Glycogen storage disease (GSD) type VI is one of the rare GSD variants characterised by PYGL muta... more Glycogen storage disease (GSD) type VI is one of the rare GSD variants characterised by PYGL mutation. It leads to liver phosphorylase deficiency, thus causing glycogenolysis disorder. Classic manifestations are mild hypoglycaemia, abdominal distension, growth retardation, hepatomegaly, elevated liver transaminases, hyperlipidaemia, and normal lactate and uric acid. This proband is a 14-year-old Malay girl who was the second child of non-consanguineous parents. At four years old, she was referred for the incidental finding of hepatomegaly with deranged liver enzymes. Clinically, there was hepatomegaly (4 cm below the right costal margin) without any growth retardation. Subsequently, at 11 and 13 years old, she experienced a twisted left ovarian cyst and acute colitis, respectively. Biochemically, there was significantly increased transaminases, hypertriglyceridaemia, and mild hypoglycaemia. The liver biopsy result was consistent with GSD. This is a case report of a 47-year old man with an unknown medical illness, presented with neurol... more This is a case report of a 47-year old man with an unknown medical illness, presented with neurological deficits typical of clinical stroke. The Magnetic Resonance Imaging (MRI) findings were an acute left middle cerebral artery (MCA) territory infarction and an incidental finding of a sellar mass likely pituitary origin. The pituitary hormones showed markedly hyperprolactinaemia with the level of 21146 ng/ml (n 4.04 – 15.2 ng/ml), low Follicle-stimulating hormone (FSH) with a level of 0.929 IU/L (n 1.5-12.4 IU/L), and low luteinizing hormone (LH) with a level of 1.11 IU/L (n 1.7-8.6 IU/L). Other investigations of serum cortisol, growth hormone (GH), thyroid-stimulating hormone (TSH), platelet count and coagulation profiles were normal. The patient was treated conservatively in the ward for acute ischaemic stroke and later was started on dopamine agonist cabergoline for hyperprolactinaemia.

INTRODUCTION: Analyte stability time is challenging to employ in clinical settings. The diversity... more INTRODUCTION: Analyte stability time is challenging to employ in clinical settings. The diversity of processing time may affect the accuracy of results. This study evaluates the maximum time delay permissible before sample centrifugation and analysis. MATERIALS AND METHODS: We evaluated 15 serum electrolytes in 40 samples centrifuged and analysed using an automated analyser. The time was divided into 1, 2, 6, and 24 hours. RESULTS: Most analytes studied remained stable for up to 24 hours before centrifugation. However, delayed processing affected the value of potassium, magnesium, LDH, and calcium after 2 hours up to a maximum of 6 hours at room temperature for total protein, phosphate, and sodium. CONCLUSIONS: Most analytes were unaffected by a delay in centrifugation at room temperatures, but several critical analytes were severely affected.

This is a double-blind randomised controlled trial, conducted among 78 patients with primary knee osteoarthritis. Ornithine transcarbamylase (OTC) deficiency (OTCD), the most common urea cycle disorder, is an X-... more Ornithine transcarbamylase (OTC) deficiency (OTCD), the most common urea cycle disorder, is an X-linked genetic disorder due to complete or partial lack of the OTC enzyme. Its clinical presentation depends on the degree of enzyme deficiency and ranges from an acute neonatal metabolic crisis with a high mortality rate through to an asymptomatic adult. We present a case of a newborn baby boy who presented with poor feeding, vomiting, lethargy, and respiratory distress. Laboratory investigations revealed severe hyperammonaemia, hyperglutaminaemia, hyperalaninaemia, absence of citrulline, and marked orotic aciduria. Family screening confirmed the presence of an OTC disease-causing mutation in his mother. It was a heterozygous mutation, c.316G>A. p. Gly106Arg in exon 4.

A cross sectional pilot study was carried out to look into the usefulness of percent free prostat... more A cross sectional pilot study was carried out to look into the usefulness of percent free prostate specific antigen (fPSA) in the diagnosis of prostatic cancer in HUSM patients. All patients who attended surgical clinic and admitted to surgical wards with signs and symptoms of prostate problems during the study period were taken as the study subjects. Total prostate specific antigen (tPSA) was estimated by immunoassay technique and those values of 4 ng/mL or more were proceeded for estimation of fPSA. Using the cut-off value of less than 25% fPSA for diagnosing patients with prostate cancer, our study showed that majority of the prostate cancer patients have a ratio of fPSA:tPSA more than 25% and a significantly higher level of total prostate specific antigen (P<0.005) when compared with patients with benign prostatic hyperplasia (BPH). Insulinoma, a rare pancreatic endocrine tumour is the most common cause of hypoglycemia in patients without systemic illness. Most insulinomas are small, solitary and benign, which can be treated effectively by surgical removal of the tumour. Diagnosis is based on clinical features, laboratory tests and radiological investigation to localize the tumour. However, small size of this tumour leads to difficulty in localization of the disease, especially in occult insulinoma. Intra-arterial calcium stimulation with hepatic venous sampling can be utilised for preoperative localization of the tumour. Insulinoma, a rare pancreatic endocrine tumour is the most common cause of hypoglycemia in patients without systemic illness. Most insulinomas are small, solitary and benign, which can be treated effectively by surgical removal of the tumour. Diagnosis is based on clinical features, laboratory tests and radiological investigation to localize the tumour. However, small size of this tumour leads to difficulty in localization of the disease, especially in occult insulinoma. Intra-arterial calcium stimulation with hepatic venous sampling can be utilised for preoperative localization of the tumour.
