The adoption of good practices for the economic valuation of environmental services (ES) has stro... more The adoption of good practices for the economic valuation of environmental services (ES) has strong implications in the evaluation and design of a Payment for Environmental Services program. People’s willingness to pay for an ES is useful to evaluate whether money collected from users will be enough to cover both the providers’ opportunity costs and other costs generated by the institutional arrangements required for implementation. In this article, we use a numerical certainty scale to adjust answers to a valuation question aiming to correct for hypothetical bias associated with stated preference methods. Following this approach, the mean willingness to pay decreases by approximately 70%. Values that are more conservative could assure greater political and social support for the program because more ES users would want to participate in the program; simultaneously, however, it might suggest that the project is not completely funded.

Generally, classification societies rules are used to predict the structural strength of the main deck of FPSO vessels. However, these classification societies rules are limited to estimate the variation of the structural performance of the main deck caused by the operation of offshore cranes under different hydrodynamic conditions. Here, we present a methodology to determine the alteration of the structural behavior of a main deck of FPSO vessel due to different operation conditions of a board offshore crane. This paper describes the development of a user interface for its implementation in the naval Engineering Educational, is designed with structured and modular programming using open source software MyOpenLab©. The goal of this work is calculated longitudinal resistance of the boats, obtaining the diagrams of shear force and bending moment.

Este artículo presenta una propuesta de instalación de una grúa sobre orugas sobre la cubierta principal de un buque de apoyo a plataformas petroleras, el cual realizará operaciones de mantenimiento en mar adentro. Esta propuesta incluye los resultados del análisis estructural de la cubierta principal, donde se considera el refuerzo con dos vigas NVA36. Este análisis estructural incorpora la integridad de la cubierta principal considerando la grúa y las cargas mediante modelos del método de elementos finitos (MEF). Los resultados obtenidos muestran que la cubierta principal requiere el refuerzo propuesto para disminuir las tensiones máximas de Von Mises hasta 218,71 MPa para la estructura de la cubierta principal y 201,67 MPa en las vigas de refuerzo. These failures can be generated by the barge and cargo weights, and wave load effects on the midships sections. These load types must be considered in the design of the barge midship sections. Here, we present the structural analysis of a barge midship section that has decreased up to 36.4% of its deck thickness caused by corrosion. This analysis is developed using finite element method (FEM) models that include the barge and cargo weights, and wave load effects. The FEM models regarded three cargo tanks in the midship section, containing the main longitudinal and transverse structural elements. BACKGROUND Adult-onset Still's disease (AOSD) is a rare inflammatory disease, typically chara... more BACKGROUND Adult-onset Still's disease (AOSD) is a rare inflammatory disease, typically characterized by spiking fever, skin rash, and arthralgia or arthritis. Its conventional treatment includes NSAIDs and corticosteroids, and DMARDs as second-line therapy. Frequently, IL-1 inhibitors are also required, mainly in patients refractory to traditional therapy. Canakinumab is a monoclonal antibody that binds IL-1β with high affinity and specificity, making it appropriate for therapeutic purposes in AOSD. OBJECTIVE The aim of this systematic review was to identify and compile the current data on the efficacy and safety of canakinumab in the treatment of AOSD. METHODS Following the guidelines established by the PRISMA statement, we searched Scopus, Web of Science, Pubmed, and Cochrane Library for relevant literature up to March 2021. The inclusion criteria comprised: randomized controlled trials, pooled analyses, observational studies, case series, and case reports. RESULTS Seventeen studies published from 2012 to 2021 were evaluated; 11 of these correspond to case series or case reports, four observational studies, one placebo-controlled phase II trial, and one analysis of pooled systemic juvenile idiopathic arthritis data. In general, out of a total of 99 patients, 68.7% of these presented a complete remission of the systemic and arthritic manifestations at the end of the observation period, while 16.2% of the patients showed a partial improvement of the symptoms and the remaining (15.1%) did not show clinical improvement or were excluded. Moreover, 210 adverse events were reported in 69 patients during canakinumab treatment, of which the majority correspond to respiratory tract infections, arthralgia, disease flares, abdominal pain, nausea, and diarrhea, whereas the most common severe adverse events included macrophage activation syndrome and serious infections. Also, a corticosteroid-sparing effect was observed in a large percentage of patients. CONCLUSION More studies with solid evidence are needed to support the efficacy of canakinumab in AOSD, although its use is encouraged by the increasing favorable results reported and the efficacy of other IL-1 inhibitors. It was also associated with an acceptable safety profile, similar to expected in IL-1 inhibitor therapy. However, future studies with well-defined endpoints are warranted to examine further the usefulness of canakinumab in AOSD.

En este trabajo se presentan los análisis de esfuerzos y de fractura en el ánima de un cañón de 3... more En este trabajo se presentan los análisis de esfuerzos y de fractura en el ánima de un cañón de 3"/50 calibre, sometido a presión interna de 95 MPa y con dos tipos de grietas utilizando los criterios de falla de la mecánica de la fractura linealmente elástica. En todos los casos se demostró que se podían aplicar estos criterios. Para el estudio se proponen modelos geométricos en 2-D y 3-D, considerando el rayado interno del ánima, mediante el método de elemento finito (MEF) y con la ayuda de un soft ware comercial. La presencia de grietas en el modelo 2-D, registró un incremento significativo del 68.35% en los esfuerzos de Von Mises, pero sin superar el esfuerzo de fluencia del acero ASTM A723. Se determinaron los factores de intensidad de esfuerzos (FIEs) en las grietas de los modelos geométricos y se realizó un estudio de la zona plástica. Los modelos geométricos propuestos del ánima de un cañón reducen el tiempo de cómputo y son de fácil ejecución para la obtención de una aproximación aceptable de los esfuerzos y el FIE que le ocasionan grietas internas.

Background: Diffuse large B-cell lymphoma (DLBCL) is classified into germinal center-like (GCB) a... more Background: Diffuse large B-cell lymphoma (DLBCL) is classified into germinal center-like (GCB) and non-germinal center-like (non-GCB) cell-of-origin groups, entities driven by different oncogenic pathways with different clinical outcomes. DLBCL classification by immunohistochemistry (IHC)-based decision tree algorithms is a simpler reported technique than gene expression profiling (GEP). There is a significant discrepancy between IHC-decision tree algorithms when they are compared to GEP. Methods: To address these inconsistencies, we applied the machine learning approach considering the same combinations of antibodies as in IHC-decision tree algorithms. Immunohistochemistry data from a public DLBCL database was used to perform comparisons among IHC-decision tree algorithms, and the machine learning structures based on Bayesian, Bayesian simple, Naïve Bayesian, artificial neural networks, and support vector machine to show the best diagnostic model. We implemented the linear discriminant analysis over the complete database, detecting a higher influence of BCL6 antibody for GCB classification and MUM1 for non-GCB classification. Results: The classifier with the highest metrics was the four antibody-based Perfecto-Villela (PV) algorithm with 0.94 accuracy, 0.93 specificity, and 0.95 sensitivity, with a perfect agreement with GEP (κ = 0.88, P < 0.001). After training, a sample of 49 Mexican-mestizo DLBCL patient data was classified by COO for the first time in a testing trial. Conclusions: Harnessing all the available immunohistochemical data without reliance on the order of examination or cutoff value, we conclude that our PV machine learning algorithm outperforms Hans and other IHC-decision tree algorithms currently in use and represents an affordable and time-saving alternative for DLBCL cell-of-origin identification.

Several drugs are available for the treatment of this parasitosis; unfortunately, all of them have variable efficacies and adverse effects. Bursera fagaroides has been known for its anti-inflammatory and antidiarrheal properties in Mexican traditional medicine. We investigated the in vitro anti-giardial activities of four podophyllotoxin-type lignans from Bursera fagaroides var. fagaroides, namely, 5'-desmethoxy-β-peltatin-A-methylether (5-DES), acetylpodophyllotoxin (APOD), burseranin (BUR), and podophyllotoxin (POD). All lignans affected the Giardia adhesion and electron microscopy images revealed morphological alterations in the caudal region, ventral disk, membrane, and flagella, to different extents. Inmediatamente se comenzaron medidas de reanimación, con bolsa, pero al no mejorar se decide intubación traqueal. Se planteó malformación de vías respiratorias por imposibilidad de lograrlo; al intentar traqueotomía, la tráquea está ausente, planteándose el diagnóstico de agenesia traqueal. A pesar de las medidas terapéuticas tomadas, el paciente fallece 8 horas después del nacimiento. En el estudio patológico se comprobó la presencia de agenesia traqueal tipo II, de acuerdo a la clasificación de Floyd.

La mayoría de las enfermedades renales asociadas con gammaatías monoclonales resultan del depósit... more La mayoría de las enfermedades renales asociadas con gammaatías monoclonales resultan del depósito de inmunoglobulinas onoclonales o de sus fragmentos en distintos compartimentos del iñón1. La enfermedad por depósitos de inmunoglobulinas monolonales (EDIM) no amiloide se define por depósitos de cadenas igeras y/o pesadas monoclonales en membranas basales de gloérulos, túbulos y vasos2. Se conocen 3 subtipos de EDIM, la nfermedad por depósitos de cadenas ligeras (EDCL), la enfermead por depósitos de cadenas pesadas (EDCP) y la enfermedad por epósitos de cadenas ligeras y pesadas (EDCLP)3. Dentro de las DIM, la EDCL es la más prevalente4 y es la cadena kappa la que e deposita con mayor frecuencia5. Su expresión clínica se caraceriza por insuficiencia renal con proteinuria, tiene un pronóstico rave y suelen acompañarse de un mieloma múltiple u otro proeso linfoproliferativo6. A continuación describimos el caso de un nciano con una EDCL tipo lambda cuya manifestación clínica fue n síndrome nefrótico severo. Se trataba de un varón de 84 años con antecedentes personaes de hipertensión arterial sin tratamiento, estenosis del canal ervical y lumbar, coxartrosis, crisis de ausencia diagnosticada en 992 en tratamiento con lamotrigina, colelitiasis múltiple asinomática, aneurisma de aorta abdominal infrarrenal, adenoma de róstata, hiperparatiroidismo por adenoma intervenido en 2002, erniorrafia inguinal bilateral en 2005, y apendicectomizado. No enía alergias a medicamentos y era independiente para actividades ásicas de la vida diaria, sin antecedentes de deterioro cogniivo (puntuación en las escalas de Cruz Roja funcional y mental e 0/0). El paciente fue remitido por su médico de cabecera a la conulta de Nefrología por el hallazgo de una proteinuria de 3,84 g/día. iez meses antes había estado ingresado en el Servicio de Mediina Interna, por aumento de tamaño de miembro inferior derecho fue etiquetado de linfedema, recibiendo tratamiento con furoseida y heparina de bajo peso molecular. Revisada la historia de ese ngreso en el sistemático de orina ya se constató la presencia de una roteinuria. En la exploración física en la consulta externa destacaba: peso 8 kg, talla 176 cm, presión arterial 120/60 mmHg, frecuencia caríaca 68 lpm. En la auscultación cardiopulmonar presentaba un oplo sistólico i/iv localizado en foco mitroaórtico e hipoventilación n ambas bases pulmonares. Tenía edemas con fóvea en miemros inferiores, más en la extremidad derecha, siendo el resto de la xploración anodina.

The elderly are begining to use more and more new technologies, although several times they find ... more The elderly are begining to use more and more new technologies, although several times they find these are difficult to interact with, especially when they are accessed by a keyboard or the screen is too small. This work proposes a middleware architecture for TV-based applications for the elderly, with a focus on social interaction services like webmail. The elderly feel very confident with their TVs since it is something they use on a daily basis, and the screen is sufficiently large. The architecture contains a Context Manager with an Ontology that models the elderly user impairments and actual context information collected from sensors located at the user's home, and an Interface Adaptation Engine which generates the adapted interface for a particular user, according to his profile and context of use. This architecture is part of an ongoing EU funded project: MyUI 1 and a first prototype is expected by mid 2011.

Enucleación laparoscópica de páncreas. ANALYSIS OF THE SPANISH NATIONAL REGISTRY OF LAPAROSCOPIC PANCREATIC SURGERY Introduction. The reported experience with laparoscopic pancreatic surgery remains limited to case reports or small series of patients. A recent European multicenter study has allowed the limits and results of this technique to be known. This article presents an analysis of the results of the Spanish National Registry of Laparoscopic Pancreatic Surgery. Material and methods. A total of 132 patients with lesions in the left pancreas were included in this series. The final diagnosis included 42 neuroendocrine tumors, 40 cystic neoplasms, 24 cysts and pseudocysts, 8 inflammatory tumors, 8 ductal carcinomas, 7 intraductal papillary mucinous tumors, 1 acinar carcinoma and 2 solid pseudopapillary tumors. Results. The conversion rate was 9.7%. Tumor enucleation was performed only in patients with insulinomas. The most frequent technique was spleen-preserving distal pancreatectomy. There were no postoperative deaths. The overall rate of postoperative pancreatic-related complications was 16%. Conclusions. Although only a few Spanish hospitals participated in the registry, a greater number of hospitals are expected to enroll patients in the very near future.

Este artículo presenta una propuesta de instalación de una grúa sobre orugas sobre la cubierta principal de un buque de apoyo a plataformas petroleras, el cual realizará operaciones de mantenimiento en mar adentro. Esta propuesta incluye los resultados del análisis estructural de la cubierta principal, donde se considera el refuerzo con dos vigas NVA36. Este análisis estructural incorpora la integridad de la cubierta principal considerando la grúa y las cargas mediante modelos del método de elementos finitos (MEF). Los resultados obtenidos muestran que la cubierta principal requiere el refuerzo propuesto para disminuir las tensiones máximas de Von Mises hasta 218,71 MPa para la estructura de la cubierta principal y 201,67 MPa en las vigas de refuerzo. These failures can be generated by the barge and cargo weights, and wave load effects on the midships sections. These load types must be considered in the design of the barge midship sections. Here, we present the structural analysis of a barge midship section that has decreased up to 36.4% of its deck thickness caused by corrosion. This analysis is developed using finite element method (FEM) models that include the barge and cargo weights, and wave load effects. The FEM models regarded three cargo tanks in the midship section, containing the main longitudinal and transverse structural elements. BACKGROUND Adult-onset Still's disease (AOSD) is a rare inflammatory disease, typically chara... more BACKGROUND Adult-onset Still's disease (AOSD) is a rare inflammatory disease, typically characterized by spiking fever, skin rash, and arthralgia or arthritis. Its conventional treatment includes NSAIDs and corticosteroids, and DMARDs as second-line therapy. Frequently, IL-1 inhibitors are also required, mainly in patients refractory to traditional therapy. Canakinumab is a monoclonal antibody that binds IL-1β with high affinity and specificity, making it appropriate for therapeutic purposes in AOSD. OBJECTIVE The aim of this systematic review was to identify and compile the current data on the efficacy and safety of canakinumab in the treatment of AOSD. METHODS Following the guidelines established by the PRISMA statement, we searched Scopus, Web of Science, Pubmed, and Cochrane Library for relevant literature up to March 2021. The inclusion criteria comprised: randomized controlled trials, pooled analyses, observational studies, case series, and case reports. RESULTS Seventeen studies published from 2012 to 2021 were evaluated; 11 of these correspond to case series or case reports, four observational studies, one placebo-controlled phase II trial, and one analysis of pooled systemic juvenile idiopathic arthritis data. In general, out of a total of 99 patients, 68.7% of these presented a complete remission of the systemic and arthritic manifestations at the end of the observation period, while 16.2% of the patients showed a partial improvement of the symptoms and the remaining (15.1%) did not show clinical improvement or were excluded. Moreover, 210 adverse events were reported in 69 patients during canakinumab treatment, of which the majority correspond to respiratory tract infections, arthralgia, disease flares, abdominal pain, nausea, and diarrhea, whereas the most common severe adverse events included macrophage activation syndrome and serious infections. Also, a corticosteroid-sparing effect was observed in a large percentage of patients. CONCLUSION More studies with solid evidence are needed to support the efficacy of canakinumab in AOSD, although its use is encouraged by the increasing favorable results reported and the efficacy of other IL-1 inhibitors. It was also associated with an acceptable safety profile, similar to expected in IL-1 inhibitor therapy. However, future studies with well-defined endpoints are warranted to examine further the usefulness of canakinumab in AOSD.

En este trabajo se presentan los análisis de esfuerzos y de fractura en el ánima de un cañón de 3... more En este trabajo se presentan los análisis de esfuerzos y de fractura en el ánima de un cañón de 3"/50 calibre, sometido a presión interna de 95 MPa y con dos tipos de grietas utilizando los criterios de falla de la mecánica de la fractura linealmente elástica. En todos los casos se demostró que se podían aplicar estos criterios. Para el estudio se proponen modelos geométricos en 2-D y 3-D, considerando el rayado interno del ánima, mediante el método de elemento finito (MEF) y con la ayuda de un soft ware comercial. La presencia de grietas en el modelo 2-D, registró un incremento significativo del 68.35% en los esfuerzos de Von Mises, pero sin superar el esfuerzo de fluencia del acero ASTM A723. Se determinaron los factores de intensidad de esfuerzos (FIEs) en las grietas de los modelos geométricos y se realizó un estudio de la zona plástica. Los modelos geométricos propuestos del ánima de un cañón reducen el tiempo de cómputo y son de fácil ejecución para la obtención de una aproximación aceptable de los esfuerzos y el FIE que le ocasionan grietas internas.

Background: Diffuse large B-cell lymphoma (DLBCL) is classified into germinal center-like (GCB) a... more Background: Diffuse large B-cell lymphoma (DLBCL) is classified into germinal center-like (GCB) and non-germinal center-like (non-GCB) cell-of-origin groups, entities driven by different oncogenic pathways with different clinical outcomes. DLBCL classification by immunohistochemistry (IHC)-based decision tree algorithms is a simpler reported technique than gene expression profiling (GEP). There is a significant discrepancy between IHC-decision tree algorithms when they are compared to GEP. Methods: To address these inconsistencies, we applied the machine learning approach considering the same combinations of antibodies as in IHC-decision tree algorithms. Immunohistochemistry data from a public DLBCL database was used to perform comparisons among IHC-decision tree algorithms, and the machine learning structures based on Bayesian, Bayesian simple, Naïve Bayesian, artificial neural networks, and support vector machine to show the best diagnostic model. We implemented the linear discriminant analysis over the complete database, detecting a higher influence of BCL6 antibody for GCB classification and MUM1 for non-GCB classification. Results: The classifier with the highest metrics was the four antibody-based Perfecto-Villela (PV) algorithm with 0.94 accuracy, 0.93 specificity, and 0.95 sensitivity, with a perfect agreement with GEP (κ = 0.88, P < 0.001). After training, a sample of 49 Mexican-mestizo DLBCL patient data was classified by COO for the first time in a testing trial. Conclusions: Harnessing all the available immunohistochemical data without reliance on the order of examination or cutoff value, we conclude that our PV machine learning algorithm outperforms Hans and other IHC-decision tree algorithms currently in use and represents an affordable and time-saving alternative for DLBCL cell-of-origin identification.

Bursera fagaroides has been known for its anti-inflammatory and antidiarrheal properties in Mexican traditional medicine. We investigated the in vitro anti-giardial activities of four podophyllotoxin-type lignans from Bursera fagaroides var. fagaroides, namely, 5'-desmethoxy-β-peltatin-A-methylether (5-DES), acetylpodophyllotoxin (APOD), burseranin (BUR), and podophyllotoxin (POD). All lignans affected the Giardia adhesion and electron microscopy images revealed morphological alterations in the caudal region, ventral disk, membrane, and flagella, to different extents. Inmediatamente se comenzaron medidas de reanimación, con bolsa, pero al no mejorar se decide intubación traqueal. Se planteó malformación de vías respiratorias por imposibilidad de lograrlo; al intentar traqueotomía, la tráquea está ausente, planteándose el diagnóstico de agenesia traqueal. A pesar de las medidas terapéuticas tomadas, el paciente fallece 8 horas después del nacimiento. En el estudio patológico se comprobó la presencia de agenesia traqueal tipo II, de acuerdo a la clasificación de Floyd.

La enfermedad por depósitos de inmunoglobulinas monolonales (EDIM) no amiloide se define por depósitos de cadenas igeras y/o pesadas monoclonales en membranas basales de gloérulos, túbulos y vasos2. Se conocen 3 subtipos de EDIM, la nfermedad por depósitos de cadenas ligeras (EDCL), la enfermead por depósitos de cadenas pesadas (EDCP) y la enfermedad por epósitos de cadenas ligeras y pesadas (EDCLP)3. Dentro de las DIM, la EDCL es la más prevalente4 y es la cadena kappa la que e deposita con mayor frecuencia5. Su expresión clínica se caraceriza por insuficiencia renal con proteinuria, tiene un pronóstico rave y suelen acompañarse de un mieloma múltiple u otro proeso linfoproliferativo6. A continuación describimos el caso de un nciano con una EDCL tipo lambda cuya manifestación clínica fue n síndrome nefrótico severo. Se trataba de un varón de 84 años con antecedentes personaes de hipertensión arterial sin tratamiento, estenosis del canal ervical y lumbar, coxartrosis, crisis de ausencia diagnosticada en 992 en tratamiento con lamotrigina, colelitiasis múltiple asinomática, aneurisma de aorta abdominal infrarrenal, adenoma de róstata, hiperparatiroidismo por adenoma intervenido en 2002, erniorrafia inguinal bilateral en 2005, y apendicectomizado. No enía alergias a medicamentos y era independiente para actividades ásicas de la vida diaria, sin antecedentes de deterioro cogniivo (puntuación en las escalas de Cruz Roja funcional y mental e 0/0). El paciente fue remitido por su médico de cabecera a la conulta de Nefrología por el hallazgo de una proteinuria de 3,84 g/día. iez meses antes había estado ingresado en el Servicio de Mediina Interna, por aumento de tamaño de miembro inferior derecho fue etiquetado de linfedema, recibiendo tratamiento con furoseida y heparina de bajo peso molecular. Revisada la historia de ese ngreso en el sistemático de orina ya se constató la presencia de una roteinuria. En la exploración física en la consulta externa destacaba: peso 8 kg, talla 176 cm, presión arterial 120/60 mmHg, frecuencia caríaca 68 lpm. En la auscultación cardiopulmonar presentaba un oplo sistólico i/iv localizado en foco mitroaórtico e hipoventilación n ambas bases pulmonares. Tenía edemas con fóvea en miemros inferiores, más en la extremidad derecha, siendo el resto de la xploración anodina.

The elderly are begining to use more and more new technologies, although several times they find ... more The elderly are begining to use more and more new technologies, although several times they find these are difficult to interact with, especially when they are accessed by a keyboard or the screen is too small. This work proposes a middleware architecture for TV-based applications for the elderly, with a focus on social interaction services like webmail. The elderly feel very confident with their TVs since it is something they use on a daily basis, and the screen is sufficiently large. The architecture contains a Context Manager with an Ontology that models the elderly user impairments and actual context information collected from sensors located at the user's home, and an Interface Adaptation Engine which generates the adapted interface for a particular user, according to his profile and context of use. This architecture is part of an ongoing EU funded project: MyUI 1 and a first prototype is expected by mid 2011.

Enucleación laparoscópica de páncreas. ANALYSIS OF THE SPANISH NATIONAL REGISTRY OF LAPAROSCOPIC PANCREATIC SURGERY Introduction. The reported experience with laparoscopic pancreatic surgery remains limited to case reports or small series of patients. A recent European multicenter study has allowed the limits and results of this technique to be known. This article presents an analysis of the results of the Spanish National Registry of Laparoscopic Pancreatic Surgery. Material and methods. A total of 132 patients with lesions in the left pancreas were included in this series. The final diagnosis included 42 neuroendocrine tumors, 40 cystic neoplasms, 24 cysts and pseudocysts, 8 inflammatory tumors, 8 ductal carcinomas, 7 intraductal papillary mucinous tumors, 1 acinar carcinoma and 2 solid pseudopapillary tumors. Results. The conversion rate was 9.7%. Tumor enucleation was performed only in patients with insulinomas. The most frequent technique was spleen-preserving distal pancreatectomy. There were no postoperative deaths. The overall rate of postoperative pancreatic-related complications was 16%. Conclusions. Although only a few Spanish hospitals participated in the registry, a greater number of hospitals are expected to enroll patients in the very near future.
