Northern river ecosystems worldwide are under increasing environmental stress from degrading deve... more Northern river ecosystems worldwide are under increasing environmental stress from degrading developments that influence water quality and associated ecological integrity. In particular, contaminant-related threats to these systems are rising from enhanced industrial and municipal effluent discharges along with elevated non-point source inputs related to land-use activities such as forestry, agriculture, mining and long-range atmospheric transport. In this regard, the contaminants program of the Northern River Basins Study (NRBS) in western Canada identified key contaminant sources to the Athabasca, Slave and Peace river basins (particularly related to pulp-mill developments) and assessed their environmental fate and distribution in water and sediments. The study also developed and employed new analytical approaches and generated improved models to predict contaminant transport and fate in the aquatic environment and related food webs. Consequently the study focused on those contaminant families identified in characterization studies as arising from key point-and non-point sources within the basins or as being of greatest toxicological significance. These included resin acids, polychlorinated dioxins and furans, polychlorinated biphenyls, chlorinated phenolics, polyaromatic hydrocarbons and selected heavy metals such as mercury. Low or non-detectable concentrations of a number of contaminant groups were found in the ambient water phase including chlorinated phenolics, some chlorinated dioxins and furans and some resin acids. For both suspended and depositional sediments, significant declines were observed over the study period for the major chlorinated contaminant groups tested, correlating directly with the implementation of improved effluent treatment in many of the pulp mills located in the basins. In general, the environmental levels of chlorinated organic and metal contaminants in water or sediments were low and within Canadian health or environmental guidelines. It is hoped that the approaches used and lessons learned from the NRBS will be of use to others assessing contaminant and multiple stressor issues in other large river ecosystems.

The mean age of this cohort was approximately a decade younger than the average atherothromboembolic stroke victim and women were more commonly involved than men. The frequency of conventional stroke risk factors was lowest in the group of stroke patients with the highest levels of IgG cardiolipin immunoreactivity. Other serological abnormalities associated with aPL (false‐positive Veneral Disease Research Laboratory test, thrombocytopenia, prolonged activated partial thromboplastin time [aPTT]) were more common in the group with over 100 GPL units (high positive). Patients with the highest IgG anticardiolipin titers had the shortest times to subsequent thrombo‐occlusive events. This is an excellent review for the interested clinician.) Visual loss in the patient sustaining facial trauma mandates prompt evaluation, diagnosis, and treatment.l A high index of suspicion is necessary when evaluating these patients because untreated injuries may lead to permanent visual loss and blindness. An evaluation by the ophthamology service often should be performed in any patient who has sustained an orbital or facial fracture because of the potential for an ocular injury until proven otherwise.2 The diagnoses leading to vision loss are numerous and include ocular injury or traumatic optic neuropathy (TON).3,4 TON is associated with head injury (0.5% to 5.0%) and results from an insult to the peripheral optic nerve that is distinctly diagnosed by a relative afferent pupillary defect (RAPD) in light of a normal ocular examination. TON can present immediately after the injury or can be delayed in onset. Once identified, treatment for TON should start promptly. Studies have shown that patients treated with high-dose corticosteroids and/or surgical decompression fare much better than those patients left untreated.5s6 Given the potentially morbid consequences

ABSTRACT This work presents the diagnostic criteria for vestibular migraine, jointly formulated b... more ABSTRACT This work presents the diagnostic criteria for vestibular migraine, jointly formulated by the Committee for Classification of Vestibular Disorders of the Bárány Society and the Migraine Classification Subcommittee of the International Headache Society. The classification includes vestibular migraine and probable vestibular migraine. Vestibular migraine will feature in an appendix of the third edition of the International Classification of Headache Disorders as a first step for new entities, in accordance with the usual procedures of the International Headache Society. Probable vestibular migraine may be included in a later version of the International Classification of Headache Disorders, when further evidence has been gathered. The diagnosis of vestibular migraine is based on recurrent vestibular symptoms, a history of migraine, a temporal association between vestibular symptoms and migraine symptoms and on the exclusion of other causes of vestibular symptoms. The symptoms required for a diagnosis of vestibular migraine include various types of vertigo, as well as head motion-induced dizziness with nausea. Symptoms must be of moderate or severe intensity. The duration of acute episodes is limited to a window of between 5 min and 72 h.

We present herein characteristics of an in-situ forming hydrogel prepared from carboxymethyl chit... more We present herein characteristics of an in-situ forming hydrogel prepared from carboxymethyl chitosan and oxidized carboxymethyl cellulose for interventional therapies. The gelation, owing to the formation of Schiff base, occurred both with and without the presence of a radiographic contrast. The hydrogel exhibited a highly porous internal structure (pore diameter: 17 ± 4 µm), non-cytotoxicity to human umbilical vein endothelial cells, hemocompatibility with human blood, and degradability in lysozyme solutions. Drug release from hydrogel loaded with a sclerosant, tetracycline, was measured at pH 7.4, 6 and 2 at 37 o C, and showed that tetracycline was more stable at acidic conditions with a lower release rate observed at pH 6. Moreover, an anticancer drug, doxorubicin, was loaded into the hydrogel and a cumulative release of 30% was observed over 78 h in PBS at 37 o C. Injection of the hydrogel precursor through a 5-F catheter into a fusiform aneurysmal model was feasible, leading to complete filling of the aneurysmal sac, which was visualized under fluoroscopy. Level of occlusion was compared between hydrogel precursors (1.8% and 2.1%) and standard microspheres (100-300 µm) in a rabbit renal model. Embolization with hydrogel precursors was performed without clogging and the hydrogel achieved effective occlusion in more distal arteries than calibrated microspheres. In conclusion, this hydrogel possesses promising characteristics potentially beneficial for a wide range of interventional vascular procedures that involve embolization and drug delivery.
