La Nutrición Parenteral Periférica (NPP) es el objeto de esta revisión. En ella se analizan críti... more La Nutrición Parenteral Periférica (NPP) es el objeto de esta revisión. En ella se analizan críticamente las técnicas, las indicaciones y los resultados. Los catéteres de nueva generación, junto con una mejora del conocimiento del metabolismo intermediario, permiten la utilización de NPP en muchas situaciones clínicas durante períodos cortos de tiempo. La NPP es una alternativa a la Nutrición Parenteral Total (NPT) y es también un complemento a la Nutrición Enteral y a la vía oral. El progreso en el diseño de catéteres y de sus materiales, las técnicas de infusión y un incremento del conocimiento de los nutrientes óptimos ha hecho de la NPP una terapia eficaz, segura y útil para tratar pacientes durante determinados períodos de tiempo.

Objective: To evaluate the relationship between serum albumin, total cholesterol and total lympho... more Objective: To evaluate the relationship between serum albumin, total cholesterol and total lymphocyte count with two nutritional assessment methods, to verify if their use is justified in nutritional screening tools. Methods: 101 patients admitted to medical/surgical wards underwent the SGA and the Full Nutritional Assessment (FNA). Blood test which included serum albumin, total cholesterol and total lymphocyte count (TLC), were made. Percentage of weight loss and BMI were calculated. An Anova test was done to measure the differences in the mean levels of the three parameters for the nutritional status evaluated by SGA and FNA. The probability of a patient being malnourished in the four ranges established for each parameter was calculated, as well as the relationship between the ranges and the percentage of weight loss and BMI. A small group of editors of general medical journals met informally in Vancouver, British Columbi... more A small group of editors of general medical journals met informally in Vancouver, British Columbia, in 1978 to establish guidelines for the format of manuscripts submitted to their journals. The group became known as the Vancouver Group. Its requirements for manuscripts, including formats for bibliographic references developed by the USA National Library of Medicine, were first published in 1979. The Vancouver Group expanded and evolved into the International Committee of Medical Journal Editors (ICMJE), which meets annually. The ICMJE gradually has broadened its concerns to include ethical principles related to publication in biomedical journals. The total content of the Uniform Requirements for Manuscripts Submitted to Biomedical Journals may be reproduced for educational, not-for-profit purposes without regard for copyright; the committee encourages distribution of the material.

These enzymes for the normal digestion of food and a deficit, whether due to a seduced secretion (chronic pancreatitis, cystic fibrosis), or dysfunction in the chronology of their secretion (following truncal vagotomy) will cause malabsorption which often develops, in clinical terms, into malnutrition sometimes this functional deficit is secondary to other pathologies or surgical operations which alter the digestive tract physiology, preventing the correct combination of nutrients and pancreatic enzymes together with the biliary salts. The outcome is malabsorption, mainly of fast and fat soluble vitamins. As the onset is slow and forms part of a chronic, pathology, diagnosis is difficult, for which reason it is included in the general group of malabsorption due to pancreatic insufficiency or surgical sequelae, finally resulting in pancreatic failure or problems following surger...

Bariatric surgery has increased substantially in the last years. However, there are controversies... more Bariatric surgery has increased substantially in the last years. However, there are controversies of the long-term efficacy and safety. To evaluate the long-term efficacy and safety of Roux-en-Y gastric bypass (BGRY) and gastric banding (Band) in randomized controlled trials and cohort studies. We searched for randomized controlled trials and cohort studies of BGRY and Band, with a follow-up > 48 months, with description of weight loss, complications and mortality. Four randomized trials and one cohort study met the inclusion criteria. Mean weight loss was greater with BGRY (27%-39%) than with Band (13%-35%). In both procedures the complication rate is high. Second operation rate is higher with Band. The outcomes observed from this revision are not sufficient to generalize the efficacy and safety of bariatric surgery to population attended at different health care systems.

Fatty acids from the diet or from IV fat emulsions are incorporated into the plasma and cell memb... more Fatty acids from the diet or from IV fat emulsions are incorporated into the plasma and cell membrane phospholipids and act as substrates in the synthesis of eicosanoids. This study reports the effect of 2 parenteral lipid emulsions in plasma phospholipids fatty acids. A total of 83 patients aged 18 to 75 years were randomized to receive long-chain triglycerides (LCT) or 50/50 mix of long- and medium-chain triglyceride emulsion (LCT/MCT). Blood samples were collected at baseline and at weekly intervals for 28 days. Plasma phospholipid fatty acids were measured by gas chromatography. Patients receiving LCT versus MCT/LCT emulsion have an increase in 18:2n6 and a decrease in 20:4n6 and 22:4n6 after 7, 14, and 21 days of treatment with parenteral nutrition. Phospholipid fatty acids at 15 days of treatment with parenteral nutrition with LCT versus MCT/LCT for 18:2n6 were 17.30% versus 22,90% (p < .05), for 20:4n6 10.44% versus 8.38% (p < .05), and for 22:4n6 0.51% versus 0.40% (p < .05). The 20:4n6 percentage inversely correlated with the percentage of 18:2n6 on days 7, 14, and 21: regression coefficients: -7.40 (p < .001), -7.39 (p < .001), and 5.70 (p < .001), respectively. Parenteral lipid emulsions modify fatty acid profiles in plasma phospholipids. MCT/LCT emulsions produce in phospholipids a fatty-acid profile that is closer to normality than that achieved with LCT emulsions. These changes in phospholipid fatty acids are suggestive of an inhibition of A-5-desaturase in patients who received LCT emulsions.
El fallo hepático fulminante (FHF) es un síndrome clínico muy grave asociado con una alta mortali... more El fallo hepático fulminante (FHF) es un síndrome clínico muy grave asociado con una alta mortalidad 1 a pesar de los grandes avances que se han producido en los campos tanto del manejo de los cuidados intensivos como del trasplante hepático 2,3. Desde que en 1970 Trey y Davidson 4 propusieron una definición del FHF, algunos autores han aportado distintas clasificaciones con el objetivo de establecer un pronóstico y una acción terapéutica en cada caso basándose fundamentalmente en la duración del periodo de tiempo transcurrido entre el inicio de los síntomas clínicos o la aparición de ictericia y el desarrollo de encefalopatía. Así, en el King's College Hospital de Londres, Resumen El fallo hepático fulminante (FHF) es un síndrome clínico muy grave, asociado con alta mortalidad, a pesar de los grandes avances que se han producido en los últimos años en la terapia tanto del manejo de los cuidados intensivos mediante diversos soportes hepáticos bioartificiales como del trasplante hepático. Tanto el conocimiento como el tratamiento del FHF han estado limitados por la falta de modelos animales satisfactorios. Así, han sido muchos los intentos de desarrollar un modelo adecuado, entre los que se incluyen los modelos quirúrgicos, tales como la hepatectomía y desvascularización total y/o parcial, la utilización de sustancias químicas con toxicidad hepática tales como el acetaminofeno, azoximetano, galactosamina, tioacetamida, entre otras. Ahora bien, la mayor parte de estos modelos no reflejan de modo idóneo el patrón de la enfermedad humana de FHF y todos ellos presentan importantes limitaciones. A pesar de que la hepatitis vírica es una de las etiologías más frecuentes de FHF, el uso de agentes víricos para desarrollar modelos animales ha sido escaso y desafortunado. Nuestro grupo ha desarrollado recientemente un modelo animal mediante la inoculación de conejos con el virus de la enfermedad hemorrágica del conejo que presenta características bioquímicas, histológicas y signos clínicos compatibles con el FHF del hombre. En el trabajo se resumen los modelos animales más utilizados asi como las ventajas e inconvenientes más reseñables de cada uno de ellos.

In Spain, the prevalence of malnutrition in hospitalized patients has been reported from 30% to 50%. Objectives: Main purposes of this consensus document were to establish recommendations that facilitate decision-making and action to prevent and early-diagnose disease-related hospital malnutrition, on the management of nutritional support methods and actions to evaluate nutritional treatment compliance and efficacy. Methods: A systematic bibliographical search of authors was performed, complemented by updated bibliography by author references up to 2010. From this review, some recommendations were defined, modified and critically evaluated by the representatives of scientific societies in a consensus conference (Dec 2010) following a structured brainstorming technique: the Metaplan ® technique. A double validation process was undertaken until final recommendations were obtained. Results: 30 consensus recommendations for the prevention and management of hospital malnutrition are presented in this document. Recommendations cover all clinical care settings as well as prevention, screening, diagnosis, treatment and follow-up of disease-related malnutrition. Conclusions: Nutritional screening is strongly recommended at all clinical settings when nutritional risk factors are identified or there is clinical suspicion of malnutrition. Nutritional assessment should be designed and performed according to centers' resources, but clearly identified protocols should be available.

La Sociedad Espanola de Nutricion Enteral y Parenteral (SENPE) es una sociedad multidisciplinar f... more La Sociedad Espanola de Nutricion Enteral y Parenteral (SENPE) es una sociedad multidisciplinar formada por profesionales reunidos en torno a la Nutricion Clinica. Los Grupos de Trabajo constituyen uno de los ejes que vertebran la actividad corporativa de la Sociedad. Son un lugar de encuentro que permite amplificar las voces de sus componentes, los socios de SENPE, que con toda generosidad dedican su tiempo al servicio de los objetivos del grupo al que pertenecen. En la mayoria de las ocasiones los objetivos son muy concretos analizando temas de interes clinico, de investigacion, de formacion y de gestion, asi como estandarizando medidas de la practica clinica, elaborando documentos de consenso en sinergias con otras sociedades, definiendo un posicionamiento institucional en una determinada cuestion etc. En definitiva, siendo utiles a los miembros de nuestra sociedad cientifica y, porque no decirlo, a la sociedad civil. In this way, Kaplan and Norton propose performance measurement through several perspectives with a logical sequence: internal processes and learning impact client services, so that financial performance is affected. The aim of the present paper is to analyze the main characteristics of balanced scorecard when it is applied to non-for-profit companies and, specifically to the health sector in the clinical nutrition field. This model improves the economic vision of management with clinical indicators that represent healthcare professional's perspective. The balanced scorecard would allow a proper monitoring and tracking system for the main healthcare indicators. This contributes to a better control in comparison with standards that are associated with adequate quality assistance.
