnose and treat patients is strongly suggested. The effect of specific activity of meta[I-123]iodobenzylguanidine ([I-123]MIBG) on neuroblastoma ... more The effect of specific activity of meta[I-123]iodobenzylguanidine ([I-123]MIBG) on neuroblastoma uptake was studied in a nude mouse model (NMRI nu/nu) xenografted subcutaneously with SK-N-SH cells. Groups of eight animals received [I-123]MIBG intravenously with a specific activity of greater than or equal to 260 GBq/mu mol (no-carrier-added), 3.7 GBq/mu mol, 37 MBq/mu mol, and 0.37 MBq/mu mol, respectively. All animals in the group injected with 0.37 MBq/mu mol died immediately after the injection. Al 4 and 24 h, there was no significant effect of specific activity on tumor uptake of [I-123]MIBG in the different groups. The uptake of non-tumor tissue was in general lower with 37 MBq/mu mol compared to higher specific activities. The differences in blood, heart, liver, spleen and lungs were statistically significant at 24 h, whereas at 4 h significant differences were only present in the heart, liver and lungs. The results suggest that for the treatment of children with neuroblastoma a lower specific activity of radioiodinated MIBG may minimize the radiation exposure to non-tumor tissue but not to the tumor. Higher mass of MIBG &amp;amp;gt;0.5 mu mol/g, however, is considered as lethal dose in our nude mice model and corresponding doses may cause toxic side effects in human.

Acute bone crisis and global indolent bone marrow displacement could both be assessed by bone marrow scintigraphy, whereas MRI could better clarify the corti-comedullary alteration after bone infarction. Thus, MRI and bone marrow scintigraphy could be used as complementary imaging methods in the management of patients with Gaucher disease.

BODY: Objectives : The diagnostic performance of the first European PEM is evaluated in 79 females with 124 suspicious breast lesions. Methods : PEM tests of the breast were performed in 79 females with 124 suspicious breast lesions or known breast cancer (BC) 90 min after i.v. application of 3.5 MBq/kg F-18-FDG per kg body weight. The maximum PEM uptake value (PUV) was derived from a ROI around the target lesion and was correlated with a corresponding non-target ROI in the contra lateral healthy breast to determine the target/non-target ratio (PUV-ratio). Data were analyzed by 2 experienced independent readers and compared to histopathology in all cases. The between group analyses for all malignant, benign and corresponding non-target lesions were calculated by paired Student t-Test. The mean target/non-target ratio in patients with BC compared to healthy patients was calculated by independent Student t-Test. Significance level was considered at p value &lt;0.05. Receiver operating characteristic (ROC) analyses were employed to determine associations with PUV and PUV-ratio. Results : A total of 22 out of 124(17.7%) lesions indicates BC. Mean PUV was estimated to be 4.3±2.7 in BC and 1.2±0.4 for the contra-lateral healthy breast (p&lt;0.001). The mean PUV-ratio in patients with BC of 3.7±1.5 was significantly higher as compared to benign lesions 1.2±0.3 (p&lt;0.001). The area under the ROC curve was 0.997(0.000-1.000) for PUV and 0.986(0.965-1.000) for PUV-ratio. PEM was true-positive in 21 out of 22(1 false positive in case of papilloma), resulting in sensitivity of 95%, specificity of 99%, positive predictive value of 95%, and a negative predictive value of 99%. Conclusions : The different European approach in PEM using personalized FDG-dosage enables the comparability of FDG-metabolism in patients, resulting in highest accuracy of PEM tests also. A low stimulated Tg level (&lt;1 ng/ml) associated with normal neck ultrasonography is considered as the most reliable criteria for complete remission in low risk patients. We are reporting a case of pediatric thyroid cancer with diffuse skeletal metastasis associated with low serum Tg level bringing a point of caution in evaluation of patients with low Tg.
