Pour le praticien, il est beaucoup plus difficile de bien décider que de bien agir. L'action n'étant le plus souvent que l'application de protocoles simples et reproductibles. Rien de mieux pour illustrer des difficultés de décision que des situations concrètes. La discussion de cas cliniques constituera la trame du congrès du Collège National d'Occlusodontologie (C.N.O.) de mars 2007. Le cas clinique suivant n'est qu'une illustration ponctuelle des nécessaires interrogations que se pose le praticien face à une situation concrète.

The novelty of the All-on-4 concept for a mandibular implant-supported fixed dental prosthesis is... more The novelty of the All-on-4 concept for a mandibular implant-supported fixed dental prosthesis is the inclination of the posterior implants. Typically, the anterior implants are placed lingually relative to the canine/incisor teeth and perpendicular relative to the occlusal plane. According to the laws of elementary biomechanics, the long axis of the implant unit should be aligned to the axis of the occlusal loading forces during clenching in the maximal intercuspal position. When several implants are connected by a prosthesis, the mean axis of the overall occlusal loading must be taken into account. The objective of this report was to propose a different position for anterior implants by tilting them labially to counterbalance the distal inclination of the posterior implants.

ABSTRACT The determinants of the morphology of the mandibular tubercle of the Temporomandibular J... more ABSTRACT The determinants of the morphology of the mandibular tubercle of the Temporomandibular Joint (TMJ) have been the subject of numerous investigations but remain poorly understood. It has been hypothesized that the lingual surfaces of the maxillary anterior teeth could influence the growth, the development, and the shape of the mandibular tubercle. Nonetheless, there is disagreement concerning the existence of a functional relationship between the condylar path and the anterior guidance of the mandible. The objective of the study was to find a correlation between the mean Functional Incisal Path (FIP) of the maxillary anterior teeth and the Functional Condylar Path (FCP). MATERIALS AND METHODS: Subjects included in the study consisted of randomly selected dental students (n = 50). For each student, the following was performed: – The tracing of multiple cuts of silicone of maxillary anterior block which were analyzed by an odontometry software program. – A mechanical axiography to register the protrusive path bilaterally. RESULTS: A statistically significant correlation (r = 0.8; p = 0.0000; FCP = PIF * 0.661 + 9.799) between anterior guidance on the incisors (FIP mean value = 56.96 ± 7.86) and sagittal condylar path inclination (FCP mean value = 47.46 ± 6.56) was found during the protruding movement. CONCLUSION: The clinical application is interesting in the areas of both anterior restorative reconstruction and orthodontic treatment, classically by increasing by 10° the anterior guidance in relation to the condylar path or more precisely by using a regression formula.

Unlike a flat reconstruction, a correct occlusal curvature allows mandibular translation without occlusal interferences in the posterior segment, Furthermore masticatory efficiency on the working side is not lost. In this way, a better distribution of the axial load and protection against T.M.J. overloading is achieved.

Occlusal splint are defined as intra-oral devices mostly indicated to modify the occlusal relatio... more Occlusal splint are defined as intra-oral devices mostly indicated to modify the occlusal relationship between maxillar and mandibular dental arches. Among the different shapes of occlusal splint, an updating seemed necessary to the authors. The main indications for occlusal splint are represented by temporomandibular disorders and teeth protection. Occlusal splints are usually made of hard resin and are, generally, carried on the mandibular jaw. Total occlusal splint are preferable to partial occlusal splint, except in some emergent cases. The smooth occlusal splints are needed for musculo-articular disorders when the indentated splints are reserved for mandibular repositioning in articular temporomandibular joint disorders.
