By means of a 2-week intensive multidisciplinary training & treatment course in small gro... more By means of a 2-week intensive multidisciplinary training & treatment course in small groups (ISBP), young adults with atopic dermatitis may be able to achieve better self-management of their disease and reduce their number of doctor visits. Patients aged 18-35 with moderate to severe atopic dermatitis (SCORAD > 20) were randomized in a treatment (ISBP) group of n = 31 and a control group of n = 20. Follow-up was 9 months. The outcome was assessed using validated primary and secondary parameters, both specific for atopic dermatitis and more general. Participants in the ISBP scored significantly better at follow-up in the Marburger atopic dermatitis-specific questionnaire and the self-care parameter, needed less time for medical consultations, and used more emollients without corticosteroids. Absence from work/sick leave was less at 10 weeks follow-up, but equal at 9 months. The ISBP program can be judged successful because both the patients and their doctors perceive their interactions as more efficient, less time time-consuming and more satisfying.

De organisatoren van de conferentie waren erin geslaagd de 450 deelnemers een aantrekkelijk programma te bieden, waarbij zij zich overigens niet al te strikt aan het thema hadden gehouden. De conferentie werd geopend met twee plenaire lezingen van zeer verschillend gehalte.

From the University Hospital Groningen 193 patients with the diagnose sprain of the neck filled out a questionnaire. Of this group 100 subjects did not have complaints before the accident and were therefore at risk for the development of complaints as a result of sprain of the neck. Quality of life and psychological functioning were assessed using the RAND-36 and the SCL-90, respectively. Personality was assessed by means of the Dutch Personality Questionnaire. The authors reviewed data involving the impact of providing psychotherapy for psychiatric disorde... more The authors reviewed data involving the impact of providing psychotherapy for psychiatric disorders on costs of care. In a search of the MEDLINE database limited to peer-reviewed papers published from 1984 through 1994, 686 articles were identified. Forty-one articles, covering 35 studies, were found in which the intervention tested was psychotherapeutic and the study included measures of outcome that had some implications for cost. The exclusion criteria for reviewing these studies included absence of a comparison group, a focus on medical disorders instead of psychiatric illnesses, and outcomes that did not include cost data or measures from which costs could be inferred. On this basis, 18 of the 35 studies were selected for analysis. The studies were categorized according to whether or not subjects were randomly assigned to study groups. De opzet van de conferentie was enigszins anders dan in de voorgaande jaren. Dit jaar waren er een openingslezing en een slotlezing, waarbij de sprekers niet alleen op veel spreektijd, maar ook op een omvangrijk publiek konden rekenen, aangezien op die tijden geen andere programmaonderdelen waren gepland. Nadeel van deze wijziging in de opzet was wel dat op de overige dagdelen veel parallelsessies plaatsvonden.

Individuals who had been involved in traffic accidents and had initiated compensation claim procedures with a Dutch insurance company were sent questionnaires (Q1) containing complaint-related questions and the Self-Rating Scale for PTSD. Of the 997 questionnaires that were dispatched, 617 (62%) were returned. Only car accident victims were included in this study (n=240). Complaints were monitored using additional questionnaires that were administered 6 months (Q2) and 12 months (Q3) after the accident. PTSD was related to the presence and severity of concurrent post-whiplash syndrome. This study investigated differences in psychological distress and coping styles between fathers a... more This study investigated differences in psychological distress and coping styles between fathers and mothers of pediatric cancer patients, over a 1-year time period. Also examined were (dis)similarities in couples in distress and coping, and the relationship between (dis)similarities in coping and psychological functioning of both members of a couple. Parents (n= 124, 62 couples) were assessed at diagnosis, at 6 and 12 months. Fathers and mothers experienced higher levels of psychiatric symptomatology and psychological distress at diagnosis than men and women of a normgroup. Distress declined significantly with time. Although parents did not report more symptoms than the normgroup 12 months post-diagnosis, they still were psychologically out of balance. Contrary to findings in the general population, no differences were found between fathers and mothers in psychiatric symptoms or psychological distress on any of the measurements. Only a few gender differences in coping were found. Fathers used more active-problem focusing at diagnosis and a less palliative reaction pattern at 12 months than did mothers. Mothers used more social-support seeking on all measurements. A tendency for similarity in the use of the coping styles within couples was found. Discrepancies in coping in couples were positively related to distress in fathers at diagnosis. However, 12 months later, the more discrepant the couples were in their coping preferences the more distress the mothers indicated.

Introduction: Strategies in case of high risk of breast cancer in BRCA1/2 mutation carriers are e... more Introduction: Strategies in case of high risk of breast cancer in BRCA1/2 mutation carriers are either intensive breast cancer screening or risk-reducing mastectomy (RRM). Both options have a high physical and psychosexual impact. The aim of this study is to investigate who chooses when to undergo RRM. Methods: BRCA1/2 mutation carriers have been prospectively registered at the family cancer clinic between 1994 and 2011. Analyses were performed to assess the relation between characteristics of the BRCA1/2 mutation carriers and an earlier decision for RRM. Results: A cumulative percentage of 35.6% of all women chose to undergo RRM within the first five years after disclosure of DNA test results. Women needed less time to choose for RRM measured from the first visit, if they were younger than 50 years of age (hazard ratio (HR) = 2.67, 95% confidence interval (CI) = 1.30-5.48) or had a mother who had had breast cancer (HR = 1.51 95% CI = 1.04-2.18). Also, women needed less time to choose for RRM in case of a previous breast cancer (HR = 2.25, 95% CI = 1.55-3.27). After a previous unilateral therapeutic mastectomy as a treatment for breast cancer, women needed less time to choose for RRM of the contralateral breast (HR = 2.69, 95% CI = 1.29-5.62) compared to women who had had breast-conserving therapy. Conclusion: BRCA1/2 mutation carriers aged under 50, having a mother with breast cancer, who had previous unilateral breast cancer and previous unilateral therapeutic mastectomy chose more often and earlier for RRM.

After a previous unilateral therapeutic mastectomy as a treatment for breast cancer, women needed less time to choose for RRM of the contralateral breast (HR = 2.69, 95% CI = 1.29-5.62) compared to women who had had breast-conserving therapy. Conclusion: BRCA1/2 mutation carriers aged under 50, having a mother with breast cancer, who had previous unilateral breast cancer and previous unilateral therapeutic mastectomy chose more often and earlier for RRM.
