Objectives Chronic insomnia is common in late adulthood. A non-pharmacological approach should ta... more Objectives Chronic insomnia is common in late adulthood. A non-pharmacological approach should take priority in the treatment of insomnia for the elderly. Many studies have shown the efficacy of Cognitive Behavioral Therapy for Insomnia (CBT-I) for elderly diagnosed with insomnia. However the effect of CBT-I on mild insomnia among older adults in community settings has not been ascertained. We conducted a randomized controlled trial to evaluate the effectiveness of a brief CBT-I delivered by nurses, which is feasible in community settings, to improve sleep quality and decrease the dose of hypnotics use for older adults.Methods Participants aged 60 years and over were enrolled in this study. The participants in the intervention group were administrated the brief CBT-I consisting of a group session (60 min) and an individual session (30 min). The primary outcomes were the score differences in the Pittsburgh Sleep Quality Index (PSQI) and Insomnia Severity Index (ISI). The secondary outcomes were the change in the proportion of people diagnosed with insomnia and the dose of hypnotics used. The follow-up period was 3 months.Results The score on PSQI in the intervention group (n=41) significantly decreased compared to the control group (n=38). The effect size (Cohen's d) was 0.56 (95% Confidence interval [CI], 0.07 to 1.05). The score on ISI also decreased significantly and Cohen's d was 0.77 (95%CI, 0.27 to 1.26). According to subgroup analysis, Number Needed to Treat (NNT) for improvement of insomnia was 2.8 (95%CI, 1.5-17.2) and NNT for decreasing of dose of hypnotics use was 2.8 (95%CI, 1.5-45.1).Conclusion The present results have demonstrated that the brief CBT-I significantly improved subjective evaluation of sleep quality and insomnia symptoms among the elderly. In addition, the brief CBT-I decreased the usage of hypnotics. Further studies are needed in terms of the procedure and the effects of brief CBT-I for older adults living in a community.

The purpose of this study is to develop a new screening test for detecting the demented elderly i... more The purpose of this study is to develop a new screening test for detecting the demented elderly in the early stage in communities. The test is easy to apply for consultation, guidance and care and is capable of administering differential diagnoses. Based on the 9 dementia rating scales used in Japan, Europe and the US., a new test was completed after investigating and modifying the design 5 times. The test consists of 20 items. This test was given to 203 subjects (59 males and 144 females) including normal elderly as well as those suspected of suffering from dementia. The internal consistency, reliability and validity were studied using clinical diagnoses (diagnostic criteria of Diagnostic and Statistical Manual of Mental Disorders, Third Edition (DSM-III) and Karasawa's Criteria for Judging Senility), Hasegawa's Dementia Rating Scale and Mental Status Questionnaire (MSQ) as external criteria. The present test was confirmed to have sufficient effectiveness as the screening test for dementia.

e purpose of this study was to examine the cross-sectional association between quality of life (Q... more e purpose of this study was to examine the cross-sectional association between quality of life (QOL) in older adults and their interest in and standard of selection of clothing. e QOL of older adults in this study was assessed by the sense of life worth living (in Japanese, Ikigai). We hypothesized that having an interest in clothing and dressing behaviors may help maintain and enhance the QOL of older adults. In January 2010, a questionnaire survey including demographic factors (age, sex, and activities of daily living), interest in clothing, standard of selection of clothing, and sense of life worth living was distributed to 850 older Japanese people (aged 70-95 years) who were registered with a survey company. Responses from 499 people (256 men and 243 women; response rate, 58.7%) were analyzed. For standard of selection of clothing, all scores for women were higher than they were for men; no signi cant age-group di erences were observed, however. Furthermore, scores for interest in one s own and others dressing behavior and interest in fashion were signi cantly greater in those who were younger or independent. e hierarchical multiple regression analyses also showed that the clothing-related variables signi cantly explained variance of the sense of life worth living (Ikigai) in older people (from 10% to 23%). We suggest that selecting personal taste in dress and/or an interest in clothing contribute to maintaining and enhancing QOL in older adults.

Methods: A questionnaire survey was distributed that determined age, sex, physical health, exercise level, SE for exercise, and personality traits of 1,515 older Japanese adults. Of these 1,515 people, 876 aged 60-92 years completed the questionnaire survey. Results: Extraversion and conscientiousness were signi cantly and positively associated with exercise level a er controlling for age, sex, and physical health. All domains of personality traits (extraversion, openness, agreeableness, and conscientiousness were positive, but neuroticism was negative) were signi cantly correlated with SE for exercise. Structural equation modeling showed that only extraversion was directly a ected by exercise level, all personality traits showed signi cant pathways to SE for exercise, and SE for exercise was the greatest predictor of exercise level in all pathways in the nal model. Conclusion: Extraversion is directly associated with exercise level, extraversion, and other personality traits that are a ected by SE for exercise. Additionally, SE for exercise is the greatest predictor of exercise level in older Japanese adults.

In 1990 a functional fimess test for the elderly was proposed by the American Alliance for Health... more In 1990 a functional fimess test for the elderly was proposed by the American Alliance for Health, Physical Education,Recreation and Dance (AAHPERD). This study was undertaken to determine the applicability of that test for dderly Japanese adults. The fimess parameters of the test were "total body flexibility","agility and dynamic balance","coordination of eye, arm and hand movements","strength and endurance",and "aerobic capacity". The sample consisted of five hundred thirty-four healthy normal dderly adults between the ages of 60 and 90. The results indicated that each test item possessed rdatively high reliability and validity. It would be useful to give this test to the elderly Japanese adults in order to determine their functional fitness levels measured by each test item individually and also to assess the relationships between their functional fitness levels and their activities of daily living, particularly those involving cognitive and social abilities.

This evidence-based practice paper attempts to reflect learning of a particular problem through q... more This evidence-based practice paper attempts to reflect learning of a particular problem through qualitative inquiry of working with individual, families, and group of institutionalized elderly women. It reflects the elderly problems, intervention process, and outcome of the social work practice. It attempted to analysis the planned use of empirically supported assessment and intervention methods combined with the use of monitoring and evaluation strategies for the purpose of improving the psychosocial well being of the elderly women. It recommends that enhancement of family social work to strengthen the interpersonal and intergenerational relationships in both aging and elderly care. The family’s role as a provider of social welfare may become salient in the promoting psychosocial wellbeing of the elderly.

This evidence-based practice paper attempts to reflect learning of a particular problem through q... more This evidence-based practice paper attempts to reflect learning of a particular problem through qualitative inquiry of working with individual, families, and group of institutionalized elderly women. It reflects the elderly problems, intervention process, and outcome of the social work practice. It attempted to analysis the planned use of empirically supported assessment and intervention methods combined with the use of monitoring and evaluation strategies for the purpose of improving the psychosocial well being of the elderly women. It recommends that enhancement of family social work to strengthen the interpersonal and intergenerational relationships in both aging and elderly care. The family’s role as a provider of social welfare may become salient in the promoting psychosocial wellbeing of the elderly.

e purpose of this study was to examine the cross-sectional association between quality of life (Q... more e purpose of this study was to examine the cross-sectional association between quality of life (QOL) in older adults and their interest in and standard of selection of clothing. e QOL of older adults in this study was assessed by the sense of life worth living (in Japanese, Ikigai). We hypothesized that having an interest in clothing and dressing behaviors may help maintain and enhance the QOL of older adults. In January 2010, a questionnaire survey including demographic factors (age, sex, and activities of daily living), interest in clothing, standard of selection of clothing, and sense of life worth living was distributed to 850 older Japanese people (aged 70-95 years) who were registered with a survey company. Responses from 499 people (256 men and 243 women; response rate, 58.7%) were analyzed. For standard of selection of clothing, all scores for women were higher than they were for men; no signi cant age-group di erences were observed, however. Furthermore, scores for interest in one s own and others dressing behavior and interest in fashion were signi cantly greater in those who were younger or independent. e hierarchical multiple regression analyses also showed that the clothing-related variables signi cantly explained variance of the sense of life worth living (Ikigai) in older people (from 10% to 23%). We suggest that selecting personal taste in dress and/or an interest in clothing contribute to maintaining and enhancing QOL in older adults.
