OBJECTIVES To analyse the outcomes of Robotic Sacrocolpopexy (RSC) on resolution of Lower urinary... more OBJECTIVES To analyse the outcomes of Robotic Sacrocolpopexy (RSC) on resolution of Lower urinary tract Symptoms (LUTS) associated to pelvic organ prolapses (POP) and to determine the risk factors for preoperative LUTS presence. MATERIAL AND METHODS A longitudinal study was carried out on 51 females mean aged (± standard deviation) 66 ± 9 years, who underwent RSC. A preoperative evaluation was made on the degree and type of POP, and the presence of the LUTS stress urinary incontinence (SUI), urgency and voiding difficulty. An urodynamic study was also performed. The presence of LUTS was evaluated again six months after being operated on. The statistical test used were the Mcnermar test for dependent variables, the Fisher's exact test and the t Student's mean comparison test. The signification level was set at p < 0.05 bilaterally. Prostate cancer is the most common cancer amongst men, and one of its treatments in the localized... more Prostate cancer is the most common cancer amongst men, and one of its treatments in the localized stage is radical prostatectomy (RP). This technique achieves a high success rate regarding oncological results, but one of its main drawbacks are sexual disorders, especially erectile dysfunction, ranging from 14-90% of the cases. Therefore, surgery advancements in the last decade have been focused on minimizing the impact on patient’s functioning. Now is time for a step forward in the recovery of functional outcomes after RP. Bioengineering solutions such as stem-cell therapy and tissue engineering hold great opportunities for the treatment of all tissues and organs dysfunctions, which may help in neuroprotection and neuro-regeneration. Background: Stroke is one of the diseases affecting the nervous system which controls the lower u... more Background: Stroke is one of the diseases affecting the nervous system which controls the lower urinary tract. Lower urinary tract dysfunction is a broad term of subjective urinary tract symptoms and may vary according to the course of disease. The presence of Lower urinary tract dysfunction is a negative prognostic indicator for a variety of health and rehabilitation outcomes across the spectrum of care. This study was carried out to find whether severity of lower urinary tract dysfunction has an influence on functional status among ambulant stroke survivors. Methodology: 20 ambulant stroke survivors with the mean age of 68.55±8.02 were evaluated using Core Lower Urinary Tract Symptom Score to assess the severity of lower urinary tract dysfunction and Barthel Index to assess the functional status. Conclusion: This pilot study found weak negative correlation (r=-0.26) between Core lower urinary tract symptom score and functional status; however it was found to be statistically non-significant (p>0.05).

As of April 13, 2021, 137 million cases of COVID-19 and 2.95 million deaths have been reported wo... more As of April 13, 2021, 137 million cases of COVID-19 and 2.95 million deaths have been reported worldwide. On December 21, 2020, the Pfizer-BioNTech vaccine was approved for use in the European Union, with efficacy of 95% protection against COVID-19 infection. Several other vaccines are at different stages of assessment by the European Medicines Agency. In addition to the elderly, oncology patients are a vulnerable population in which COVID-19 infection may be more severe. However, owing to the design of the initial studies, evidence on the safety and efficacy of vaccination against SARS-CoV-2 in these patients is scarce and recommendations are based on the opinion of associations, stakeholders, and experts via extrapolation of information and experience for other vaccines, especially influenza vaccines. Despite the limited evidence, the consensus is that SARS-CoV-2 vaccines are safe and vaccination of oncology patients and their close relatives is recommended, although efficacy may be lower in patients with an impaired immune response and the need for additional booster doses is not yet clear. Recommendations include avoiding the use of vaccines based on viral vectors for patients with an impaired immune response, deferring vaccination for immunosuppressed patients or administering the vaccine before immunosuppression, and avoiding chemotherapy receipt between the two doses of a vaccine or on the same day that the vaccine is administered. These recommendations can be extrapolated to urology patients and although evidence is lacking, there should not be greater interference with SARS-CoV-2 vaccines from androgen deprivation therapy or intravesical bacillus Calmette-Guérin. However, large studies to provide strong evidence for uro-oncology patients are needed. Patient summary We looked at the effects of COVID-19 vaccination for patients with urological cancers. The consensus is that the vaccines are safe, and vaccination of cancer patients and their close relatives is recommended.

Twenty-eight cases of testicular cancer diagnosed from 1987 to 1991 and their corresponding anato... more Twenty-eight cases of testicular cancer diagnosed from 1987 to 1991 and their corresponding anatomopathological data are presented. According to our series, this tumor type ranks third and accounts for 16% of the male urogenital neoplasms. The clinical records were reviewed in order to obtain the necessary data for tabulation. The treatment, results, complications and prognosis were analyzed and compared with those reported in the literature. Of the testicular cancers, there is a higher incidence of seminoma and it is more frequent between the second and third decades of life.

It increased its options after the incorporation of MRI in the 90s. The association of real-time ultrasound with anatomical and functional data of multiparametric magnetic resonance imaging (mpMRI) has changed the prostate cancer scenario and presumably will modify the therapeutic alternatives. OBJECTIVE To study the influence of clean intermittent catheterization (CIC) on the lower urinary... more OBJECTIVE To study the influence of clean intermittent catheterization (CIC) on the lower urinary tract function in patients with urinary retention (UR) due to detrusor underactivity (DU). MATERIAL AND METHODS A longitudinal study was carried out on 49 patients (28 men, 21 women) of mean age 55years, who underwent CIC for UR secondary to DU. The mean CIC frequency was 3.15 times/day. Patients' clinical data were collected, and they underwent urodynamic study before and after CIC, with a mean interval of 4years. Fisher's exact test was used for the analysis of categorical variables and Student's t test for parametric variables. The level of significance was set at 0.05 for a two-tailed test. RESULTS The second urodynamic study showed a significantly increased bladder compliance, the Bladder Outlet Obstruction Index (BOOI) and the Bladder Contractility Index (BCI) also increased but without reaching statistical significance. There was a significantly higher percentage of benign prostatic hyperplasia (BPH) and acontractile detrusor cases among the group of patients whose BCI improved after CIC, with significantly lower CIC time. CONCLUSIONS CIC improved bladder compliance in the patients of our series. The BCI improved in BPH patients and in patients with acontractile detrusor.

A retrospective study was performed in a cohort of 318 patients with BPH which underwent silodosine treatment, during at least 12 weeks. Univariate analysis demonstrated that the variables in relationship with a decrease of urinary symptoms` punctuation postreatment (measured with the IPSS Questionnaire), a maximum peak flow (Q max) postreatment equal or superior to 15 ml/s, a postreatment postvoid residual lower to 100 ml, and a postreatment obstruction index ( Bladder Outlet Obstruction Index: BOOI) equal or lower to 20 cm H2O, presented such both as a clinical and urodynamic character. OBJECTIVES To determine the current status of the training of Urology Resident's in Laparosco... more OBJECTIVES To determine the current status of the training of Urology Resident's in Laparoscopic surgery, robotics and endourology. METHODS We performed a survey among Spanish Urology Residents between 2011 and may 2012. We designed a survey that was disseminated through the web page of the Spanish Society of Laparoscopic and Robotic surgery (SECLA) (www.seclaendosurgery.com) to all residents from every Spanish hospital. The survey was divided in four blocks: General data, data of the Training Hospital, data of the surgical procedures (endoscopic, laparoscopic and robotic) and training features. We performed a descriptive study of the results. RESULTS 36 Urology residents out of a total of 384 answered the survey (9,3%). In reference to endoscopic procedures 25% of the residents had never been involved on a percutaneous nephrostomy (n=36), and did not expect to do it. On the contrary, 77% have performed ureterorrenoscopy as first surgeon (n=36) and 25% have participated in more than 10 procedures. 54.4% of the participants took part as surgeons in percutaneous nephrolithotomies (n=28) and 79% looked forward to do them. Participation of residents in Da Vinci Robot assisted laparoscopic procedures is low. Laparoscopic procedures are very attractive and their participation is reduced as procedure complexity increases. CONCLUSIONS 41.6% of participants think their training is adequate whereas 58,3% think it is not. 88% think their training could improve through courses and seminars and acquiring a greater degree of responsibility.

Carcinoembryonic antigen (CEA), described by Gold and Freedman, was the first fetal tumor antigen... more Carcinoembryonic antigen (CEA), described by Gold and Freedman, was the first fetal tumor antigen isolated in tumor homogenates. The present study investigated the biological behaviour of CEA in superficial transitional cell carcinoma of the bladder, deep localized and disseminated. The study comprised 100 subjects; 30 carefully-selected healthy subjects comprised the first group and 70 patients with a diagnosed transitional cell carcinoma of the bladder comprised the second group. Serum CEA was determined by enzyme immunoassay (CEA, EIA, Roche). Our results suggest that serum CEA determination affords no diagnostic benefit in this type of malignant tumor.

The results of cell culture studies using nephrectomy specimens are presented. Surgical specimens from 7 patients (3 males, 42.9%; 4 females, 57.1%) aged 43-79 years (mean 62.14), who had undergone nephrectomy for renal adenocarcinoma between April 1997 and February 1998 were utilized. The tumor cell samples were obtained during surgery, after the kidney had been excised. The samples were cut, washed in a balanced saline solution and enzymatically dissociated in a trypsin solution (0.1% Hank's solution) for 30 minutes at 37 degrees, centrifuged, placed in Multiwell plates, covered with 0.1 mg/ml polylysine (ICN) or 3 mg/ml collagen S (Boehringer) and cultured in RPMI 1640 (Sigma) supplemented with 5% fetal calf serum (reagent), which was changed every 2-3 days for one month. Robotic surgery can provide a useful minimally invasive tool. Objectives: The study aims to evaluate the robotic-assisted laparoscopic prostatectomy (RALP) in kidney transplant recipients, describing intra/postoperative complications, renal and oncological outcomes. Methods: This is a retrospective study conducted in a single institution of 84 RALP. Four of them were kidney transplant recipients. Side of the renal graft, clinical stage, intra/postoperative events, operating time, creatinine levels before/after intervention and oncologic follow-up were recorded. A systematic review of the literature was performed. Results: Aged 61.25 ± 7.76 years, interval between renal transplantation and RALP: 10 ± 3.16 years. In order to perform a safe and effective surgery is essential a good radiological study. 3D printing is a new tool that allows the creation of virtual and physical trustworthy 3D reproduction of organs or anatomical structures of patients. METHOD Non systematic review of PubMed database about the usefulness of 3D printing in renal surgery. RESULTS 3D printing has a potential use in surgery planning, resident education and improving the communication and understanding of our patients. CONCLUSION Today the number of publications and the use of these technique are still low. 3D printing is a useful tool in order to improve communication with patients and has a potential role in resident education. 3D printing technology could be useful in surgically complex cases since provides an exact model of the organ and the lesion before the intervention allowing a better surgery planning to the surgeon.

Robot-assisted radical prostatectomy (RARP) has become the most widely used surgical approach for radical prostatectomy (RP) in the treatment of localized prostate cancer. However, the current available evidence on the benefit of RARP compared to open radical prostatectomy (ORP) is still under debate. Moreover, recent studies have proposed technical modifications of RARP to improve functional outcomes and postoperative quality of life of prostate cancer patients. We designed a survey that was disseminated through the web page of the Spanish Society of Laparoscopic and Robotic surgery (SECLA) (www.seclaendosurgery.com) to all residents from every Spanish hospital. The survey was divided in four blocks: General data, data of the Training Hospital, data of the surgical procedures (endoscopic, laparoscopic and robotic) and training features. We performed a descriptive study of the results. RESULTS 36 Urology residents out of a total of 384 answered the survey (9,3%). In reference to endoscopic procedures 25% of the residents had never been involved on a percutaneous nephrostomy (n=36), and did not expect to do it. On the contrary, 77% have performed ureterorrenoscopy as first surgeon (n=36) and 25% have participated in more than 10 procedures. 54.4% of the participants took part as surgeons in percutaneous nephrolithotomies (n=28) and 79% looked forward to do them. Participation of residents in Da Vinci Robot assisted laparoscopic procedures is low. Laparoscopic procedures are very attractive and their participation is reduced as procedure complexity increases. CONCLUSIONS 41.6% of participants think their training is adequate whereas 58,3% think it is not. 88% think their training could improve through courses and seminars and acquiring a greater degree of responsibility.

Carcinoembryonic antigen (CEA), described by Gold and Freedman, was the first fetal tumor antigen... more Carcinoembryonic antigen (CEA), described by Gold and Freedman, was the first fetal tumor antigen isolated in tumor homogenates. The present study investigated the biological behaviour of CEA in superficial transitional cell carcinoma of the bladder, deep localized and disseminated. The study comprised 100 subjects; 30 carefully-selected healthy subjects comprised the first group and 70 patients with a diagnosed transitional cell carcinoma of the bladder comprised the second group. Serum CEA was determined by enzyme immunoassay (CEA, EIA, Roche). Our results suggest that serum CEA determination affords no diagnostic benefit in this type of malignant tumor.

The results of cell culture studies using nephrectomy specimens are presented. Surgical specimens from 7 patients (3 males, 42.9%; 4 females, 57.1%) aged 43-79 years (mean 62.14), who had undergone nephrectomy for renal adenocarcinoma between April 1997 and February 1998 were utilized. The tumor cell samples were obtained during surgery, after the kidney had been excised. The samples were cut, washed in a balanced saline solution and enzymatically dissociated in a trypsin solution (0.1% Hank's solution) for 30 minutes at 37 degrees, centrifuged, placed in Multiwell plates, covered with 0.1 mg/ml polylysine (ICN) or 3 mg/ml collagen S (Boehringer) and cultured in RPMI 1640 (Sigma) supplemented with 5% fetal calf serum (reagent), which was changed every 2-3 days for one month. Robotic surgery can provide a useful minimally invasive tool. Objectives: The study aims to evaluate the robotic-assisted laparoscopic prostatectomy (RALP) in kidney transplant recipients, describing intra/postoperative complications, renal and oncological outcomes. Methods: This is a retrospective study conducted in a single institution of 84 RALP. Four of them were kidney transplant recipients. Side of the renal graft, clinical stage, intra/postoperative events, operating time, creatinine levels before/after intervention and oncologic follow-up were recorded. A systematic review of the literature was performed. Results: Aged 61.25 ± 7.76 years, interval between renal transplantation and RALP: 10 ± 3.16 years. In order to perform a safe and effective surgery is essential a good radiological study. 3D printing is a new tool that allows the creation of virtual and physical trustworthy 3D reproduction of organs or anatomical structures of patients. METHOD Non systematic review of PubMed database about the usefulness of 3D printing in renal surgery. RESULTS 3D printing has a potential use in surgery planning, resident education and improving the communication and understanding of our patients. CONCLUSION Today the number of publications and the use of these technique are still low. 3D printing is a useful tool in order to improve communication with patients and has a potential role in resident education. 3D printing technology could be useful in surgically complex cases since provides an exact model of the organ and the lesion before the intervention allowing a better surgery planning to the surgeon.
