The aim of this study was to assess the impact of low-volume metastasis (LVM) on disease-free sur... more The aim of this study was to assess the impact of low-volume metastasis (LVM) on disease-free survival (DFS) in women with apparent early-stage endometrial cancer (EC) who underwent sentinel lymph node (SLN) mapping. Patients with pre-operative early-stage EC were retrospectively collected from an international collaboration including 13 referring institutions. A total of 1428 patients were included in this analysis. One hundred and eighty-six patients (13%) had lymph node involvement. Fifty-nine percent of positive SLN exhibited micrometastases, 26.9% micrometastases, and 14% isolated tumor cells. Seventeen patients with positive lymph nodes did not receive any adjuvant therapy. At a median follow-up of 33.3 months, the disease had recurred in 114 women (8%). Patients with micrometastases in the lymph nodes had a worse prognosis of disease-free survival compared to patients with negative nodes or LVM. ObjectiveSentinel lymph node (SLN) mapping represents the standard approach in uterine confined e... more ObjectiveSentinel lymph node (SLN) mapping represents the standard approach in uterine confined endometrial cancer patients. The aim of this study was to evaluate the anatomical distribution of SLNs and the most frequent locations of nodal metastasis.MethodsThis was an observational retrospective multicenter study involving eight high volume gynecologic cancer centers in Italy. We reviewed 1576 patients with a histologically confirmed diagnosis of endometrial cancer from September 2015 to June 2020. All patients underwent total hysterectomy with salpingo-ophorectomy and SLN mapping.ResultsA total of 3105 SLNs were mapped and removed, 2809 (90.5%) of these were bilateral and 296 (9.5%) unilateral. The overall detection rate was 93.4% (77.9% bilateral and 15.5% unilateral). The majority of SLNs (80%) and positive SLNs (77.8%) were found at the external iliac and obturator level in both endometrioid and non-endometrioid endometrial cancer. Endometriosis is a common gynecologic disease, affecting up to 11 % of premenopausal women. Typic... more Endometriosis is a common gynecologic disease, affecting up to 11 % of premenopausal women. Typical symptoms of endometriosis include dysmenorrhea, dyspareunia and chronic pelvic pain. In its deep invasive form, endometriosis has the potential to infiltrate organs and ligaments, including the recto-vaginal septum, the parametria, the rectosigmoid, the ureters and the bladder. These anatomical structures are in strict contiguity to several autonomic nerves that provide their neural supply to the pelvis, particularly to the vagina, the anal sphincter and the bladder. In order to preserve the function of these organs, surgical treatment for deep infiltrating endometriosis should take into account the distribution of the ortho- and para-sympathetic fibers to the pelvis and in particular the routes of the hypogastric nerves, the pelvic splancnic nerves and the pelvic plexus. Based on the strong recommendation of the World Health Organization to implement testing for SARS... more Based on the strong recommendation of the World Health Organization to implement testing for SARS-CoV-2, at our Department of Obstetrics and Gynecology in northeast Italy we started performing universal screening in the obstetric population prior to hospitalization. All pregnant women admitted at our department were tests for SARS-CoV-2 with a nasopharyngeal swab test based on the specific SARS-CoV-2 sequences amplification with reverse transcription polymerase chain reaction (RT-PCR) assay. From April 10th to May 25th (45 days), 473 women were tested, and 2 (0.42%) out of 473 patients were found positive to SARS-CoV-2 during the first three weeks when the numbers of infection were higher and social contacts reduced. No patients reported either respiratory or gastrointestinal symptoms.
