A healthy workforce is fundamental for China to pursue its Healthy China 2030 strategy and the UN... more A healthy workforce is fundamental for China to pursue its Healthy China 2030 strategy and the UN Sustainable Development Goals. The Law of the People's Republic of China on the Prevention and Control of Occupational Diseases (the Chinese OD Law), which has been amended four times since its adoption in 2002, is the core and cornerstone of the entire regulatory system for occupational health. In addition to discussing the new institutional reforms, which are relevant to China's regulatory system for occupational diseases (ODs) because of their marked impact on healthcare system reform, we present further evidence, mainly drawn from desk and field research. Our study focuses on the barriers that employers face in complying with the Chinese OD Law and explores the optimized employer-initiated management system, which requires further amendments to the Chinese OD Law, as the State Council of China has prioritized this issue as a top challenge. We first summarize the key characteristics of the Chinese OD Law and illustrate employers' essential obligations, and then analyze the critical challenges that employers face regarding OD prevention and control. We demonstrate good practices and experiences in China, and, finally, propose recommendations for further amendments to the Chinese OD Law in the near future. These recommendations can be achieved only through integrating the healthcare system reform with mechanism innovation. At the beginning of 2018, China drew up a major plan to deepen institutional reform. 5,6 Accordingly, the new National Health Commission was established with the aim of promoting the implementation of the Healthy China 2030 strategy and ensuring the delivery of comprehensive health services to all Chinese people throughout their lives. It is also the ultimate goal of healthcare system reform in China. The regulatory functions regarding occupational OS & H, previously handled by the former National Health and Family Planning Commission and the former State Administration of Work Safety, will be fully incorporated into the National Health Commission. Naturally, this institutional reform will have a marked impact on healthcare system reform in China. Along with this institutional reform, a new round of amendments to the relevant laws will soon be made, with special reference to the Chinese OD Law. Research and technical support for the implementation of the Chinese OD Law has therefore become necessary and urgent. However, the research and evidence base for this task is insufficient. Even fewer research articles on ODs were published in Chinese scientific journals during the past five years than were published in the English literature. 7-10

Proposal for eliminating ARDs A complete ban on all manufacture and use of asbestos worldwide is the only strategy to end the global pandemic of ARDs. The possibility of securing an asbestos ban in the USA was rekindled in 2016 with passage of new chemical safety legislation. Asbestos is listed under this law as a priority chemical. Responsibility for enforcement rests with the Trump Administration.

The original Helsinki criteria (criteria) were published in 1997 (2). The Helsinki criteria updates of 2014 consisted of preparatory work in the form of reviews by four work groups on pre-selected topics (subject areas). This was thus not a systematic re-evaluation of the entire criteria and major portions of the 1997 criteria are unaffected by the updates. Based on the reviews, a set of recommendations for each subject area was formulated by the updating group, as a whole, at a meeting in Helsinki. At the beginning of the meeting, declarations of interest information were requested and obtained from all the participants. Simultaneously with the meeting of the Helsinki criteria updating group, the "International Conference on Asbestos Monitoring and Health Surveillance in Individuals" took place at the same venue. Finland is used as a model in attempts to study the possible association of the incidence of lung... more Finland is used as a model in attempts to study the possible association of the incidence of lung cancer and exposure of population to fossil fuel combustion products. Unfortunately because of great geographical variation of unknown origin in the incidence of lung cancer in Finland, detailed studies of the possible role of an individual exposure in the lung cancer risk are not possible. This background variation in the incidence is much greater than variation carried by any known etiological factor and does not clearly correlate with the degree of urbanization, industrialization, regional use of fossil fuels, number of motor vehicles or smoking habits. To get more precise information on the possible association of lung cancer incidence with exposure to fossil fuel combustion products, occupational studies serve as powerful tools. The definition of population is more reliable and the measurement of exposures can be done more precisely; moreover the management of confounding and modifying factors is more effective than in community studies. So far the studies carried out among the Finnish working population exposed to PAH compounds reveal an association between the lung cancer risk and exposure to PAHs.

The global macro-trends of work life, such as globalization of economies, introduction of new tec... more The global macro-trends of work life, such as globalization of economies, introduction of new technologies, 24-h economy, increased speed and pace of communication, growing time pressure and psychological stress, and mentally demanding work affect drastically also the conditions of work. They are shifting the perspectives and challenges to research on occupational health, safety, occupational hygiene, and other fields of health and safety. The growing dynamics of change, internationalization, short-term work contracts, mobility of workforce , growing role of the service sector, and working in contact with clients, changes in work organization and company structure (merging, downsizing, splitting) all result in breakdown and fragmentation of previously stable continuity typical for traditional industrial settings.

The 2.4 billion working people in the developing countries often have to endure employment condit... more The 2.4 billion working people in the developing countries often have to endure employment conditions, which do not meet even basic occupational safety and health (OSH) standards. The lack of work safety, excessive work loads, and occupational physical, chemical and biological exposures result in occupational diseases, injuries and as many as 1.2 million fatalities each year. Furthermore, as little as 15% of workers in the developing countries have access to occupational health and safety services. Some collaboration between the industrialized and developing countries in the field of OSH has been practiced for 30 years but its volume has been modest compared to other sectors of development assistance. The lessons learned from 30 years of experience are that the most important OSH improving factors in developing countries include legal and policy instruments, national OSH programs, infrastructure for OSH implementation, and OSH services.
