Castleman disease is an uncommon lymphoproliferative disorder that usually presents asymptomatica... more Castleman disease is an uncommon lymphoproliferative disorder that usually presents asymptomatically at a single site, most commonly the mediastinum; it is rare in the head and neck. Although Castleman disease may be mistaken for a malignant process such as lymphoma, it is a benign condition that is curable with complete surgical resection. We report the case of a 43-year-old man with the hyaline vascular type of unicentric Castleman disease who presented with an isolated neck mass.

BACKGROUND: Detailed information about how patients with head and neck carcinoma (HNC) are treate... more BACKGROUND: Detailed information about how patients with head and neck carcinoma (HNC) are treated across practice settings does not exist. The authors conducted a prospective, observational study to examine the patterns of care for a series of patients with newly diagnosed HNC in the United States and to test 2 hypotheses: 1) There is no difference in the pattern of care between community and academic settings; and 2) the results of major randomized clinical trials will change the pattern of care in both practice settings within 1 year of publication in peer-reviewed journals. METHODS: Patients aged !18 years were enrolled in the Longitudinal Oncology Registry of Head and Neck Carcinoma (LORHAN) after providing written informed consent if they had a confirmed diagnosis of new HNC and were scheduled to receive treatment other than surgery alone. RESULTS: Between 2005 and 2010, 100 centers enrolled 4243 patients, including 2612 patients (62%) from academic investigators and 1631 patients (38%) from community centers. Initial treatments were radiation with concurrent chemotherapy (30%) or cetuximab (9%), adjuvant radiotherapy (21%), induction chemotherapy (16%), and other (24%). Intensity modulated radiation therapy was the dominant radiation technique (84%). Singleagent cisplatin was prescribed in nearly half of patients and more often in academic centers (53% vs 43% of patients; P < .0001). Single-agent cetuximab was the next most common drug used (19%) and was prescribed more frequently in community settings (24% vs 17%; P ¼ .0001). The data rejected the 2 prospective hypotheses. CONCLUSIONS: LORHAN documented differences in patient characteristics and treatments between community and academic settings for a large series of patients in the United States. Orbital involvement may occur via direct or perineural spread from a lacrimal gland or sinonasal source. Primary orbital adenoid cystic carcinoma without involvement of the lacrimal gland is rare. The authors report a 53-year-old woman who was examined for insidious monocular vision loss and was found to have a primary adenoid cystic carcinoma of the orbital apex and cavernous sinus. Systemic workup for a primary source, including ipsilateral lacrimal gland biopsy, was negative. One must maintain a high index of suspicion for adenoid cystic carcinoma when evaluating orbital tumors.

An MR lymphangiogram demonstrated a lymphatic varix at the confluence of the left upper extremity lymphatic ducts, confirmed by intranodal axillary lymphangiography. After successful catheterization of the feeding lymphatic vessels, the varix was successfully embolized with detachable microcoils and an autologous blood patch. The patient has been free from symptoms on subsequent outpatient follow-up.

Hibernomas are rare, benign tumors that contain neoplastic cells that are histologically similar ... more Hibernomas are rare, benign tumors that contain neoplastic cells that are histologically similar to those of brown adipose tissue. The most common sites of these tumors have been the back and thighs; head and neck involvement has been rare. We describe the case of a 54-year-old woman who presented with a 1-year history of a large, asymptomatic, lateral neck mass. Based on imaging findings and fine-needle aspiration cytology, a hibernoma was suspected, although a liposarcoma could not be ruled out prior to surgery. The mass was completely resected. On postoperative analysis, the tumor was grossly and microscopically consistent with a cervical hibernoma. Although hibernomas are rare, they are important entities and should be considered in the differential diagnosis of a soft-tissue mass of the head or neck.

Its occurrence in the parotid gland is very rare. Such a presentation must be properly evaluated in order to rule out malignancy. We describe the diagnostic evaluation and surgical management of a parotid dermoid cyst in an 18-year-old man. We also review the embryologic pathogenesis of the tumor and the usefulness of the physical examination, preoperative fine-needle aspiration cytology, magnetic resonance imaging, and computed tomography in evaluating the extent of the tumor, its histologic features, and its cystic characteristics. Finally, we discuss the role of surgical management in terms of preserving facial nerve integrity and preventing recurrence.

We present 3 cases in which a unilateral pericranial flap was used to repair defects in the media... more We present 3 cases in which a unilateral pericranial flap was used to repair defects in the medial orbital and upper lateral nasal areas. All three patients had undergone previous excision of malignant tumors from the upper septum and ethmoid areas. Postoperatively, they all received radiation and then developed wound breakdown with fistula formation, particularly in the area of the incisional site. All 3 patients had undergone previous unsuccessful repair of the fistula. Closure of such fistulas requires well-vascularized nonradiated tissue, which can be provided by a unilateral pericranial flap. This flap is flexible, thin, and well suited to the sinonasal area. Use of this flap avoids the need for a paramedian forehead or other musculocutaneous flap. Cosmetic results were excellent, and there have been no flap breakdowns or complications to date.

We undertook a study to determine the usefulness and effectiveness of a relatively new, portable ... more We undertook a study to determine the usefulness and effectiveness of a relatively new, portable 980-nm wavelength diode laser with a fiberoptic delivery system. We tested the laser in several clinical situations, both in the operating room and in the office. We used it while performing 14 turbinate reduction procedures, one nasal polypectomy, one ablation of an oral papilloma, and one photocoagulation of nasal telangiectasias. Our preliminary findings indicate that the use of this laser was helpful in alleviating nasal congestion in the patients with turbinate dysfunction and in controlling epistaxis in the patient with telangiectasias. It was also effective in treating the polyp and papilloma patients. We did not experience any intra- or postoperative complications. The laser's flexible fiber delivery system is compatible with hollow instruments, allows for coaxial vision, and is ideally suited for intranasal use. A review of bibliographies of these studies was also performed. Criteria for inclusion in this meta-analysis consisted of the availability of extractable data from studies presenting a defined group of patients who had undergone primary vestibular schwannoma removal and for whom the presence and absence of cerebrospinal fluid leakage was reported. Studies reporting combined approaches were excluded. No duplications of patient populations were included. Twenty-five studies met the inclusion criteria. Quality of the studies was determined by the design of each study and the ability to combine the data with the results of other studies. All of the studies were biased by their retrospective, nonrandomized nature. Significance (p &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05) was determined using the chi2 test. Cerebrospinal fluid leak occurred in 10.6% of 2,273 retrosigmoid surgeries, 9.5% of 3,118 translabyrinthine surgeries, and 10.6% of 573 middle fossa surgeries. The type of cerebrospinal fluid leak was not associated with surgical approach. Meningitis was significantly associated with cerebrospinal fluid leak (p &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05). Age and tumor size were not associated with cerebrospinal fluid leak.

Prior studies examining the role of fistula formation (FF) at the time of TL to reduce the incidence of PCF are conflicting. We studied our institutional experience, where in the last few years a more routine approach to using FFs with TLs has been adopted irrespective of the size of the mucosal defect resulting from the TL. It is hypothesized that this routine approach reduces the incidence of PCFs.

The static and kinetic frictional coefficients of commercially pure titanium brackets were evalua... more The static and kinetic frictional coefficients of commercially pure titanium brackets were evaluated in the passive configuration in the dry and wet states against stainless steel, nickel-titanium, and beta-titanium arch wires. For comparison, stainless steel brackets were evaluated under identical conditions. Titanium brackets were grayer in color and rougher in texture than the stainless steel brackets. Bracket slots were up to 0.002 inch greater than the nominally stated values. Remarkably, the static and kinetic frictional coefficients of the couples formed by titanium and stainless steel brackets were comparable. When evaluated against stainless steel and nickel-titanium arch wires in the dry state at 34 degrees C, the static coefficient averaged.12 and.20, respectively, independent of bracket alloy. When evaluated against stainless steel and nickel-titanium wires in the wet state at 34 degrees C using human saliva, the static coefficient averaged.15 and.20, respectively, independent of bracket alloy. Only the beta-titanium arch wires increased by about 15%, when tested in either the dry or the wet state against titanium versus stainless steel brackets. Noteworthy, too, was the decrease of both coefficients in the beta-titanium wire couples from their previously reported values. Analyses of electron spectroscopy for chemical analysis spectra and depth profiles show that these new brackets are titanium only in the bulk. Indeed the immediate surfaces are composed of, at least, 80 atomic percent (at.%) carbon and oxygen; whereas, the titanium that is present (&gt;11 at.%) is mostly in the form of titanium dioxide. The presence of this quite thin passivating layer, which resides on top of an oxygen-hardened titanium substrate, reduces the galling and fretting that would normally be expected in such materials. Pending the outcome of future angulation tests, these frictional measurements show that titanium brackets are not only comparable to stainless steel brackets but also are more biocompatible with nickel having been eliminated from their constitution.

MEDLINE and PubMed literature search using the terms &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot;acoustic neuroma&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot; or &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot;vestibular schwannoma,&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot; and &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot;cerebrospinal fluid leak&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot; or &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot;cerebrospinal fluid fistula&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot; covering the period from 1985 to the present in the English language literature. A review of bibliographies of these studies was also performed. Criteria for inclusion in this meta-analysis consisted of the availability of extractable data from studies presenting a defined group of patients who had undergone primary vestibular schwannoma removal and for whom the presence and absence of cerebrospinal fluid leakage was reported. Studies reporting combined approaches were excluded. No duplications of patient populations were included. Twenty-five studies met the inclusion criteria. Quality of the studies was determined by the design of each study and the ability to combine the data with the results of other studies. All of the studies were biased by their retrospective, nonrandomized nature. Significance (p &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05) was determined using the chi2 test. Cerebrospinal fluid leak occurred in 10.6% of 2,273 retrosigmoid surgeries, 9.5% of 3,118 translabyrinthine surgeries, and 10.6% of 573 middle fossa surgeries. The type of cerebrospinal fluid leak was not associated with surgical approach. Meningitis was significantly associated with cerebrospinal fluid leak (p &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05). Age and tumor size were not associated with cerebrospinal fluid leak.

The static and kinetic frictional coefficients of commercially pure titanium brackets were evalua... more The static and kinetic frictional coefficients of commercially pure titanium brackets were evaluated in the passive configuration in the dry and wet states against stainless steel, nickel-titanium, and beta-titanium arch wires. For comparison, stainless steel brackets were evaluated under identical conditions. Titanium brackets were grayer in color and rougher in texture than the stainless steel brackets. Bracket slots were up to 0.002 inch greater than the nominally stated values. Remarkably, the static and kinetic frictional coefficients of the couples formed by titanium and stainless steel brackets were comparable. When evaluated against stainless steel and nickel-titanium arch wires in the dry state at 34 degrees C, the static coefficient averaged.12 and.20, respectively, independent of bracket alloy. When evaluated against stainless steel and nickel-titanium wires in the wet state at 34 degrees C using human saliva, the static coefficient averaged.15 and.20, respectively, independent of bracket alloy. Only the beta-titanium arch wires increased by about 15%, when tested in either the dry or the wet state against titanium versus stainless steel brackets. Noteworthy, too, was the decrease of both coefficients in the beta-titanium wire couples from their previously reported values. Analyses of electron spectroscopy for chemical analysis spectra and depth profiles show that these new brackets are titanium only in the bulk. Indeed the immediate surfaces are composed of, at least, 80 atomic percent (at.%) carbon and oxygen; whereas, the titanium that is present (&gt;11 at.%) is mostly in the form of titanium dioxide. The presence of this quite thin passivating layer, which resides on top of an oxygen-hardened titanium substrate, reduces the galling and fretting that would normally be expected in such materials. Pending the outcome of future angulation tests, these frictional measurements show that titanium brackets are not only comparable to stainless steel brackets but also are more biocompatible with nickel having been eliminated from their constitution.
