Correct diagnosis of the bipolar patient, along with effective treatment, adequate prevention, an... more Correct diagnosis of the bipolar patient, along with effective treatment, adequate prevention, and reduction of the socioeconomic impact and stigma associated with bipolar disease, is one of the key objectives of any public mental health service. Although we look hopefully to the incipient contributions of new technologies (genomics, proteomics, and neuroimaging), detection of the disorder continues to be primarily based on the clinical presentation, i.e., on the descriptive psychopathology. For many years, the distinction between bipolar disorder and schizophrenia was an area of great difficulty, but changes in diagnostic criteria have shifted interest to early differentiation between unipolar and bipolar depression and between the different entities of the bipolar spectrum. In addition, the peculiarities of the presentation of bipolar disorder in special cases (children and adolescents, pregnancy and postpartum, and the elderly), and the high comorbidity, especially with impulsivity disorders, make the diagnostic process a complicated task requiring conceptual clarification.
