OBJECTIVE Measure the effect of the strategy to fight the Covid-19 pandemic, based on containment... more OBJECTIVE Measure the effect of the strategy to fight the Covid-19 pandemic, based on containment, on the practice of orthopedic and trauma surgery in Tunisia. METHODS This is a comparative study, concerning the impact of Covid-19 on the flow of patients, operated in orthopedic surgery and trauma departments, during the containment period from March 12, 2020 to April 30, 2020, comparing it to the same period of the year 2019, in the Central-East region of Tunisia (Kairouan, Sousse, Mahdia, Monastir). RESULTS A significant decrease of 27% in the flow of patients admitted to orthopedic surgery departments in central-eastern Tunisia, during the 2020 containment period, compared to the same period of the previous year was documented. This decrease was generally widespread, notified in three orthopedic departments: Sousse (39%; p<10-3), Monastir (29%; p<10-3) and Mahdia (27%; p<10-3). It focused on the following three groups: infections (45%; p<10-3), wounds (30%; p<10-2), and fractures (20%; p<10-3). CONCLUSION The collateral effects of the strategy to fight the Covid-19 pandemic, are "obvious" in orthopedic surgical practice. A health crisis management plan (including disasters and pandemics) should be better prepared in orthopedic surgery departments.

Purpose: Aneurysmal bone cysts are benign and expansile osteolytic lesions that can occur in any ... more Purpose: Aneurysmal bone cysts are benign and expansile osteolytic lesions that can occur in any location in the spine. This report describes clinical characteristics and treatment results of 9 young patients with aneurysmal bone cyst of the spine. Methods: Between 1997 and 2015, 9 young patients with aneurysmal bone cyst of the spine were surgically treated in our department. The clinical records, radiographs, and operative reports were analysed. Results: This study included 9 paediatric cases (6 girls, 3 boys) managed for aneurysmal bone cyst. The mean age was 11 years. Posterior spinal elements were affected in all cases. The lesion affected the cervical spine (1 case), thoracic spine (3 cases), and lumbar region (5 cases). The radiological aspect was typical in all cases. MRI imaging was very suggestive of the diagnosis showing specific features of the lesion. It was described as an expansil lytic process arising from vertebral posterior elements. All patients underwent a surgical removal of the tumor following a surgical biopsy. A segmental instrumented fusion in order to establish a proper spinal stability was made in 5 cases. Histological report confirmed the diagnosis of aneurysmal cyst in all patients. The mean follow-up was 7, 7 years. The follow-up course was uneventful. Conclusion: The presented study demonstrates that lesional resection and structural spinal integrity made by local spinal fusion allow good outcomes with a low rate of complications and low recurrence.

This treatment can be a high tibial osteotomy or a joint replacement that can be a partial unicompartmental knee arthroplasty (UKA) or total knee replacement (TKR). The aim of our study is the long-term evaluation of functional outcome of partial unicompartmental knee arthroplasty (UKA) in elderly patients under than 60 years. METHODS This is a retrospective study of 25 unicompartmental knee arthroplasties in 22 patients, collected between1993 and 2003 whose age was less than 60 years. The minimum follow-up was 10 years. At last follow, an analysis of IKS score (International Knee Score) with assessment of both articular and functional components have been established. This intervention needs to identify some pearls and pitfalls. AIM To evaluate results of total knee arthroplasty after closing wedge high tibial osteotomy. METHOD Fourty total knee arthroplasty following closing wedge high tibial osteotomy were identified. Demographic features, tibiofemoral alignment and radiolucent lines were documented on postoperatively. Clinical and functional evaluations were performed preoperatively and postoperatively according IKSS score. RESULTS The average duration of follow-up was 11,5 years. The mean function score increased from 39 points to 70,4 points postoperatively. The mean knee score increased from 40 points to 84,9 points postoperatively. The mean average tibio-femoral angle was 177,7° in preoperatively and 178° (min 176°, max 185°) at the last follow-up. Introduction: Villonodular synovitis (SVN) is a rare benign pseudotumoral proliferation of the sy... more Introduction: Villonodular synovitis (SVN) is a rare benign pseudotumoral proliferation of the synovial joint, of unknown etiology. In general, it reaches the big joints, especially the knee. Localization at the subtalar level is extremely rare, with only a few cases published in the literature. Case Report: We report the case of an SVN of the subtalar in a 60-year-old patient without particular histories, who consulted for a swelling of the external border of the right foot evolving for 2 years. Biology, as well as the standard radiographs, was without anomalies. The diagnosis was evoked on magnetic resonance imaging (MRI) and confirmed by surgical biopsy and excision. The operative follow-up was simple with no recurrence after 3 years. Conclusion: SVN of the subtalar is rare. Surgical treatment is based mainly on synovectomy, and the prognosis depends mainly on the osteocartilaginous lesion and the quality of excision.

The purpose of this study was to identify the main clinical features of radial neck fracture in children and to evaluate the anatomical and functional results of the Metaizeau technique. Although total knee arthroplasty is a successful procedure, postoperative residual knee pain afte... more Although total knee arthroplasty is a successful procedure, postoperative residual knee pain after total knee arthroplasty is still the major factor that contributes to patient dissatisfaction. Even after resurfacing the patella, residual anterior knee pain is the most frequent complaint. Numerous etiologies have been associated with residual pain, including mid-flexion instability, aseptic loosening, infection, and patellar maltracking due to component malalignment. Other rare etiologies like patellar clunk syndrome could also cause residual knee pain after total knee arthroplasty. Through this case report of a patellar clunk syndrome successfully treated with arthroscopic resection, we discuss the etiopathogeny, diagnostic challenge, and treatment of this rare entity.

Introduction Les syndromes de conflit de la cheville peuvent etre anterieurs ou posterieurs. Les ... more Introduction Les syndromes de conflit de la cheville peuvent etre anterieurs ou posterieurs. Les conflits anterieurs ont ete initialement decrits chez les athletes, mais peuvent aussi etre observes en post-traumatique. L’objectif de notre etude est de determiner les caracteristiques epidemiologiques et cliniques du conflit anterieur de la cheville, de determiner la place de l’arthroscopie dans la prise en charge de ces lesions et d’evaluer les resultats anatomiques et fonctionnels apres traitement arthroscopique. Materiel et methodes Il s’agit d’une etude retrospective menee sur 5 patients traites pour conflits anterieurs de la cheville par arthroscopie. Notre etude a ete menee sur une periode de 3 ans. Resultats Tous nos patients avaient des conflits antero-externes dont 3 sont d’origine tissulaire et 2 d’origine osseuse. L’âge moyen de nos patients etait de 36,4 ans. Le sexe-ratio etait de 1,5. La notion de traumatisme etait presente dans tous les cas. De bons et d’excellents resultats etaient notes dans 80 % des cas. Discussion et conclusion La manifestation clinique du conflit anterieur est dominee par un syndrome douloureux et le diagnostic est avant tout clinique. La radiographie standard et l’echographie confirment le diagnostic et on a rarement besoin de completer les explorations par une TDM ou une IRM. On distingue 2 types de conflit anterieur : les conflits d’interposition tissulaire qui sont les plus frequents et les conflits osseux. Le conflit anterieur pur est rarement isole ; on a souvent des conflits antero-externes ou antero-internes. Le traitement medical est instaure de premiere intention et le traitement chirurgical n’est envisage qu’en cas d’echec du traitement medical. Actuellement, le traitement arthroscopique des conflits anterieurs de la cheville est bien etabli. La bonne indication therapeutique et la maitrise de la technique arthroscopique sont les principaux moteurs de la reussite therapeutique.

McKeever first reported on the use of a patellar prosthesis in 1955. Blazina first reported on pa... more McKeever first reported on the use of a patellar prosthesis in 1955. Blazina first reported on patellofemoral replacement in 1979. The success rate of the procedure ranges from 70% to 94% depending on the author. We are reporting a retrospective study of 38 knees treated with patellofemoral arthroplasty between 1980 and 1997. Patients were assessed by questionnaire and clinical examination. Objective measures using the knee society score, the Hungerford-Kenna score and WOMAC score were performed. Radiological examination, as well as a survivorship analysis were performed. The average age at operation was 58 years with an average follow up of 9 years. Surgery was performed for isolated patellofemoral osteoarthritis. Seventy one percent of patients were classed as successful. The best results scored in patients with patellofemoral dysplasia. Failures resulted from mal-alignment, resulting in instability and progression of femoral-tibial osteoarthritis. Careful selection of patients is essential and the femoral-tibial arthritis is a relative contra-indication.
Congenital constriction of the limbs is usually due to amniotic band syndrome, which often causes... more Congenital constriction of the limbs is usually due to amniotic band syndrome, which often causes damage to the skin and soft tissues. We report an unusual case in which a neonate had recent fractures of both leg eywords: ongenital constriction ring ecent fracture ibia bones with an amniotic band encircling the limb. Non-operative treatment was successful. Challenges to the management of neonatal limb constriction include the absence of a consensus about the best treatment and the high frequency of damage to vessels and nerves. © 2016 Published by Elsevier Masson SAS.
